RESUMO

PURPOSE: To compare the production efficiency of selective laser sintering (SLS) to traditional casting for the fabrication of metal prosthodontic frameworks in a prospective pilot evaluation in a hospital prosthodontic laboratory setting. MATERIALS AND METHODS: The time taken to complete each of the identified stages in the production of 50 removable partial denture frameworks made using either SLS (n = 25) or casting (n = 25) workflows was measured. The mean time for production was calculated for each workflow, and the difference was tested for statistical significance. RESULTS/CONCLUSION: The results indicate that an SLS workflow may be more time-efficient, and further cost-effectiveness research is indicated.

Historically, oral and dental issues for head and neck cancer patients were often not considered until after cancer treatment was complete. As a result, outcomes for oral rehabilitation were sometimes suboptimal. Inconsistencies in service delivery models and qualification, training and experience of staff delivering dental care often compounded this problem, making research and audit almost impossible. Collaborative working by consultants in restorative dentistry from all over the UK as part of a Restorative Dentistry-UK (RD UK) subgroup, renamed more recently as the RD-UK Head and Neck Cancer Clinical Excellence Network (CEN), has re-emphasised the importance of specialist restorative dentistry intervention at the outset of the head and neck cancer pathway to optimise outcomes of patient care. The CEN has driven several initiatives, reflecting Getting It Right First Time (GIRFT) principles aimed at reducing unwarranted variation. This improved consistency in approach and optimised collaborative working of the team now presents a better environment for multicentre audit and research. Ultimately, this should result in a continued improvement in patient and carer experience.

RESUMO

Oral rehabilitation of head and neck cancer patients is an integral component of the care pathway. Maxillectomy procedures can cause significant defects, such as oronasal fistulas, loss of support for the cheek and lip, aesthetic defects in the middle third of the face and functional impairments. Orofacial rehabilitation plays a fundamental role in restoring aesthetics and functional capabilities, such as speech, mastication and deglutition.Rehabilitation of maxillectomy patients poses a challenge for both clinicians and patients. This paper utilises case examples to demonstrate the treatment options for the oral rehabilitation of these patients. We will summarise the treatment pathways for conventional obturators, delayed (secondary) implant retained fixed rehabilitation following composite free flap and early rehabilitation using a zygomatic implant perforated flap technique. This paper aims to highlight the challenges in treatment planning and the importance of a multidisciplinary approach in improving patient outcomes.

Research from human and animal studies has found that after responding has been successfully reduced following treatment it can return upon exposure to certain contexts. An individual in recovery from alcohol use disorder, for example, might relapse to drinking upon visiting their favourite bar. However, most of these data have been derived from experiments involving a single (active) response, and the context-dependence of returned responding in situations involving choice between multiple actions and outcomes is less well-understood. We thus investigated how outcome-selective reinstatement - a procedure involving choice between two actions and outcomes - was affected by altering the physical context in rats. In Experiment 1, rats were trained over 6 days to press a left lever for one food outcome (pellets or sucrose) and a right lever for the other outcome. Then, rats received an extinction session in either the same context (A) as lever press training, or in a different context (B). Rats were tested immediately (5 min) after extinction in Context A or B such that there were four groups in total: AAA, ABB, ABA, and AAB. Reinstatement testing consisted of one food outcome being delivered 'freely' (i.e. unearned by lever pressing and unsignalled by cues) to the food magazine every 4 min in the following order: Sucrose, Pellet, Pellet, Sucrose. Selective reinstatement was considered intact if pellet delivery increased pressing selectively on the pellet lever, and sucrose delivery selectively increased pressing on the sucrose lever. This result (Reinstated > Nonreinstated) was observed for rats in group AAA and ABB, but not rats in groups ABA and AAB. Experiment 2 was conducted identically, except that rats received two extinction sessions over two days and tested one day later. This time, all groups demonstrated intact outcome-selective reinstatement regardless of context. Analysis of c-Fos expression in several brain regions revealed that only c-Fos expression in the posterior dorsomedial striatum (pDMS) was related to intact reinstatement performance. Overall, these results suggest that outcome-selective reinstatement is predominantly context-independent, and that intact reinstatement is related to neuronal activity in the pDMS.

