Ischaemic cardiovascular disease is associated with tissue hypoxia as a significant determinant of angiogenic dysfunction and adverse remodelling. While cord blood-derived endothelial colony-forming cells (CB-ECFCs) hold clear therapeutic potential due to their enhanced angiogenic and proliferative capacity, their impaired functionality within the disease microenvironment represents a major barrier to clinical translation. The aim of this study was to define the specific contribution of NOX4 NADPH oxidase, which we previously reported as a key CB-ECFC regulator, to hypoxia-induced dysfunction and its potential as a therapeutic target. CB-ECFCs exposed to experimental hypoxia demonstrated downregulation of NOX4-mediated reactive oxygen species (ROS) signalling linked with a reduced tube formation, which was partially restored by NOX4 plasmid overexpression. siRNA knockdown of placenta-specific 8 (PLAC8), identified by microarray analysis as an upstream regulator of NOX4 in hypoxic versus normoxic CB-ECFCs, enhanced tube formation, NOX4 expression and hydrogen peroxide generation, and induced several key transcription factors associated with downstream Nrf2 signalling. Taken together, these findings indicated that activation of the PLAC8-NOX4 signalling axis improved CB-ECFC angiogenic functions in experimental hypoxia, highlighting this pathway as a potential target for protecting therapeutic cells against the ischaemic cardiovascular disease microenvironment.

Background: SEAs are infrequent; however, panspinal infections are even rarer, especially when GBS infection is involved. The cornerstone of treatment is based on early diagnosis and use of targeted antimicrobial therapy; in case of cord compression or neurological compromise, urgent surgical intervention should be pursued. Overall, it is an infrequent condition and therefore requires prospective multicenter studies. Case Presentation. We describe a case who presented with diabetic lower extremity wounds; however, soon the patient developed bowel and bladder incontinence in the setting of back pain, secondary to panspinal epidural abscess. The patient's case is unique in two aspects: firstly, it is panspinal, and secondly, its causative agent is GBS. Conclusion: Prompt diagnosis of SEA is critical in the preservation of neurological function. Anyone presenting with fevers, back pain, and neurological changes should have urgent MRI evaluation of the spine.

A 74-year-old man with chronic obstructive pulmonary disease on home oxygen and coronary artery disease was transferred from an outside facility to obtain an inguinal lymph node biopsy to rule out malignancy. He underwent an uncomplicated procedure and was discharged the same day. While waiting for transportation, he had sudden-onset dyspnea and collapsed. After resuscitation, patient had return of spontaneous circulation and was admitted but was provided comfort care and soon expired. Autopsy showed metastatic squamous cell carcinoma with multiple bilateral tumor emboli. Pulmonary tumor embolism is a rare cause of dyspnea in cancer population. Most of the cases are diagnosed with autopsy after sudden death; however, few cases have been reported antemortem. Tumor embolism is rare and difficult to diagnose without an autopsy with a poor outcome.

Extracelluar matrix (ECM) proteins create complex networks of macromolecules which fill-in the extracellular spaces of living tissues. They provide structural support and play an important role in maintaining cellular functions. Identification of ECM proteins can play a vital role in studying various types of diseases. Conventional wet lab-based methods are reliable; however, they are expensive and time consuming and are, therefore, not scalable. In this research, we propose a sequence-based novel machine learning approach for the prediction of ECM proteins. In the proposed method, composition of k-spaced amino acid pair (CKSAAP) features are encoded into a classifiable latent space (LS) with the help of deep latent space encoding (LSE). A comprehensive ablation analysis is conducted for performance evaluation of the proposed method. Results are compared with other state-of-the-art methods on the benchmark dataset, and the proposed ECM-LSE approach has shown to comprehensively outperform the contemporary methods.

