RESUMO

Background: Older adult patients with post-COVID-19 syndrome present greater physical impairment accompanied by frailty than younger patients, which is why multicomponent exercise programs (MEP) are recommended for their positive effects on improving frailty and physical capacity. The aim of this study was to evaluate the effects of a short MEP (Vivifrail; <4 weeks) on improving frailty in post-COVID-19 older adults after intensive care units. Methods: To develop a retrospective cohort study, data were collected from the functional gait training program based on selected Vivifrail MEP in a single-group and applied to patients admitted with a diagnosis of post-COVID-19 functional impairment. The MEP was carried out for 3 weeks, with daily sessions lasting 40 min. Patients included were assessed at the beginning and at the end of the protocol by using the Short Performance Physical Battery (SPPB), the number of falls in the last year, the number of falls with medical attention, the Timed Up and Go (TUG) test, the presence of dementia, the Trunk Control Test (TCT), the Tinetti balance and gait test, Barthel Index, Medical Research Council Sum Score (MRCSS) and handgrip strength dynamometry. Results: The results of this study show statistically significant improvements in physical fitness and frailty with increases in the Short Physical Performance Battery (Z = 9.12, p < 0.001) by means of the MET applied in its short version (<4 weeks) showing even clinically relevant improvements (>2.5 points). Statistically significant improvements were also found in Medical Research Council Sum Score (Z = 12.345, p < 0.001), Barthel Index Score (Z = 12.272, p < 0.001), Trunk Control Test (Z = 12. 36, p < 0.001), Tinetti−POMA (Z = 12.293, p < 0.001) including the balance (Z = 12.11, p < 0.001), gait (Z = 12.164, p < 0.001) subscales and in the hand dynamometry (Z = 12.172, p < 0.001). Conclusions: The selected Vivifrail MEP is effective and safe for improving frailty in post-COVID-19 older adult's patients.

BACKGROUND: : Few studies have investigated the psychometric properties of the Scale for Contraversive Pushing (SCP) in depth, and none have evaluated its ability to establish differential diagnosis between pusher behavior (PB) and thalamic astasia (TA). OBJECTIVES: : To study the ability of the SCP to establish differential diagnosis, its reliability, content, construct, and internal validity in the assessment of subacute stroke patients. METHODS: : 120 individuals were evaluated using the SCP over a four-week period of treatment. Intra- and inter-observer reliability, floor and ceiling effects, minimum detectable change (MDC), internal validity and sensitivity to change were explored. In addition, the Barthel Index and the Trunk Control Test were used to study their correlations with the SCP. RESULTS: : Discriminant validity provides evidence that the correlation between SCP items was large or moderate. Convergent validity demonstrated that the correlation of each item with the total score of the scale was high (at around 0.8). Sensitivity to change was large (W = 0.274). Intra- and inter-observer reliability were excellent (Intraclass Correlation Coefficient > 0.9; k > 0.8), except for items B standing and C sitting (k > 0.7). The MDC was 1.39, and ceiling (8.333%) and floor (15.833%) effects were adequate. Cronbach's alpha (α) was equal to 0.901 (0.874-0.924) and McDonald's Omega (ω) was equal to 0.883 (0.856-0.973), showing excellent internal consistency. CONCLUSIONS: : The SCP is a reliable and valid tool which can successfully establish differential diagnosis between PB and TA and evaluate the changes generated by physiotherapy treatment.

The wild edible mushroom 'digüeñe' (Cyttaria espinosae) is a mushroom with interesting flavor and nutritional properties, but highly perishable with a consumption very limited, generating inevitable economic losses for their producers. Because of that, the aim of this study was the evaluation of two drying methods (hot-air drying and freeze-drying) in its bioactive and physicochemical composition for its preservation. The best performances of antioxidant capacity were 62 µmol TE g-1 (ORAC) and 14.21 µmol TE g-1 (DPPH), being higher compared with other vegetable sources. Its protein (5.33 %), fiber (5.58 %) and K (2.82 mg g-1 ) content also stands out. ß-Carotene retention was 35.02 and 23.25 % after freeze drying and hot-air, respectively. Freeze-drying seems a good strategy for digüeñe, bioactive compounds, color and structure were better maintained. This work represents the first characterization of dehydrated digüeñe, constituting approaches for its valorization and use in the food industry.

