RESUMEN

Among holographic imaging configurations, inline holography excels in its compact design and portability, making it the preferred choice for on-site or field applications with unique imaging requirements. However, effectively holographic reconstruction from a single-shot measurement remains a challenge. While several approaches have been proposed, our novel unsupervised algorithm, the physics-aware diffusion model for digital holographic reconstruction (PadDH), offers distinct advantages. By seamlessly integrating physical information with a pre-trained diffusion model, PadDH overcomes the need for a holographic training dataset and significantly reduces the number of parameters involved. Through comprehensive experiments using both synthetic and experimental data, we validate the capabilities of PadDH in reducing twin-image contamination and generating high-quality reconstructions. Our work represents significant advancements in unsupervised holographic imaging by harnessing the full potential of the pre-trained diffusion prior.

To investigate the prognostic impact of serum beta-2 microglobulin (B2M) in adult lymphoma-associated hemophagocytic lymphohistiocytosis (HLH). The clinical and laboratory characteristics of 326 adult patients in a multicenter cohort with lymphoma-associated HLH with available baseline serum B2M levels were retrospectively analyzed. A total of 326 cases were included in this study, and the median serum B2M level was 5.19 mg/L. The optimal cut-off of serum B2M was 8.73 mg/L, and the cases with serum B2M level >8.73 mg/L were older and had a more advanced stage, lower levels of platelets, albumin, and fibrinogen, and higher creatinine level. The serum B2M >8.73 mg/L, creatinine ≥133 µmol/L, fibrinogen ≤1.5 g/L, agranulocytosis (<0.5 × 109/L), severe thrombocytopenia (<50 × 109/L), and high Epstein-Barr virus DNA copy number were found to have independent prognostic values in all patients, and the serum B2M >8.73 mg/L was also an independent prognostic factor in patients with creatinine <133 µmol/L. Finally, a prognostic scoring system was established based on independent prognostic factors of all patients and categorized the patients into three groups with significant prognostic differences. This study confirmed that the serum B2M level can be an independent prognostic factor in lymphoma-associated HLH and established a prognostic scoring system to predict patients' survival.

Elderly patients with lymphoproliferative diseases (LPD) are vulnerable to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Here, we retrospectively described the clinical features and outcomes of the first time infection of Omicron SARS-CoV-2 in 364 elderly patients with lymphoma enrolled in Jiangsu Cooperative Lymphoma Group (JCLG) between November 2022 and April 2023 in China. Median age was 69 years (range 60-92). 54.4% (198/364) of patients were confirmed as severe and critical COVID-19 infection. In univariable analysis, Age > 70 years (OR 1.88, p = 0.003), with multiple comorbidities (OR 1.41, p = 0.005), aggressive lymphoma (OR 2.33, p < 0.001), active disease (progressive or relapsed/refractory, OR 2.02, p < 0.001), and active anti-lymphoma therapy (OR 1.90, p < 0.001) were associated with severe COVID-19. Multiple (three or more) lines of previous anti-lymphoma therapy (OR 3.84, p = 0.021) remained an adverse factor for severe COVID-19 in multivariable analysis. Moreover, CD20 antibody (Rituximab or Obinutuzumab)-based treatments within the last 6 months was associated with severe COVID-19 in the entire cohort (OR 3.42, p < 0.001). Continuous BTK inhibitors might be protective effect on the outcome of COVID-19 infection (OR 0.44, p = 0.043) in the indolent lymphoma cohort. Overall, 7.7% (28/364) of the patients ceased, multiple lines of previous anti-lymphoma therapy (OR 3.46, p = 0.016) remained an adverse factor for mortality.