PURPOSE: The COVID-19 pandemic has caused 1.4 million deaths globally and is associated with a 3-4 times increase in 30-day mortality after a fragility hip fracture with concurrent COVID-19 infection. Typically, death from COVID-19 infection occurs between 15 and 22 days after the onset of symptoms, but this period can extend up to 8 weeks. This study aimed to assess the impact of concurrent COVID-19 infection on 120-day mortality after a fragility hip fracture. METHODS: A multi-centre prospective study across 10 hospitals treating 8% of the annual burden of hip fractures in England between 1st March and 30th April, 2020 was performed. Patients whose surgical treatment was payable through the National Health Service Best Practice Tariff mechanism for "fragility hip fractures" were included in the study. Patients' 120-day mortality was assessed relative to their peri-operative COVID-19 status. Statistical analysis was performed using SPSS version 27. RESULTS: A total of 746 patients were included in this study, of which 87 (11.7%) were COVID-19 positive. Mortality rates at 30- and 120-day were significantly higher for COVID-19 positive patients relative to COVID-19 negative patients (p < 0.001). However, mortality rates between 31 and 120-day were not significantly different (p = 0.107), 16.1% and 9.4% respectively for COVID-19 positive and negative patients, odds ratio 1.855 (95% CI 0.865-3.978). CONCLUSION: Hip fracture patients with concurrent COVID-19 infection, provided that they are alive at day-31 after injury, have no significant difference in 120-day mortality. Despite the growing awareness and concern of "long-COVID" and its widespread prevalence, this does not appear to increase medium-term mortality rates after a hip fracture.

New materials are continuously being developed and advanced for the provision of partial dentures. However, the rate at which they are developing makes it hard for the clinician to discern which would be worth adopting into their practice.This paper looks at some of those materials, including the earlier alternatives to conventional acrylic resins, polyamides and acetal resin, followed by a review of the newer novel polymers that are currently in use, including aryl-ketone-polymers and the polymer group polyaryletherketones.The newer polymers have many similarities, in both properties and aesthetics, such as increased elasticity that allows the dentures to engage deeper undercuts, while maintaining high strength. A significant benefit of the novel polymers is in their ability to fit into a digital workflow. Computer-aided design and manufacture confer a high degree of consistency, efficiency and accuracy in the prosthesis. The ability to use precise digital tools to create frameworks and clasps of accurate parameters limits the introduction of technical error that has the potential to be introduced in the construction of a conventional cobalt-chrome framework.The novel high-performance polymers show great promise. Research is needed on these materials investigating the effects on oral health, longevity and survival of the appliances, as well as studies looking at patient satisfaction and the impact on quality of life.

RESUMO

While the conventional fabrication of removal partial dentures through the lost-wax technique is a time-tested technique, it is recognised as a 'complicated, error-prone, time-consuming and expensive process'. This has financial implications and can lead to increased chair time for the patient and dentist, resulting from low precision and ill-fitting frameworks. The use of computer-aided design and computer-aided manufacture brings removable prosthodontics into the digital era and opens up a host of benefits to the practitioner and laboratory technician. This paper introduces the benefits and downsides to adopting a digital workflow, and looks at the various alternative manufacturing techniques, which allow the clinician and technician to streamline their processes and make use of novel denture materials.

The aim of this study is to compare laparoscopic and conventional techniques following Total Pharyngo-laryngo-esophagectomy (TPLE) with respect to perioperative morbidity and mortality and postoperative recovery in post cricoid cancer patients. This is a prospective study, which was undertaken in Gujrat Cancer Research Institute (GCRI) in the period of July 2007 to March 2010. Fifteen consecutive patients who underwent laparoscopic TPLE were compared to that of 18 consecutive patients who underwent open TPLE. Laparoscopic and open TPLE procedure were compared with respect to patient characteristics, intra operative and complications present. The average duration was observed to be 3.5 h in the MIS (Minimally Invasive Group) group and was 5.3 h in the open group. The average blood loss was 300 mL in the MIS group and 500 mL in the open group. Average duration of the hospital stay in the MIS group was 13 days and 16 days in the open group. In the MIS group, one patient (6.7%) had a pneumonic complication and two patients (13%) had wound complications. In the open group, six patients (33%) had pneumonic consolidation and four patients (22%) had wound infections. In both groups, one patient each suffered mortality. Laparoscopic TPLE has been found to be much safer with less morbidity as compared with open surgery.

