BACKGROUND: The different bony and soft tissue reference points and the micro and macroscopic structures of the knee continue to be the object of focused study and analysis. Upon reviewing the most recent literature, we saw the wide spectrum of studies that seek to define the different anatomical aspects of the anterior cruciate ligament (ACL). PURPOSE: The purpose of this paper is to review the most recent publications on the ACL and its morphology in which its microscopic composition and macroscopic anatomy are addressed. RESULTS: The ACL consists of typeI (90%) and typeIII (10%) collagen matrix. Its length ranges from 27 to 38mm and its width from 10 to 12mm. The ACL cross-section area measures an average of 44mm2, and its shape resembles that of an hourglass or a bow tie. ACL bundles have been defined as anteromedial, intermediate, and posterolateral. Femoral and tibial footprints were seen to present a high degree of variability in shape and size. Furthermore, the blood supply is given by the medial genicular artery and innervation by the tibial nerve branches. Additionally, the ACL functionally prevents anterior translation of the tibia and stabilizes against the internal rotation of the tibia and valgus angulation of the knee. CONCLUSIONS: There is great variability in the anatomy of the ACL as well as its attachment sites. At the same time, the shape and size of its footprint has become a factor in determining individualized ACL reconstruction. The persistence of morphological variability in the aging of the ACL and important aspects of surgical planning and decision making with respect to anatomical risk factors suggest that further studies are called for.

INTRODUCTION AND OBJECTIVE: Artificial intelligence is fully present in our lives. In education, the possibilities of its use are endless, both for students and teachers. MATERIAL AND METHODS: The capacity of ChatGPT has been explored when solving multiple choice questions based on the exam of the subject «Anatomopathological Diagnostic and Therapeutic Procedures¼ of the first call of the 2022-23 academic year. In addition, to comparing their results with those of the rest of the students presented the probable causes of incorrect answers have been evaluated. Finally, its ability to formulate new test questions based on specific instructions has been evaluated. RESULTS: ChatGPT correctly answered 47 out of 68 questions, achieving a grade higher than the course average and median. Most failed questions present negative statements, using the words «no¼, «false¼ or «incorrect¼ in their statement. After interacting with it, the program can realize its mistake and change its initial response to the correct answer. Finally, ChatGPT can develop new questions based on a theoretical assumption or a specific clinical simulation. CONCLUSIONS: As teachers we are obliged to explore the uses of artificial intelligence and try to use it to our benefit. Carrying out tasks that involve significant consumption, such as preparing multiple-choice questions for content evaluation, is a good example.

INTRODUCTION: In a pathological anatomy service, the workload in medical time is analyzed based on the complexity of the samples received and its distribution among pathologists is assessed, presenting a new computer algorithm that favors an equitable distribution. METHODS: Following the second edition of the Spanish guidelines for the estimation of workload in cytopathology and histopathology (medical time) according to the Spanish Pathology Society-International Academy of Pathology (SEAP-IAP) catalog of samples and procedures, we determined the workload units (UCL) per pathologist and the overall UCL of the service, the average workload of the service (MU factor), the time dedicated by each pathologist to healthcare activity and the optimal number of pathologists according to the workload of the service. RESULTS: We determined 12 197 total annual UCL for the chief pathologist, as well as 14 702 and 13 842 UCL for associate pathologists, with an overall of 40 742 UCL for the whole service. The calculated MU factor is 4.97. The chief pathologist devoted 72.25% of his working day to healthcare activity while associate pathologists dedicated 87.09% and 82.01% of their working hours. The optimal number of pathologists for the service is found to be 3.55. CONCLUSIONS: The results demonstrate medical work overload and a non-equitable distribution of UCLs among pathologists. We propose a computer algorithm capable of distributing the workload in an equitable manner. It would be associated with the laboratory information system and take into account the type of specimen, its complexity and the dedication of each pathologist to healthcare activity.

INTRODUCTION AND OBJECTIVES: This study aimed to retrospectively analyze the anatomical characteristics and classification of multiple coronary artery fistulas (MCAFs), and to compare the outcomes of transcatheter closure between MCAFs and single fistulas. METHODS: All patients who underwent attempts at transcatheter closure of coronary artery fistulas (CAFs) at Fuwai Hospital from 2010 to 2023 were retrospectively reviewed. Patients were categorized into single fistula and MCAFs groups, and anatomical characteristics and transcatheter closure outcomes were compared between the 2 groups. RESULTS: This retrospective study included 146 patients who underwent attempted transcatheter closure of CAFs, with a 14.38% failure rate. Among the 146 patients with CAFs, 32.19% were identified as having MCAFs, with types I, II, and III constituting 40.43%, 42.55%, and 17.02%, respectively. Unlike single fistulas, which predominantly originated from the right coronary artery and terminated in the left ventricle, MCAFs mainly had simultaneous origins from the right coronary artery and left anterior descending artery (29.79%), and predominantly drained into the pulmonary artery (70.21%), with a notable prevalence of plexus-like morphology (38.3% vs 2.02%, P<.001). The success rate of transcatheter closure was significantly lower for multiple fistulas compared with single fistula (64.29% vs 84.34%, P=.011). Multivariate regression analysis indicated that the risk of closure failure for MCAFs was 2.64 times that of single fistulas. CONCLUSIONS: MCAFs are common among CAFs and can be classified into 3 types based on the number and location of their origins and terminations. The risk of failure of transcatheter closure is significantly higher in MCAFs than in single fistulas.

