RESUMO

Partial Body Cryostimulation (PBC) involves a very cold air flow directed to the body of subjects with minimal clothing. PBC is performed in a rapid timeframe, inside an on-purposed designed cryo-cabin. Recently, cryo-cabins have been built with different energy systems, however a validation study on relative thermal responses is missing. This study was aimed at comparing thermal responses following a PBC in an electrically powered cryo-cabin based on forced convection or into a standard nitrogen-fueled cryo-cabin. In a randomized crossover fashion, thirty-six subjects (F=20; M=16) underwent both cryo-exposures lasting 150 s each. Thermal responses were assessed before and immediately after completing each PBC session. Mixed model analysis of variance revealed a significantly colder temperature after electric PBC in all the body regions (except for the thighs) with respect to a standard nitrogen based PBC (F: 16.4 ± 1.4 vs 18 ± 5.8 °C; M: 16.4 ± 1.7 vs 20.9 ± 4 °C). Moreover, a significant lower thermal discomfort was perceived at the end of electric PBC as compared to that one felt following standard PBC. For the first time, the safety and thermo-effectiveness of an electric cryo-cabin based on forced convection was ensured. This methodology can be viable for practitioners of PBC and clinicians.

In the mild subtype of inherited carnitine palmitoyltransferase II (CPTII) deficiency, muscular mitochondrial fatty acid ß-oxidation is impaired. In this condition, interventions involve daily dietary restriction of fats and increase of carbohydrates, whereas physical exercise is commonly contraindicated due to the risk of muscle pain and rhabdomyolysis. We present the case of a 14-year-old female with CPTII deficiency who underwent a 1-h session of unsupervised exercise training for 6 months, 3 days per week, including interval and resistance exercises, after diet assessment and correction. Before and after intervention, the resting metabolic rate (RMR) and respiratory quotient (RQ) were measured by indirect calorimetry, and a cardiopulmonary exercise test (CPET, 10 W/30 s to exhaustion) was performed. Interval training consisted of a 1 min run and a 5 min walk (for 15 min progressively increased to 30 min). During these efforts, the heart rate was maintained over 70% HR max corresponding to respiratory exchange ratio (RER) of 0.98. Resistance training included upper/lower split workouts (3 sets of 8 repetitions each, with 2 min rest between sets). Blood CK was checked before and 36 h after two training sessions chosen randomly without significant difference. After training, RMR increased (+8.1%) and RQ lowered into the physiological range (from 1.0 to 0.85). CPET highlighted an increase of peak power output (+16.7%), aerobic performance (VO2 peak, 8.3%) and anaerobic threshold (+5.7%), oxygen pulse (+4.5%) and a much longer isocapnic buffering duration (+335%). No muscle pain or rhabdomyolysis was reported. Results from our study highlight that training based on short-duration high-intensity exercise improves overall metabolism and aerobic fitness, thus being feasible, at least in a case of CPTII deficiency.

Post-activation potentiation (PAP) has been defined as a major enhancement of muscular performance following a preload stimulus. The eccentric actions seem to cause a potentiating effect on subsequent explosive exercises. The aim of this study was to determine whether a protocol of squat exercise using an inertial flywheel could have a potentiating effect on jump performance. Sixteen physically active volunteers participated in the study (age: 21.8 ± 2.7 years; body mass index: 23.6 ± 3). All participants completed two different protocols on separate days: a Traditional Protocol (using a half squat with a guided barbell) and an Inertial Flywheel Protocol (using a half squat with an inertial flywheel). Both protocols were similar and consisted of 3 x 6 reps at the load that maximized power, with a 3-minute rest interval between sets. The squat jump (SJ) was measured by a contact platform at baseline, and four, eight and twelve minutes after the PAP stimulus. A two-way ANOVA with repeated measures was performed to analyze significant differences over time. There were significant increases of SJ height (p = 0.004, d = 0.665), velocity (p = 0.003, d = 0.688) and power (p = 0.004, d = 0.682) from baseline after the inertial flywheel protocol. A significant interaction effect (time x protocol) was observed, showing that the inertial flywheel protocol had a potentiating effect on the jump performance compared to the traditional protocol, more specifically at 4 and 8 minutes after the PAP stimulus. In conclusion, the inertial flywheel protocol showed a potentiating effect on the squat jump performance, thus this pre- conditioning activity could be useful during the warm-up before the competition.