OBJECTIVE: It is claimed that a hydrophilic primer, called the moisture insensitive primer (MIP), can be used with both dry and wet enamel surfaces. This study sought to assess the shear bond strength (SBS) of APC Plus adhesive coated appliance system to enamel using MIP in wet and dry conditions. MATERIALS AND METHODS: This in vitro experimental study evaluated 24 extracted maxillary premolars with intact buccal enamel. The teeth were randomly divided into two groups (n=12), and APC Plus premolar brackets were bonded to their buccal surface using moisture insensitive primer (MIP) under wet and dry enamel conditions. The SBS values were measured by a universal testing machine. The adhesive remnant index (ARI) score was also determined under a stereomicroscope. Data were analyzed using t-test and Mann-Whitney test at P<0.05 level of significance. RESULTS: The SBS in wet condition was significantly lower than that in dry condition (mean value of 18.37MPa versus 25.5MPa, P<0.001). The two groups had no significant difference regarding the ARI scores (P>0.05). However, in both groups, less adhesive was left on the tooth surface. This suggests that bond failure occurred at the adhesive-enamel interface. CONCLUSIONS: APC Plus adhesive coated appliance system in combination with hydrophilic MIP can be effective in clinical conditions with high risk of moisture contamination.

Introduction Patients with scaphoid nonunion and wrist pain may have a wide spectrum of potential concomitant pathologies that may be diagnosed and potentially managed arthroscopically. The aim of this study is to assess the usefulness of wrist arthroscopy in the assessment and treatment of scaphoid nonunion and any associated injuries. Materials and Methods We retrospectively reviewed 34 consecutive patients with established scaphoid nonunion between January 2006 and December 2012 who had undergone arthroscopic assessment. The average age of the patients was 40 years (range: 25-64), and all the patients had arthroscopic assessment of the wrist joint before definitive surgery. The patients with associated intra-articular problems, which could be addressed along with the scaphoid open reduction internal fixation (ORIF) and bone grafting (BG), had definite procedure in the same sitting. However, if the patients had major intra-articular pathology that needed change in the management plan, they had staged definitive treatment after discussing with them about the arthroscopic findings. Results Arthroscopic assessment of the 34 joints showed varying degrees of arthritis affecting radioscaphoid joint (41%) followed by injuries to the triangular fibrocartilage complex (TFCC) (35%), lunotriquetral ligament (LTL) tears (32%), and scapholunate ligament (SLL) injuries (26%). Concomitant procedures performed during the wrist arthroscopy included debridement of synovitis (62%), TFCC debridement (32%), loose body removal (17%), and DRUJ stabilization and TFCC repair (3%). Twenty-nine patients had arthroscopy and definitive procedure in the same sitting, and the remaining had staged or delayed definitive treatment. Conclusion Our study highlights the usefulness of wrist arthroscopy in assessment and management of the scaphoid nonunion and associated pathologies. Besides in 18% of our patients, the initial management plan changed after arthroscopy. Level of Evidence This is a Level IV study.

PURPOSE: To investigate differences in the performance of polyether ether ketone (PEEK) frameworks vs traditional metal frameworks for removable partial dentures (RPDs) in terms of OHRQoL, patient preference, periodontal indices, and denture satistfaction. MATERIALS AND METHODS: Twenty-six participants were recruited to a randomized controlled crossover trial and were provided with either PEEK or cobalt-chromium (Co-Cr) RPDs. After 4 weeks, the participants were provided with the other RPD. After both 4-week testing periods, the participants chose their preferred RPD and were followed up again at 6 months and 1 year. The primary outcome measure was effect on OHIP-20 score, which was compared among patients using repeated measures ANOVA. Secondary outcomes were participant preference compared using chi-square analysis; Plaque Index and Gingival Bleeding Index (PI and GBI, respectively) compared using repeated measures ANOVA; and McGill Denture Satisfaction Questionnaire (MDSQ) score compared between the two 4-week follow-ups using paired-samples t test. RESULTS: Both Co-Cr and PEEK frameworks resulted in a significant improvement in OHIP-20 score (P < .001), but the material was not a significant factor in changes over 1 year (P = .87). There were no statistically significant differences in participant preference at 1 year (P = .491), nor between RPD materials in their effect on GBI (P = .476), PI (P = .967), or MDSQ (P = .368). CONCLUSION: Both Co-Cr and PEEK RPDs improved OHRQoL to a degree greater than the minimum clinically important difference at 4 weeks, 6 months, and 1 year compared to baseline. No significant preference or improved denture satisfaction score was seen for either material. PEEK frameworks seem to be associated with similar degrees of periodontal effects as Co-Cr frameworks.

