RESUMO

AIM: The aim of this study was to determine the clinical, socio-demographic, and therapeutic variables associated with the length of hospitalization in a psychiatric emergency hospital in Buenos Aires City. METHOD: The present retrospective analytical study included 350 consecutively admitted patients aged 18-65, from June 2013 until December 2017 in a public psychiatric hospital in Buenos Aires City. Data collected included socio-demographic, clinical and discharge conditions. RESULTS: Variables that predicted the length of hospitalization were: diagnosis of psychosis, the use of lithium and anticonvulsants, unemployment, no economic autonomy, not have formed a family and have modified the living support group during the hospitalization. CONCLUSION: The representative of social and economic vulnerability variables were associated with the utilization of psychiatric inpatient beds. Public policies are requested to interrupt the relationship between poverty and mental pathology.

RESUMO

This case report describes a case of a woman with treatment-resistant schizophrenia, who experienced neutropenia induced by olanzapine and clozapine, and reached symptomatic stabilization with a combination of two depot antipsychotics. This report presents a brief review about the incidence of haematologic events by antipsychotics and the evidence of antipsychotic combination in the treatment.

Premenstrual syndrome is characterized by physical, cognitive, affective and behavioral symptoms that occur cyclically during the luteal phase of the menstrual cycle and resolved quickly at the beginning or within few days after menstruation started. The most severe form is represented by premenstrual dysphoric disorder included in the DSM 5. Over 40% of women experience emotional symptoms such as irritability, tension, emotional lability; and physical symptoms such as breast tenderness, fatigue, and abdominal distension. It has not yet been established the underlying pathophysiological mechanism, although hypotheses related to sex hormones and the GABAergic and serotonergic regulation have been postulated. Treatment includes nonpharmacologic and pharmacologic interventions, being serotonin reuptake inhibitors the first-line treatment.

Altered eating behaviors are a consequence of cognitions and emotions generated by low self-esteem and dissatisfaction with body image and schema. It is possible, that a negative body image, and in many cases, distorted, may be the most difficult feature to approach and improve in this disorders. But the dissatisfaction to one's own body would not be related only to altered eating behaviors. Several studies show that body image and one's own body perception are an essential feature in sexual experiences, especially in women's sexuality. This article describes the relation and influences between body image and women's sexuality, and the sexuality in women with eating disorders.

UNLABELLED: Since 2003, international health regulatory agencies warned about the use of antidepressants in children and adolescents. These warnings were based upon reports from clinical trials in which there was an increased risk of "suicidality" with these drugs. In 2007, this warning was extended to young adults. Given the widespread use of antidepressants, these warnings have been very controversial. Numerous clinical trials have been made to investigate this subject, finding some difficulties: 1) the lack of consistency of the term "suicidality", 2) the complexity of the differential diagnoses of children depression, given the high rates of bipolarity in this population, 3) the difference between "activation syndrome" and mixed states induced by antidepressants, both with high suicide risk, 4) the methodological difficulties to evaluate suicide. CONCLUSIONS: In treatment with antidepressants there are some reports about an increment in suicide attempts (most at the initial phase of treatment). There is no evidence of an increase of completed suicide rates. It is of utmost importance to make a proper diagnose and a close follow up of depressed patients even if they do not receive pharmacological treatment.
