RESUMEN

The effects of blue light at 455 nm were investigated on the bacterial composition of human dental plaque in vivo. Eleven subjects who refrained from brushing for 3 days before and during phototherapy participated in the study. Light with a power density of 70 mW/cm(2) was applied to the buccal surfaces of premolar and molar teeth on one side of the mouth twice daily for 2 min over a period of 4 days. Dental plaque was harvested at baseline and again at the end of 4 days from eight posterior teeth on both the exposed side and unexposed sides of the mouth. Microbiological changes were monitored by checkerboard DNA probe analysis of 40 periodontal bacteria. The proportions of black-pigmented species Porphyromonas gingivalis and Prevotella intermedia were significantly reduced on the exposed side from their original proportions by 25 and 56 %, respectively, while no change was observed to the unexposed side. Five other species showed the greatest proportional reduction of the light-exposed side relative to the unexposed side. These species were Streptococcus intermedius, Fusobacterium nucleatum ss. vincentii, Fusobacterium nucleatum ss. polymorphum, Fusobacterium periodonticum, and Capnocytophaga sputigena. At the same time, the percentage of gingival areas scored as being red decreased on the side exposed to light from 48 to 42 %, whereas the percentage scored as red increased on the unexposed side from 53 to 56 %. No adverse effects were found or reported in this study. The present study proposes a new method to modify the ecosystem in dental plaque by phototherapy and introduces a new avenue of prophylactic treatment for periodontal diseases.

OBJECTIVE: This study examined the use of color measurement to determine tooth color in a study of bleaching with peroxide and light used separately and in combination. Two measurement protocols were evaluated for their ability to reveal differences between treatments and confirm tooth shade evaluations. METHODOLOGY: Before and after treatment, 43 subjects were evaluated for tooth color using a shade guide and a chromameter. Two measurement procedures were used: one measured a single location on each tooth using a fabricated plastic stent, and the other measured nine locations across the buccal surface. Values from the two procedures were compared to each other, and to shade guide measurements. RESULTS: Tooth color, as measured by both procedures, significantly correlated with shade evaluation. Statistically significant differences between treatment groups were found more often with data from the multiple-site procedure than from the single-site procedure. Incisal-gingival color gradation on tooth surfaces was diminished as a result of whitening treatments. Chromameter measurement data showed that tooth whitening procedures reduced tooth yellowness (b) more reproducibly than they increased whiteness (L). CONCLUSION: When more sites per surface were measured, tooth color measurement by the chromameter more closely matched shade with less variability and greater statistical power. Chromameter-derived values can be used to estimate shade values to a reasonable level of accuracy. Decreasing yellowness, particularly at the gingival margin, was an important component of the tooth whitening effect. Following tooth whitening, teeth were more uniform in their color, and this may contribute substantially to the subjective patient recognition of whiter teeth.

BACKGROUND: The authors tested the adjunctive use of light with a 15 percent peroxide gel as a single-visit, in-office tooth whitening system. METHODS: Subject (N = 87) with stained (> shade D4, Vita Zahnfabrik, Bad Säckingen, Germany) anterior teeth were randomly assigned to test (peroxide and light), peroxide control (peroxide gel) or light control (placebo gel and light) groups and were treated for one hour. The researchers evaluated tooth shade, color and subject response at baseline and posttreatment and at three and six months posttreatment. RESULTS: The initial shade unit reduction of combined light and peroxide treatment (8.4) was greatest compared with that of peroxide alone (5.9) and of light alone (4.9). Approximately 88 percent of these effects persisted for six months. Lightness was increased and yellowness decreased to a significantly greater extent in the test group than in either control. These findings were corroborated by subject evaluation. One week after treatment, moderate to greatly increased tooth sensitivity occurred in 20 percent of test subjects, 21.7 percent of peroxide control subjects and none of the light control subjects. Neither tooth sensitivity nor gingival redness was present at the three- and six-month visits. CONCLUSIONS: Peroxide and light treatment significantly lightened the color of teeth to a greater extent than did peroxide or light alone, with a low and transient incidence of tooth sensitivity. CLINICAL IMPLICATIONS: Light can increase the tooth-whitening effect of peroxide, thereby increasing the effectiveness of tooth-whitening procedures.
