RESUMEN

HIV transmitted through cosmetic injection services via contaminated blood has not been previously documented. During summer 2018, the New Mexico Department of Health (NMDOH) was notified of a diagnosis of HIV infection in a woman with no known HIV risk factors who reported exposure to needles from cosmetic platelet-rich plasma microneedling facials (vampire facials) received at a spa in spring 2018. An investigation of the spa's services began in summer 2018, and NMDOH and CDC identified four former spa clients, and one sexual partner of a spa client, all of whom received HIV infection diagnoses during 2018-2023, despite low reported behavioral risks associated with HIV acquisition. Nucleotide sequence analysis revealed highly similar HIV strains among all cases. Although transmission of HIV via unsterile injection practices is a known risk, determining novel routes of HIV transmission among persons with no known HIV risk factors is important. This investigation identified an HIV cluster associated with receipt of cosmetic injection services at an unlicensed facility that did not follow recommended infection control procedures or maintain client records. Requiring adequate infection control practices and maintenance of client records at spa facilities offering cosmetic injection services can help prevent the transmission of HIV and other bloodborne pathogens and ensure adequate traceback and notification in the event of adverse clinical outcomes, respectively.

BACKGROUND: Vesicular stomatitis virus (VSV), a vector-borne pathogen of livestock, emerges periodically in the western US. In New Mexico (NM), US, most cases occur close to the Rio Grande River, implicating black flies (Simulium spp.) as a possible vector. In 2020, VS cases were reported in NM from April to May, although total black fly abundance remained high until September. We investigated the hypothesis that transience of local VSV transmission results from transient abundance of key, competent black fly species. Additionally, we investigated whether irrigation canals in southern NM support a different community of black flies than the main river. Lastly, to gain insight into the source of local black flies, in 2023 we collected black fly larvae prior to the release of water into the Rio Grande River channel. METHODS: We randomly sub-sampled adult black flies collected along the Rio Grande during and after the 2020 VSV outbreak. We also collected black fly adults along the river in 2021 and 2022 and at southern NM farms and irrigation canals in 2022. Black fly larvae were collected from dams in the area in 2023. All collections were counted, and individual specimens were subjected to molecular barcoding for species identification. RESULTS: DNA barcoding of adult black flies detected four species in 2020: Simulium meridionale (N = 158), S. mediovittatum (N = 83), S. robynae (N = 26) and S. griseum/notatum (N = 1). Simulium robynae was only detected during the VSV outbreak period, S. meridionale showed higher relative abundance, but lower absolute abundance, during the outbreak than post-outbreak period, and S. mediovittatum was rare during the outbreak period but predominated later in the summer. In 2022, relative abundance of black fly species did not differ significantly between the Rio Grande sites and farm and irrigation canals. Intriguingly, 63 larval black flies comprised 56% Simulium vittatum, 43% S. argus and 1% S. encisoi species that were either extremely rare or not detected in previous adult collections. CONCLUSIONS: Our results suggest that S. robynae and S. meridionale could be shaping patterns of VSV transmission in southern NM. Thus, field studies of the source of these species as well as vector competence studies are warranted.