BACKGROUND: Continuous peripheral nerve blocks are commonly used for pain management. However, the incidence of catheter dislodgement or migration is unclear, and may be underestimated and underreported. Our objective was to assess suprascapular catheter tip positioning before and after routine simulated shoulder physiotherapy manipulation in an anatomical cadaver model. METHOD: Eight ultrasound-guided continuous suprascapular nerve block catheters were placed in cryopreserved fresh cadavers. Computed tomography (CT) confirmed the location of the catheter tip after injection of 1 ml of contrast medium. We performed a series of standardized shoulder movements during a simulated shoulder physiotherapy session in cadavers. Following this, we administered 1 ml of methylene blue through the catheters, and then performed anatomical dissections to accurately identify the location of the catheter tips and compare them to their placement prior to the 'physiotherapy'. RESULT: CT imaging confirmed the location of the catheter tips at the suprascapular notch in all cases. However, following physiotherapy, 2 catheters (25%) were found to have migrated - specifically, 1 was located in the supraspinatus muscle, and the other was located in the trapezius muscle. CONCLUSION: Our findings suggest that catheter dislodgement may occur in approximately 25% of cases following simulated physiotherapy manipulation. However, further research is needed to determine the read incidence of catheter dislodgement in clinical practice.

INTRODUCTION AND OBJECTIVES: The objective of the study was: (1) to describe changes in the shape of the atlas during growth, including gender and side differences; (2) to assess the dimension essential for identification of the optimal entry point; (3) to determine the age limit for a safe insertion of 3.5-mm screws into the lateral masses according to our own limiting parameters. MATERIALS AND METHODS: Dimensions of the atlas were measured on 200 CT scans of the craniocervical junction in individuals aged 0-18 years and on 34 anatomical specimens of the first cervical vertebra (aged 2.5-18 years). Both series were divided according to the gender and age. The values measured on CT scans were used for statistical comparison of data in boys and girls and comparison of the right and left sides. RESULTS: The atlas reaches its maximum growth rate between 0 and 2 years of age, then the growth decelerates and continues until the age of 18 years. The proportion of dimensions of C1 vertebral foramens changes with age. The youngest children show a relatively greater distance from the left to the right medial pedicle; around the age of 5 the values get even and subsequently the distance from the inner wall of anterior to posterior arch gets relatively greater. The transverse foramen has a slightly oval shape throughout the period of growth. Statistically significant differences between boys and girls were observed primarily between 12 and 18 years of age. CONCLUSION: The study has proved adequate size of lateral masses for insertion of 3.5-mm screws in all patients from the age of 5 years. In younger children, the patient´s anatomy should be respected and the surgical technique tailored accordingly.

La presente investigación documental con diseño bibliométrico pretender caracterizar la producción científica del Dr. Hugo Ariel Hernández-Parada, fundador del actual Instituto de Anatomía, Histología y Patología de la Universidad Austral de Chile (UACh). Por medio de la búsqueda electrónica en seis bases de datos se identificaron 42 estudios (41 artículos y 1 tesis), cuyo análisis bibliométrico determinó que su quehacer dentro de la investigación en el campo de la Anatomía Humana estuvo centrado en el estudio del Sistema Nervioso Autónomo, seguida del Sistema Nervioso Periférico, Miología y Anatomía de cabeza y cuello. La mayoría de sus obras fueron realizadas en muestras humanas (fetos), publicadas en idioma español, en revistas chilenas, y como autor principal en 41 de los 42 registros encontrados. Por medio de esta investigación, se pretende recordar y rendir homenaje a este destacado anatomista, cuya obra contribuyó al desarrollo y difusión de la Anatomía Humana en Chile.


SUMMARY: This documentary research via bibliometric design, aims to characterize the scientific contribution of Dr. Hugo Ariel Hernández-Parada, the founder of the current Institute of Anatomy, Histology and Pathology of the Universidad Austral de Chile (UACh). An electronic search was carried out in six databases, 42 studies (41 articles and 1 thesis) were identified. Their bibliometric analysis determined that the work of these research studies in the field of Human Anatomy was focused on the study of the Autonomous Nervous System, followed by the Peripheral Nervous System, Myology and Anatomy of the head and neck. Most of his works were carried out on human samples (fetuses), published in Spanish in Chilean science journals, and as the main author in 41 of the 42 records found. Through this research, it is intended to remember and offer tribute to this outstanding anatomist, whose work contributed to the development and diffusion of human anatomy in Chile.

