RESUMEN

We discuss invisibility cloaking of metal grid electrodes on Lambertian light emitters by using dielectric free-form surfaces. We show that cloaking can be ideal in geometrical optics for all viewing directions if reflections at the dielectric-air interface are negligible. We also present corresponding white-light proof-of-principle experiments that demonstrate close-to-ideal cloaking for a wide range of viewing angles. Remaining imperfections are analyzed by ray-tracing calculations. The concept can potentially be used to enhance the luminance homogeneity of large-area organic light-emitting diodes.

In this paper, we experimentally demonstrate simultaneous wavelength and orbital angular momentum (OAM) multiplexing/demultiplexing of 10 Gbit/s data streams using a new on-chip micro-component-tunable MEMS-based Fabry-Perot filter integrated with a spiral phase plate. In the experiment, two wavelengths, each of them carrying two channels with zero and nonzero OAMs, form four independent information channels. In case of spacing between wavelength channels of 0.8 nm and intensity modulation, power penalties relative to the transmission of one channel do not exceed 1.45, 0.79 and 0.46 dB at the hard-decision forward-error correction (HD-FEC) bit-error-rate (BER) limit 3.8 × 10-3 when multiplexing a Gaussian beam and OAM beams of azimuthal orders 1, 2 and 3 respectively. In case of phase modulation, power penalties do not exceed 1.77, 0.54 and 0.79 dB respectively. At the 0.4 nm wavelength grid, maximum power penalties at the HD-FEC BER threshold relative to the 0.8 nm wavelength spacing read 0.83, 0.84 and 1.15 dB when multiplexing a Gaussian beam and OAM beams of 1st, 2nd and 3rd orders respectively. The novelty and impact of the proposed filter design is in providing practical, integrable, cheap, and reliable transformation of OAM states simultaneously with the selection of a particular wavelength in wavelength division multiplexing (WDM). The proposed on-chip device can be useful in future high-capacity optical communications with spatial- and wavelength-division multiplexing, especially for short-range communication links and optical interconnects.

We demonstrate an on-chip device capable of wavelength-selective generation of vortex beams, which is realized by a spiral phase plate integrated onto a microelectromechanical system (MEMS) tunable filter. This vortex MEMS filter, being capable of functioning simultaneously in both wavelength and orbital-angular-momentum (OAM) domains at the 1550 nm wavelength regime, is considered as a compact, robust, and cost-effective solution for simultaneous OAM- and wavelength-division multiplexed optical communications. The experimental OAM spectra for azimuthal orders 1, 2, and 3 show an OAM state purity >92% across a wavelength range of more than 30 nm.

We present a light-extraction approach allowing for single-pass and omniangle outcoupling of light from light-emitting diodes (LED). By using transformation optics, we perceive a feasible graded-index structure that is a transition from the LED exit facet to a low refractive index region with expanded space that represents air. Apart from the material dispersion of the constituents, our approach is wavelength independent. The suggested extractor is geometrically compact with size parameters comparable to the width of an LED and therefore well adapted for pixelated LEDs. A beam-expanding functionality is possible while fully preserving the outcoupling efficiency by applying index and geometry truncation.

INTRODUCTION: Succinylcholine and rocuronium are widely used to facilitate rapid sequence induction (RSI) intubation in intensive care. Concerns relate to the side effects of succinylcholine and to slower onset and inferior intubation conditions associated with rocuronium. So far, succinylcholine and rocuronium have not been compared in an adequately powered randomized trial in intensive care. Accordingly, the aim of the present study was to compare the incidence of hypoxemia after rocuronium or succinylcholine in critically ill patients requiring an emergent RSI. METHODS: This was a prospective randomized controlled single-blind trial conducted from 2006 to 2010 at the University Hospital of Basel. Participants were 401 critically ill patients requiring emergent RSI. Patients were randomized to receive 1 mg/kg succinylcholine or 0.6 mg/kg rocuronium for neuromuscular blockade. The primary outcome was the incidence of oxygen desaturations defined as a decrease in oxygen saturation ≥ 5%, assessed by continuous pulse oxymetry, at any time between the start of the induction sequence and two minutes after the completion of the intubation. A severe oxygen desaturation was defined as a decrease in oxygen saturation ≥ 5% leading to a saturation value of ≤ 80%. RESULTS: There was no difference between succinylcholine and rocuronium regarding oxygen desaturations (succinylcholine 73/196; rocuronium 66/195; P = 0.67); severe oxygen desaturations (succinylcholine 20/196; rocuronium 20/195; P = 1.0); and extent of oxygen desaturations (succinylcholine -14 ± 12%; rocuronium -16 ± 13%; P = 0.77). The duration of the intubation sequence was shorter after succinycholine than after rocuronium (81 ± 38 sec versus 95 ± 48 sec; P = 0.002). Intubation conditions (succinylcholine 8.3 ± 0.8; rocuronium 8.2 ± 0.9; P = 0.7) and failed first intubation attempts (succinylcholine 32/200; rocuronium 36/201; P = 1.0) did not differ between the groups. CONCLUSIONS: In critically ill patients undergoing emergent RSI, incidence and severity of oxygen desaturations, the quality of intubation conditions, and incidence of failed intubation attempts did not differ between succinylcholine and rocuronium. RESUMEN

A 49-year-old male patient presented with a paravalvular leakage 18 years after aortic valve replacement. The patient received a mechanical valve and was weaned uneventfully from cardiopulmonary bypass. After a drop in the pressure curve of the left radial artery an additional line was placed in the proximal ascending aorta showing normal systemic pressure. Transesophageal echocardiography showed a massive circular thrombosis of the distal aortic arch. The patient received a covered stent to expand the residual aortic lumen and left the hospital in good health.