Robust tick surveillance enhances diagnosis and prevention of tick-borne pathogens, yet surveillance efforts in the United States are highly uneven, resulting in large surveillance vacuums, one of which spans the state of New Mexico. As part of a larger effort to fill this vacuum, we conducted both active and passive tick sampling in New Mexico, focusing on the southern portion of the state. We conducted active tick sampling using dragging and CO2 trapping at 45 sites across Hidalgo, Doña Ana, Otero, and Eddy counties between June 2021 to May 2022. Sampling occurred intermittently, with at least one sampling event each month from June to October 2021, pausing in winter and resuming in March through May 2022. We also conducted opportunistic, passive tick sampling in 2021 and 2022 from animals harvested by hunters or captured or collected by researchers and animals housed in animal hospitals, shelters, and farms. All pools of ticks were screened for Rickettsia rickettsii, Rickettsia parkeri, Rickettsia amblyommatis, Ehrlichia ewingii, and Ehrlichia chaffeensis. Active sampling yielded no ticks. Passive sampling yielded 497 ticks comprising Carios kelleyi from pallid bats, Rhipicephalus sanguineus from dogs, mule deer, and Rocky Mountain elk, Otobius megnini from dogs, cats, horses, and Coues deer, Dermacentor parumapertus from dogs and black-tailed jackrabbits, Dermacentor albipictus from domesticated cats, mule deer and Rocky Mountain elk, and Dermacentor spp. from American black bear, Rocky Mountain elk, and mule deer. One pool of D. parumapterus from a black-tailed jackrabbit in Luna County tested positive for R. parkeri, an agent of spotted fever rickettsiosis. Additionally, a spotted fever group Rickettsia was detected in 6 of 7 C. kelleyi pools. Two ticks showed morphological abnormalities; however, these samples did not test positive for any of the target pathogens, and the cause of the abnormalities is unknown. Passive surveillance yielded five identified species of ticks from three domestic and six wild mammal species. Our findings update tick distributions and inform the public, medical, and veterinary communities of the potential tick-borne pathogens present in southern New Mexico.

No published studies have examined the community service priorities and harm reduction perspectives of unhoused people in Santa Fe, New Mexico. We conducted a mixed methods pilot study of 56 unhoused people at community locations in Santa Fe to: (1) assess the current prevalence of chronic medical conditions and substance use; (2) highlight community service priorities; and (3) explore views of innovative community harm reduction programs. Our first hypothesis was there would be high prevalence of chronic medical conditions, for which we found high prevalence of post-traumatic stress disorder, major depression, substance use disorders, chronic pain, and hypertension. Our second hypothesis was that we would find top community service priorities of housing, food, and health care. We found long- and short-term housing and food, but not healthcare, top priorities. Our third hypothesis was that we would find mixed support for community harm reduction initiatives like managed alcohol programs and overdose prevention centers. We found positive, not mixed, support for these community harm reduction programs among Santa Fe's unhoused. Unhoused study participants ranged in age 27-77 years, with lifetime years unhoused from less than one year to 63 years. Study limitations included small sample size, convenience sampling, and descriptive results. Policies and program initiatives supporting additional Housing First options, managed alcohol programs, and overdose prevention centers in the Santa Fe community are clearly indicated to increase engagement with this vulnerable population. Future research should focus on inclusion of the perspectives of the unhoused in the design, conduct, evaluation, and dissemination of community programs to meet the needs of the unhoused, with re-defined outcomes to include changes in quality of life, program engagement, demarginalization, and future goals and plans, beyond currently utilized health and social service program outcome measures.

Orthohantaviruses are diverse zoonotic RNA viruses. Small mammals, such as mice and rats are common chronic, asymptomatic hosts that transmit the virus through their feces and urine. In North America, hantavirus infection primarily causes hantavirus cardiopulmonary syndrome (HCPS), which has a mortality rate of nearly 36%. In the United States of America, New Mexico (NM) is leading the nation in the number of HCPS-reported cases (N = 129). However, no reported cases of HCPS have occurred within eastern NM. In this study, we assessed the prevalence of Sin Nombre virus (SNV) in rodent assemblages across eastern NM, using RT-qPCR. We screened for potential rodent hosts in the region, as well as identified areas that may pose significant infection risk to humans. We captured and collected blood and lung tissues from 738 rodents belonging to 23 species. 167 individuals from 16 different species were positive for SNV RNA by RT-qPCR, including 6 species unreported in the literature: Onychomys leucogaster (Northern grasshopper mouse), Dipodomys merriami (Merriam's kangaroo rat), Dipodomys ordii (Ord's kangaroo rat), Dipodomys spectabilis (Banner-tailed kangaroo rat), Perognathus flavus (Silky pocket mouse), and Chaetodipus hispidus (Hispid pocket mouse). The infection rates did not differ between sexes or rodent families (i.e., Cricetidae vs. Heteromyidae). Generalized linear model showed that disturbed habitat types positively influenced the prevalence of SNV at sites of survey. Overall, the results of this study indicate that many rodent species in east New Mexico have the potential to maintain SNV in the environment, but further research is needed to assess species specific infectivity mechanisms and potential risk to humans.