SUMMARY: The quest for aesthetic procedures is experiencing a steady increase in popularity, concomitant with the expanding array of available treatment options. Of notable interest is the burgeoning trend in the use of minimally invasive techniques. Among the various aspects of facial anatomy, the platysma stands as a pivotal element that significantly influences the aesthetic appearance of the neck region. It has garnered particular attention as a strategic focal point in various treatments geared towards enhancing the neck's visual appeal. Additionally, the versatility of the platysma extends beyond the realm of cosmetic improvements. Its functional significance is recognized in reconstructive surgical procedures, where it may be harnessed for specific maneuvers. Furthermore, the muscle serves as a critical access point for minimally invasive endoscopic surgeries in the neck region. While these developments hold great promise, it is crucial to underscore that safety should always take precedence in any medical or surgical approach. This applies equally to the neck region, which presents a complex and intricate anatomical landscape. An in-depth understanding and meticulous investigation of the platysma in all its diverse aspects are paramount to ensuring the success and safety of any procedure conducted in this region. This comprehensive review aspires to provide a contemporary understanding of the platysma, offering an in-depth analysis that encompasses its intricate anatomy, histological characteristics, and multifaceted clinical implications. La búsqueda de procedimientos estéticos ha experimentado un aumento constante en popularidad, junto con la creciente gama de opciones de tratamientos disponibles. De notable interés es la creciente tendencia en el uso de técnicas mínimamente invasivas. Entre los diversos aspectos de la anatomía facial, el platisma destaca como un elemento fundamental que influye significativamente en la apariencia estética de la región del cuello. Ha ocasionado especial atención como punto focal estratégico en varios tratamientos orientados a mejorar el atractivo visual del cuello. Además, la versatilidad del platisma se extiende más allá del ámbito de las mejoras cosméticas. Su importancia funcional se reconoce en procedimientos quirúrgicos reconstructivos, donde se puede aprovechar para maniobras específicas. Además, el músculo sirve como punto de acceso crítico para cirugías endoscópicas mínimamente invasivas en la región del cuello. Si bien estos avances son muy prometedores, es fundamental subrayar que la seguridad siempre debe tener prioridad ante cualquier abordaje médico o quirúrgico. Esto se aplica igualmente a la región del cuello, que presenta un aspecto anatómico complejo e intrincado. Una comprensión profunda y una investigación meticulosa del platisma en todos sus diversos aspectos son fundamentales para garantizar el éxito y la seguridad de cualquier procedimiento realizado en esta región. La presente revisión narrativa trata de caracterizar como ha sido el proceso de enseñanza de la anatomía en la Universidad Austral de Chile (UACh), presentando una visión histórica desde los inicios de la Universidad a los tiempos actuales y una reflexión sobre el futuro de la Anatomía Humana. Se describen los orígenes del Instituto de Anatomía de la UACh, primeros académicos, logros institucionales, convenios interinstitucionales; además, se detallan los métodos y estrategias educativas utilizadas en la actualidad para la enseñanza anatómica y se presenta una visión panorámica del porvenir como instituto de docencia e investigación en el campo de la Anatomía humana.


SUMMARY: This narrative review tries to characterize how the anatomy teaching process has been carried out at the Universidad Austral de Chile (UACh), presenting a historical vision from the beginnings of the University to the present time and a reflection on the future of Human Anatomy. The origins of the Human Anatomy Institute at the UACh, first academics, institutional achievements, inter-institutional agreements are described; in addition, the educational methods and strategies currently used for anatomical teaching are detailed and a panoramic vision of the future as a teaching and research institute in the field of human anatomy is presented.

La presente revisión narrativa propone caracterizar las estrategias didácticas actualmente utilizadas en el Instituto de Anatomía, Histología y Patología (IAHP) de la Universidad Austral de Chile (UACh), para la enseñanza-aprendizaje de la anatomía humana en los alumnos de las escuelas de Enfermería, Kinesiología, Medicina, Obstetricia y Puericultura, Odontología y Terapia Ocupacional de su Facultad de Medicina. Se listan las diversas estrategias didácticas implementadas y se explica la manera en que se han venido aplicando en esta institución a fin de perfeccionar la enseñanza anatómica, considerando los retos y desafíos que demandan las generaciones actuales de estudiantes y la dinámica del mundo contemporáneo.


