RÉSUMÉ

In 45 patients with chronic digestive tract diseases (gastric and/or duodenal peptic ulcer, cholecystolithiasis, conditions after gastrectomy and cholecystectomy) the biochemical parameters serving as indicators of the nutritional status of the organism (Hb, Bc, Fe, Ca, alkaline phosphatase) were determined. In the group with postgastrectomy syndrome the serum calcium level was decreased. The other parameters were normal in all groups of patients.

RÉSUMÉ

Forty-two patients with postgastroresection peptic ulcers of the zone of gastrojejunal anastomoses were examined. The cause of the appearance of the ulcers was the action of aggressive content of the gastric stump on the jejunum in the area of anastomosis. "Vagal" peptic ulcers were found by the gastrocepin test. After operations reducing the stump acidity healing of all the ulcers was noted. In achlorhydria of the stump the digestion in it is realized due to reflux at the expense of pancreas proteinases.

For the purpose of studying digestion and absorption disorders following stomach surgery, particularly with respect to membrane digestive functional disorders, morphological and enzyme-histochemical observation of the small intestinal mucosa was performed, and a comparison was done between the partial gastrectomy group (59 cases), and the total gastrectomy group (52 cases). The following results were obtained: 1) The small intestinal mucosa showed a tendency to significant villous atrophy in post-operative groups. 2) There was a significant decrease in the disaccharidase activity, especially in the small intestine of the total gastrectomy group. ALP, LAP and gamma-GPT activity also decreased, or disappeared in the small intestines showing villous atrophy. 3) In the atrophied mucosa which showed a marked decrease in membraneous enzyme activity, poorly-formed, irregularly sized microvilli accompanied by glycocalyx irregularity and disappearance were recognized under the electron microscope. The above mentioned morphological and enzyme-histological findings in the small intestinal mucosa, were strongly reflected in both post-operative progress and clinical symptoms.

RÉSUMÉ

To study post-gastrectomy metabolic bone disorders, we measured the radial bone mineral content (BMC), serum levels of calcium, inorganic phosphorus, alkaline phosphatase, and 25-hydroxyvitamin D(25-OH-D) in 59 patients with partial- and 39 patients with total gastrectomy. Total gastrectomy patients manifested a higher incidence of decreased BMC levels than did partial gastrectomy patients (56 per cent vs. 25 per cent). Patients subjected to the Billroth II procedure, especially females, manifested abnormally low BMC values. The decline in BMC was age-related; it was pronounced in females. At 10 years postoperatively, many of the partial gastrectomy patients manifested markedly low BMC levels; in totally gastrectomized patients this finding was made at less than 5 years postoperatively. Approximately 30 per cent of our patients showed abnormalities in serum minerals, alkaline phosphatase or 25-OH-D.

RÉSUMÉ

Activity of the enzymes of local and overall biological actions was examined in a group of patients who had suffered gastric resection for peptic ulcer. The studies revealed enzymopathies of 2 types. The type I enzymopathy arises from deficiency of the enzymes implicated in digestion of food, while the type II enzymopathy is determined by abnormality of the mediator-enzymatic balance in the neurohumoral system. The enzymopathy pattern should be taken into consideration while deciding on a pathogenetically validated treatment.

RÉSUMÉ

No microbial growth in platings of the gastric juice of patients with gastric ulcer and chronic anacidic gastritis was observed. It means that the absence of hydrochloric acid in the gastric juice does not deprive it of any antimicrobial action. The possible role of lysozyme in providing sterility of the proximal part of the gastro-intestinal tract was studied. Eighty patients with chronic diseases of the digestive organs were observed. It was noted that the levels of lysozyme in the gastric juice was high and markedly exceeded the maximum concentrations required for lysis of organisms most resistant to it. The maximum concentration was determined at pH of the gastric juice equal to 7.0-7.5 (265 gamma/ml+/-28). No lysozyme in the content of the duodenum and jejunal juice was found in most cases. Its presence in the above parts of the gastro-intestinal tract was mainly associated with microbial growth. The maximum concentration of lysozyme (40 gamma/ml) in the jejunal juice was observed in a female patient with chronic enterocolitis and significant microbial proliferation in the thin colon (more than 10(4) microbial bodies per 1 ml of the juice). Such parallelism between the presence of lysozyme in the gastric juice and microbial proliferation in it may be considered as a protective-adoptive reaction of the host.