BACKGROUND: An isokinetic moment curve (IMC) pattern-damaged structure prediction model may be of considerable value in assisting the diagnosis of knee injuries in clinical scenarios. This study aimed to explore the association between irregular IMC patterns and specific structural damages in the knee, including anterior cruciate ligament (ACL) rupture, meniscus (MS) injury, and patellofemoral joint (PFJ) lesions, and to develop an IMC pattern-damaged structure prediction model. METHODS: A total of 94 subjects were enrolled in this study and underwent isokinetic testing of the knee joint (5 consecutive flexion-extension movements within the range of motion of 90°-10°, 60°/s). Qualitative analysis of the IMCs for all subjects was completed by two blinded examiners. A multinomial logistic regression analysis was used to investigate whether a specific abnormal curve pattern was associated with specific knee structural injuries and to test the predictive effectiveness of IMC patterns for specific structural damage in the knee. RESULTS: The results of the multinomial logistic regression revealed a significant association between the irregular IMC patterns of the knee extensors and specific structural damages ("Valley" - ACL, PFJ, and ACL + MS, "Drop" - ACL, and ACL + MS, "Shaking" - ACL, MS, PFJ, and ACL + MS). The accuracy and Macro-averaged F1 score of the predicting model were 56.1% and 0.426, respectively. CONCLUSION: The associations between irregular IMC patterns and specific knee structural injuries were identified. However, the accuracy and Macro-averaged F1 score of the established predictive model indicated its relatively low predictive efficacy. For the development of a more accurate predictive model, it may be essential to incorporate angle-specific and/or speed-specific analyses of qualitative and quantitative data in isokinetic testing. Furthermore, the utilization of artificial intelligence image recognition technology may prove beneficial for analyzing large datasets in the future.

RESUMEN

Volumetric reconstruction of a three-dimensional (3D) particle field with high resolution and low latency is an ambitious and valuable task. As a compact and high-throughput imaging system, digital holography (DH) encodes the 3D information of a particle volume into a two-dimensional (2D) interference pattern. In this work, we propose a one-stage network (OSNet) for 3D particle volumetric reconstruction. Specifically, by a single feed-forward process, OSNet can retrieve the 3D coordinates of the particles directly from the holograms without high-fidelity image reconstruction at each depth slice. Evaluation results from both synthetic and experimental data confirm the feasibility and robustness of our method under different particle concentrations and noise levels in terms of detection rate and position accuracy, with improved processing speed. The additional applications of 3D particle tracking are also investigated, facilitating the analysis of the dynamic displacements and motions for micro-objects or cells. It can be further extended to various types of computational imaging problems sharing similar traits.

OBJECTIVES: Nursing factors have been found to be associated with a reduction in readmission rates. Nevertheless, few attentions have been given to the effect of nursing factors on nursing home (NH) residents. This study was to assess the impact of nursing factors on the hospital readmissions and length of stay (LOS) of the elderly residents from the NH. STUDY DESIGN: This was a cross-sectional study. METHODS: Data were extracted from the NH of the LTCFocus.org data set between 2011 and 2018. The study included residents aged ≥55 years who were admitted to NH in the United States, following a hospitalization event. The nursing factors included facility-level data elements and medical care personnel. An unsupervised machine learning algorithm (K-means) was used to cluster NH according to readmission rate and LOS. Multivariate logistic regression analysis was performed. RESULTS: This study consisted of 107,000 NH-year observations. The median readmission rate was 17%, with a median LOS was 28.00 days. Three clusters were identified: cluster 1 was a high readmission rate with high LOS, cluster 2 was a low readmission rate with low LOS, and cluster 3 was a high readmission rate with low LOS. Multifacility and admission/bed were associated with a reduction in readmission rate and LOS in both cluster 1 vs cluster 2 and cluster 3 vs cluster 2. The special care unit and registered nurses' ratio were associated with decreased readmission rate and LOS in cluster 1 vs cluster 2. Total beds and Alzheimer unit decreased the readmission rate and LOS, whereas certified nursing assistant increased the readmission rate and LOS in cluster 3 vs cluster 2. NH for profit was associated with elevated readmission rate and LOS in cluster 1 vs cluster 2 and decreased readmission rate and LOS in cluster 3 vs cluster 2. Based on the subgroup analysis, the certified nursing assistant decreased readmission rate and LOS in cluster 1 vs cluster 2 and increased readmission rate and LOS in cluster 3 vs cluster 2 (all P < 0.005). CONCLUSION: This study indicates the importance of the improvement of nurse number and level and the inputs of facility characteristics in NH.

