Introducción: La intususcepción es una patología abdominal idiopá-tica o secundaria a procesos intesti-nales que actúan como puntos de partida para la invaginación. Se han descrito casos de arrastre de estruc-turas que derivan en otros procesos inflamatorios como la apendicitis aguda. Caso clínico: Niño 3 años, con dolor abdominal de 6 horas de evolución. Al examen físico se pre-senta pálido, somnoliento, taqui-cárdico y deshidratado. El abdo-men con signos apendiculares posi-tivos, con palpación en masa en fosa iliaca derecha. Taller diagnóstico: Leucocitos 9690 u/mm3, neutrófilos 58.1%. Ecografía con imagen sugerente de intususcepción intestinal con cam-bios inflamatorios en la grasa me-sentérica. Se realiza tomografía abdominal que reporta intususcep-ción ileocolónica de 47 x 50 mm, con múltiples ganglios reactivos mesentéricos, con imagen apendicu-lar en dirección pélvica, con apendi-colito en su interior. Evolución: El manejo quirúrgico incluyó una laparotomía explorato-ria con desinvaginación manual y apendicectomía convencional. El reporte de patología fue apendicitis aguda supurativa. El paciente 48 horas hospitalizado, recibió Ampici-lina + Sulbactam y analgesia. Al mejorar la función abdominal fue dado de alta. Conclusiones: En este caso la apendicitis aguda fue la causa de intususección intestinal con el signo ecográfico de la "diana" en un paciente de 3 años de edad.


Introduction: Intussusception is an idiopathic abdominal pathology or secondary to intestinal processes that act as starting points for intussusception. Cases of dragging of structures that lead to other inflammatory processes, such as acute appendicitis, have been described. Clinical case: 3-year-old boy with abdominal pain of 6 hours of evolution. On physical examination, he appears pale, drowsy, tachycardic, and dehydrated. The abdomen with positive appendiceal signs, with palpation of a mass in the right iliac fossa. Diagnostic workshop: leukocytes 9690 u/mm3, neutrophils 58.1%. Ultrasound with image suggestive of intestinal intussusception with inflammatory changes in the mesenteric fat. An abdominal tomography was performed that reported ileocolonic intussusception of 47 x 50 mm, with multiple mesenteric reactive nodes, an appendicular image in the pelvic direction, and an appendicolith inside. Evolution: Surgical management included an exploratory laparotomy with manual evagination and conventional appendectomy. The pathology report was acute suppurative appendicitis. The patient was hospitalized for 48 hours and received Ampicillin + Sulbac-tam and analgesia. When abdominal function improved, he was discharged. Conclusions: In this case, acute appendicitis was the cause of intestinal intussusception with the ultrasound sign of the "target" in a 3-year-old patient.

Magnetic wood is a composite material that achieves harmony between both woody and magnetic functions through the active addition of magnetic characteristics to the wood itself. In addition to showing magnetic characteristics, magnetic wood offers low specific gravity, humidity control and acoustic absorption ability. It has potential for broad applications in the fields of electromagnetic wave absorption, electromagnetic interference shielding, furniture, etc. This work reports on the synthesis of Fe3O4 nanoparticles (NPs) in wood from three tropical species (Pinus oocarpa, Vochysia ferruginea and Vochysia guatemalensis) using a solution of iron (III) hexahydrate and iron (II) chloride tetrahydrate with a molar ratio of 1.6:1 at a concentration of 1.2 mol/L ferric chlorate under 700 kPa pressure for 2 h. Afterward, the wood samples were impregnated with an ammonia solution with three different immersion times. The treated wood (wood composites) was evaluated for the weight gain percentage (WPG), density, ash content and Fe3O4 content by the Fourier transform infrared spectroscopy (FTIR) spectrum, X-ray diffraction (XRD) and vibrating sample magnetometry (VSM). The results show that the species P. oocarpa had the lowest values of WPG, and its density decreased in relation to the untreated wood, with lower ash and Fe3O4 NP content. The XRD and some FTIR signals associated with changes in the wood component showed small differences from the untreated wood. Fe3O4 NPs presented nanoparticles with the smallest diameter of (approx. 7.3 to 8.5 nm), and its saturation magnetization (Ms) parameters were the lowest. On the other hand, V. guatemalensis was the species with the best Ms values, but the wood composite had the lowest density. In relation to the different immersion times, the magnetic properties were not statistically affected. Finally, the magnetization values of the studied species were lower than those of the pure Fe3O4 nanoparticles, since the species only have a certain amount of these nanoparticles (NPs), and this was reflected proportionally in the magnetization of saturation.