This narrative review aims to describe the didactic strategies currently employed by the Institute of Anatomy, Histology, and Pathology at the Universidad Austral de Chile (UACh) for teaching Human Anatomy to students in the Nursing, Kinesiology, Medicine, Obstetrics and Childcare, Dentistry, and Occupational Therapy programs of its Faculty of Medicine. The implemented teaching strategies for improving anatomical teaching in this institution are listed and explained, considering the challenges posed by current generations of students and the dynamics of the contemporary world.

RESUMO Introdução: A anatomia humana desempenha um papel central na formação de profissionais de saúde, fornecendo a base fundamental para compreender o corpo humano e suas complexidades. Isso é particularmente relevante no contexto do ensino médico, sobre o qual as Diretrizes Curriculares Nacionais (DCN) de 2014 mencionam a importância do conteúdo curricular relacionado à estrutura, de modo a influenciar a execução da anamnese, do exame físico e do diagnóstico nosológico. Desenvolvimento: Quanto ao currículo, o ensino de anatomia vem passando por mudanças significativas nas últimas décadas, buscando equilibrar abordagens tradicionais e ativas, promover a integração curricular e ampliar perspectivas bioéticas e biopsicossociais no estudo. O ensino da anatomia tem evoluído, mas ainda é frequentemente baseado em aulas expositivas e práticas em laboratório de dissecação, com foco na memorização. No entanto, há um consenso crescente de que abordagens mais interativas, práticas e integradas são necessárias, incluindo o uso de tecnologias educacionais. Sobre os métodos de aprendizagem, os recursos de estudo incluem livros-texto, atlas, peças anatômicas, tecnologias digitais e softwares especializados, e a dissecação em cadáveres e peças anatômicas naturais ainda é altamente valorizada, mas enfrenta desafios logísticos e bioéticos. Os desafios da aprendizagem incluem a grande quantidade de informações a serem memorizadas, a complexidade da nomenclatura anatômica, a necessidade de disponibilidade de horários para práticas, o aumento e a diversificação das avaliações, e a intervenção na falta de motivação discente em relação ao estudo de anatomia. Ainda, há iniciativas de integração do ensino, pesquisa e extensão na anatomia que devem ser incentivadas para os estudantes se envolverem em projetos de pesquisa desde o início do curso e desenvolverem atividades com a comunidade, como a partir de museus de ciências da vida. Considerações Finais: O ensino de anatomia está em constante evolução, e as tendências investigativas contemporâneas têm o potencial de melhorar significativamente os processos de ensino e aprendizagem da morfologia humana. A pesquisa em ensino de anatomia, especialmente no contexto brasileiro, é essencial para entender a evolução dessa disciplina e adaptá-la às mudanças nas perspectivas educacionais e tecnológicas.


ABSTRACT Introduction: human anatomy plays a central role in the training of healthcare professionals, providing the fundamental basis for understanding the human body and its complexities. This is particularly relevant in the context of medical education, where the 2014 National Curricular Guidelines (DCN) mention the importance of structure-related curricular content, influencing the execution of anamnesis, physical examination and nosological diagnosis. Development: as far as the curriculum is concerned, anatomy teaching has undergone significant changes in recent decades, seeking to balance traditional and active approaches, promote curricular integration and expand bioethical and biopsychosocial perspectives in the study. Anatomy teaching has evolved, but is still often based on lectures and practical classes in the dissection laboratory, with a focus on memorization. However, there is a growing consensus that more interactive, practical and integrated approaches are needed, including the use of educational technologies. In terms of study methods, resources include textbooks, atlases, anatomical pieces, digital technologies and specialized software; dissection on cadavers and natural anatomical pieces remains highly valued, but is faced with logistical and bioethical challenges. Learning challenges include the large amount of information to be memorized, the complexity of anatomical nomenclature, the need to have time available for practices, increased and diversified assessments and interventions in the lack of student motivation in relation to the study of anatomy. Furthermore, there are initiatives to integrate teaching, research and community outreach in anatomy that should be encouraged for students to involve themselves in research projects from the start of the course and develop activities with the community, such as life science museums. Final Considerations: anatomy teaching is constantly evolving, and contemporary investigative trends have the potential to significantly improve the teaching and learning processes of human morphology. Research into anatomy teaching, especially in the Brazilian context, is essential to understand the evolution of this discipline and adapt it to changes in educational and technological perspectives.

