Fundamento: la elevada frecuencia de recién nacidos con peso inferior al adecuado, en relación con la anemia materna, la atención prenatal insuficiente y la condición de madre soltera, puede tener importantes repercusiones en la salud de las madres y los niños. Objetivo: identificar la relación entre el control prenatal, la anemia materna y la condición de ser madre soltera con el bajo peso al nacer de los infantes. Método: se realizó un estudio observacional, analítico y retrospectivo de casos y controles en gestantes atendidas en un hospital de la región Sierra de Ecuador durante el año 2022. La muestra fue de 280 pacientes. El grupo de casos lo conformaron las gestantes con hijos con bajo peso al nacer (n = 70) y el grupo de control por gestantes con recién nacidos con peso normal (n = 210). Se realizó la prueba de Chi cuadrado de independencia y se calculó el Odds Ratio y su intervalo de confianza del 95 % para el nivel de asociación entre las variables. Resultados: la relación de la variable asistencia a controles prenatales con respecto al bajo peso al nacer mostró una asociación protectora (OR = 0,5). Se identificaron como factores de riesgo, la anemia materna (OR = 3,1) y la condición de madre soltera (OR = 2,3) por sus altos valores de asociación positiva y directa. Conclusiones: el control prenatal, la anemia materna y la condición de madre soltera resultaron tener un nivel de asociación significativa con el bajo peso al nacer.
Foundation: the high frequency of newborns with less than adequate weight, in relation to maternal anemia, insufficient prenatal care and single mother status, can have important repercussions on the health of mothers and children. Objective: to identify the relationship between prenatal control, maternal anemia and the condition of being a single mother with low birth weight of infants. Method: an observational, analytical and retrospective case-control study was carried out in pregnant women treated in a hospital in the Sierra region of Ecuador during the year 2022. The sample was 280 patients. The case group was made up of pregnant women with children with low birth weight (n = 70) and the control group was made up of pregnant women with newborns with normal weight (n = 210). The Chi square test of independence was performed and the Odds Ratio and its 95 % confidence interval were calculated for the level of association between the variables. Results: the relationship between the variable attendance at prenatal check-ups and low birth weight showed a protective association (OR = 0,5). Maternal anemia (OR = 3,1) and being a single mother (OR = 2,3) were identified as risk factors due to their high values of positive and direct association. Conclusions: prenatal control, maternal anemia and single mother status turned out to have a significant level of association with low birth weight.

Background: Chronic obstructive pulmonary disease (COPD) remains one of the most prevalent pathologies in the world and is among the leading causes of mortality and morbidity, partially due to underdiagnosis. The use of clinical questionnaires to identify high-risk individuals to take them to further diagnostic procedures has emerged as a strategy to address this problem. Objective: To compare the performance of the COULD IT BE COPD, CDQ, COPD-PS, LFQ, and PUMA questionnaires for COPD diagnosis. Methods: A cross-sectional study was carried out on subjects who underwent spirometry in the third-level center. Data were collected between January 2015 and March 2020. Bivariate analysis was performed between the study variables and the presence of COPD. The area under the receiver operating characteristics curve (AUC-ROC), sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (LR+), and negative likelihood ratio (LR-) for each questionnaire were calculated. The AUC-ROCs were compared with the DeLong test, considering a p value <0.05 statistically significant. Results: 681 subjects met the inclusion criteria and were taken to the final analysis. The prevalence of COPD was 27.5% (187/681). The mean age of the subjects was 65.9 years (SD ± 11.79); 46.3% (315/681) were female, and 83.6% (569/681) reported respiratory symptoms. Statistically significant relationship was found for COPD diagnosis with male sex, older age, respiratory symptoms, and exposure to wood smoke (p value <0.05). The AUC-ROCs of the questionnaires were between 0.581 and 0.681. The COULD IT BE COPD questionnaire had a lower discriminatory capacity AUC-ROC of 0.581, concerning the other scores (DeLong test, p = 0.0002). Conclusion: The CDQ, COPD-PS, LFQ, PUMA, and COULD IT BE COPD questionnaires have acceptable performance for the diagnosis of COPD together with low sensitivity and specificity. Therefore, its use must be complemented with other diagnostic tests or techniques such as pulmonary function tests.

