RESUMEN

Este trabajo tiene como objetivo relatar el abordaje individualizado, inclusivo y humanizado de un niño de 4 años y tres meses, con diagnóstico de neurofibromatosis tipo 1, atendido en la Carrera de Especialización en Odontopediatría de la Escuela de Graduados, Facultad de Odontología, Universidad de la Repú blica, Montevideo, Uruguay. En el examen clínico se pudo observar cicatrices derivadas del tratamiento quirúrgico del neurofibroma plexiforme en la zona cervical y la presencia de hipotonicidad muscular oral. Al examen clínico intraoral, se observaron amplias zonas desdentadas a consecuencia de exodoncias de quince dientes deciduos. Presentaba lesiones activas de caries en los dientes remanentes, acompañado de biopelícula generalizada y sangrado gingival provocado. Se realizó un abordaje integral del paciente y la terapia rehabilitadora mediante el uso de prótesis parcial removible superior e inferior. Concluímos que el tratamiento odontológico educativo, preventivo, restaurador y rehabilitador fue satisfactorio y la empatía, como proceso dinámico, involucró mecanismos cognitivos, afectivos y conductuales, y fue reconocida por sus efectos positivos en la promoción de la salud bucal del paciente.


Este trabalho tem como objetivo relatar uma abordagem odontológica individualizada, inclusiva e humanizada, enfatizando a importância do tratamento de uma criança de 4 anos e três meses, com diagnóstico de neurofibromatose tipo 1, que foi encaminhada e tratada en la clínica de la Carrera de Especialización en Odontopediatria de la Escuela de Graduados, Facultad de Odontología, Universidad de la República. O exame clínico revelou cicatrizes decorrentes do tratamento cirúrgico do neurofibroma plexiforme que acometia o pescoço da criança e hipotonicidade muscular oral. Ao exame clínico intraoral, quinze dentes decíduos estavam ausentes e cinco com lesões de cárie na primeira infância, além de presença generalizada de biofilme microbiano nas superfícies dentais e sangramento gengival provocado. Foi realizado tratamento odontológico necessário ao paciente, com os devidos cuidados e terapia reabilitadora com prótese parcial removível superior e inferior. Concluímos que o tratamento odontológico educacional, preventivo, restaurador e reabilitador foi satisfatório e a empatia, como processo dinâmico, envolveu mecanismos cognitivos, afetivos e comportamentais e foi reconhecida por seus efeitos positivos na promoção da saúde bucal do paciente.


This paper aims to report an individualized, inclusive, and humanized dental approach, emphasizing the importance of treating a child aged four years and three months with neurofibromatosis type 1. The patient was referred and treated at the Clinic of the Specialization Course in Pediatric Dentistry of the Graduate School, School of Dentistry, Universidad de la República, Montevideo, Uruguay. The clinical examination revealed scars from the surgical treatment of the plexiform neurofibroma and neck and oral muscle hypotonicity. The intraoral clinical examination showed large edentulous areas as 15 deciduous teeth were missing. The patient had active carious lesions, generalized microbial biofilm on the tooth surfaces, and gingival bleeding. A comprehensive treatment was provided, as well as restorative therapy with upper and lower removable partial dentures. We conclude that the dental educational, preventive, restorative, and rehabilitation treatment was satisfactory. Furthermore, empathy, as a dynamic process, involved cognitive, affective, and behavioral mechanisms. It also helped enhance the patient's oral health.

We examined alveolar bone samples in the area of on-bone fixation of a free gingival graft performed during surgery in patients aged 37-55 years with a diagnosis of secondary partial adentia of the upper and lower jaws. Six months after fixation of the graft in the alveolar bone, foci of neoosteogenesis were found in the contact zone. They were characterized by the appearance of appositional lines, cords of basophilic osteoblasts, and growing osteons. An immunohistochemical study revealed an increase in the number of CD44+, CD29+, and osteocalcin+ cells in the layer of the outer circumferential lamellae, primary osteons, and the lining of the Haversian canals. TGF-ß1+ cells were located in the intertrabecular reticular tissue and wall of microvessels. The results indicate activation of mesenchymal stem cells in the area of localization of the graft and differentiating osteoblasts. The observed osteoinductive effect of free gingival graft is associated with its participation in reorganization in MSC and induction of morphogenetic molecules.