BACKGROUND: Radical regulations to improve air quality, including traffic control, were implemented prior to and during the 2008 Beijing Olympic Games. Consequently, ambient concentrations of nitrogen dioxide (NO2) and particular matter 10 micrometers or less (PM10), were reduced in a distinct and short window of time, which presented a natural experiment for testing the relationships between maternal exposure to PM10 and NO2 during pregnancy and adverse birth outcomes. METHODS: We estimated the effect of PM10 and NO2 exposure during each trimester of gestation on the risk of preterm birth among live births and the birth weight among term babies. The data were based on 50,874 live births delivered between January 1, 2006 and December 31, 2010 at the Beijing Haidian Maternal and Child Health Hospital. Air monitoring data for the same period were obtained from the Beijing Municipal Environmental Monitoring Center. RESULTS: Among full-term births, maternal exposure to NO2 in the third trimester predicted birth weight, with each 10-unit increment (per 10 ug/m(3)) in NO2 concentration associated with a 13.78 g (95 % confidence interval: -21.12, -6.43; p < 0.0001) reduction in birth weight. This association was maintained after adjusting for other pollutants, including carbon monoxide (CO), sulfur dioxide (SO2), and PM10. No relationship was found between the concentration of PM10 and low birth weight among full-term births. Neither PM10 nor NO2 concentrations predicted the risk of premature birth. CONCLUSIONS: Exposure to ambient air pollution during certain periods of pregnancy may decrease birth weight, but the effect size is small.

OBJECTIVE: To explore the restrictive use of episiotomy for low forceps delivery. METHODS: A total of 311 low forceps delivery women at ≥37 weeks of gestation with live singleton cephalic pregnancies were recruited from June 2013 to December 2013 at our hospital. Among whom, 117 women underwent no episiotomy another 194 had mediolateral episiotomy. The maternal and neonatal outcomes of two types of episiotomy were compared. RESULTS: The amount of intra and post-partum hemorrhage, I-II perineal tearing, time of perineal suturing, perineal pain severity of post-partum 24 h significantly decreased than control group (P<0.05). No statistical significant inter-group differences existed in perineal hematoma, postnatal infection, urinary retention or length of stay after childbirth (P>0.05). And no statistically significant inter-group differences existed in incidence rates of neonatal asphyxia, neonatal birth trauma and newborns into neonatal intensive care unit (NICU) (P>0.05). CONCLUSION: During low forceps delivery, restrictive use of episiotomy may decrease the rate of episiotomy, reduce the amount of hemorrhage, minimize maternal injury, relieve pain and have no adverse effects on neonatal morbidities. And it improves the quality of vaginal delivery and demonstrate the concept of mini-invasiveness so that it is worthy of wider promotions.

