OBJECTIVES: Nonsuicidal self-injury (NSSI) occurs more frequently in the prisoner population than in the general population. Monitoring and management of this behavior is challenging because NSSI may present in diverse ways. People often use more than one method of NSSI, and there are many possible combinations of these behaviors. We used latent class analysis (LCA) to identify subgroups of male inmates based on methods and frequency of NSSI. METHODS: A total of 1042 male prisoners in China (Mage = 38.45, SD = 10.67) completed measures of hopelessness, sensation seeking, identity integration, and suicidal ideation, and NSSI was assessed in a structured interview. RESULTS: Results of the LCA supported a three-class model: high-NSSI (1.8%), moderate-NSSI (8.0%), and no-or-negligible NSSI (90.2%). Multinomial logistic regression analyses showed that prisoners in the high-NSSI and moderate-NSSI classes were significantly more likely than those in the no-or-negligible NSSI class to show high hopelessness, high sensation seeking, and low identity. Furthermore, suicidal ideation was significantly higher in the high-NSSI and moderate-NSSI classes than in the no-or-negligible NSSI class. CONCLUSIONS: Classifying groups of prisoners based on features of NSSI is potentially useful for understanding risk factors and for developing tailored prevention and treatment strategies.

Traumatic events are stressful episodes that exceed an individual's coping resources and have been related to physical and mental health problems, particularly posttraumatic stress disorder. This study aims to compare the prison population and the general population in terms of trauma and psychopathological symptoms derived from exposure to traumatic events in the context of Ecuador. It is a descriptive-comparative and cross-sectional study. The study sample consisted of 99 incarcerated individuals and 84 persons from the general population of Ambato-Ecuador. The results indicate that persons from the prison population have experienced multiple traumas and re-victimization to a greater extent and present a higher percentage of PTSD and psychopathological symptoms than the general population. In that sense, it is a population that requires more significant intervention to reduce symptoms and prevent violent behavior.


Los eventos traumáticos son hechos estresantes que exceden los recursos de afrontamiento de un individuo y han sido relacionados con problemas de salud física y mental, particularmente con el trastorno de estrés postraumàtico. El objetivo de este estudio es comparar las experiencias traumáticas y los síntomas psicopatológicos entre la población penitenciaria y la población general. Es un estudio descriptivo-comparativo de corte transversal. La muestra del estudio estuvo conformada por 99 individuos encarcelados y 84 individuos de la población general de Ambato (Ecuador). Los resultados indican que los participantes de la población penitenciaria han experimentado en mayor medida múltiples traumas y revictimización; presentan un mayor número de síntomas psicopatológicos y un porcentaje más elevado de TEPT que la población general. Las diferencias que se presentan entre la población penitenciaria y la población general pueden deberse a que las personas encarceladas son más vulnerables a estar expuestas a eventos potencialmente traumáticos por su propia situación social. La población penitenciaria requiere una mayor intervención para reducir los síntomas psicopatológicos provocados por el trauma.

Introducción: La población penitenciaria debido a su condición vulnerable está expuesta a sufrir el deterioro de su salud. El estado es quien debe prestar y garantizar los servicios básicos. El objetivo fue describir las condiciones de acceso a la salud bucal de los internos recluidos en centros penitenciarios del Perú. Material y métodos: Es un estudio descriptivo, longitudinal y se recogió los datos en una ficha documental de fuentes secundarias entre el periodo 2015 al 2021 proporcionada por el Instituto Nacional Penitenciario del Perú. La muestra fue de 87245 internos recluidos en 64 establecimientos y distribuidos en ocho regiones. Para la recolección de datos y el análisis estadístico se empleó el paquete profesional software estadístico IBM SPSS Statistics versión 26 para Windows (SPSS Inc., de Estados Unidos). El análisis de datos es descriptivo, se realizó utilizando tablas de frecuencias, en las cuales se trabajó con la frecuencias absolutas y relativas. Resultados: Los resultados demuestran que solo 43 internos (67,19%) cuentan con servicios odontológicos, de los cuales 29 tienen asignado un personal odontólogo. Discusión: En cuanto a las medidas de prevención, ninguno de los centros penitenciarios destina presupuesto para este fin. Durante el año 2021, 22364 internos no recibieron atención bucal. Gran parte de la población penitenciaria no tiene acceso a los servicios básicos de odontología y otros reciben servicios precarios. Se pone en riesgo la salud bucal de los internos y se les priva de sus derechos básicos de acceso a la salud.