OBJECTIVE: The aim of this study was to compare the effects of dry needling (DN) versus pressure release over scalene muscle trigger points (TrPs) on pain, related disability, and inspiratory vital capacity in individuals with neck pain. METHODS: In this randomized, single-blind trial, 30 patients with mechanical neck pain and active TrPs in the scalene musculature were randomly allocated to trigger point dry needling (TrP-DN; n = 15) or pressure release (n = 15) groups. The DN group received a single session of DN of active TrPs in the anterior scalene muscles, and the pressure release group received a single session of TrP pressure release over the same muscle lasting 30 s. The primary outcome was pain intensity as assessed by a numerical pain rate scale (NPRS, 0-10). Secondary outcomes included disability (neck disability index, NDI) and inspiratory vital capacity. Outcomes were assessed at baseline and 1 day (immediately post), 1 week, and 1 month after the treatment session. Data were expressed as mean score difference (Δ) and standardized mean difference (SMD). RESULTS: Patients receiving DN exhibited a greater decrease in pain intensity than those receiving TrP pressure release at 1 month (Δ 1.2 (95% CI-1.8, -0.6), p = 0.01), but not immediately (1 day) or 1 week after. Patients in the DN group exhibited a greater increase in inspiratory vital capacity at all follow-up time points (Δ 281 mm (95% CI 130, 432) immediately after, Δ 358 mm (95% CI 227, 489) 1 week after, and Δ 310 mm (95% CI 180, 440) 1 month after treatment) than those in the pressure release group (p = 0.006). Between-group effect sizes were large at all follow-up time points (1.1 > SMD > 1.3) in favor of DN. CONCLUSION: This trial suggests that a single session of DN over active TrPs in the scalene muscles could be effective at reducing pain and increasing inspiratory vital capacity in individuals with mechanical neck pain. Future studies are needed to further confirm these results.

Resumen Debido a que los fenómenos de adsorción de tinturas sobre óxidos metálicos son de gran importancia en la búsqueda de sistemas de conversión de energía solar, en este trabajo se presenta el comportamiento espectroscópico y electroquímico del complejo [Fe(pytpy2)] (PF6)2 (donde pytpy es 4'-(piridin-4-il)-2,2':6',2"-terpiridina) en solución fluida y anclado sobre una película delgada de nanocristales de dióxido de titanio. La constante de formación del aducto se estimó utilizando el modelo isotérmico de Langmuir y se encontró un valor de 1,03x105 M-1 para dicho complejo. El anclaje del complejo sobre la superficie del óxido metálico semiconductor se debe a la interacción del residuo de piridina con los sitios ácidos de Lewis presentes en la superficie del TiO2. Se observó que las propiedades espectroscópicas y electroquímicas del complejo no se ven modificadas por la adsorción.


Abstract Because the phenomena of adsorption of tinctures on metal oxides are of great importance in the search for solar energy conversion systems, this paper presents the spectroscopic and electrochemical behavior of the complex [Fe(pytpy2)](PF6)2 (where pytpy is 4'-(4-pyridyl)-2,2':6',2''-terpyridine) in a fluid solution and anchored on a thin film of nanocrystals of titanium dioxide. The adduct formation constant was estimated using the Langmuir isothermal model, with a value of 1.03x105 M-1 for the complex. The anchoring of the complex on the surface of the semiconductor metal oxide is due to the interaction of the pyridine residue with the Lewis acid sites presented on the TiO2 surface. Results indicated that the spectroscopic and electrochemical properties of the complex are not modified by the adsorption.