A high-fat diet (HFD) during pregnancy promotes fat accumulation and reduces docosahexaenoic acid (DHA) levels in the liver of the offspring at postnatal ages, which can depend on fetal sex. However, the prenatal mechanisms behind these associations are still unclear. Thus, we analyzed if an HFD alters DHA content and the expression of molecules related to fatty acid (FA) metabolism in the fetal liver. Female C57BL/6 mice were fed a control diet or HFD for 4-6 weeks before pregnancy until the gestational day (GD) 17.5. A subgroup of each diet received DHA (100 mg/Kg) orally from GD 6.5 until 16.5. On GD 17.5, maternal livers, placentas, and livers from male and female fetuses were collected for FA profiling with gas-chromatography and gene expression of molecules related to FA metabolism using qPCR. PPAR-α protein expression was evaluated using Western blot. The gene expression of placental FA transporters was also assessed. An HFD increased eicosapentaenoic acid (EPA) and decreased DHA levels and protein expression of PPAR-α in the fetal livers of both sexes. DHA increased the gene expression of Ppara, Cpt1, and Acsl1 in the livers of female fetuses. Therefore, an HFD reduces DHA levels and PPAR-α, a master regulator of gene expression, in the fetal liver. In turn, the livers of female fetuses seem to be more sensitive to DHA action.

BACKGROUND: This study aims to identify which risk factors are associated with the appearance of an incisional hernia in a stoma site after its closure. This in the sake of identifying which patients would benefit from a preventative intervention and thus start implementing a cost-effective protocol for prophylactic mesh placement in high-risk patients. METHODS: A systematic review of PubMed, Cochrane library, and ScienceDirect was performed according to PRISMA guidelines. Studies reporting incidence, risk factors, and follow-up time for appearance of incisional hernia after stoma site closure were included. A fixed-effects and random effects models were used to calculate odds ratios' estimates and standardized mean values with their respective grouped 95% confidence interval. This to evaluate the association between possible risk factors and the appearance of incisional hernia after stoma site closure. RESULTS: Seventeen studies totaling 2899 patients were included. Incidence proportion between included studies was of 16.76% (CI95% 12.82; 21.62). Out of the evaluated factors higher BMI (p = 0.0001), presence of parastomal hernia (p = 0.0023), colostomy (p = 0,001), and end stoma (p = 0.0405) were associated with the appearance of incisional hernia in stoma site after stoma closure, while malignant disease (p = 0.0084) and rectum anterior resection (p = 0.0011) were found to be protective factors. CONCLUSIONS: Prophylactic mesh placement should be considered as an effective preventative intervention in high-risk patients (obese patients, patients with parastomal hernia, colostomy, and end stoma patients) with the goal of reducing incisional hernia rates in stoma site after closure while remaining cost-effective.

BACKGROUND: A healthy maternal diet must consider an appropriate supply of long-chain polyunsaturated fatty acids (LCPUFAs) precursors to ensure adequate growth and development of the fetus. In this regard, n-6 PUFAs, predominantly linoleic (C18:2 n-6, LA) and arachidonic acid (C20:4 n-6), have a central role in the development of the central nervous system because they are part of the membrane structure and participate in the metabolism and signal transduction of cells. Nevertheless, they can also be transformed into inflammatory metabolites promoting the pathogenesis of cardiovascular diseases, cancer, and autoimmune or inflammatory conditions. In modern westernized societies, there is a high dietary consumption of foods rich in n-6 PUFAs which could have detrimental consequences for the fetus and neonate due to excessive exposure to these fatty acids (FAs). OBJECTIVE: To summarize the evidence of maternal, placental, and fetal alterations that an excessive intake of n-6 polyunsaturated FAs (PUFAs), LA, and AA), could produce during pregnancy. METHODS: A thorough review of the literature regarding the effects of n-6 PUFAs during pregnancy and lactation including in vivo and in vitro models, was carried out using the PubMed database from the National Library of Medicine-National Institutes of Health. RESULTS: An elevated intake of n-6 PUFA, specifically LA, during pregnancy influences children's motor, cognitive, and verbal development during infancy and early childhood. Similarly, they could harm the placenta and the development of other fetal organs such as the fat tissue, liver, and cardiovascular system. CONCLUSION: Maternal diet, specifically LA intake, could have significant repercussions on fetal development and long-term consequences in the offspring, including the possibility of future metabolic and mental diseases. It would be necessary to focus on the prevention of these alterations through timely dietary interventions in the target population.