Food security has a bearing on national development and people's livelihoods and is an important guarantee of social stability for national development. The problems of arable land abandonment and non-grain are becoming more and more serious, and national food security is difficult to guarantee, which will seriously hinder the forward development of China's society and economy. Taking Ruijin City of Jiangxi Province as an example, this study calculated the abandonment level and non-grain level of arable land in Ruijin City respectively from two aspects, and explored the spatial differentiation law of farmland abandonment and non-grain level in the hilly and mountainous areas of southern Jiangxi Province by using spatial autocorrelation and cold and hot spot analysis methods, and the causes of arable land abandonment and non-grain spatial differentiation in the hilly mountainous areas of Gannan were revealed by the methods of Geodetector factor detection and interaction detection. Conclusions of the study: (1) Ruijin City, the abandoned area was 1216.73 hm2, the abandonment rate of each village ranged from 0.01 % to 50.62 %, and the comprehensive abandonment rate was 4.90 %; the area of non-grain was 2937.27 hm2, and the rate of non-grain of each village ranged from 0.01 % to 100.00 %, and the comprehensive non-grain rate was 11.83 %. The area of non-grain was 2937.27 hm2, and the rate of non-grain in each village ranged from 0.01 % to 100.00 %, and the comprehensive rate of non-grain was 11.83 %. (2) The phenomenon of abandonment of arable land and non-grain in Gannan hilly and mountainous areas has a certain clustering and driving effect in space. Globally, the phenomena of arable land abandonment and non-grain in Ruijin City are positively correlated, with the global Moran's I of arable land abandonment rate being 0.05, and the global Moran's I of arable land non-grain being 0.73. (3) Whether or not arable land in the hilly mountainous areas of Gannan is abandoned is affected by the combination of socioeconomics, natural resources, farming conditions, and economic location, with elevation, the degree of arable land contiguity, and population density being the dominant factors. The interaction of elevation, degree of concentration and contiguity, field regularity, and per capita arable land area increased the spatial variability of arable land abandonment in the hilly mountainous areas of Gannan. Whether the phenomenon of non-grain occurs or not is affected by socio-economic conditions, farming conditions and economic location, of which the proportion of paddy fields, land transfer price, arable land area, and urban-rural gradient are the dominant factors. The proportion of paddy land, the price of land transfer, the area of arable land, and the urban-rural gradient interact with each other, and the tendency of arable land to be planted with non-grain crops is more serious.

OBJECTIVE: The recently developed noninvasive prenatal test (NIPT) presents a new era of prenatal screening. Previously reported studies were primarily conducted on high-risk and advanced maternal age (AMA) pregnancies. We sought to evaluate the performance of NIPT for detection of fetal aneuploidies in a Chinese cohort of women younger than 35 years old in a prospective clinical setting. METHODS: Maternal plasma samples were sequenced to identify the aneuploidies. The results were compared against the serum screening results and validated by karyotyping through invasive procedures and birth follow-up. RESULTS: A total of 1916 prospectively collected maternal plasma samples were sequenced, among which 73 samples (3.8%) failed the sequencing quality control. Birth follow-up missed 111 samples (5.8%). The remaining 1741 samples were analyzed. Sequencing reported 15 aneuploidy samples, including all the T21, T18, and T13 cases. Sequencing performed moderately in identifying sex chromosome aneuploidies, detecting two out of four samples, with a specificity of 99.88% (95% CI 99.53% to 99.98%). CONCLUSIONS: Noninvasive prenatal detection of common fetal aneuploidies is a more sensitive and specific method than triple maternal serum screening. It has a remarkable low false positive rate and is applicable to women younger than 35 years old.

RESUMEN

Few-shot relation extraction is used to solve the problem of long tail distribution of data by matching between query instances and support instances. Existing methods focus only on the single direction process of matching, ignoring the symmetry of the data in the process. To address this issue, we propose the bidirectional matching and aggregation network (BMAN), which is particularly powerful when the training data is symmetrical. This model not only tries to extract relations for query instances, but also seeks relational prototypes about the query instances to validate the feature representation of the support set. Moreover, to avoid overfitting in bidirectional matching, the data enhancement method was designed to scale up the number of instances while maintaining the scope of the instance relation class. Extensive experiments on FewRel and FewRel2.0 public datasets are conducted and evaluate the effectiveness of BMAN.

Exploring the cross-sensitivity between land use transformation and ecological service values in rare earth mining areas is of great significance for the development of ecological protection and restoration in rare earth mining areas. To study the impact of land use changes on ecosystem service functions in rare earth mining areas, firstly, the land use change trends in the study area from 2009 to 2019 were analyzed using the land transfer matrix; then the distribution of ecosystem service values and the flow direction of ecosystem service values in the study area were measured based on the ecosystem service value equivalents; a spatial autocorrelation analysis was done on the ecosystem service values to explore their spatial distribution patterns; and finally, the cross-sensitivity coefficient was used to quantitatively assess the extent and direction of the impact of land use change on ecosystem service values. The results show that the land use types in the study area are mainly forest land and farmland, with woodland accounting for the highest proportion of the study area. The ESV changes in the study area are consistent with the trend of land use transformation, with the overall increase and decrease being comparable, and the decrease in ESV is mainly concentrated in the areas with a large increase in mining land and construction land; during the study period, the study area was significantly reduced with low-low cluster areas and the ecological environment was improved; from 2009 to 2014, the ecological sensitivity coefficient is more variable, and is more sensitive to the net conversion between water and desert, from 2014 to 2019, the ecological sensitivity coefficient is less variable, and the most sensitive is the net conversion between cultivated land and water. The study area should be reasonably developed for rare earth resources and the ecological environment around the mining area should be reasonably protected to build an ecological security pattern.