ABSTRACT Knowledge of root canal internal anatomy and its variations is important forproper endodontic treatment. It is therefore necessary to investigate morphological aspects among different dental groups in the same patient to define the best protocol for the case. Aim To evaluate the morphology and symmetry of homologous incisors, premolars and mandibular molars using cone beam computed tomography (CBCT). Materials and Method Descriptive statistical analysis was performed for the frequency of categorical variables, and a chi-square test or Fisher 's exact test was used to test whether gender and side were associated with number of roots, number of canals, and Vertucci's classification. Forty-five CBCT scans were evaluated, and 444 mandibular teeth were analyzed. The number of roots, number of canals, classification of the canals in each root according to Vertucci and presence of a symmetrical relationship between pairs of posterior teeth were analyzed. Results The resuls showed that 74% of mandibular central incisors had type I root canal, 26% of mandibular lateral incisors had type I and, with a significant difference in the number of canals between males and females (p < 0.05). In mandibular first premolars, 70.5% had type I; and in mandibular second premolars, 98.5% had type I. Mandibular first molars had two roots in 98% of the cases. Second mandibular molars had two roots in 92.5% of the cases, one root in 6%, and three roots in 1.5%. Symmetry between central incisors was higher in females than in males. Conclusión Teeth of the same group can have different morphologies in the same patient.


RESUMO 0 conhecimento da anatomia interna e suas variagoes anatómicas é fator importante para o adequado tratamento endodóntico. Portanto, é necessário investigar esses aspectos morfológicos entre diferentes grupos dentários de um mesmo paciente para definir o melhor protocolo para o caso. Objetivo Avaliar a morfologia e simetria de incisivos, pré-molares e molares inferiores homólogos por meio de tomografia computadorizada de feixe cónico (TCFC). Materiais e Método Foi realizada análise estatística descri-tiva para a frequéncia das variáveis categóricas e foi utilizado o teste do qui-quadrado ou teste exato de Fisher para testar a relagao entre sexo e lado em comparagao com número de raízes, número de canais e classificagao de Vertucci. Quarenta e cinco TCFC foram avaliadas e 444 dentes inferiores foram analisados. Foram considerados: o número de raízes, o número de canais, o tipo dos canais acordo com a classificagao de Vertucci e a presenga de relagao simétrica entre pares de dentes posteriores. Resultados Os resultados mostraram que 74% dos incisivos centrais inferiores tinham um canal radicular tipo 1 e 26% tinham dois canais; 73% dos incisivos laterais inferiores, 26%oeram do tipo I, tinham um canal e 27% tinham dois canais, com diferenga significativa no número de canais entre os grupos masculino e feminino (p < 0,05). Nos primeiros pré-molares inferiores, tipo I, um canal foi detectado em 70,5% e dois canais em 29,5%; nos segundos pré-molares inferiores, tipo I, um único canal foi detectado em 98,5%. O primeiro molar inferior foi observado com duas raízes em 98% e tres raízes em 2%o. O segundo molar inferior tinha duas raízes em 92,5% dos casos, uma raiz em 6% e tres raízes em 1,5%. A simetria foi maior nas mulheres em comparagao aos homens nos incisivos centrais. Conclusao Pode-se concluir que dentes de um mesmo grupo podem apresentar morfologias diferentes no mesmo paciente.

SUMMARY: Head and cervical spine movements cause narrowing or widening of neuroforamina. In healthy individuals these movements do not cause symptoms of radiculopathy. This implies a compensating volume-regulating mechanism of the neuroforamina. Such a mechanism has been postulated in the years before CT and MRI for the neuroforaminal veins. Dural sac indentations with emptying and refilling of the internal vertebral venous plexus (IVVP) were postulated in the lumbar region using myelography. Emptying of the IVVP occurs in the lumbar spine when moving towards maximal extension and refilling while moving towards maximal flexion. Such indentations have not been shown in the cervical region. With MRI this mechanism has been demonstrated during axial rotation in the C1-C2 segment. It consists of emptying and refilling of the IVVP and thus prevents dural sac compression. During spinal surgery, the IVVP and connecting neuroforaminal veins may be damaged. Because the clinical implications of dysfunction of this protecting mechanism of the IVVP and its neuroforaminal venous connections are not clear, the consequences of such damage are unknown. Therefore, these venous structures should be examined by studying the cervical spine in supine position and, if possible, in different postures (flexion, extension and axial rotation) using MRI with contrast-enhancement and fat suppression. These images may be a basis for future advancement of clinical care.


Los movimientos de la cabeza y la columna cervical provocan un estrechamiento o ensanchamiento de las neuroforaminas. En individuos sanos estos movimientos no causan síntomas de radiculopatía. Esto implica un mecanismo compensador de regulación del volumen de las neuroforaminas. Este mecanismo se ha postulado en los años anteriores a la TC y la RM para las venas neuroforaminales. Mediante mielografía se postularon hendiduras del saco dural con vaciado y llenado del plexo venoso vertebral interno (PVVI) en la región lumbar. El vaciado del PVVI se produce en la columna lumbar cuando se mueve hacia la máxima extensión y se rellena mientras se mueve hacia la máxima flexión. En la región cervical no se han observado tales depresiones. Con resonancia magnética se ha demostrado este mecanismo durante la rotación axial en el segmento C1-C2. Consiste en vaciar y rellenar la PVVI y así evitar la compresión del saco dural. Durante la cirugía de columna, la PVVI y las venas neuroforaminales que las conectan pueden dañarse. Debido a que las implicaciones clínicas de la disfunción de este mecanismo protector de la PVVI y sus conexiones venosas neuroforaminales no están claras, se desconocen las consecuencias de dicho daño. Por tanto, estas estructuras venosas deben examinarse estudiando la columna cervical en decúbito supino y, si es posible, en diferentes posturas (flexión, extensión y rotación axial) mediante resonancia magnética con contraste y supresión grasa. Estas imágenes pueden ser una base para futuros avances de la atención clínica.