Introduction: The prison population, due to its vulnerable condition, is exposed to deteriorating health. The state is the one who must provide and guarantee basic services. The objective was to describe the conditions of access to oral health for inmates confined in penitentiary centers in Peru. Material and methods: It is a descriptive, longitudinal study and it collected the data in a documentary file from secondary sources between 2015 to 2021 provided by the Peru's National Penitentiary Institute. The sample consisted of 87,245 inmates confined in 64 establishments and distributed in eight regions. For data collection and statistical analysis, the professional statistical software package IBM SPSS Statistics version 26 for Windows (SPSS Inc., United States) was used. The data analysis is descriptive, it was carried out using frequency tables, in which we worked with the absolute and relative frequencies. Results: The results show that only 43 inmates (67.19%) have dental services, of which 29 are assigned a dental staff. Discussion: Regarding prevention measures, none of the prisons allocate a budget for this purpose. During the year 2021, 22,364 inmates did not receive oral care. A large part of the prison population does not have access to basic dental services and cons receive precarious services. The oral health of inmates is put at risk and they are deprived of their basic rights to access to health.

The food served in Canadian penitentiaries was scrutinized following food service reform where Correctional Service Canada (CSC) created a standardized menu to feed incarcerated male individuals. Food in prison is a complex issue because penitentiaries are responsible for providing adequate nutrition to the prison population, who are vulnerable to poor health outcomes but are often seen as undeserving. This study aimed to analyse the national menu served in Canadian penitentiaries, in order to compare them with Dietary Reference Intakes (DRIs) for male adults and the internal nutritional assessment reported by CSC. The goal was to verify if the menu served was adequate and to validate CSC's nutritional assessment. The diet analysis software NutrificR was used to analyse the 4-week cycle menu. Both analyses were within range for DRIs for most nutrients. However, some nutrients were not within target. The sodium content (3404.2 mg) was higher than the Tolerable Upper Intake Levels (UL) of 2300 mg, the ω-6 (linolenic acid) content (10.8 g) was below the AI of 14 g, and the vitamin D content (16.2 µg) was below the target of 20 µg for individuals older than 70 years. When these outliers were analysed in-depth, the menu offering was consistent with the eating habits of non-incarcerated individuals. Based on this nutritional analysis and interpretation of the results in light of the complex nature of prison food, this study concludes that CSC meets its obligation to provide a nutritionally adequate menu offering to the general population during incarceration.

Background: The eleven-item Drug Use Disorder Identification Test (DUDIT) is a recommended screening tool, but its length may impede its use in prison intake assessments. Hence, we examined the performance of eight brief DUDIT screeners against the full DUDIT, employing a sample of male inmates. Methods: Our study included male participants in the Norwegian Offender Mental Health and Addiction (NorMA) study who reported pre-prison drug use and who had been incarcerated three months or less (n = 251). We performed receiver operating characteristic curve (ROC) analyses and estimated the area under the curve (AUROC) to assess the performance of DUDIT-C (four drug consumption items) and five-item versions that consisted of DUDIT-C and one additional item. Results: Almost all (95%) screened positive on the full DUDIT (scores ≥6) and 35% had scores that were indicative of drug dependence (scores ≥25). The DUDIT-C performed very well in detecting likely dependence (AUROC=0.950), but some of the five-item versions performed significantly better. Of these, the DUDIT-C + item 5 (craving) had the highest AUROC (0.097). A cut-point of ≥9 on the DUDIT-C and ≥11 on the DUDIT-C + item 5 identified virtually all (98% and 97%, respectively) cases of likely dependence, with a specificity of 73% and 83%, respectively. At these cut-points, the occurrence of false positives was modest (15% and 10%, respectively) and only 4-5% were false negatives. Conclusions: The DUDIT-C was highly effective in detecting likely drug dependence (according to the full DUDIT), but some combinations of DUDIT-C and one additional item performed better.