Bronchoscopy is a common and safe procedure with low mortality rates and complications. The risk of pneumothorax (PTX) post bronchoscopy is estimated to be 0.1% but increases to 1-6% with the addition of transbronchial lung biopsy (TBB) to the procedure. Studies have shown that a short observation period is adequate after TBB, and the usual practice is to perform a portable chest radiograph (CXR) to rule out PTX. Delayed PTX is a rare complication post-TBB and very few cases have been reported in the literature. In this report, we discuss a patient with delayed PTX 48 hours post-TBB. A 71-year-old male with a history of malignancy of unknown primary with metastasis to the sacrum and vertebral column presented with lower limb weakness status post-palliative radiation to the spine. His sacral lesion biopsy was inconclusive. He was currently on oral steroids. He was noted to have a left upper lobe lung nodule on a CT scan of the chest. He underwent bronchoscopy with TBB to determine if it was a primary lung malignancy. He was stable post-procedure with an unremarkable CXR for PTX and was discharged with outpatient follow-up. Two days later, he presented to the emergency department with shortness of breath and hypoxemia. A CXR was performed, which showed a left-sided PTX. A chest tube was placed, and a follow-up CXR showed lung immediate re-expansion. The chest tube was removed after two days and the patient was discharged home after a total of four days of hospitalization. Iatrogenic PTX can be due to diagnostic and/or therapeutic interventions. PTX after procedures can be classified as acute (one to four hours post-procedure) or delayed (>4 hours post-procedure). It is recommended to have a CXR within an hour post-TBB. To our knowledge, very few cases of delayed PTX post-TBB have been reported, mostly among lung transplant patients and those with chronic infections such as tuberculosis. In prior reports, it has been speculated that a visceral pleural defect might occur during a biopsy, but is protected by blood clot formation in the proximal bronchus. A PTX then occurs after fibrinolysis of the blood clot. Low immunity and poor wound healing due to chronic inflammation or steroid use can play a role in causing a delayed PTX. Also, the use of pain drugs such as opioids is associated with iatrogenic PTX. Patients with underlying lung disease such as emphysema are more prone to developing a PTX. Another hypothesis is that a tissue flap is created after the biopsy, which obstructs the airflow during exhalation, thereby resulting in a PTX. On the other hand, it is known that lung malignancies, either primary or metastatic, can increase the risk of secondary PTX. In our case, the temporal relationship of the delayed PTX with bronchoscopy makes it more likely that it was related to the lung biopsy (in our case, poorly differentiated non-small cell carcinoma). The underlying malignancy with low immunity, chronic tissue inflammation, and current steroid use may have resulted in delayed lung healing at the biopsy site. This case report highlights the importance of considering delayed PTX among high-risk patients who undergo such procedures. Delayed PTX is a rare complication post-TBB and should be considered in patients who are stable post-procedure but present with dyspnea and/or hypoxemia even days after the procedure.

Zinc oxide nanoparticles (ZnONPs) were the targets of numerous biological syntheses to attain their precious values in various biomedical fields. The phycosynthesis of ZnONPs were innovatively investigated using cell-free extract of the macroalgae, Ulva fasciata Delile. The phycosynthesized U. fasciata-zinc oxide nanoparticles (UFD-ZnONPs) had 77.81 nm mean size, with flower and sphere shapes and positive zeta potential. The UFD-ZnONPs infra-red analysis indicated their basic components' cross-linkage. The antibacterial potentialities of UFD-ZnONPs were confirmed, qualitatively and quantitatively, against foodborne microorganisms (Escherichia coli plus Staphylococcus aureus); the bactericidal action was higher for UFD-ZnONPs than the annealed phycosynthesized ZnONPs. The scanning micrographs of S. aureus and E. coli cells treated with UFD-ZnONPs indicated the severe action of nanoparticles to destroy bacterial cells in time-dependent manners. Peeled shrimps (Fenneropenaeus indicus) were biopreservated through refrigerated storage (4 °C) with UFD-ZnONPs based solution for six days. The microbial examination of UFD-ZnONPs -treated shrimps displayed decrease in microbial loads throughout the storage days. Moreover, the UFD-ZnONPs-treated shrimps showed acceptable sensorial attributes (appearance, odor, color and texture) compared to untreated shrimps. UFD-ZnONPs nanocomposite concentration of 3% and 5% could be remarkably suggested as efficient procedure for shrimps' biopreservation during refrigerated storage regarding sensorial quality and microbial profile of product.