Maternal obesity and the imbalance in linoleic acid (C18:2 n-6, LA) and alpha-linolenic acid (C18:3 n-3, ALA) levels are related with hepatic disturbances in the offspring. However, whether these alterations are present during fetal life is not well understood. Obese and normal weight pregnant women were recruited to determine fatty acids (FAs) consumption, FAs profile (in maternal erythrocytes, placenta and neonatal very low-density lipoproteins VLDL) and biomarkers of fetal liver function, such as gamma-glutamyl transferase (GGT), alpha-fetoprotein (AFP) and albumin, in umbilical cord blood. Stearic acid (C18:0, ST) was lower, and total n-3 FAs tended to be lower in umbilical cord VLDLs of obese women compared to controls. Independently of maternal obesity, GGT levels in umbilical cord blood was positively correlated with the LA content and negatively correlated with the ALA content in maternal erythrocytes. We conclude that maternal obesity and its imbalance of LA and ALA are associated with changes in biomarkers of fetal liver function.

SCOPE: Nonalcoholic fatty liver disease (NAFLD) has a high and growing prevalence globally. Mitochondria are fundamental in regulating cell energy homeostasis. Nevertheless, mitochondria control mechanisms can be exceeded in this context of energy overload. Damaged mitochondria worsen NAFLD progression. Diet and lifestyle changes are the main recommendations for NAFLD prevention and treatment. Some polyphenols have improved mitochondrial function in different NAFLD and obesity models. OBJECTIVE: The study aims to discuss the potential role of polyphenols as a nonpharmacological approach targeting mitochondria to prevent and treat NAFLD, analyzing the influence of polyphenols' chemical structure, limitations and clinical projections. METHODS: In vivo and in vitro NAFLD models were considered. Study searches were performed using the following keywords: nonalcoholic fatty liver disease, liver steatosis, mitochondria, mitochondrial activity, mitochondrial dynamics, mitochondrial dysfunction, mitochondrial morphology, mitochondrial cristae, fusion, fission, polyphenols, flavonoids, anthocyanins, AND/OR bioactive compounds. CONCLUSION: Polyphenols are a group of diverse bioactive molecules whose bioactive effects are highly determined by their chemical structure. These bioactive compounds could offer an interesting non-pharmacological approach to preventing and treating NAFLD, regulating mitochondrial dynamics and function. Nevertheless, the mitochondria' role in subjects with NAFLD treatment is not fully elucidated. The dosage and bioavailability of these compounds should be addressed when studied.

Poly(amide-triazole) and poly(ester-triazole) synthesized from d-galactose as a renewable resource were applied for the synthesis of nanoparticles (NPs) by the emulsification/solvent evaporation method. The NPs were characterized as stable, spherical particles, and none of their components, including the stabilizer poly(vinyl alcohol), were cytotoxic for normal rat kidney cells. These NPs proved to be useful for the efficient encapsulation of cilostazol (CLZ), an antiplatelet and vasodilator drug currently used for the treatment of intermittent claudication, which is associated with undesired side-effects. In this context, the nanoencapsulation of CLZ was expected to improve its therapeutic administration. The carbohydrate-derived polymeric NPs were designed taking into account that the triazole rings of the polymer backbone could have attractive interactions with the tetrazole ring of CLZ. The activity of the nanoencapsulated CLZ was measured using a matrix metalloproteinase model in a lipopolysaccharide-induced inflammation system. Interestingly, the encapsulated drug exhibited enhanced anti-inflammatory activity in comparison with the free drug. The results are very promising since the stable, noncytotoxic NP systems efficiently reduced the inflammation response at low CLZ doses. In summary, the NPs were obtained through an innovative methodology that combines a carbohydrate-derived synthetic polymer, designed to interact with the drug, ease of preparation, adequate biological performance, and environmentally friendly production.

Hemochromatosis (HC) is a disorder that alters the body's ability to metabolize iron, increasing its absorption, causing iron overload, and consequently an accumulation of the mineral in multiple organs such as the liver, heart, and pancreas. The amount of total iron in the body is 2-4 g in healthy individuals and remains within these limits throughout life thanks to the control of intestinal absorption. In patients with CH, this amount is increased by at least 10 times, which translates into body deposits of 20-40 grams of iron on average. Factors that increase the risk of having HC: having two copies of the mutated HFE gene, family history, ethnicity or ancestry from Northern Europe (less common in blacks, Hispanics, and Asians), and male gender.