Mutations in VPS13 gene have been recently reported as a genetic cause of Parkinson's disease (PD). In this study, we isolated the skin fibroblasts from a PD patient harboring VPS13A gene mutation (c. 4282_4289delinsA) and reprogrammed the fibroblasts to a novel patient-specific induced pluripotent stem cell (iPSC) line LCPHi002-A using transgene-free episomal plasmids to express OCT3/4, SOX2, KLF4, L-MYC, and LIN28. The LCPHi002-A line showed the normal karyotype, expression of pluripotency markers, and had multi-lineage differentiation capacity in vivo. This iPSC line of LCPHi002-A could be used for studying pathogenic mechanisms of PD.

In California, a novel closterovirus was detected in "Redglobe" grapevine, associated with graft incompatibility and given a trivial name "Grapevine rootstock stem lesion associated virus (GRSLaV)." The biological properties of the putative virus were ascertained when asymptomatic yet infected Redglobe scion buds were graft-inoculated onto test plants of Cabernet Sauvignon propagated on 18 different rootstocks. It proved lethal on test plants growing on rootstocks 1616C, 5BB, 5C, 3309C, and 1103 P, whereas latent infections occurred on the remaining scion-rootstock combinations. In contrast, GLRaV-2 type (type strain) produced only typical leafroll symptoms. In a different experiment, GLRaV-2 type was successfully sap-transmitted to N. benthamiana, whereas sap transmission of GRSLaV was unsuccessful. Double-stranded RNA was extracted from infected Redglobe grapevines, cloned, sequenced, and determined a genome length of 16,527 nucleotides. Computer-assisted analysis of open-reading frames (ORFs) revealed a genome organization typical of monopartite viruses in the genus Closterovirus with nine ORFs (range 71-79% identity) with GLRaV-2 type, the closest similar virus species within the family Closteroviridae. Also the 3'-UTR of GRSLaV consisted of 223 nucleotides with an extended oligo(A) tract similar to that of GLRaV-2 type, Beet yellow stunt virus, and Beet yellows virus. Recombinant GRSLaV coat protein was expressed in E. coli, purified, and immunized a rabbit to produce polyclonal antiserum. Serological data matched the molecular data, whereby exposed plant tissue extracts of grapevines infected by both viruses (GRSLaV and GLRaV-2) reacted positively with homologous and heterologous viral antisera but not with healthy grapevine extracts in ELISA and Western blot tests. Based on the comparative sequence data and shared antigens, GRSLaV is now considered a strain of GLRaV-2 and redesignated as Grapevine leafroll associated virus-2 Redglobe (GLRaV-2RG). Primers specific for GLRaV-2RG were developed, which did not amplify GLRaV-2 type strain. When both sets of specific primers were used in assays of different grapevine collections, the incidence of the respective viruses varied considerably, e.g., 1.7 and 13.5%, respectively, for GLRaV-2RG and GLRaV-2 type.