The main challenge of interventions in penitentiary institutions is the re-education and reintegration of inmates, i.e., that inmates have the intention and ability to live law-abiding lives. Therefore, an increase in self-control or, on the contrary, the decrease or elimination of impulsive versus compulsive behaviors becomes necessary. This study aimed to evaluate the role of impulsivity versus compulsivity in treatment adherence in the prison population. The study included 134 men from the Penitentiary Center of Granada who were divided into two groups. Group 1 was considered treatment adherent, and Group 2 was considered non-adherent to treatment. The following instruments were used: Symptom Inventory (SCL-90-R), Addiction Severity Index (EuropASI), Impulsivity Scale (UPPS-P), and Compulsive Belief Questionnaire (OBQ-44). Statistically significant differences were found in impulsivity in the dimensions of negative urgency, sensation seeking, and positive urgency, with higher scores in all cases for the non-adherent group than for the adherent group. We also found statistically significant differences in responsibility/inhibition, perfectionism/uncertainty, and importance/control, with higher scores for the non-adherent group compared to the adherent group. Treatment adherence is inversely related to impulsive and compulsive behaviors.

BACKGROUND: The reduction of misconduct levels and its prevention are two important objectives in prison settings, for which several strategies and control mechanisms are implemented. OBJETIVE: We explore the relationship between a set of socio-demographic, judicial, affective, and personality characteristics and various expressions of aggression. METHOD: An Ecuadorian sample of 675 sentenced male prisoners was assessed using the Spanish adaptation of the Personality Assessment Inventory (PAI). Three subscales of the PAI were used as outcome measures: Aggressive Attitude (AGG-A), Verbal Aggression (AGG-V), and Physical Aggression (AGG-P). RESULTS: Using path analysis, three models were obtained, each of which had four indicators. Borderline personality features and antisocial personality features emerged as indicators that were common to the three models. Anxiety-related disorders and non-support were indicators specific to AGG-A, whilst schizophrenia and non-support were indicators of AGG-V, and drug problems and crimes against property were indicators of AGG-P. CONCLUSION: This study indicates that each expression of aggression has a different structure, which suggests its usefulness for detecting the tendency of an inmate to predominantly express one form of aggression or another.

BACKGROUND AND PURPOSE: The purpose of this study was to evaluate the psychometric properties of the Brief Symptom Inventory (BSI) hostility and anxiety subscales in prison inmates. METHODS: Reliability and construct validity of the BSI hostility and anxiety subscales were examined using Cronbach's alphas, factor analysis, and hypothesis testing. RESULTS: Of the 373 male inmates, 63% were White with a mean age of 36 years. Cronbach's alphas for hostility and anxiety were 0.83 and 0.81, supporting internal consistency. Factor analysis demonstrated two dimensions, hostility and anxiety. Supporting construct validity, inmates with higher hostility as well as those with higher anxiety had a greater number of mentally unhealthy days and lower perceived control. CONCLUSION: The BSI hostility and anxiety subscales are reliable and valid for use in prison inmates.

BACKGROUND: NHS Psychiatric beds comprise mental illness and intellectual disability beds. Penrose hypothesised that the number of psychiatric in-patients was inversely related to prison population size. AIMS: To ascertain whether the Penrose hypothesis held true in England between 1960 and 2018-2019. METHOD: A time-series analysis explored the association between total prison population and NHS psychiatric beds; this was also tested for the male and female prison populations, using non-psychiatric beds as a comparator. Associations were explored with time lags of up to 20 years. Linear regression was conducted to estimate the size of the effect of bed closures. RESULTS: NHS psychiatric beds decreased 93% and the prison population increased 208%. A strong (r =-0.96) and highly significant negative correlation between these changes was found. Annual reduction in psychiatric bed numbers was associated with an increase in prison population, strongest at a lag of 10 years. The closure of mental illness and intellectual disability beds was associated with increases in female prisoners 10 years later. The only significant explanatory variable for the increase in male prison population was intellectual disability bed reduction. CONCLUSIONS: The Penrose hypothesis held true between 1960 and 2018-2019 in England: psychiatric bed closures were associated with increases in prison population up to 10 years later. For every 100 psychiatric beds closed, there were 36 more prisoners 10 years later: 3 more female prisoners and 33 more male prisoners. Our results suggest that the dramatic increase in the female prison population may relate to the closure of NHS beds.