OBJECTIVESTo analyze the epidemiological, demographic, clinical, laboratory, radiographic and treatment outcome trends in non-US-born individuals with TB in New Mexico.DESIGNSWe retrospectively analyzed TB data from New Mexico TB surveillance system from (1993-2021), comparing variables between non-US-born and US-born individuals.RESULTSOf the 1,512 TB cases, 876 (56.5%) were non-US-born and 653 (43.3%) were US-born. The incidence rate among non-US-born patients declined from 15.3/100,000 (1993) to 7.8/100,000 (2021) (54.6% reduction), while among US-born patients it declined from 3.3/100,000 (1993) to 0.5/100,000 (2021) (84.8% reduction). The majority of non-US-born individuals were from Mexico (n = 482, 73.5%). Non-US-born were typically younger adults (median age: 54 vs. 61), predominantly male (64.8% vs. 59.4%), less likely to consume excess alcohol and have extrapulmonary TB. However, they were more likely to exhibit resistance to standard TB drugs (P < 0.01). Non-US-born individuals were less likely to die (7.8% vs. 15.4%), but more likely to be lost to follow-up (P < 0.007). Treatment by providers outside the Department of Health was associated with noncompletion (OR 0.18, 95% CI 0.09-0.35; P < 0.001).CONCLUSIONThese results highlight the need for a detailed understanding of the impact of migration on TB epidemiology and the development of tailored interventions to improve treatment outcomes..

BACKGROUND: The incidence of thyroid cancer in the United States has risen dramatically since the 1970s, driven by an increase in the diagnosis of small tumors. There is a paucity of published New Mexico (NM) specific data regarding thyroid cancer. We hypothesized that due to New Mexico's unique geographic and cultural makeup, the incidence of thyroid cancer and tumor size at diagnosis in this state would differ from that demonstrated on a national level. METHODS: The New Mexico Tumor Registry (NMTR) was queried to include all NM residents diagnosed with thyroid cancer between 1992 and 2019. For 2010 to 2019, age-adjusted incidence rates were calculated via direct method using the 2000 United States population as the adjustment standard. Differences in incidence rate and tumor size by race/ethnicity and residence (metropolitan vs non-metropolitan) were assessed with rate ratios between groups. For 1992 to 2019, temporal trends in age-adjusted incidence rates for major race/ethnic groups in NM [Non-Hispanic White (NHW), Hispanic, and American Indian (AI)] were assessed by joinpoint regression using National Cancer Institute software. RESULTS: Our study included 3,161 patients for the time period 2010 to 2019, including NHW (1518), Hispanic (1425), and AI (218) cases. The overall incidence rates for NM AIs were lower than those for Hispanics and NHWs because of a decreased incidence of very small tumors (<1.1 cm). The incidence rates for large tumors (>5.1 cm) was equivalent among groups. In the early 2000s, Hispanics also had lower rates of small tumors when compared to NHWs but this trend disappeared over time. CONCLUSION: AIs in New Mexico have been left out of the nationwide increase in incidental diagnosis of small thyroid tumors. This same pattern was noted for Hispanics in the early 2000s but changed over time to mirror incidence rates for NHWs. These data are illustrative of the health care disparities that exist among New Mexico's population and how these disparities have changed over time.