RESUMEN

ABSTRACT The purpose of this review is to highlight the most important aspects of the anatomical and functional uniqueness of the human brain. For this, a comparison is made between our brains and those of our closest ancestors (chimpanzees and bonobos) and human ancestors. During human evolution, several changes occurred in the brain, such as an absolute increase in brain size and number of cortical neurons, in addition to a greater degree of functional lateralization and anatomical asymmetry. Also, the cortical cytoarchitecture became more diversified and there was an increase in the number of intracortical networks and networks extending from the cerebral cortex to subcortical structures, with more neural networks being invested in multisensory and sensory-motor-affective-cognitive integration. These changes permitted more complex, flexible and versatile cognitive abilities and social behavior, such as shared intentionality and symbolic articulated language, which, in turn, made possible the formation of larger social groups and cumulative cultural evolution that are characteristic of our species.


RESUMO Esta revisão se propõe a relatar os aspectos mais importantes da singularidade anatômica e funcional do cérebro humano. Para isso, faz-se uma comparação entre o nosso cérebro e os de nossos parentes evolutivos mais próximos (chimpanzés e bonobos) e os ancestrais humanos. Durante a evolução humana ocorreu aumento absoluto do tamanho do cérebro e do número de neurônios corticais cerebrais, maior grau de lateralização funcional e assimetria anatômica cerebral, citoarquitetura cortical mais diversificada e aumento das redes neurais intracorticais e do córtex cerebral para as estruturas subcorticais acompanhada de mudança em direção ao investimento de redes neurais na integração multissensorial e sensório-motora-afetiva-cognitiva. Essas mudanças possibilitaram capacidades cognitivas e comportamentos sociais complexos, flexíveis e versáteis, destacando-se a intencionalidade compartilhada e a linguagem articulada simbólica, que permitiram a formação de grupos sociais maiores e a evolução cultural cumulativa característica de nossa espécie.

SUMMARY: Currently, training in the field of anatomy requires the implementation of learning and knowledge technologies (TAC). Therefore, the objective of this work was to use digital images taken of plastinated canine brains, hearts, and kidneys to create an interactive atlas that facilitates the teaching-learning of the anatomy of these organs. The research was carried out in 3 phases. In the first, canine brains, hearts and kidneys were obtained using the cold-temperature silicone plastination. In the second stage, photographs were taken, the images were edited with Adobe Photoshop and converted to SVG format using Adobe Illustrator. During the last phase, the 2D atlas was created using MongoDB and Node.js for the backend and Vue.js as the framework for the frontend. In addition, it was used three.js to render the 3D models. As a result, the 'Interactive Canine Atlas', ATINCA, was created. The atlas comprises 27 interactive images and 27 in atlas mode view (28 of the brain, 20 of the heart, and 6 of the kidney). Furthermore, the atlas features 3D models of the three organs. The developed atlas constitutes the first digital tool created in Ecuador based on local institutional needs, including a 3D format. Consequently, ATINCA will be integrated into the curricula as a digital material that will facilitate significant autonomous and collaborative learning of canine anatomical knowledge.


Actualmente, la formación en el campo de la anatomía requiere la implementación de tecnologías del aprendizaje y el conocimiento (TAC). Por ello, el objetivo del trabajo fue utilizar imágenes digitales tomadas de cerebros, corazones y riñones caninos plastinados para crear un atlas interactivo que facilite la enseñanza-aprendizaje de la anatomía de estos órganos. La investigación se llevó a cabo en tres fases. En la primera se obtuvieron cerebros, corazones y riñones caninos mediante la técnica de plastinación con silicona al frío. En la segunda etapa se tomaron fotografías, las imágenes se editaron con Adobe Photoshop y se convirtieron a formato SVG con Adobe Illustrator. Durante la última fase, se creó el atlas 2D usando MongoDB y Node.js para el backend y Vue.js como framework para el frontend. Además, se utilizó three.js para renderizar los modelos 3D. Como resultado se creó el Atlas Interactivo del Canino, ATINCA. El atlas cuenta con 27 imágenes interactivas y 27 en vista modo atlas (28 del cerebro, 20 del corazón y 6 del riñón). Además, el atlas presenta modelos 3D de los tres órganos. El atlas desarrollado constituye la primera herramienta digital creada en el Ecuador con base en necesidades institucionales locales y donde se incluye el formato 3D. Con lo cual, ATINCA podrá incorporarse en las mallas curriculares como material digital que facilitará el aprendizaje autónomo y colaborativo significativo de conocimientos anatómicos de los órganos caninos.