BACKGROUND: Information on the comorbidity of mental health problems in prison populations is scarce. The aim of the present study was to assess the prevalence of comorbidities at intake to prison between three diagnostic groups: severe mental illnesses (SMIs), personality disorders (PDs), and substance use disorders (SUDs). The co-occurrence of those disorders in prison populations may require the integration of differential treatment approaches and novel treatment trials. METHODS: A consecutive sample of N = 427 (229 male and 198 female) individuals committed to imprisonment in Santiago de Chile was assessed with the Mini Neuropsychiatric Interview and the Structured Clinical Interview for DSM-IV (module for borderline personality disorders) on arrival at prison. Diagnoses were a priori grouped as SMI including psychosis, bipolar disorder and major depression, PD including borderline and antisocial PD and SUD including alcohol and drug abuse or dependence. Sex stratified multivariate binary logistic regression analyses were conducted to assess sociodemographic, criminal and treatment characteristics of individuals with at least one diagnosis from each of the three diagnostic groups. RESULTS: The triad of SMI, PD, and SUD was present in n = 138 (32.3%; 95% IC: 28.0-36.9) study participants, n = 105 (45.9%; 95% CI: 39.4-52.4) of the men and n = 33 (16.7%; 95% CI: 12.1-22.6) of the women. Among those with the disorder triad, n = 129 (30.2%; 95% CI: 26.0-34.8) had major depression, PD and SUD; n = 54 (12.6%; 95% CI: 9.8-16.2) had psychosis, PD and SUD. The disorder triad was more common in men (OR = 4.86; 95% IC: 2.63-8.95), younger age (OR = 0.94; 95% CI: 0.91-0.97), and participants with lower educational levels (OR = 1.69; 95% CI: 1.01-2.82). The disorder triad was significantly associated with previous incarcerations (OR 2.60; 95% CI: 1.55-4.34) and histories of psychiatric hospitalizations (OR 2.82; 95% CI: 1.27-6.28). DISCUSSION: The complex triad of disorders from different diagnostic groups is common in prison populations, especially among young men. Successful treatment interventions may have the potential to break a cycle of repeat institutionalization in prisons and psychiatric institutions.

Introduction: Innovative and interdisciplinary approaches are needed to improve mental health and psychosocial outcomes of people with criminal justice involvement and their families. Aim of the study was to assess effects of the participation in a theatre project on the mental health problems of people with criminal justice involvement and relatives. Methods: We conducted structured diagnostic interviews and in-depth qualitative interviews with five participants performing Shakespeare's Richard III in Chile. Three participants had been imprisoned prior to the project, and two were the parents of a person who died in a prison fire. Qualitative interviews followed a topic guide. Data were transcribed, and a six-phase approach for thematic analysis of the data was used. Results: Substance use disorder or major depression was identified in all the participants. Participation in the theatre project was experienced by the respondents as having a positive effect on the mental health conditions. The research registered the positive experiences of role identification, emotional expression, commitment with group processes, improved skills to socially interact, to be heard by the general public and society, and positive perceptions of the audience (including relatives). Discussion: The study raises the possibility that there may be improvements of depression and substance use problems through the participation of people with criminal justice involvement in a drama project. Wider scale research is recommended on the possible effects. The approach may be an alternative to psychotherapy and medication for some individuals.