With the intensity and frequency of wildfires increasing rapidly, the need to study the ecological effects of these wildfires is also growing. An understudied aspect of fire ecology is the effect fires have on parasite-host interactions, including ectoparasites that might be pathogen vectors. Although some studies have examined the impacts of fire on ticks, studies on other ectoparasites, including pathogen vectors, are rare. To help address this knowledge gap, we examined the abiotic and biotic factors that predict the likelihood and extent of parasitism of deer mice (Peromyscus maniculatus) by fleas within a landscape of unburned and recovering burned (>9 yr postfire) mixed conifer forests. We sampled 227 individual deer mice across 27 sites within the Jemez Mountains of northern New Mexico in 2022 and quantified measures of parasitism by fleas (primarily Aetheca wagneri). These sites were distributed in both unburned areas (n = 15) and recovering burned areas (n = 12), with the latter derived from 2 large fires, the Las Conchas fire (2011) and the Thompson Ridge fire (2013). Using these data, we tested for differences in prevalence, mean abundance, and mean intensity of fleas on deer mice, focusing on the predictive importance of host sex and fire history. We also created generalized linear mixed-effects models to investigate the best host and environmental predictors of parasitism by fleas. Approximately a decade postfire, we found minimal evidence to suggest that fire history influenced either the presence or intensity of fleas on deer mice. Rather, at the current forest-regeneration stage, the extent of parasitism by fleas was best predicted by measures of host sex, body condition, and the trapline's ability to accumulate water, as measured through topography. As host body condition increased, the probability of males being parasitized increased, whereas the opposite pattern was seen for females. Male mice also had significantly greater flea loads. Among potential abiotic predictors, the topographic wetness index or compound topographic index (a proxy for soil moisture) was positively related to flea intensity, suggesting larger flea populations in burrows with higher relative humidity. In summary, although fire may potentially have short-term impacts on the likelihood and extent of host parasitism by fleas, in this recovering study system, host characteristics and topographic wetness index are the primary predictors of parasitism by fleas.

INTRODUCTION: Although Hispanic White (HW) females have a lower incidence of breast cancer than non-Hispanic White (NHW) females, breast cancer risk is unclear for HW females after benign breast disease (BBD). METHODS: We compared BBD characteristics and subsequent breast cancer risk among HW and NHW females in New Mexico using a population-based collection of benign breast biopsies (1996-2007). BBD was categorized as nonproliferative disease (NPD), proliferative disease without atypia (PDWA), or atypical hyperplasia (AH). Breast cancer risk was assessed as absolute risk (AR) using cumulative incidence and RR by comparing the number of breast cancer events in BBDs to non-BBD. RESULTS: This study included 3,684 HW and 6,587 NHW females with BBD. HW females had similar proportions of NPD (58.6% vs. 54.3%), PDWA (21.4% vs. 23.5%), and AH (3.6% vs. 3.3%) as NHW females. Breast cancer risk among all females with BBD was higher than population-based expected rates (RR, 1.87) and was similar for HW and NHW subgroups (RR = 1.99 vs. 1.84). As expected, breast cancer risk increased with increasing BBD severity, both overall [RR, 1.81 (NPD), 1.85 (PDWA), and 3.10 (AH)] and in the HW and NHW subgroups. Adjusted AR of breast cancer at 5 years also increased with the severity of BBD (HW vs. NHW; NPD: 1.4% vs. 2.1%; PDWA: 1.5% vs. 2.7%; AH: 6% vs. 4.8%). CONCLUSIONS: We found similar breast cancer RRs and ARs in HW and NHW. Risk counseling should ensure that HW females receive breast cancer clinical management warranted by their similar absolute risks. IMPACT: The present population-based provides evidence for the clinical management of HW females with BBD for the prevention of breast cancer.

The indigenous North American mammalian schistosome Heterobilharzia americana has recently attracted attention for causing outbreaks in dogs in states outside of its southeastern U.S. distribution. Although H. americana has yet to be reported in New Mexico, we examined 2 New Mexico isolates of Galba snails to determine their susceptibility to experimental infection with an isolate of H. americana from Utah. One of the Galba isolates from the Rio Grande bosque in the Albuquerque suburb of Corrales was identified as Galba humilis, and like specimens of the same taxon from Utah, proved susceptible to H. americana (27.6% of exposed surviving snails positive). The second Galba isolate sourced from the northern mountains of New Mexico, which surprisingly was revealed to be Galba schirazensis based on cytochrome c oxidase 1, 16S rRNA, and the internal transcribed spacer 2 markers, was also susceptible to H. americana (56.3% of exposed surviving field-derived snails and 46.4% first generation [F1] snails positive). This is the first report of the latter snail being a compatible snail host for H. americana. As G. schirazensis has a wide, albeit spotty, distribution and is considered an invasive species, it provides yet another opportunity for H. americana to expand its known range, potentially including the state of New Mexico as well.

