RESUMO

Two types of possible adaptive response of the photosynthetic apparatus of diatoms to the changes in growth temperature conditions are shown. The first type is a temperature-dependent change in the content of chlorophyll in the cell, aimed at matching the rates of light and dark reactions of photosynthesis (noted in Phaeodactylum tricornutum and Nitzschia sp. 3). At the limiting light intensity, a temperature decrease from 20 to 5°C leads to an increase in the initial slope of light dependence of the C/Chl ratio; under the optimal light conditions at a temperature decrease from 20 to 10°C and from 10 to 5°C, the C/Chl ratio increases 1.5-fold in both species. The second type of response to the changes in growth temperature conditions was observed in Skeletonema costatum, for which the chlorophyll content in the cell does not depend on the temperature in the range of 10-20°C. The adaptation of the photosynthetic apparatus in this case probably occurs due to the changes in the activity of enzyme systems and in the rate of enzyme processes. The potential productivity of all studied species of algae at 10°C, calculated as the increase in biomass per unit of chlorophyll per day, does not differ significantly. Under the conditions of light inhibition, a temperature decrease leads to a progressive decrease in the content of chlorophyll in the cells of all algal species under study due to a decrease in the rate of pigment synthesis against the background of its intense photooxidation.

AIM: To develop a rationale treatment strategy for elective isolated and simultaneous interventions in geriatric patients with benign prostatic hyperplasia (BPH) aimed at decreasing of complications rate and grade, based on current approach to indications and contraindications. MATERIALS AND METHODS: From 2000 to 2015 yy. a total of 639 elderly and senile patients with BPH were undergone to isolated simple prostatectomy, transurethral prostatic resection and simultaneous surgery. Two timeframe were allocated, from 2000 to 2006 and from 2007 to 2015 yy. During the first period, the conventional indications and contraindications were used for determination of the type and volume of the the surgery. During the second period, a novel authors approach to the structure indications and contraindications for the surgical treatment was introduced. RESULTS: For the first time in the geriatric urology a modern structure of the indications and contraindications for elective interventions in patients with BPH has been developed, which allowed to clarify the treatment strategy on the individual basis. The new criteria for absolute and relative contraindications are included as well as, for the first time, temporary, specific and organizational contraindications for the simple prostatectomy and TURP. There is a need for review some conventional contraindications for the TURP in elderly and senile patients, considering that in specialized geriatric urological hospital for patients with severe concomitant diseases (therapeutic, neurologic, etc.), TURP should be the method of choice regardless prostate volume, often exceeding 60 cc. The use of a new structure of indications and contraindications allowed to establish absolute contraindications for the elective surgery in 19%, relative contraindications in 21% and temporary contraindications in 8% cases, respectively. These patients were undergone to palliative surgeries or drug treatment which helped to prevent possible postoperative complications and mortality. CONCLUSION: In order to determine the treatment strategy in elective isolated and simultaneous interventions in geriatric patients with BPH it is necessary to consider the current general and specific indications as well as absolute, relative, specific, temporary and organizational contraindications. The introduction of the novel structure of the indications and contraindications for the elective isolated and simultaneous surgeries with priority of TURP had a pronounced clinical effect, contributing to the marked reduction of postoperative complications and deaths in geriatric patients with BPH.

Reproductive function and treatment results have been analysed for 59 patients aged 17 to 32 years after herniotomy. 28 and 31 of them underwent standard and endosurgical interventions, respectively. The control group consisted of 30 healthy males matched for age. As shown by 6-month follow-up, the endosurgical herniotomy was not associated with such complications as hydroscheocele, hydrocele, testicular atrophy, diminution of blood flow along the testicular arteries. Ejaculate, levels of sex hormones in the blood, cremasteric reflex remained intact. After routine herniotomy there appeared defects in scrotal and testicular microcirculation with partial testicular atrophy in 25% of cases, testicular circulation fell 2.2-2.5-fold, deterioration of spermogram and cremasteric reflex parameters, reduction of serum testosterone were observed.

RESUMO

The anonymous survey of the population for the presence of human immunodeficiency virus (HIV) carried out in Moscow in 1987 revealed 4 seropositive persons among 10, 117 persons subjected to examination. These 4 persons belonged to typical risk groups with respect to the acquired immunodeficiency syndrome (AIDS). The questioning of the persons coming for examination made it possible to find out that a large percentage of them really had a risk of contacting HIV infection; besides, a considerable proportion of the visitors proved to have signs of AIDS phobia.

RESUMO

Serological screening for HIV infection revealed the presence of antibodies to HIV in 26 Soviet citizens and 200 foreigners from 33 countries. Among the Soviet citizens, the risk factors were homosexual relations with homosexual partners (3 men), homosexual and heterosexual relations with partners found to be HIV carriers (5 men and 7 women respectively), as well as sexual relations with foreigners not examined for the presence of HIV infection (4 women); besides, 5 persons were infected as the result of the transfusion of donor blood containing antibodies to HIV and 1 child was born of a mother with antibodies to HIV.