Background and Study Objective: To estimate the prevalence of chronic edema (CO) and wounds within two vulnerable populations, a male high security prison in the East Midlands (United Kingdom) and residential and nursing homes in the United Kingdom and Australia. Methods and Results: Methods for screening for CO and wounds were adapted from the main LIMPRINT methodology. Prison Population: In total, 195 inmates were recruited with 22 (11%) having CO. While the majority were white Caucasian (156/83.4%) a further 20 (10.7%) were dark skinned with 11 (5.95%) from other minority populations. Comorbidities included 123 (63%) smokers, 22 (11%) alcohol dependant, 60 (31%) with mental health problems, and 35 (18%) a history of self-harm. Only three had a current wound with 30 (16%) having had a traumatic stab wound. Residential and Nursing Homes (United Kingdom and Australia): In the United Kingdom, the total population available for inclusion was 189 with only 137 (73%) recruited. Seventy-two of the 137 (52%) suffered from CO and a further 16 (23%) had a history of cellulitis. Results from the Australian residential care facilities have been published in full. In summary, of the 37 participants 20 (54%) experienced CO with 25 (68%) having comorbidities and 11 (30%) having a concurrent wound. Conclusion: Obtaining an accurate picture of the prevalence and impact of CO in vulnerable populations is extremely challenging due to issues of access and consent. Lack of reliable data for these populations will contribute to poor service provision.

BACKGROUND: Around 25% of prisoners meet diagnostic criteria for attention-deficit/hyperactivity disorder (ADHD). Because ADHD is associated with increased recidivism and other functional and behavioural problems, appropriate diagnosis and treatment can be a critical intervention to improve outcomes. While ADHD is a treatable condition, best managed by a combination of medication and psychological treatments, among individuals in the criminal justice system ADHD remains both mis- and under-diagnosed and consequently inadequately treated. We aimed to identify barriers within the prison system that prevent appropriate intervention, and provide a practical approach to identify and treat incarcerated offenders with ADHD. METHODS: The United Kingdom ADHD Partnership hosted a consensus meeting to discuss practical interventions for youth (< 18 years) and adult (≥18 years) offenders with ADHD. Experts at the meeting addressed prisoners' needs for effective identification, treatment, and multiagency liaison, and considered the requirement of different approaches based on age or gender. RESULTS: The authors developed a consensus statement that offers practical advice to anyone working with prison populations. We identified specific barriers within the prison and criminal justice system such as the lack of adequate: staff and offender awareness of ADHD symptoms and treatments; trained mental health staff; use of appropriate screening and diagnostic tools; appropriate multimodal interventions; care management; supportive services; multiagency liaison; and preparation for prison release. Through discussion, a consensus was reached regarding prisoners' needs, effective identification, treatment and multiagency liaison and considered how this may differ for age and gender. CONCLUSIONS: This practical approach based upon expert consensus will inform effective identification and treatment of offenders with ADHD. Appropriate intervention is expected to have a positive impact on the offender and society and lead to increased productivity, decreased resource utilization, and most importantly reduced rates of re-offending. Research is still needed, however, to identify optimal clinical operating models and to monitor their implementation and measure their success. Furthermore, government support will likely be required to effect change in criminal justice and mental health service policies.

BACKGROUND: Attention-deficit hyperactivity disorder (ADHD) and head injury (including traumatic brain injury (TBI)) manifest in high levels across prison samples and guidance from the National Institute for Health and Care Excellence notes that people with acquired brain injury may have increased prevalence of ADHD. We aimed to examine the association of ADHD with TBI and the impact of the association upon health-related quality of life (HRQoL) and service use among imprisoned adults. METHODS: An observational study was performed in 2011-2013, at Porterfield Prison, Inverness, United Kingdom (UK). The all male sample included 390 adult prison inmates with capacity to consent and no history of moderate or severe intellectual disability. Head injury was measured with a series of self-reported questions, addressing history of hits to the head: frequency, severity, loss of consciousness (LOC), and sequelae. Participants were interviewed using the Diagnostic Interview for ADHD in Adults 2.0. The Health Utilities Index Mark 3 was used to measure health status, and to calculate attribute specific HRQoL and Quality-Adjusted Life Year (QALY) scores. RESULTS: 72% of prisoners sampled reported at least one head injury in their lifetime. Among those, 70% of head injuries occurred before age 16 and 70% experienced LOC. Prisoners with ADHD were nearly twice more likely to have TBI. Prisoners with ADHD-only and ADHD with co-morbid TBI had significantly lower scores in several HRQoL attributes, compared with TBI only or the absence of either condition. Adjusted logistic regression models indicated an average reduction of 0.20 QALYs in inmates with ADHD-only and 0.30 QALY loss in those with ADHD with co-morbid TBI compared with inmates with neither condition. CONCLUSIONS: There is a robust association between ADHD and TBI, and ADHD with co-morbid TBI confers significantly greater impairment in terms of HRQoL. Managing the short and long-term consequences of TBI is essential to improving care for prisoners and to addressing the criminogenic factors related to them.