Se presentó un paciente masculino de 8 años, con antecedentes personales de disnea, al que en el examen físico se le detectó soplo corto en foco aórtico accesorio. Se realizó ecocardiograma transtorácico. El síndrome de Marfan es una patología poco común causada por una mutación genética de fibrilina 1, imprescindible para la síntesis de fibras elásticas del tejido conectivo. Se caracteriza por una alta penetrancia y marcada heterogeneidad fenotípica. Entre las diferentes manifestaciones clínicas, la afectación cardiovascular merece una consideración especial. El diagnóstico requiere una evaluación clínica completa de múltiples órganos y sistemas. Por su ampliada sintomatología, la toma de decisiones es compleja, por tanto, cuando se sospeche síndrome de Marfan debe aplicarse la revisión de los criterios de Ghent. Dado el impacto del pronóstico y el manejo, la terapia médica temprana y la intervención quirúrgica oportuna, el paciente mejoró sustancialmente la calidad de vida.
An eight years-old male patient was presented, with a personal history of dyspnea, and who in the physical examination was found, in the Erb´s point, to have a small heart murmur. A transthoracic echocardiogram was performed. Marfan Syndrome is a rare pathology caused by a genetic mutation in fibrillin 1, essential for the synthesis of elastic connective tissue. It is associated with a high penetrance and marked phenotypic heterogeneity. Among the different clinical manifestations, cardiovascular involvement deserves special attention. Diagnosis requires a complete clinical evaluation of multiple organs and systems. Because of its extensive symptomatology, decision making is complex, therefore, when Marfan Syndrome is suspected, the revised Ghent criteria should be applied. Given the impact of prognosis and management, early medical therapy and timely surgical intervention, the patient's quality of life improved substantially.
Paciente do sexo masculino, 8 anos de idade, com história pessoal de dispnéia. Ao exame físico, detectou-se sopro curto em foco aórtico acessório. Foi realizado ecocardiograma transtorácico. A síndrome de Marfan é uma patologia rara causada por uma mutação genética da fibrilina 1, essencial para a síntese das fibras elásticas do tecido conjuntivo. É caracterizada por alta penetrância e marcada heterogeneidade fenotípica. Dentre as diferentes manifestações clínicas, o envolvimento cardiovascular merece consideração especial. O diagnóstico requer uma avaliação clínica completa de múltiplos órgãos e sistemas. Devido aos seus sintomas extensos, a tomada de decisão é complexa, portanto, quando houver suspeita de síndrome de Marfan, a revisão dos critérios de Ghent deve ser aplicada. Dado o impacto do prognóstico e tratamento, terapia médica precoce e intervenção cirúrgica oportuna, a qualidade de vida do paciente melhorou substancialmente.

Introducción: El síndrome de Angelman es un trastorno del neurodesarrollo hereditario poco frecuente que afecta a 1 de cada 10 mil a 24 mil nacimientos. Esta condición incluye retraso del desarrollo, discapacidad intelectual, discapacidad severa para hablar, problemas con el movimiento y el equilibrio (ataxia), epilepsia y cabeza muy pequeña. Las personas con síndrome de Angelman parecen estar siempre de buen humor y sonríen mucho. Objetivo: Sistematizar los conocimientos sobre las características del síndrome de Angelman, los aspectos clínicos y genéticos de la enfermedad y las estrategias de tratamientos actuales. Método: Se realizó una revisión bibliográfica en la Universidad Regional Autónoma de los Andes, mediante la búsqueda en bases de datos tanto nacionales como internacionales, como PubMed, Scopus, SciELO, Web of Science y Google Scholar. Para la investigación se empleó una estrategia de búsqueda. Se encontraron 45 artículos, de los cuales 15 fueron seleccionados para esta revisión. Resultados: Se elaboró un texto sintetizado donde se abordaron aspectos tales como: etiología, diagnóstico, principales síntomas clínicos y tratamiento de este trastorno genético. Conclusiones: Por su naturaleza de necesidades clínicas que no son satisfechas en cuanto al área motora, la comunicación, el sueño y el comportamiento, el síndrome de Angelman hace necesario que los profesionales de enfermería desarrollen un plan de acción que permita un diagnóstico precoz y desarrollen un plan de cuidados específico para el individuo y el entorno íntimo de actuación para responder a las necesidades a demanda.
Introduction: Angelman Syndrome is a rare inherited neurodevelopmental disorder that affects 1 in 10,000 to 24,000 newborns. This condition includes developmental desability, intellectual disability, severe speech disability, movement and balance problems (ataxia), seizures and very small head. People with Angelman Syndrome always seem to be in a good mood and smile a lot. Objective: To systematize knowledge about the characteristics of Angelman Syndrome, clinical and genetic aspects of the disease and current treatment strategies. Method: A bibliographic review was carried out at the Universidad Regional Autónoma de los Andes, by searching in national and international databases such as PubMed, Scopus, SciELO, Web of Science and Google Scholar. For the investigation a search strategy was used. Forty-five articles were found, of which 15 were selected for this review. Results: A synthesized text was elaborated where aspects such as etiology, diagnosis, main clinical symptoms and treatment of this genetic disorder were addressed. Conclusions: Due to its nature of clinical needs that are not met in terms of the motor area, communication, sleep and behavior, Angelman syndrome makes it necessary for nursing professionals to develop an action plan that allows an early diagnosis and develop a plan specific care for the individual and the intimate environment of action to respond to the needs on demand.

