ABSTRACT

ObjectiveTo establish a qualitative and quantitative analysis method for chemical constituents in Liu Junzitang(LJZT), and to clarify its material basis. MethodThe chemical constituents in LJZT were analyzed by ultra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS), and the resulting compounds were identified by using databases, such as MassBank, PubChem, ChemSpider, Traditional Chinese Medicine Systems Pharmacology Database and Analytical Platform(TCMSP), and by combining with relevant literature. UPLC was used to establish a quantitative method for analysis of 9 compounds in LJZT, including liquiritin, hesperidin, lobetyolin, liquiritigenin, glycyrrhizic acid, nobiletin, tangeretin, atractylenolide Ⅱ and Ⅰ. ResultBy combining the relevant literature, database and MS information, a total of 79 compounds were identified from LJZT, including 31 flavonoids, 15 terpenoids, 14 nitrogen-containing compounds, 6 phenylpropanoids, 6 organic acids and 7 other compounds. The established quantitative analytical method for the nine representative components showed good linearity within their respective linear ranges, and the precision, stability, reproducibility and recovery were in accordance with the requirements. The quantitative results showed that the contents of liquiritin, hesperidin, lobetyolin, liquiritigenin, glycyrrhizic acid, nobiletin, tangeretin, atractylenolide Ⅱ and Ⅰ in LJZT were 0.376 5, 2.602 1, 0.082 6, 0.128 1, 1.778 6, 0.015 7, 0.006 7, 0.030 4, 0.003 2 mg·g-1, respectively. ConclusionThe established method can quickly, sensitively and accurately analyze the chemical constituents in LJZT, clarify that the material basis of LJZT is mainly flavonoids, terpenoids and nitrogen-containing compounds, and simultaneously determine the contents of the 9 components, which can lay a foundation for the research on quality control, mechanism and clinical application of LJZT.

ABSTRACT

Brillouin fiber-optic gyroscope (BFOG) is a fiber ring resonator (FRR)-based ring laser gyroscope with a large dynamic range and high sensitivity. Previous BFOGs operated under simultaneous resonance of the pump and Brillouin laser light in the FRR, resulting in complex control systems. This article introduces an asymmetric prism Mach-Zehnder interferometer coupler (APMZIC)-based FRR system, in which the coupling ratio of pump light is large, while the Brillouin laser resonates in the cavity with a negligible coupling loss, resulting in a simple control system. We analyzed the characteristics of the APMZIC and its combined resonator system both theoretically and experimentally and verified the feasibility of connecting the APMZIC with the resonant cavity of a Brillouin laser. This coupler system is compact, easy to fabricate, highly stable, and exhibits good repeatability, which are necessary for miniaturizing lasers and gyroscopes.

ABSTRACT

The performance appraisal for tertiary public hospitals provides means for the rational management of drug use in public hospitals. Starting from October 2021, Hospital Z had established a drug performance appraisal system based on the relevant requirements for rational drug use in the performance appraisal for tertiary public hospitals, in view of the actual situation. The system included four indexes: weighted outpatient per capita drug cost, weighted inpatient per capita drug cost, rational drug use rate, and intensity of antibacterial drug use. Different scores were assigned to each indexi and scoring methods were developed. By dividing all drugs in the hospital medication catalog into three categories A, B, and C and assigning them different weights, as well as setting reasonable target values for each indexi, the hospital guided clinical departments to use drugs rationally. The proportion of drugs in hospital Z decreased from 28.75% in 2020 to 25.44% in 2022, the proportion of medical service revenue increased from 22.97% to 28.40%, the per capita outpatient drug cost decreased from 170 yuan to 166 yuan, and the per capita inpatient drug cost decreased from 7 318 yuan to 5 983 yuan. The number of departments with a 100% reasonable drug use rate increased from 11 in September 2021 to 16 in September 2022, and the intensity of antibacterial drug use decreased by 35.84%. The opplication of this drug performance appraisal system partly optimized the hospital′s income structure, controlled the growth of drug costs, promoted the rational use of drugs, so as to provid references for the rational drug use management of tertiary public hospitals.

ABSTRACT

Objective:To explore the efficacy of tislelizumab combined with umbilical cord blood transplantation (UCBT) in relapsed/refractory acute myeloid leukemia (R/R AML) patients. Methods:The diagnosis and treatment of 1 patient with R/R AML who received tislelizumab bridging to UCBT after the failure of re-induction treatment in the First Affiliated Hospital of Soochow University in November 2021 was retrospectively analyzed. Results:The 59-year-old male patient with R/R AML achieved a complete remission after initial induction chemotherapy regimen of decitabine and venetoclax, and then additional consolidation therapy regimens of decitabine and middle-dose cytarabine, middle-dose cytarabine and idarubicin were performed. The patient relapsed 16 months later and failed to achieve a second remission after re-induction therapy regimens of cladribine, azacitidine, venetoclax combined with chemotherapy, and homoharringtonine, cytarabine combined with granulocyte colony-stimulating factor. Tislelizumab significantly reduced tumor burden and the patient achieved the complete remission after bridging to UCBT. After transplantation, the patient was given maintenance treatment with azacitidine and he had sustained remission without severe transplant-related complications during 9-month follow-up.Conclusions:The use of tislelizumab bridging UCBT can be a potential therapeutic strategy for R/R AML patients.