It is a known fact that a disabled person is a greater burden to society than a dead person. Krukenberg operation, first described by German army surgeon in 1917, Hermann Krukenberg, converts a forearm stump into a pincer, so as to retain some dexterity of the hand rather than it being reduced to a mere stump. We report this case of Krukenberg operation performed in a tertiary care setup, wherein an amputated forearm stump was converted into a functional pincer that can result in huge advantage for poor amputee patients in developing countries who rely heavily on the functionality of their hands to earn their everyday meal and are unable to bear the expense of costly prosthesis. One such patient, a 25-year-old male hailing from a very poor background who came to us with traumatic amputation of his dominant hand.

Though diagnosis can be made on the basis of the classic clinical and radiological features, it is often missed due to variable presentations. Therefore, it is important to know about this syndrome to reach correct diagnosis.

This study was designed to evaluate efficacy and stability of corneal collagen crosslinking (CXL) in halting the progression of post-laser in situ keratomileusis (LASIK) ectasia and provide long-term follow-up results with an average of 80mo. Patients with post-LASIK ectasia were treated with CXL between December 2007 and January 2012. Main outcome measures were uncorrected distance visual acuities (UDVA) and corrected distance visual acuities (CDVA), minimum and maximum keratometry (K) values, spherical and cylindrical refraction, and corneal thickness. The study evaluated 17 eyes for 13 patients (8 men, 5 women) with mean age of 31y (range 23 to 39) and mean follow-up of 80.7±15 (range 57 to 102)mo. UDVA and CDVA improved from logMAR 0.53±0.36 (20/63) to 0.49±0.4 (20/50) (P=0.43) and from 0.18±0.17 (20/28) to 0.16±0.16 (20/27) (P=0.55) respectively. In 15 eyes UDVA and in 13 eyes CDVA either remained stable or improved ≥1 Snellen lines (88.2%) and (76.5%) respectively. Although statistically insignificant, spherical and cylindrical refraction decreased post-CXL from -1.26±2.87 to -0.38±2.32 diopters (D) (P=0.054) and from -3.80±2.47 to -3.04±2.18 D (P=0.13) respectively. Kmax significantly decreased from 44.23±3.76 to 42.85±3.08 D (P=0.013) and Kmin decreased from 41.07±3.61 to 40.00±2.65 D (P=0.057). Corneal thickness decreased from 470±42 to 460±41 µm, but was statistically non-significant (P=0.063). Therefore, CXL is effective in halting and partially reversing the progression of post-LASIK ectasia on the long-term (mean follow-up of more than 80mo), thus highlighting the stability and maintained effect of CXL for such cases.