Bone grafting is important to preserve the alveolar bone ridge height and volume for dental implant placement. Even though implant-supported overdentures present highly successful outcomes, it seems that a great number of edentulous individuals have not pursued implant-based rehabilitation. The cost of the treatment is one of the reasons of discrepancy between highly successful therapy and its acceptance. Therefore, the development of biomaterials for bone grafting with comparable characteristics and biological effects than those renowned internationally, is necessary. In addition, domestic manufacture would reduce the high costs in public health arising from the application of these biomaterials in the dental feld. The purpose of this clinical case report is to provide preliminary clinical evidence of the efficacy of a new bovine bone graft in the bone healing process when used for sinus floor elevation. (AU)


El uso de injertos óseos es importante para preservar la altura y el volumen de la cresta alveolar para la colocación de implantes dentales. Si bien las sobredentaduras implanto-soportadas presentan resultados altamente exitosos, la mayoría de las personas desdentadas no han sido rehabilitadas mediante implantes dentales. Uno de los principales motivos por los cuales los pacientes no aceptan este tipo de tratamiento, altamente exitoso, es el elevado costo del mismo. Por ello, es necesario el desarrollo de biomateriales de injerto óseo con características y efectos biológicos comparables a los reconocidos internacionalmente. Asimismo, la fabricación nacional reduciría los altos costos en Salud Pública derivados de la aplicación de estos biomateriales en el campo dental. El objetivo de esta comunicación es presentar un caso clínico a fin de proporcionar evidencia preliminar acerca de la eficacia de un nuevo injerto de hueso bovino en el proceso de cicatrización ósea en el levantamiento del piso del seno maxilar. Down syndrome, known as trisomy 21, is the most common chromosomal disorder. The disorder affects mental and systemic development as well as oral structure, including dental anomalies, high susceptibility of periodontal disease, and poor quality of alveolar bone. This report presents a case of dental rehabilitation by means of dental implants of a patient with Down syndrome. Two titanium dental implants were placed in the maxilla, and three titanium dental implants were installed in the mandible. One implant was lost during the osseointegration period. The prosthetic rehabilitation was performed with implant-retained maxillary and mandibular overdentures with the Locator attachment system. After a 2-year follow-up period, the patient was doing well, and all implants were clinically stable with no signs of bone loss or inflammation. The present study emphasizes that implant-retained overdentures with Locator attachment system could be a therapeutic option even for patients with Down syndrome. This therapy prevents crestal bone loss around the implants, improves functional and esthetic outcomes, and provides optimum oral hygiene for patients with mild mental impairment. Careful patient selection and education of patients and caregivers are essential considerations for a successful and safe treatment with dental implants in Down syndrome patients.

This clinical report describes a digital workflow using an intraoral digital impression and cone beam computed tomography (CBCT) volumetric data to create a three-dimensional (3D) virtual model of the dentition, defect area, and soft and hard tissue of a patient who had undergone a hemimaxillectomy. Stereolithography (SLA) 3D printing technology was used to produce a resin mold, from which an obturator prosthesis was fabricated through a conventional technique.