ABSTRACT

Objective:To investigate the predictive value of visceral adipose tissue (VAT) in occult peritoneal metastasis (OPM) of gastric cancer. Methods:A total of 93 patients with gastric cancer admitted to the First Affiliated Hospital of Zhengzhou University from October 2018 to October 2021 were retrospectively collected. None of the patients had typical peritoneal metastasis on CT. Patients were divided into OPM group (31 cases) and non-OPM group (62 cases) according to laparoscopic exploration. The clinical, pathological and CT features were recorded. The parameters related to adipose tissue (VAT and subcutaneous adipose tissue) within the range of 15 mm and 25 mm below the largest layer of gastric cancer lesions in preoperative CT images were measured, including the volume, average CT attenuation and standard deviation. The independent-sample t test, Wilcoxon rank-sum test, χ 2 test or Fisher′s exact probability were used to compare the clinical, pathological and CT imaging parameters between OPM and non-OPM groups. Multivariate logistic regression analysis was used to explore the independent risk factors for OPM of gastric cancer and establish a combined model. The receiver operating characteristic curve was used to evaluate the efficacy of each indicator and the combined model in predicting OPM of gastric cancer. Results:There were statistically significant differences in age, pathological type, CA125, ascites, cT stage, the thickest diameter of lesion, average CT attenuation of 15 mm VAT and 25 mm VAT between the OPM group and the non-OPM group ( P<0.05). Multivariate analysis showed that ascites, cT stage and average CT attenuation of 25 mm VAT were independent risk factors for OPM of gastric cancer, with the OR (95%CI) of 4.940 (1.287-18.967), 4.284 (1.270-14.455), and 1.149 (1.013-1.303), respectively. A combined model was established. The area under the curve (AUC) of ascites, cT stage, average CT attenuation of 25 mm VAT, average CT attenuation of 15 mm VAT and combined model were 0.685, 0.718, 0.703, 0.674 and 0.813, respectively. There were statistically significant differences in AUC between the combined model and each four single indicators above ( Z=2.98, 2.63, 2.09, 2.54, P=0.003, 0.009, 0.037, 0.011). Conclusions:The ascites, cT stage and average CT attenuation of 25 mm VAT are independent risk factors for OPM in gastric cancer. The combined model based on the above three indicators has the best performance in predicting OPM in gastric cancer.

ABSTRACT

Objective:To prepare the sustained release system of icariin(ICA)@ gelatin nanoparticles (GNPs)-polyactic-co-glycolic acid(PLGA)(ICA @ GNPs-PLGA), and to optimize the conditions. Methods:ICA@GNPs-PLGA sustained release system was prepared using two-step desolvation method and S/O/W emulsion solvent-evaporation technique. The effects of different conditions, such as the PLGA:GNPs mass ratio and the total quality of ICA added on the entrapment efficiency(EE)of ICA@GNPs-PLGA composite microspheres were detected to optimize the preparation process. The surface morphology of GNPs and ICA@GNPs-PLGA composite microspheres were observed by SEM.The EE and the release results of ICA@GNPs-PLGA in the sample were determined with HPLC.Results:The prepared composite microspheres and nanocomplex were were white powder. The SEM results showed that the composite microspheres and nanocomplexs were spherical, the surfaces were smoothy, and the particle size distribution range was 4-12 μm and 150-200 nm,respectively, relatively uniform. At a GNPs mass fraction of 6 mg,the critical concentration of PLGA in DCM ranged within 0.5%-1.0%.At a GNPs mass fraction of 12 mg,the critical concentration of PLGA in DCM ranged within 1.0%-2.0%.However,at a critical PLGA mass fraction lower than 0.25%,no fully formed composite microspheres were observed.Within the critical concentration, the average EE of ICA@GNPs-PLGA microspheres was higher than(62.00±1.25)%.In addition, the EE of ICA in the microspheres was negatively correlated with the quality of ICA added. The accumulative release rate was less than in 24 h and it was 65.21% in 40 d.Conclusion:The ICA@GNPs-PLGA microspheres with homogeneous particle size distribution, high EE,low initial burst and without agglomeration can be acquired under the optimized conditions.

ABSTRACT

Tumor cell line culture technologies emerged as a strong impetus to the development of tumor biology research. However, cell line tumor model presented limited predictive value and instable properties in the clinical translational research of new cancer therapies.Recently Patient-derived Xenograft Model (PDX) has aroused researcher's attention and been widely applied to new cancer therapy research. This new model could highly retain the stroma heterogeneity、histological characteristics and molecular diversity of the original tumor, and is expected to bring new breakthroughs in individualized treatment of tumor patients.