Hispanic and American Indian (AI) women experience lower breast cancer incidence than non-Hispanic White (NHW) women, but later-stage diagnoses and lower survival rates, suggesting issues with screening and healthcare access. Between 1999-2015, NHW breast cancer incidence decreased by 10% but increased by 8% for AI women. This study used 2016 and 2018 Behavioral Risk Factor Surveillance System data for Arizona and New Mexico to explore mammography screening. Analyses included Hispanic, AI, and NHW women aged ≥40 years (n = 12,830) to calculate age-specific compliance by race/ethnicity, logistic regression, and adjusted and sample-weighted evaluated associations between compliance and socio-economic covariates. In total, 75.1% of Hispanic women aged 50-74 reported mammography in the past two years (United States Preventive Services Task Force compliant) compared to 73.9% of NHW and 71.0% of AI women. Women who reported doctor visits in the past 12 months were likelier to comply than those without (AOR = 4.2 for Hispanic, 2.9 for AI, and 3.2 for NHW women). Reporting access to a personal doctor was related to compliance, except for AI women. While screening compliance was over 74%, visiting a healthcare provider in the past 12 months was essential. AI women reported issues that suggest unique challenges when deciding on mammography.

Objetivo: Describir el contexto en el que los síndromes culturales de las madres de ascendencia mexicana creían que afectaban a la salud infantil y describir cómo ellas mejoraban la salud cuando estos síndromes afectaban a sus hijos. Diseño: Los resultados de este estudio se basan en una amplia investigación que se centró en la promoción de la salud y las prácticas de protección usadas por las madres de ascendencia mexicana en la ciudad de Texas. Se seleccionó un diseño naturalista, mediante las técnicas de entrevistas etnográficas de Spradley y observaciones de los participantes, para explorar y describir la promoción de la salud infantil y las prácticas de protección, incluyendo los síndromes culturales usados por las madres de origen mexicano. Método: La recopilación de los datos consistió en 21 entrevistas etnográficas mejoradas por el enfoque de las observaciones en hogares con 9 madres de habla hispana. Resultados: Para este grupo de mujeres, los síndromes culturales del "empacho", "mal de ojo" y "susto" podían provocar trastornos alimenticios y del sueño, causando un desajuste en el bienestar del niño. Además, las participantes creían que ellas tenían que ser conscientes de los síndromes culturales que afectaban a la salud de sus hijos y cuidar aquellos síndromes usando remedios tradicionales para promocionar y proteger la salud de sus niños. Conclusión: Los resultados de este estudio ofrecen una descripción en profundidad de los síndromes culturales y los remedios tradicionales que las madres de ascendencia mexicana usaban para promocionar y proteger la salud de sus niños. Este conocimiento ofrece un marco para que los profesionales de la salud lo usen cuando trabajen con madres de ascendencia mexicana que pueden estar usando este tipo de medios para cuidar a sus hijos (AU)


Purpose: To describe the context in which culture-bound syndromes that mothers of Mexican descent believed affected child wellness and describe how they restored health when these syndromes affected their children. Design: The findings of this come from a larger study that focused on the health promotion and protection practices used by mothers of Mexican descent in urban Texas A naturalistic design, using Spradley’s ethnographic interview techniques and participant observations, was selected to explore and describe the child health promotion and protection practices, including culture-bound syndromes, used by mothers of Mexican origin. Method: Data collection consisted of 21 ethnographic interviews enhanced by focused home observations with nine Spanish speaking mothers. Results: To these group of mothers, the culture-bound syndromes of empacho, fright and evil eye could affect children’s eating and sleep patterns, thereby causing an imbalance in a child’s wellbeing. Therefore the participants believed that they had be mindful of culture-bound syndromes that affected their children’s health and take care of those syndromes by using folk remedies to restore balance in their children’s’ wellbeing. Conclusion: The findings of this study provide an in-depth description of culture-bound syndromes and the folk remedies which mother of Mexican descent used to promote and protect the health of their preschool children. This knowledge provides a framework for healthcare professionals to use when working with mothers of Mexican descent who may be using folk healing to promote and protect the health of their children (AU)