Purpose The purpose of this paper is to investigate if any exposure to segregation minimal association in a single male prison population had any association with an increased risk of vitamin D deficiency. Design/methodology/approach A retrospective case study was undertaken with all inmates who had a 25-hyrdoxy-vitamin D test taken during the study period deemed eligible. Hand searching of the medical records by an independent party identified eligible participants whose data were recorded for analysis. Findings In total, 124 prisoners were deemed eligible for inclusion; 67 were vitamin D sufficient and 57 were vitamin D deficient by Australian standards. Time in segregation minimal association was shown not to be significant, however, smoking (OR 2.93, 95% CI 1.27-6.81, p=0.012) and having Asian ethnicity (OR 4.16, 95% CI 1.56-11.10, p=0.004) independently significantly increased the risk of vitamin D deficiency. Research limitations/implications This research is limited by its study design, small sample size and single location. Originality/value This paper presents the first published research into vitamin D levels in a prison population in Australia, and provides a basis for a larger prospective cohort study.

RESUMEN

The increasing ageing prison population is becoming a pressing issue throughout the criminal justice system. Alongside the rising population, are a host of health and wellbeing issues that contribute to older offenders needs whilst in prison. It has been recommended that meaningful activities can have positive effects on this population and therefore this paper uniquely reviews older offenders accounts of taking part in an arts based project, Good Vibrations, whilst imprisoned. The Good Vibrations project engages individuals in Gamelan music making with an end of project performance. This study used independent in-depth interviews to capture the voices of older offenders who took part in an art based prison project. The interview data was analysed using thematic analysis, which highlighted themes that were consistent with other populations who have taken part in a Good Vibrations project, along with specific age relating issues of mobility, motivation, identity and wellbeing.

Chile presenta un alarmante incremento de suicidios, tanto en població n general como intrapenitenciaria, lo que preocupa a las autoridades de justicia y salud. Objetivo: analizar los suicidios consumados por prisioneros en cárceles chilenas durante los años 2006-2015, para obtener el perfil de características sociocriminó genas del acto suicida y de los centros penitenciarios. Metodología: de un total de 162 suicidios, se analizaron los 132 casos examinados por la Policía de Investigaciones de Chile. Resultados: 97,7% de los suicidios ocurrieron en hombres de todas las edades (de 16 a 74 años); dos terceras partes (66,7%) sucedieron en personas con ingresos previos a prisió n, aunque la mayoría no tenían antecedentes penales ni condenas anteriores (97,7%). Una mayoría (65,1%) ocurrieron durante el primer año de ingreso. El 73,5% eran solteros, 47% solo tenían estudios básicos, 84,8% no poseían oficio estable. En 43,2% se reportó estado depresivo como desencadenante del suicidio, y el método más común fue ahorcamiento (97%). Se encontraron diferencias con estadísticas significativas de ciertas correlaciones, como entre nivel de estudios y motivo manifestado para el suicidio, y entre nivel de estudios e ingresos previos a prisió n; asimismo, una correlació n relevante que muestra que a menor edad, más ingresos a prisió n y menor nivel de estudios