Flower-like titanium dioxide (TiO2) nanostructures are successfully synthesized using a hybrid sol-gel and a simple hydrothermal method. The sample was characterized using various techniques to study their physicochemical properties and was tested as a photocatalyst for methyl orange degradation and as an antibacterial material. Raman spectrum and X-ray diffraction (XRD) pattern show that the phase structure of the synthesized TiO2 is anatase with 80-100 nm in diameter and 150-200 nm in length of flower-like nanostructures as proved by field emission scanning electron microscope (FESEM). The energy-dispersive X-ray spectroscopy (EDS) analysis of flower-like anatase TiO2 nanostructure found that only titanium and oxygen elements are present in the sample. The anatase phase was confirmed further by a high-resolution transmission electron microscope (HRTEM) and selected area electron diffraction (SAED) pattern analysis. The Brunauer-Emmett-Teller (BET) result shows that the sample had a large surface area (108.24 m2/g) and large band gap energy (3.26 eV) due to their nanosize. X-ray photoelectron spectroscopy (XPS) analysis revealed the formation of Ti4+ and Ti3+ species which could prevent the recombination of the photogenerated electron, thus increased the electron transportation and photocatalytic activity of flower-like anatase TiO2 nanostructure to degrade the methyl orange (83.03%) in a short time (60 minutes). These properties also support the good performance of flower-like titanium dioxide (TiO2) nanostructure as an antibacterial material which is comparable with penicillin which is 13.00 ± 0.02 mm inhibition zone against Staphylococcus aureus.

OBJECTIVE: To investigate the therapeutic effects of ipriflavone on postmenopausal syndrome and osteoporosis in women. METHODS: A randomized and double-blind study was conducted. Sixty postmenopausal women with osteoporosis were chosen and they were randomly divided into three groups: Treatment group I was given oral compound calcium acid chelate and Vitamin AD guttate; treatment group II was given oral compound calcium acid chelate, Vitamin AD guttate and ipriflavone; Control group was given placebo and compound calcium acid chelate. The postmenopausal syndrome, bone mineral density (BMD), and bone biochemical markers were assessed 6 and 12 months after the treatment. RESULTS: In treatment group II, hot flush and ostalgia syndromes were dramatically relieved, BMD and serum calcium level increased markedly and alkaline phosphatase, parathyroid hormone and tartrate-resistant acid phosphatase decreased markedly, comparing with treatment group I and control group (p < 0.05). CONCLUSION: Ipriflavone could inhibit bone resorption and promote bone formation. It is an effective drug for the prevention and treatment to menopausal syndrome and osteoporosis. Ipriflavone could be used as a supplement to estrogen replacement treatment.