Objetivo: o tratamento de pacientes desdentados com reabsorção alveolar severa é uma temática desafiadora na reabilitação protética. Identificada por Kelly em 1972, a síndrome da combinação é uma manifestação intraoral encontrada em pacientes totalmente desdentados na arcada superior, em oposição a uma prótese mandibular de extensão distal bilateral. Relato de caso: este estudo revisa e discute, por meio de um caso clínico, as características clínicas presentes em um paciente com manifestações da síndrome da combinação. Além disso, apresenta uma proposta de tratamento reabilita¬dor, restabelecendo a função fonética e a estética do paciente. Considerações finais: diagnosticar a síndrome e estabelecer um plano de tratamento adequado às necessidades do paciente, criando condições clínicas para o restabelecimento da saúde, são imprescindíveis para obter resultado e, assim, impedir o avanço do quadro clínico. Objective: treatment of patients with reactivity with respiratory problems. Identified by Kelly in 1972, a combination syndrome and an intraoral manifestation found in fully edentulous patients in the upper arch as opposed to a mandibular prosthesis of bilateral distal extension. Case report: the present study reviews and discusses, through a clinical case, as clinical features present in a patient with manifestations of the combination syndrome, in addition, to present a proposal of rehabilitative treatment, restoring function, phonetics and esthetics to the patient. Final considerations: diagnosing a syndrome and establishing a treatment plan appropriate to the needs of the patient and creating clinical conditions for the reestablishment of health are essential to obtain the result and thus impeding the advancement of the clinical picture. RESUMEN

The role of hard- and soft-tissue augmentation as it pertains to dental implant therapy is often underestimated. If this restorative aspect is neglected during the natural healing process and subsequent remodeling following extraction(s), the long-term outcome of treatment can be catastrophic. Three of the most common opportunities for surgeons to enhance outcomes with regenerative therapy are: reconstruction of edentulous segments of the alveolus, management of extraction sockets, and immediate implant placements. Each has its own unique nuances, which can be quite confounding without an appreciation for the potential biomaterials and techniques currently available for use in these situations. This article will focus on novel methods aimed toward the reconstruction and maintenance of hard- and soft-tissue volumes needed to support functional, hygienic, and esthetic implant restorations.

Bone and soft tissue deformities in long-standing partial edentulism are commonly treated with implant therapy. The various ceramic restorative systems available and digital technology allow for customization of design and material selection. A substructure design that mimics tooth preparation in a custom zirconia framework with a lithium disilicate suprastructure is presented as an effort to address the functional and esthetic challenges posed by long-standing posterior partial edentulism. The aim is to present several contemporary elements of design to address the functional, esthetic, and maintenance needs.

AIM: To assess the knowledge and perceptions of Malaysian government dentists regarding the shortened dental arch (SDA) concept and its application in clinical practice. BACKGROUND: The SDA concept refers to a specific type of dentition with intact anterior teeth and a reduction in posterior occlusal pairs. Dentists' knowledge and perceptions of the SDA concept can influence its application in clinical practice. METHODS: A self-administered questionnaire on the SDA concept was distributed to 326 government dentists in the states of Selangor and Kuala Lumpur, Malaysia. The data were analysed using SPSS version 22 software. RESULTS: The response rate was 84.0%. The majority of respondents had good knowledge on five of six knowledge items and good attitudes towards 10 of 17 perception items. However, only one-fifth (20.4%) reported having applied the SDA concept in the clinic. A larger number of participants who graduated locally than who graduated abroad perceived that patients <60 years of age, without molar support, can attain acceptable chewing function and that SDA treatment does not lead to loss of occlusal vertical dimension (P < 0.05). A larger number of participants with ≤5 years of work experience than with >5 years of work experience perceived that the SDA concept enables simpler treatment planning (P < 0.05). Finally, a larger number of participants who graduated abroad than who graduated locally observed that patients without molar support had temporomandibular joint problems (P < 0.05). CONCLUSION: Although Malaysian government dentists have good knowledge and perceptions of the SDA concept, it is not widely applied in the clinic. Concerted efforts in SDA training of dentists are needed to help to shorten denture waiting lists and reduce costs.

RESUMEN: El reborde móvil maxilar producido por reabsorción ósea extrema compromete la estabilidad de la prótesis total, por lo que debe ser registrado en reposo. Se describe una nueva técnica de impresión mucoestática simple y rápida que, junto al esquema oclusal y ordenamiento dentario planificados, optimiza la retención protésica. Se discuten sus resultados y se comparan con otras técnicas de impresión.