RESUMEN

Se efectuó un estudio epidemiológico de melanoma maligno mediante la revisión de todos los casos de melanomas reportados al Registro de Cáncer de Puerto Rico entre los años 1981 al 1987. Se documentaron un total de 367 casos nuevos con una incidencia anual que fluctuó entre 1.20 y 2.12 por 100,000 habitantes y un promedio de 1.59. La mayoría de los pacientes tenín entre los 40 y 80 años de edad, con una mayoría en los 60. Casi la mitad de los tumores ocurrieron en las extremidades, sobre todo en los pies, compartiendo esta tendencia con otras razas como los negros y japoneses. El tipo clínico-histológico más frecuente lo fue el melanoma superficial expansivo seguido del acral lentiginoso , el nodular y el melanoma lentigo maligna. Cerca de un tercio del total de casos no fue clasificado de acuerdo al tipo histológico, mientras que los niveles de Clark y el grosor de Breslow no fueron reportados en 44 y 84% de los casos respectivamente. Cuando comparamos nuestra data a la de un estudio previo en Puerto Rico entre los años de 1977 al 1980, la incidencia anual promedio de casos nuevos aumentó de 0.92 a 1.59 por 100,000 habitantes, documentándose un aumento de incidencia en nuestra población, pero no tan significativo como el aumento registrado en los Estados Unidos

The association of Hispanic race/ethnicity and poverty with general survival time and breast cancer survival time was examined for a total of 14,896 breast cancer patients (14,035 White and 861 Hispanic) included in the National Cancer Institute Surveillance Epidemiology and End Results (NCI SEER) program in New Mexico and San Francisco between 1975 and 1984. Variables examined included: age, marital status, stage at diagnosis, tumor histology, delay, treatment, period of diagnosis (1975-79 vs. 1980-84), and poverty. Univariate analysis of 14,896 patients indicated that a greater proportion of Hispanics (vs. Whites) with breast cancer were: younger than age 50, married, diagnosed at a later stage, diagnosed in New Mexico, lived in greater poverty, were diagnosed between 1980-84, and died from breast cancer. Univariate Cox Proportion Hazards analysis indicated that poverty was a significant predictor for reduced general survival time. Being diagnosed in the 1980-84 period was a predictor for improved general survival time. Poverty and Hispanic race/ethnicity were significant predictors of reduced breast cancer survival time. Multivariate Cox Proportional Hazards models indicated that Hispanic race/ethnicity was a significant risk factor for breast cancer survival time for women aged 50 and older. For White women: state, marital status, poverty, surgery, radiation/hormonal treatments, and histology were significant risk factors for breast cancer survival time. For Hispanic women: stage, surgery, hormonal treatment and period of diagnosis were significant risk factors for breast cancer survival time. For White breast cancer patients, period of diagnosis was not a significant risk factor for reduced breast cancer survival time; but for Hispanics, it was a significant risk factor. In the age and race/ethnicity-stratified models of breast cancer survival time, similar risk factors emerged for both Whites and Hispanics. For both younger and older Hispanics, being diagnosed in the early 1980's (vs. the late 1970's) was associated with reduced breast cancer survival time--vs. Whites, who experienced no significant change in breast cancer survival time in the same time period. Poverty was not a predictor for Hispanic survival time in any of the models; however, it was a predictor for younger Whites for breast cancer survival time. These results fueled discussion in three areas targeting breast cancer in underserved women: the development of racial/ethnic-specific cancer control guidelines, the development of a breast cancer integrated delivery system, and population management