Resumo Os fenômenos de adsorção de tinturas em óxidos metálicos são de grande importância na busca por sistemas de conversão de energia solar, motivo pelo qual neste trabalho o comportamento espectroscópico e eletroquímico do complexo [Fe(pytpy2)] (PF6)2 (onde pytpy é 4'-(piridin-4-il)-2,2':6',2"-terpiridina) em uma solução fluida e ancorada em um filme fino de nanocristais de dióxido de titânio. A constante de formação de aduto foi estimada pelo modelo isotérmico de Langmuir, com valor de 1.03x105 M-1 para o referido complexo. A ancoragem do complexo na superfície do óxido de metal semicondutor é devida à interação do resíduo de piridina com os sítios de ácido de Lewis presentes na superfície do TiO2, observando que as propriedades espectroscópicas e eletroquímicas do complexo não são modificadas pela adsorção.

Introducción: La inmunocitoquímica es de gran utilidad para la diferenciación de lesiones benignas y malignas mediante la utilización de anticuerpos específicos que diferencian las células según su origen. Este estudio se realizó con el fin de describir la experiencia de la Fundación Valle del Lili en la aplicación de la inmunohistoquímica al estudio citológico de diferentes tipos de muestra. Materiales y métodos: Se realizó un estudio observacional, descriptivo y retrospectivo de las citologías registradas en la base de datos del departamento de patología de la Fundación Valle del Lili entre diciembre de 2015 y octubre de 2017. Resultados: Se analizaron 54 citologías a las que se efectuó estudio inmunocitoquímico. Se logró realizar tanto bloque celular como botón de citología en base líquida en el 38,88% (n=21) del total de muestras, encontrándose entre los resultados de ambos tipos de citología un coeficiente kappa de Cohen de 0,80 (IC95%: 0,46-1,0), p<0,001. Los marcadores más empleados fueron: Calretinina, MOC-31, EMA, TTF1, PAX8 y calcitonina. De las citologías positivas para malignidad, se realizó diagnóstico definitivo con biopsia al 58,1% (n=25), encontrándose una concordancia muy alta, con un coeficiente kappa de Cohen de 1,0 (IC95%: 1,0-1,0), p<0,001. Discusión: La información aportada por este estudio permite implementar la citología de botón en base líquida para la realización de inmunocitoquímica en los marcadores que fueron evaluables y en los que se encontró concordancia con respecto a la citología en bloque celular. Además, invita a continuar en la búsqueda de conocimiento aportando datos a futuros estudios en esta línea de investigación

RESUMO

Onychomatricoma is a subungual tumor that is clinically characterized by banded or diffuse thickening, yellowish discoloration, splinter hemorrhages, and transverse overcurvature of the nail plate. It is often and easily misdiagnosed because the condition is not well known and rarely has been described. We report the case of a 45-year-old man with a subungual tumor on the right index finger that fulfills the criteria of onychomatricoma.

La localización de los eventos sísmicos es uno de los factores de mayor trascendecia en el monitoreo volcánico. Hoy en día muchos sismólogos dirigen sus esfuerzos para optimizar tanto el proceso como el resultado. De este parámetro. Para ello inicialmente se debe calcular un modelo de velocidades de la corteza apropiado, mediante el cual se nos permita conocer la confiabilidad tanto de la red sísmica como el de la localización misma del evento. Os autores apresentam o caso de um paciente portador da Síndrome da Imunodeficiência Adquirida (AIDS), que desenvolveu lesões papulosas, eritemato-violáceas, em membros inferiores e superiores, tronco, e nas mucosas peniana e conjuntival. O diagnóstico clínico de Sarcoma de Kaposi foi confirmado pelo exame histopatológico. Não havia comprometimento visceral que pudesse ser evidenciado à endoscopia eao raio X de tórax. Cinqüenta e oito dias depois de instituída a terapêutica para a Aids, com Zidovudina (AZT), Lamivudina (3TC) e Indinavir, houve regressão acentuada de todas as lesões do sarcoma de Kaposi. Aceitando-se como agente etiólógico do sarcoma de Kaposi um herpes-vírus (HHV-8), o mais provável é que os inibidores de protease ajam diretamente no controle do HHV-8 ao melhorar o estado imunitário do paciente, permitindo a restauração de uma resposta específica do linfócito T contra o HHV-8