RESUMEN La presente investigación tiene como objetivo analizar, a través de sus narrativas, las prácticas educativas familiares cotidianas de un grupo de madres adolescentes afrocolombianas en condición de vulnerabilidad del Distrito de Buenaventura. Este estudio se realizó con un enfoque metodológico cualitativo descriptivo, adhiriéndose al paradigma hermenéutico interpretativo. Participaron siete adolescentes que fueron madres entre los 13 y los 17 años. La selección de la muestra se realizó de manera no probabilística, por conveniencia; asimismo, la recolección de los datos se llevó a cabo a través de entrevistas semiestructuradas. Entre los hallazgos más significativos de la investigación se encontró que la práctica autoritaria es la más utilizada por estas madres para educar a sus hijos. Sin embargo, no se regían por una única práctica, pues oscilaban entre las democráticas y las permisivas. Para las madres adolescentes del estudio, las ayudas iniciales de las redes de apoyo familiar fueron de suma importancia, ya que las percibían como fundamentales para su desempeño como madres. En consecuencia, se invita a crear nuevas estrategias de implementación de prácticas educativas familiares acordes a la cultura y al contexto de las madres adolescentes, lo que facilita el ejercicio de su maternidad en consonancia con sus realidades.


ABSTRACT The purpose of this research is to analyze the methods a group of African-Colombian teenage mothers use to raise and educate their children. The sample group lives in the District of Buenaventura-Colombia, and is considered to live under vulnerable conditions. The methodological approach was qualitative-descriptive, following the hermeneutical-interpretative paradigm. The sample was non-probabilistic, by choice. Seven teens, which became pregnant between the ages of 13 and 17, were interview for this purpose. The most significant finding suggests that the practice used the most by these mothers is the authoritarian type. Although, they alternated it with the democratic and permissive types too. It was found that for these moms it was extremely important the assistance and advice they received from family support teams in order to fulfil their role as new parents. In consequence, it is advised to create new strategies and support groups that will facilitate the child raising practices of teenage families in this region, according to their cultural background and context.

INTRODUCTION: The presence of H. pylori in the stomach is associated with gastric pathologies. However, its diagnosis through culture methods is challenging because of its complex nutritional requirements and microaerophilic conditions for optimal growth. The preferred method for rapid diagnosis of H. pylori is the Rapid Urease Test (RUT) from human biopsies, which relies on the high activity of the urease enzyme present in H. pylori. However, RUT cannot say much more information about H. pylori. This makes evident the need for bacterial culture to know essential information such as the strain type, the kind of infection present and the bacteria's antibiotic susceptibility. METHODOLOGY: Gastric biopsies from 347 patients were used for H. pylori isolation. We correlated the culture results with the RUT and histological grading used at Hospital Universitario Fundación SantaFe de Bogotá (HU-FSFB), Colombia. The concordance between techniques was determined by the Cohen's Kappa coefficient (K). The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were also calculated. RESULTS: The culture standardization was successful, and it could be applied for diagnosis in the clinical practice. H. pylori was positive by culture in 88 (26.34%) patients. The concordance of RUT and culture was strong (K= 0.805), and between histology and culture was moderate (K= 0.763) as well as for the gold standard defined and culture (K= 0.80). CONCLUSIONS: We present evidence that RUT and histological methods will be better interpreted for diagnosis of H. pylori if combined with bacterial isolation in cholesterol enriched culture.

The aim of the study was to determine the current prevalence of Helicobacter pylori in symptomatic Colombian children and evaluate the presence of mutations associated with clarithromycin resistance. Biopsies from 133 children were analyzed. The gastric fragment was used for urease test and reused for polymerase chain reaction sequencing of the 23SrDNA gene. Mutations were detected by bioinformatic analysis. Polymerase chain reaction sequencing established that H pylori infection was present in 47% of patients. Bioinformatics analysis of the 62 positive sequences for 23SrDNA revealed that 92% exhibited a genotype susceptible to clarithromycin, whereas the remaining strains (8%) showed mutations associated with clarithromycin resistance. The low rate of resistance to clarithromycin (8%) suggests that conventional treatment methods are an appropriate choice for children. Recycling a biopsy that is normally discarded reduces the risks associated with the procedure. The 23SrDNA gene amplification could be used for a dual purpose: detection of H pylori and determination of susceptibility to clarithromycin.