OBJECTIVE: To investigate the incidence and relevant information of preterm birth and the outcomes of preterm infants delivered at various gestational weeks and for different causes. METHODS: Totally 955 women, who ended their pregnancies before term, and 1066 neonates of the previous mothers were enrolled in this survey, among 15,197 deliveries at Peking University First Hospital, Beijing Gynecological and Obstetric Hospital, Women's and Children's Hospital of Haidian District and Peking University Third Hospital, respectively, from December 1(st), 2006 to May 31(st), 2007. RESULTS: (1) Incidence of preterm birth: the overall incidence of preterm birth of the 4 hospitals was 6.3% (955/15 197), and it was 8.1% (125/1549) in Peking University First Hospital, 13.1% (150/1142), which was the highest (P < 0.01), in Peking University Third Hospital, 5.5% (369/6656) in Beijing Gynecological and Obstetric Hospital and 34.0% (311/5850) in Women's and Children's Hospital of Haidian District. The preterm birth rate at the two comprehensive hospitals was significantly higher than that of the two specialized hospitals [10.2% (275/2691) vs 5.4% (680/12 506), P < 0.01]. (2) Gestational weeks at delivery: the incidence of preterm birth before 34 weeks was 28.5% (272/954) and the number changed to 71.5% (682/954) for those preterm deliveries after 34 weeks. However, this number varied among the 4 hospitals. Peking University First Hospital had the highest incidence of preterm birth before 34 weeks (P < 0.05), and the lowest was found in Women's and Children's Hospital of Haidian District (P < 0.01), but no difference was found between Peking University Third Hospital and Beijing Gynecological and Obstetric Hospital. (3) Etiology of preterm birth: preterm premature rupture of membranes (PPROM) accounted for the most proportion of all preterm birth cases, followed by iatrogenic preterm birth and spontaneous preterm birth. But the causes of preterm birth in the 4 hospitals were different. Peking University Third Hospital had a higher incidence of iatrogenic preterm birth than the others (P < 0.01), and Peking University First Hospital had a higher incidence of preterm birth caused by PPROM and lower incidence of spontaneous preterm birth. The first four reasons of iatrogenic preterm birth were preeclampsia (143, 42.0%), fetal distress (58, 17.1%), placenta previa (43, 12.6%) and placenta abruption (33, 9.7%). (4) Neonatal outcomes in different hospitals: the neonatal outcomes were quite different among the 4 hospitals due to different causes and different delivery weeks. The highest neonatal mortality rate was found in Beijing Gynecological and Obstetric Hospital (5.4%, 22/408) compared to that in Women's and Children's Hospital of Haidian District (1.3%, 4/320) and Peking University Third Hospital (0.6%, 1/170) (P < 0.01), but without any difference when compared to that in Peking University First Hospital (2.4%, 3/124) (P > 0.05). (5) Neonatal outcomes at different gestational age: the recovery rate of preterm infants delivered at < 32 weeks was lower than those delivered > or = 32 weeks (P < 0.01), and this number rose to 99.6% in those delivered > or = 34 weeks. More infants delivered < 32 weeks were given up for treatment or died during the perinatal period than those delivered > or = 32 weeks, with the neonatal mortality rate of 22.1% for those delivered at < 32 weeks and only 0.3% for those delivered at > or = 34 weeks (P < 0.01). (6) Neonatal outcomes for various causes: the premature neonatal mortality rate for iatrogenic preterm births was higher than that of PPROM (4.9% vs 1.6%, P < 0.05). But the neonatal recovery rates were similar among the PPROM, spontaneous and iatrogenic preterm birth group (P > 0.05). CONCLUSIONS: Preterm birth is associated with high perinatal mortality rate, especially for those delivered before 32 weeks which would be highlighted in prevention. Reduction of the iatrogenic preterm birth, combined with proper prevention of PPROM, is an important issue in decreasing the prevalence of preterm birth.

BACKGROUND: Encephalitis is a common central nervous system inflammatory disease that seriously endangers human health owing to the lack of effective diagnostic methods, which leads to a high rate of misdiagnosis and mortality. Glutamate is implicated closely in microglial activation, and activated microglia are key players in encephalitis. Hence, using glutamate chemical exchange saturation transfer (GluCEST) imaging for the early diagnosis of encephalitis holds promise. METHODS: The sensitivity of GluCEST imaging with different concentrations of glutamate and other major metabolites in the brain was validated in phantoms. Twenty-seven Sprague-Dawley (SD) rats with encephalitis induced by Staphylococcus aureus infection were used for preclinical research of GluCEST imaging in a 7.0-Tesla scanner. For the clinical study, six patients with encephalitis, six patients with lacunar infarction, and six healthy volunteers underwent GluCEST imaging in a 3.0-Tesla scanner. RESULTS: The number of amine protons on glutamate that had a chemical shift of 3.0 ppm away from bulk water and the signal intensity of GluCEST were concentration-dependent. Under physiological conditions, glutamate is the main contributor to the GluCEST signal. Compared with normal tissue, in both rats and patients with encephalitis, the encephalitis areas demonstrated a hyper-intense GluCEST signal, while the lacunar infarction had a decreased GluCEST signal intensity. After intravenous immunoglobulin therapy, patients with encephalitis lesions showed a decrease in GluCEST signal, and the results were significantly different from the pre-treatment signal (1.34 ± 0.31 vs 5.0 ± 0.27%, respectively; p = 0.000). CONCLUSION: Glutamate plays a role in encephalitis, and the GluCEST imaging signal has potential as an in vivo imaging biomarker for the early diagnosis of encephalitis. GluCEST will provide new insight into encephalitis and help improve the differential diagnosis of brain disorders.