Chile exhibits an alarming increase in the number of suicides among both the general population and the prison community, this being a matter of concern for justice and health authorities. Objective: the analysis of suicides committed by prisoners in Chilean jails during years 2006-2015 in order to obtain the profile of both the suicide acts and penitentiary socio-criminogenic features. Methodology: out of a total of 162 suicides, the 132 cases examined by the Criminal Investigation Police of Chile were analyzed. Results: 97.7% of suicides were committed by men of all ages from 16 through 74 years. Two thirds (66.7%) were individuals with previous admissions into prison; but most of them (97.7%) did neither have criminal backgrounds nor former convictions. A majority (65.1%) occurred during the first year in prison. 73.5% prisoners were single, only 47% had basic studies, and 84.8% had no specific jobs. A depressed mood was reported in 43.2% as a trigger factor of suicide, and hanging was the most common method used (97%). Differences were found with significant statistics in certain correlations like, for example, between the study level and the motive for suicide revealed, and between the study level and previous periods of incarceration; and likewise, there is a relevant correlation where the youngest individuals show the highest number of admissions into prison and the lower study level

Resultados: 97.7% dos suicidos aconteceram nos homens de todas as idades (de 16 a 74 anos); duas terceiras partes (66.7%) aconteceram em pessoas com renda prévia à prisão, embora a maioria não tivesse os registros criminalis nem as sentenças precedentes (97.7%). Uma maioria (65.1%) aconteceu durante o primeiro ano da entrada. 73.5% eram solteiros, 47% só tinham estudos básicos, 84.8% não possuíam um ofício estável. Em 43.2% o estado depressivo foi relatado como ativador do suicido, e o método o mais comum foi o enforcamento (97%). As diferenças com estatísticas significativas de determinadas correlações foram encontradas, como entre o nível de estudos e a razão para o suicido, e entre o nível de estudos e renda prévia à prisão; também, uma correlação relevante que amostra que à idade menor, mais renda à prisão e menor nível de estudos

The investigation of the relationship between the sizes of the mental health population and the prison population, outlined in Penrose's Law, has received renewed interest in recent decades. The problems that arise in the course of the deinstitutionalization have repeatedly drawn attention to this issue. This article presents methodological challenges to the examination of Penrose's Law and retrospectively reviews historical data from empirical studies. A critical element of surveys is the sampling method; longitudinal studies seem appropriate here. The relationship between the numbers of psychiatric beds and the size of the prison population is inverse in most cases. However, a serious failure is that almost all of the data were collected in countries historically belonging to a Christian or Jewish cultural community. Only very limited conclusions can be drawn from these sparse and non-comprehensive data: a reduction in the number of psychiatric beds seems to be accompanied by increases in the numbers of involuntary admissions and forensic treatments and an accumulation of mentally ill persons in prisons. A kind of transinstitutionalization is currently ongoing. A pragmatic balance between academic epidemiological numbers and cultural narratives should be found in order to confirm or refute the validity of Penrose's Law. Unless comprehensive research is undertaken, it is impossible to draw any real conclusion.

The aim of this study was to investigate the reliability, construct and criterion validity of the Symptom Check-List-90-R (SCL-90-R) for prison inmates. A sample of 427 adult prisoners was assessed at admission to the penal justice system in the metropolitan region of Santiago de Chile using the SCL-90-R and the mini international neuropsychiatric interview. We tested internal consistency using Cronbach's alpha. We examined construct validity using Principial Components Analysis and Confirmatory Factor Analysis (PCA and CFA) as well as Mokken Scale Analysis. Receiver Operating Characteristic (ROC) analysis was conducted to examine external criterion validity against diagnoses established using structured clinical interviews. The SCL-90-R showed good internal consistency for all subscales (α=0.76-0.89) and excellent consistency for the global scale (α=0.97). PCA yielded a 1-factor structure, which accounted for 70.7% of the total variance. CFA and MSA confirmed the unidimensional structure. ROC analysis indicated useful accuracy of the SCL-90-R to screen for severe mental disorders. Optimal cut-off on the Global Severity Index between severe mental disorders and not having any severe mental disorder was 1.42. In conclusion, the SCL-90-R is a reliable and valid instrument, which may be useful to screen for severe mental disorders at admission to the prison system.