Listeria monocytogenes is a foodborne bacterial pathogen that causes serious health risks. Chitosan (Ch) is a bioactive polymer that could be effectively applied for foodstuffs biopreservation. Lycium barbarum (Goji berry) is ethnopharmaceutical fruit that have diverse health protecting benefits. Chitosan was produced from A. niger and employed with L. barbarum extract (LBE) as blends for Listeria control and quality biopreservation of African catfish mince (Clarias gariepinus). Chitosan could utterly control L. monocytogenes survival in fish mince and its efficacy was strengthened with added LBE at 0.2 and 0.4%. Blending of fish mince with Ch could effectively reduce the progress of chemical spoilage parameters and this protective effect was greatly enhanced with increased addition of LBE. The sensorial assessment of treated minces indicated panelists preferences for the entire attributes of blended samples with Ch and LBE, particularly with storage prolongation. Scanning micrographs elucidated the antibacterial action of Ch against L. monocytogenes. Results recommended the application of fungal Ch/LBE composites as biopreservatives and anti-listerial agents, through their blending with catfish mince, to eliminate bacterial growth, enhance sensory and storage attributes of preserved fish.

Fish are generous sources for providing man with his essential nutritional requirements, but the extreme susceptibility to quality deterioration hinders their optimal usage and storage. Natural derivatives are always the perfect alternatives for food preservation. The application of fungal chitosan (Ch), from Aspergillus niger, and pomegranate peel extract (PPE), in coating films for Nile tilapia (Oreochromis niloticus) fillets preservation and maintaining their microbiological, chemical and sensorial quality during cooled storage at 4°C for 30days, was investigated. Fish fillet were coated with Ch (2%) and combined Ch+PPE, at PPE percentages of 0.5, 1.0, 1.5 and 2.0%. Fillets coating resulted in sharp decrease of the entire microbial counts during storage; the increased concentrations from PPE strengthened coating film antimicrobial activity. Additionally, fillets coating could retard the chemical spoilage parameters increasing, i.e. nitrogen volatile base (TVB-N), peroxide value (PV) and reactive substances of thiobarbituric acid (TBARS), during storage period. The sensory evaluation indicated higher preferences for the odor, texture, color and overall quality of coated samples. Fish fillets coating with Ch and Ch+PPE could be recommended for shelf life extension and maintaining the microbiological, chemical and sensorial quality through the application of safe preservatives from natural origins.

OBJECTIVES: To determine sleep habits and sleep quality in medical students during their clinical years using validated measures; and to investigate associations with academic performance and psychological stress. METHODS: In this cross-sectional study, medical students (n=320) were randomly selected from a list of all enrolled clinical-year students in a Saudi medical school from 2011-2012. Students filled a questionnaire including demographic and lifestyle factors, Pittsburgh Sleep Quality Index, Epworth Sleepiness Scale, and Perceived Stress Scale. RESULTS: Students acquired on average, 5.8 hours of sleep each night, with an average bedtime at 01:53. Approximately 8% reported acquiring sleep during the day, and not during nighttime. Poor sleep quality was present in 30%, excessive daytime sleepiness (EDS) in 40%, and insomnia symptoms in 33% of students. Multivariable regression models revealed significant associations between stress, poor sleep quality, and EDS. Poorer academic performance and stress were associated with symptoms of insomnia. CONCLUSION: Sleep deprivation, poor sleep quality, and EDS are common among clinical years medical students. High levels of stress and the pressure of maintaining grade point averages may be influencing their quality of sleep.