ABSTRACT: The movable fibrous ridge caused by extreme maxillary bone resorption compromises the stability of complete dentures, so it must be registered passively. A new, simple and quick mucostatic impression technique along with the occlusal scheme and dental planning system optimizes prosthetic retention. Results are discussed and compared with various impression techniques.

PURPOSE: Restoration of partial edentulous mouth may be done using a variety of treatment options. Removable partial denture (RPD) is commonly used because of its availability. RPDs from flexible resins unlike those from acrylic engage hard and soft tissue undercuts and feel more comfortable in the mouth. The aim of the study was to compare satisfaction with dentures made from these two types of materials. MATERIALS AND METHODS: It was a quasi-experimental study among thirty patients at the Prosthetics Clinic, University College Hospital, Ibadan. Patients aged 16 years or more, requiring RPDs with one to three missing teeth in the anterior region of either the upper or lower arch participated. A modified semi-structured interviewer-administered questionnaire was used to collect data on sociodemographics and oral health variables. The level of satisfaction was assessed using a visual analogue scale. Data were analysed using descriptive and multivariate statistics at a significance level of P < 0.05. RESULTS: The participants' ages ranged between 16 and 51 years, mean age was 33.8 ± 10.01 years. Male: female ratio was 1:1 and mean duration of edentulousness was 11.37 ± 10.52 years (median - 9.50). Most 28 (93.3%) subjects delayed replacement of their missing teeth; reasons were indifference 13 (43.4%), financial constraint 10 (33.3%), ignorance 4 (13.3%) and fear of aspiration 1 (3.3%). Overall, 21 (70.0%) participants were more satisfied with the flexible dentures, 6 (20.0%) with acrylic dentures while 3 (10.0%) were equally satisfied with both types of dentures (P = 0.04). CONCLUSION: Subjects were more satisfied with the flexible RPD than the acrylic resin RPD.

BACKGROUND: End-stage renal disease (ESRD) patients are considered as a group of high risk of oral cavity diseases. One of the determinants of alveolar bone loss and increased teeth mobility in ESRD patients might be the bone abnormalities associated with chronic kidney disease-mineral and bone disorder (CKD-MBD). OBJECTIVES: The aim of the study was to compare the general health condition, number and location of teeth in a group of ESRD patients with the group of peers from general population and revealing the risk factors of tooth loss. MATERIAL AND METHODS: The ESRD group included 63 patients, 23 females and 40 males, undergoing dialysis with a mean age of 62.4 ± 15.6. The general population sample consisted of 37 people, 20 females and 17 males, applying for general practitioner visit, with a mean age of 65.5 ± 11.1. All the participants were using just public health care insurance. The data analysis was based on anamnesis, history of CKD, selected biochemical parameters of blood tests and clinical examination. RESULTS: There was no statistical difference in the prosthetic needs of patients undergoing dialysis and the general population. In both groups the situation is alarming. CONCLUSIONS: The new procedures are needed to develop complex health care for ESRD and general population patients, emphasizing prophylaxis of tooth-loss and prosthetic treatment in order to maintain good level of life quality.

This systematic review aimed to compare oral health-related quality of life (OHRQoL) between two tooth replacement strategies - the shortened dental arch (SDA) concept and conventional treatment with removable partial dental prosthesis (RPDP) or implant-supported fixed partial dental prosthesis (IFPDP) - for distal extension of edentulous space in the posterior area. We retrieved eligible randomised controlled trials (RCTs) and non-RCTs published between 1980 and November 2016 retrieved from MEDLINE and the Cochrane Central Register of Controlled Trials. The primary outcome was OHRQoL evaluated using validated questionnaires. Two reviewers independently screened and selected the articles, evaluated the risk of bias and determined the standardised weighted mean difference (SWMD) in OHRQoL scores between the two strategies using a random effects model. Two RCTs and one non-RCT involving 516 participants were included in this review. All studies employed the oral health impact profile (OHIP) for evaluation of OHRQoL. There was no statistically significant difference in OHIP summary scores between SDA and RPDP at 6 (SWMD = 0·24) or 12 (SWMD = 0·40) months post-treatment. Only one non-RCT had reported higher OHRQoL with IFPDP than with SDA; however, because of the small sample size, there was no significant difference in OHIP summary scores between the two strategies at 6 (SWMD = -0·59) or 12 (SWMD = -0·67) months post-treatment. In terms of OHRQoL in partially dentate patients, the SDA concept appears to be as feasible as RPDP restoration. Further clinical trials are required to clarify the effect of IFPDP restoration on OHRQoL.