RESUMEN

We report on a cryogenic scanning tunneling microscope (STM) designed for single molecule studies, in which the light emitted from the tunneling junction is collected by an integrated optics on the tip. Using direct laser writing, the tip and the surrounding microscopic parabolic mirror are fabricated as one piece, which is small enough to collimate the collected light directly into an optical multimode fiber fixed inside the STM. This simple and compact setup combines high collection efficiency and ease of handling while not interfering with the cryostat operation, allowing uninterrupted measurements at 1.4 K for up to 5 days with low drift.

The ability to create superoleophobic surfaces repellent toward low-surface-tension liquids is important for various applications, and has been recently demonstrated using re-entrant or doubly re-entrant microtopography. Liquid droplets on such surfaces feature composite liquid-solid-air interfaces, whereas composite liquid-lubricant-air interfaces would have potential for additional repellency. Here, the development of a novel slippery superoleophobic surface with low adhesion is demonstrated via combining doubly re-entrant microtopography with slippery lubricant-infused porous surfaces. This is realized by using 3D direct laser writing to fabricate doubly re-entrant micropillars with dedicated nanostructures on top of each pillar. The top nanostructures stabilize the impregnated slippery lubricant, while the re-entrant geometry of the micropillars prevents lubricant from spreading. The slippery layer reduces the adhesion of liquid to the pillars, as proved using scanning droplet adhesion microscopy (SDAM), while the doubly re-entrant micropillars make the surface superoleophobic. This novel interface combining two extremes, superoleophobicity and slippery lubricant-infused surface, is of importance for designing superoleophobic and superhydrophobic surfaces with advanced liquid repellent, anti-icing, or anti-fouling properties.

BACKGROUND: Keeping track of the endoscope tip in 3 planes (axial, coronal, and sagittal) while performing skull base surgeries can be difficult because the surgeon is focused most on the live video images of the endoscope. For that reason, it was the aim of this anatomical cadaver study to evaluate the usefulness of a voxel-based neuronavigation system with 3-dimensional (3D) perspective image rendering for endoscopic procedures through keyhole approaches to the skull base. METHODS: On 5 whole-body fixed cadavers, frontolateral and retrosigmoid approaches were performed bilaterally using a neuronavigation system with 3D perspective image rendering (Cbyon, Med-Surgical Services Inc., Sunnyvale, California). Target points defined on the selected target structures were approached with the navigated ∅ 4-mm 0° endoscope (Storz, Tuttlingen, Germany). Using an Endocameleon 4-mm rigid endoscope capable of changing its angle of view while remaining stationary, the surgical field was checked for injuries before and after insertion of the navigated 0° endoscope. RESULTS: The median neuronavigation registration error was 0.95 mm (range 0.6 to 1.2 mm). Evaluation showed that 100% of the defined targets were reached and visualized. Neither a target structure nor neurovascular structures or surrounding brain tissue were injured by the navigated 0° endoscope. CONCLUSIONS: A neuronavigation system with 3D voxel-based perspective image rendering could potentially improve safety during complex skull base surgeries, and possibly also help to improve surgical results. Such a system, however, cannot replace a neurosurgeon's experience nor surgical skill or anatomical knowledge. It is an excellent teaching tool for young neurosurgeons, but it also has some limitations. Therefore, clinical studies will be necessary to further evaluate the benefits of this type of neuronavigation system in a clinical setting.

BACKGROUND: The advent of electronic learning, the so-called e-learning, offers new possibilities for instruction in addition to the traditional face-to-face teaching in the education of medical students. AIM: To evaluate the additional educational value of a voluntary e-learning module in a nuclear medicine course for third-year medical students. METHODS: Twenty exemplary nuclear medicine patient cases from our department were developed for e-learning purposes and presented on the internet using the web-based training program 'CaseTrain'. Subsequently, three selected test cases were handled and evaluated by an unselected population of third-year medical students. RESULTS: One hundred and twenty-eight students studied the three patient cases and filled out the evaluation questionnaire completely. The most important result is that both the interest in and the subjective feeling of the knowledge level regarding the specialized field of nuclear medicine had increased significantly after working through the three e-learning cases. Ninety-seven percent of the evaluating students considered the use of computer-based learning useful. The subjective grading of the content of the cases and the handling of the software were graded with high marks by the participants, 1.9 and 2.0, respectively, on a linear scale with 1 being the best and 6 being the worst. CONCLUSION: The addition of e-learning to face-to-face teaching as a form of 'blended learning' is highly appreciated by medical students, and will provide an effective medium for bringing better understanding of nuclear medicine to future colleagues.