STATEMENT OF PROBLEM: Clinicians are currently unable to quantify the psychosocial, functional, and esthetic effects of prosthetic interventions to replace teeth. Understanding the effects of treatment to replace teeth on oral health-related quality of life (OHRQoL) is important for informed consent. A systematic review of the evidence of OHRQoL improvements with prosthodontic tooth replacement and a comparison of outcomes between treatment modalities is therefore indicated. PURPOSE: The purpose of this systematic review was to examine the OHRQoL of patients with partial edentulism after different dental prosthetic treatments. MATERIAL AND METHODS: Electronic database and manual searches were conducted to identify cohort studies and clinical trials reporting on the OHRQoL of individuals receiving implant-supported crowns (ISCs), implant-supported fixed dental prostheses (IFDPs), implant-supported removable dental prostheses (IRDPs), tooth-supported fixed dental prostheses (TFDPs), and removable partial dentures (RPDs). Two reviewers independently conducted article selection, data extraction, and quality assessment. Random-effects models were used to compare OHRQoL change scores (standardized mean change, 95% confidence intervals). RESULTS: Of the 2147 identified studies, 2 randomized controlled trials and 21 cohort studies met the inclusion criteria. Overall, studies were of low or moderate risk of bias. Pooled mean OHRQoL change ≤9 months was 15.3 for TFDP, 11.9 for RPD, and 14.9 for IFDP. Pooled standardized mean change OHRQoL change >9 months was 13.2 for TFDP and 15.8 for IFDP. Direct comparisons ≤9 months between TFDP against IFDP and RPD against IFDP significantly favored IFDP in both cases. CONCLUSIONS: TFDP and IFDP had short- and long-term positive effects on OHRQoL. RPDs positively affected OHRQoL in the short term. IFDP showed greater short-term improvement in OHRQoL than RPD and TFDP.

INTRODUCTION: Seborrheic keratosis is one of the most common benign epidermal cutaneous lesions encountered by dermatologists and plastic surgeons in their daily practice. PRESENTATION OF THE CASE: A 66-year-old man was presented with a large verrucous mass on the sacrum and perianal area of 10 years duration. After the diagnosis of SK was confirmed, a complete excision of the lesion was done with coverage of the defect with a partial thikness skin graft, with a good outcome. DISCUSSION: Giant Seborrheic keratosis are very rare, and their location on the perianal area is rarer still, with no more than 10 published cases of genital area involvement. CONCLUSION: Giant perianal seborrheic keratoses is a very rare presentation, that may resemble many of the skin disease of that area and should be managed with excision and biopsy to confirm the diagnosis.

INTRODUCTION: Congenital pits of the lip are uncommon and may be associated with conditions such as Van der Woude syndrome. Isolated lip pits are extremely rare developmental defects. PRESENTATION OF CASE: A 7 year old Caucasian girl presented to plastic surgery clinic in Jordan University academic hospital complaining of an upper lip pit that has been present since birth. It was associated with a single episode of whitish discharge. On examination, an isolated upper lip midline sinus was found. She is otherwise fit and healthy. Surgical excision was curative with good cosmetic outcome. DISCUSSION: The prevalence of lower lip sinuses has been estimated to be about 0.00001% of the white population. Upper lip sinuses are even more uncommon. To date, there have been several case reports of upper lip sinuses and fistulas. A total of 55 cases was found upon reviewing the English literature. No similar cases were reported in Jordan. Different presentations are mentioned in this review. Several embryologic theories are presented. CONCLUSION: Upper lip sinus formations are rare developmental events, and the pathogenesis of these lesions still needs further evaluation. Simple surgical excision is the treatment of choice.

Successful removable partial dentures (RPDs) use may be influenced by patient and denture variables including age, denture experience and number or position of teeth being replaced. Influence of patient and denture variables on OHRQoL with RPDs was investigated using ordinal logistic regression analysis. METHODS: Participants provided with RPDs were invited to complete OHIP-20 at least 4 weeks after dentures were fitted. Clinical records were retrieved to identify a number of independent variables. Variables meeting a standard of statistical significance were included into regression analysis to quantify influence on OHIP-20 scores. RESULTS: 84 patients agreed to participate. No. of missing teeth, age, gender, replacement of anterior teeth and framework material were entered into the model, which was statistically significant, p⟨0.001, with pseudo R² of 0.39. Significant variables were age (p⟨0.05), no. of missing teeth (p⟨0.01), gender (p⟨0.05) and replacement of anterior teeth (p⟨0.001). Replacing anterior teeth showed 9.68 point reduction in OHIP-20 score in patients with replaced anterior teeth. DISCUSSION: Replacing anterior teeth was associated better post-operative OHRQoL outcomes, suggesting RPDs address impacts on OHRQoL of missing anterior teeth. Success of RPDs in overcoming impacts of tooth loss on OHRQoL was worse as more teeth were lost.