OBJECTIVES: The aim was to analyze the prevalence rate of proximal contact loss (PCL) between implant-fixed prostheses (IFPs) and adjacent teeth and investigate the associated factors. MATERIAL AND METHODS: One hundred fifty participants were recruited for this prospective study from January 2009 to December 2014. Two hundred thirty-four IFPs supported by 384 implants for the posterior region were followed up until June 2016. The contact tightness had been recorded using aluminum strips of different thicknesses with a regular interval after delivery. Proximal contact was considered as lost if the contact tightness was over 50 µm, and statistical analyses were performed to estimate the prevalence rate of PCL and its influential factors. RESULTS: Among the total 299 proximal contacts of 234 IFPs, 179 were observed as a PCL (59.9%). Bone level and root configuration of the adjacent teeth, the proximal contact position and jaw position of the implant prostheses were statistically significant factors, when analyzed by the cumulative PCL rate using the log-rank test of the Kaplan-Meier method (P < 0.05) According to the Cox proportional hazard regression analysis, the proximal contact position, bone level of adjacent teeth and jaw position were revealed to be statistically significant (P < 0.05). CONCLUSION: PCL should be considered an implant prosthesis complication to which various associated factors could be related. This study revealed that the lower alveolar bone support level of the adjacent teeth, maxillary position of IFPs and mesial site of IFPs were significantly associated with a higher incidence of PCL.

STATEMENT OF PROBLEM: The number of partially dentate adults is increasing, and many patients will require replacement of missing teeth. Although current treatment options also include fixed partial dentures and implants, removable partial dentures (RPDs) can have advantages and are widely used in clinical practice. However, a significant need exists to advance materials and fabrication technologies because of the unwanted health consequences associated with current RPDs. PURPOSE: The purpose of this review was to assess the current state of and future need for prosthetics such as RPDs for patients with partial edentulism, highlight areas of weakness, and outline possible solutions to issues that affect patient satisfaction and the use of RPDs. MATERIAL AND METHODS: The data on treatment for partial edentulism were reviewed and summarized with a focus on currently available and future RPD designs, materials, means of production, and impact on oral health. Data on patient satisfaction and compliance with RPD treatment were also reviewed to assess patient-centered care. RESULTS: Design, materials, ease of repair, patient education, and follow-up for RPD treatment all had a significant impact on treatment success. Almost 40% of patients no longer use their RPD within 5 years because of factors such as sociodemographics, pain, and esthetics. Research on RPD-based treatment for partial edentulism for both disease-oriented and patient-centered outcomes is lacking. CONCLUSIONS: Future trials should evaluate new RPD materials and design technologies and include both long-term follow-up and health-related and patient-reported outcomes. Advances in materials and digital design/production along with patient education promise to further the application of RPDs and improve the quality of life for patients requiring RPDs.

RESUMEN

At the heart of prosthodontic treatment planning must be a sound understanding of the quality and reliability of the prosthodontic foundation. Implant prosthodontics relies almost entirely on the biological and mechanical properties of an osseous foundation. Initiation of therapy in the absence of a healthy and stable prosthodontic foundation risks unintended and suboptimal treatment outcomes. For conventional removable partial denture (RPD) therapy, consideration must be given to interactions between the soft and hard tissues available to support and stabilize planned prostheses. The strategic addition of implants to the partially edentulous foundation can improve the support, comfort, and esthetics of an RPD, resulting in elevated patient satisfaction and greater therapeutic success. This article discusses aspects of diagnosis, treatment planning, clinical management, laboratory execution, and maintenance that must be considered in order to obtain optimal results with implant-assisted RPDs.
