RESUMEN

Gallstone disease remains an important medical and socially significant problem due to the increase in the proportion of patients of young and working age. At the same time, along with the increase in incidence, the number of complicated forms, such as choledocholithiasis in combination with stenosing duodenal papillitis (SDP) and obstructive jaundice, is increasing, which increases the importance of surgical approaches in the complex treatment of the disease and expands the range of conservative methods of therapy. In the given clinical observation of a young patient with severe cholelithiasis, accompanied by early and late complications, including the formation of postoperative scars and ventral hernias. At the last of the described stages of treatment during allohernioplasty, a complication developed in the form of a seroma followed by phlegmon of the anterior abdominal wall in the area of the implant, which served as the basis for including both local (NPWT therapy) and general (a course of infusions of a succinate-containing drug) methods in the treatment regimen, which contributed to more pronounced positive dynamics of the patient's condition.

RESUMEN

An example of the use of 3D-modeling of 3D-printing in the diagnosis and choice of a surgical approach for the treatment of hepaticocholedochal stricture is given. The inclusion of meglumine sodium succinate (intravenous drip, once a day, 500 ml, course of 10 days) into the therapy regimen was substantiated, which contributed to the reduction of intoxication syndrome due to its antihypoxic mechanism of action and, as a result, to a reduction in the duration of hospitalization and an increase in the quality of life of the patient.

RESUMEN

A patient with Caroli disease is described. The authors used 3D modeling and 3D printing when choosing surgical strategy. Advisability of 1.5% meglumine sodium succinate 500 ml IV once a day (courses for 5 and 8 days) is justified. Thanks to antihypoxic mechanism, this drug reduced intoxication syndrome and length of hospital-stay, as well as improved quality of life.

RESUMEN

Visceral artery aneurysms are rare and occur only in 0.01-0.2% of people. According to various authors, incidence of aneurysm rupture is 10-20% with mortality rate of 20-70% depending on localization and dimensions. One of the causes of visceral artery aneurysms, in particular common hepatic artery aneurysm, is chronic pancreatitis. Incidence of this complication is 2-10%. The first clinical manifestation is often hemorrhagic shock following false aneurysm rupture and bleeding into abdominal cavity, gastrointestinal tract or retroperitoneal space. Common hepatic artery aneurysm is complicated by bleeding in 35% of cases, and mortality may be up to 75%. Treatment of visceral artery aneurysm following chronic pancreatitis and post-necrotic parapancreatic cyst includes several stages. Endovascular methods are the first stage of treatment. The second stage is elimination of the cause of visceral artery false aneurysm (surgery for chronic pancreatitis). We present 3 patients with visceral artery aneurysms and chronic pancreatitis.

RESUMEN

The chickweed (Stellaria media L.) pro-SmAMP2 gene encodes the hevein-like peptides that have in vitro antimicrobial activity against certain harmful microorganisms. These peptides play an important role in protecting the chickweed plants from infection, and the pro-SmAMP2 gene was previously used to protect transgenic tobacco and Arabidopsis plants from phytopathogens. In this study, the pro-SmAMP2 gene under control of viral CaMV35S promoter or under control of its own pro-SmAMP2 promoter was transformed into cultivated potato plants of two cultivars, differing in the resistance to Alternaria: Yubiley Zhukova (resistant) and Skoroplodny (susceptible). With the help of quantitative real-time PCR, it was demonstrated that transgenic potato plants expressed the pro-SmAMP2 gene under control of both promoters at the level comparable to or exceeding the level of the potato actin gene. Assessment of the immune status of the transformants demonstrated that expression of antimicrobial peptide pro-SmAMP2 gene was able to increase the resistance to a complex of Alternaria sp. and Fusarium sp. phytopathogens only in potato plants of the Yubiley Zhukova cultivar. The possible role of the pro-SmAMP2 products in protecting potatoes from Alternaria sp. and Fusarium sp. is discussed.