SUMMARY: The thyrohyoid muscle is one of the four infrahyoid muscles. Its role in vocalization and deglutition could be often overlooked, despite its crucial participation in these processes. Unlike other infrahyoid muscles, the thyrohyoid muscle receives innervation from the first cervical spinal nerves which contributes to its unique function. Its primary action involves hyolaryngeal elevation during swallowing, contributing to the opening of the upper esophageal sphincter. In conjunction with other muscles, it also protects the airway and facilitates the passage of food into the esophagus. Variations in the muscle's thickness may exist, and its function can be influenced by chewing habits. Weakened muscles involved in swallowing are often associated with dysphagia, a common complication in stroke and brain-injured patients. Advanced imaging techniques and sleep studies have provided insights into the dynamics and frequency of swallowing. This review explores the anatomic structures, function in action, diagnosis and clinical implications of this muscle. Overall, understanding the significance of the thyrohyoid muscle enhances our comprehension of the intricate interplay of laryngeal muscles during vocalization and deglutition.


El músculo tirohioideo es uno de los cuatro músculos infrahioideos. A menudo podría pasarse por alto su papel en la vocalización y la deglución, a pesar de su participación crucial en estos procesos. A diferencia de otros músculos infrahioideos, el músculo tirohioideo recibe inervación de los primeros nervios espinales cervicales, lo que contribuye a su función única. Su acción principal implica la elevación hiolaríngea durante la deglución, contribuyendo a la apertura del esfínter esofágico superior. Junto con otros músculos, también protege las vías respiratorias y facilita el paso de los alimentos al esófago. Pueden existir variaciones en el grosor del músculo y su función puede verse influenciada por los hábitos de masticación. Los músculos debilitados involucrados en la deglución a menudo se asocian con disfagia, una complicación común en pacientes con accidente cerebrovascular y lesión cerebral. Las técnicas de imagen avanzadas y los estudios del sueño han proporcionado información sobre la dinámica y la frecuencia de la deglución. Esta revisión explora las estructuras anatómicas, la función en acción, el diagnóstico y las implicaciones clínicas de este músculo. En general, comprender la importancia del músculo tirohioideo mejora nuestra comprensión de la intrincada interacción de los músculos laríngeos durante la vocalización y la deglución.

Aprendizajes en anatomía humana tradicionalmente han sido evaluados a través del reconocimiento de estructuras anatómicas. A pesar de su popularidad, pueden tener un componente subjetivo y memorístico. El Examen Práctico Objetivo Estructurado (EPOE) se ha presentado como una manera más global de evaluación. El objetivo de este estudio fue evidenciar el efecto en el rendimiento académico tras la implementación del EPOE en pasos prácticos. En el estudio 2312 estudiantes fueron divididos: Metodología Tradicional (n=1155) y Metodología EPOE (n=1157). A su vez, los estudiantes fueron identificados según carrera: Enfermería (n=1182); Fonoaudiología (n=185); Kinesiología (n=514) y Terapia Ocupacional (n=431). Se mantuvieron las condiciones de la asignatura en ambos grupos con tres evaluaciones prácticas. Se analizaron las calificaciones obtenidas. Evaluaciones 1 y 2 no presentaron diferencias entre grupos. Evaluación 3, que evaluó sistema nervioso, mostró disminución significativa de 0,5 puntos en el grupo EPOE. El factor tipo de metodología resultó ser significativo en evaluación 3 (p < 0,001; h2p = 0,029) y promedio de las 3 evaluaciones (p < 0,029; h2p = 0,002). El factor carrera resultó ser significativo para las tres evaluaciones. La interacción de ambos factores no mostró significancia estadística. El análisis post hoc mostró diferencias significativas entre estudiantes de Enfermería con las demás carreras, ya que obtuvieron mejores calificaciones en todas las evaluaciones (p < 0,05). Metodología EPOE no produjo drásticos cambios en el rendimiento académico de los estudiantes y tiene un amplio potencial de desarrollo por su naturaleza de evaluación integral no invasiva ni traumática. Se debe considerar la naturaleza de los contenidos en el diseño de la metodología de enseñanza-aprendizaje y en la metodología de evaluación, alineando a nivel microcurricular estos aspectos fundamentales de la formación de nuevos profesionales de la salud.


