RESUMEN
Se presenta un caso de sexo femenino, 4 años de edad, área urbana, en el Hospital Pediátrico Docente Pedro Agustín Pérez con antecedentes de haber presentado hacía 8 meses una púrpura trombocitopénica inmunológica que después de infección respiratoria alta comenzó con manifestaciones purpúricas en forma de petequias y equimosis diseminadas por todo el cuerpo, palidez cutáneo mucosa severa, siendo ambas manifestaciones clínicas expresión de trombocitopenia y anemia intensas, hepatomegalia de 2 cm y esplenomegalia de 4 cm adenopatías periféricas generalizadas de pequeño a mediano tamaño, íctero en piel y esclera, además de coluria. Se realizaron estudios hematológicos, humorales e inmunológicos. Es atendida en la unidad de Cuidados Intermedios de dicho hospital, evolucionando favorablemente con respuesta satisfactoria al tratamiento con esteroides. Se realizó la discusión diagnóstica del caso confirmándose diagnóstico de síndrome de Evans-Fisher, revisándose la bibliografía actualizada de esta entidad(AU)
A case of female, 4 years old, urban area, with a history of presenting during eight months immune and purpura thrombocytopenic after upper respiratory infection began with purpuric manifestations in the form of petechiae and bruises scattered throughout the body, severe skin pale mucosa, with both clinical expression of thrombocytopenia and severe anemia, hepatomegaly of 2 and 4 cm splenomegaly generalized peripheral lymphadenopathy small to medium size, skin and sclera jaundice, dark urine, too. Haematological humoral and immunological studies were performed. It is served in the Intermediate Care Unit of the hospital, evolved favorably with satisfactory response to steroid treatment. Diagnostic case discussion confirmed diagnosis of Evans-Fisher syndrome, reviewing the updated bibliography of this entity(AU)

Se realiza la presentación de un caso con insuficiencia renal crónica terminal secundaria, hipertensión maligna en tratamiento dialítico diagnosticándosele mieloma múltiple, asistido en Hospital General Docente Dr Agostinho Neto, de Guantánamo en 2014. Acude al servicio refiriendo dolor intenso en región cervical, disminución de fuerza muscular y febrícula, decidiéndose ingreso con diagnóstico de mielitis transversa de etiología infecciosa. Los estudios de imágenes TAC revelaron lesiones líticas en columna cervical, planteándose posibilidad de mieloma múltiple reconsiderándose diagnóstico, decidiéndose biopsia de médula ósea que revela infiltración plasmocitaria mayor del 40 por ciento considerado criterio mayor en el diagnóstico del mismo. El paciente es atendido evolucionando desfavorablemente y fallece en menos de una semana. Son identificados componentes morfológicos que caracterizan el mieloma múltiple(AU)
The presentation of a male patient of 64 years is performed with terminal chronic renal failure secondary to malignant hypertension in dialysis with multiple myeloma, assisted in the General Teaching Hospital Agostinho Neto Guantánamo, 2014. The patient came to referring intense pain in cervical region, decreased muscle strength and fever, deciding admission with a diagnosis of transverse myelitis of infectious etiology. CAT imaging studies reveal lytic lesions in cervical spine, raising possibility of multiple myeloma diagnosis reconsidering deciding bone marrow biopsy revealed increased plasma cell infiltration of 40 percent considered major criterion in the diagnosis of it. It is treated unfavorably evolving and died in less than a week. Morphological components are identified that characterize multiple myeloma(AU)