Fibrolamellar carcinoma (FLC) is an uncommon form of primary liver malignancy with unique clinical, histological, and biological characteristics. It is usually seen in young adults without underlying liver disease. Histologically, it shows large cells with abundant eosinophilic cytoplasm, large vesicular nuclei, prominent nucleoli, and lamellar type fibrosis. In contrast, classical hepatocellular carcinoma (HCC) is typically present in elderly male patients with cirrhosis. It is the most common histological subtype, and it is characterized by its resemblance to the normal liver, both in its growth pattern and its cytology. The unusual case of a liver carcinoma that presented with histological features of both FLC and classical HCC is herein reported. This was the case of a 37-year-old female complaining of diffuse abdominal discomfort and epigastric pain for two months. She was referred to us for further management after she was diagnosed with HCC in a noncirrhotic liver. She underwent a left-sided hepatectomy. A yellow nodular mass with well-defined borders and a necrotic center was present in the resection specimen. The morphological features and immunohistochemical studies were consistent with a diagnosis of FLC mixed with classical HCC. The patient was followed up for five months, and no signs of recurrence were evident.

Liver transplantation is the only available treatment for some patients with end-stage liver disease. Despite reduction in mortality rates due to advances related to surgical techniques, intensive medical management and immunosuppressive therapy, invasive fungal infections remain a serious complication in orthotopic liver transplantation. We report the case of an 18-year-old male diagnosed with autoimmune cirrhosis in 2009 who was assessed and listed for liver transplantation for massive variceal hemorrhage. One year after listing a successful orthotopic liver transplantation was performed. Uneventful early recovery was achieved; however, he developed pulmonary and neurological Aspergillus infection 23 and 40 days after surgery, respectively. Antibiotic therapy with voriconazole and amphotericin was started early, with no major response. Neuroimaging revealed multiple right frontal and right parietal lesions with perilesional edema; surgical management of the brain abscesses was performed. A biopsy with periodic acid-Schiff and Gomori stains revealed areas with mycotic microorganisms morphologically consistent with Aspergillus, later confirmed by culture. The patient developed necrotizing encephalitis secondary to aspergillosis and died. Necrotizing encephalitis as a clinical presentation of Aspergillus infection in an orthotopic liver transplant is not common, and even with adequate management, early diagnosis and prompt antifungal treatment, mortality rates remain high.

Primary laryngeal tuberculosis is a rare disease with a wide variety of clinical manifestations. We report the case of an ear-nose and throat diabetic physician with primary laryngeal tuberculosis. The diagnosis of laryngeal cancer had been considered before in another institution, and at the patient's admission, he only presented dysphonia with a six-month evolution. The vocal cord histological examination showed granulomas, giant cells, fibrosis and necrosis, and the Ziehl-Neelsen staining showed acid-alcohol resistant bacilli. The culture from vocal cord tissue was positive for mycobacteria, and IS 6110 -PCR was positive too. The Mycobacterium tuberculosis strain was sensitive to first line drugs. Treatment using directly observed therapy short-course (DOTS) was initiated. Clinical presentation, differential diagnosis, diagnostic workup, associated risk factors and a brief literature revision are discussed in this article.

La tuberculosis laríngea primaria es una enfermedad rara con una amplia gama de manifestaciones clínicas. Se reporta el caso de un médico otorrinolaringólogo con diabetes de tipo II y con diagnóstico de tuberculosis laríngea primaria. El diagnóstico de cáncer de cuerda vocal se había considerado en otra institución, y al ingreso el paciente sólo presentaba disfonía de seis meses de evolución. En el estudio histopatológico se encontraron granulomas, células gigantes multinucleadas, fibrosis y necrosis, y en la tinción de Ziehl-Neelsen, bacilos ácido-alcohol resistentes. El cultivo del tejido de cuerda vocal para micobacterias y la PCR-IS6110 fueron positivos. La cepa de Mycobacterium tuberculosis demostró sensibilidad a todos los medicamentos de primera línea. Se inició tratamiento según la estrategia DOTS ( Directly Observed Treatment, Short -course ). Se discute la presentación clínica, los diagnósticos diferenciales, el abordaje diagnóstico y los factores de riesgo asociados y se hace una breve revisión de la literatura.