SUMMARY: Learning in human anatomy has traditionally been assessed by recognizing anatomical structures. Despite their popularity, they can have a subjective component. The Objective Structured Practical Examination (EPOE) has been presented as a more global way of assessment. The aim of this study was to demonstrate the effect on academic performance after the implementation of the EPOE in practical activities. In this study 2312 students were divided into Traditional Methodology (n=1155) and EPOE Methodology (n=1157). In turn, students were identified according to career: Nursing (n=1182), Speech Therapy (n=185), Physical Therapy (n=514), and Occupational Therapy (n=431). Subject conditions were maintained in both groups with three practical evaluations. The grades obtained were analyzed. Assessments 1 and 2 showed no differences between groups. Evaluation 3, which evaluated the nervous system, showed a significant decrease of 0.5 points in the EPOE group. The type of methodology factor was significant in evaluation 3 (p < 0.001; ?2p = 0.029) and the average of 3 evaluations (p < 0.029; ?2p = 0.002). The career factor was significant for all three evaluations. The interaction of both factors did not show statistical significance. The post hoc analysis showed significant differences between nursing students and the other careers since they obtained better scores in all evaluations (p < 0.05). EPOE methodology did not produce drastic changes in students' academic performance and had a broad potential for development due to its non-invasive and non-traumatic comprehensive assessment nature. The nature of the contents should be considered in the design of the teaching-learning methodology and the evaluation methodology, aligning at the micro-curricular level with these fundamental aspects of training new health professionals.

SUMMARY: The fabella is a bony anatomical variable, which in humans is located posterior to the lateral condyle of the femur, in the tendinous portion of the lateral head of the gastrocnemius muscle. Its prevalence rate has been reported in different populations, ranging from 3% to 86%. This study aimed to create an index that classifies the fabella according to its dimensions, determine the prevalence of the fabella in the Colombian population, and identify possible differences according to the fabella index (FI), sex, age, laterality, and degree of fabellar degeneration. A retrospective study used 93 lateral knee radiographs belonging to Colombian individuals. A fabella index was established to classify fabellae into longifabellar, medifabellar, and equifabellar, and radiological classification was used to identify the degree of fabellar degeneration (DFD). The prevalence of fabella was 36.30 %, and this variant was associated with the individual's sex, with male subjects having 2.66 times the probability of presenting fabella. A higher prevalence of medifabellar fabellae (57.60 %) was observed, followed by longifabellar (24.20 %) and equifabellar (18.20 %). There was a positive and moderate correlation between the DFD and age. We consider that larger fabellae can cause entrapment of the common fibular nerve, compression of the popliteal artery, and tear of the medial meniscus, so the FI becomes relevant in clinical practice, providing information on the type of fabella and its associated pathological manifestations. This study introduces a novel fabella index and investigates the frequency of fabella in the Colombian population. The findings of this study will serve as a reference for future research and contribute to the field of knee orthopedics and surgery in this population.


La fabela es una variable anatómica ósea, que en humanos se localiza posterior al cóndilo lateral del fémur, en la porción tendinosa de la cabeza lateral del músculo gastrocnemio. Su tasa de prevalencia se ha reportado en diferentes poblaciones, cuyos valores oscilan entre 3 % y 86 %. El objetivo de este estudio fue crear un índice que clasifique la fabela según sus dimensiones, determinar la prevalencia de la fabela en población colombiana, así como identificar posibles diferencias en función del índice fabelar, sexo, edad, lateralidad y grado de degeneración de la fabela. Se desarrolló un estudio retrospectivo en el que se dio uso a 93 radiografías laterales de rodilla, pertenecientes a individuos colombianos. Se estableció un índice fabelar que clasificó las fabelas en longifabelares, medifabelares y equifabelares, además utilizamos la clasificación radiológica para identificar los grados de degeneración fabelar (GDF). La prevalencia de la fabela fue del 36,30 %, se encontró asociación de esta variante al sexo del individuo, siendo los hombres quienes tuvieron 2,66 veces más probabilidades de presentar la fabela. Se observó mayor prevalencia de fabelas tipo medifabelar 57,60 %, seguido de tipo longifabelar 24,20 % y equifabelar 18,20 %. Hubo correlación positiva y moderada entre la edad y el grado de degeneración fabelar. Consideramos que las fabelas con mayores dimensiones podrían causar atrapamiento del nervio fibular común, compresión de la arteria poplítea y desgarro del menisco medial, por lo que el índice fabelar adquiere relevancia en la práctica clínica, al dar información del tipo de fabela y sus manifestaciones patológicas asociadas. El presente estudió es el primero en proponer un índice fabelar y la frecuencia de la fabela en población colombiana, por lo que servirá de referencia para futuras investigaciones, además de brindar un aporte al área traumatológica y quirúrgica de la rodilla en esta población.