Essa condição inclui atraso no desenvolvimento, deficiência intelectual, deficiência grave da fala, problemas de movimento e equilíbrio (ataxia), epilepsia e cabeça muito pequena. Pessoas com síndrome de Angelman parecem estar sempre de bom humor e sorrir muito. Objetivo: Sistematizar o conhecimento sobre as características da síndrome de Angelman, os aspectos clínicos e genéticos da doença e as estratégias atuais de tratamento. Método: Foi realizada revisão bibliográfica na Universidad Regional Autónoma de los Andes, por meio de busca em bases de dados nacionais e internacionais, como PubMed, Scopus, SciELO, Web of Science e Google Acadêmico. Para a investigação, foi utilizada uma estratégia de busca. Foram encontrados 45 artigos, dos quais 15 foram selecionados para esta revisão. Resultados: Foi elaborado um texto sintetizado onde foram abordados aspectos como: etiologia, diagnóstico, principais sintomas clínicos e tratamento desta doença genética. Conclusões: Devido à sua natureza de necessidades clínicas não satisfeitas ao nível da área motora, comunicação, sono e comportamento, a síndrome de Angelman torna necessário que os profissionais de enfermagem desenvolvam um plano de ação que permita o diagnóstico precoce e desenvolvam um plano de cuidados específico para o indivíduo e o ambiente íntimo de ação para responder às necessidades sob demanda.

Objectives: Describe the use and findings of cardiopulmonary imaging-chest X-ray (cX-ray), echocardiography (cEcho), chest CT (cCT), lung ultrasound (LUS), and/or cardiac magnetic resonance imaging (cMRI)-in COVID-19 hospitalizations in Latin America (LATAM). Background: There is a lack of information on the images used and their findings during the SARS-CoV-2 pandemic in LATAM. Methods: Multicenter, prospective, observational study of COVID-19 inpatients, conducted from March to December 2020, from 12 high-complexity centers, in nine LATAM countries. Adults (>18 years) with at least one imaging modality performed, followed from admission until discharge and/or in-hospital death, were included. Results: We studied 1,435 hospitalized patients (64% males) with a median age of 58 years classified into three regions: Mexico (Mx), 262; Central America and Caribbean (CAC), 428; and South America (SAm), 745. More frequent comorbidities were overweight/obesity, hypertension, and diabetes. During hospitalization, 58% were admitted to the ICU. The in-hospital mortality was 28%, and it was highest in Mx (37%).The most frequent images performed were cCT (61%), mostly in Mx and SAm, and cX-ray (46%), significant in CAC. The cEcho was carried out in 18%, similarly among regions, and LUS was carried out in 7%, with a higher frequently in Mx. Abnormal findings on the cX-ray were peripheral or basal infiltrates, and in cCT abnormal findings were the ground glass infiltrates, more commonly in Mx. In LUS, interstitial syndrome was the most abnormal finding, predominantly in Mx and CAC.Renal failure was the most prevalent complication (20%), predominant in Mx and SAm. Heart failure developed in 13%, predominant in Mx and CAC. Lung thromboembolism was higher in Mx while myocardial infarction was in CAC.Logistic regression showed associations of abnormal imaging findings and their severity, with comorbidities, complications, and evolution. Conclusions: The use and findings of cardiopulmonary imaging in LATAM varied between regions and had a great impact on diagnosis and prognosis.

BACKGROUND: Multiple dietary patterns have been recommended by the 2015-2020 Dietary Guidelines for Americans for the prevention of cardiovascular disease (CVD). The adherence to these patterns and its relation with risk of CVD remain unclear in the US Hispanic/Latino population. OBJECTIVES: We aimed to evaluate 3 healthy eating patterns measured by 3 dietary pattern scores [the Alternate Mediterranean diet (aMED), the Healthy Eating Index (HEI)-2015, and the healthful Plant-based Diet Index (hPDI)] across different Hispanic/Latino backgrounds and generations. We further examined the associations of these dietary scores with incident CVD in US Hispanics/Latinos. METHODS: We included 10,293 adult participants of US Hispanics/Latinos of 6 backgrounds (Mexican, Puerto Rican, Cuban, Dominican, Central American, and South American), free of CVD or cancer at baseline, in the Hispanic Community Health Study/Study of Latinos. Dietary pattern scores were derived at the baseline visit using two 24-h dietary recalls. The primary outcome was major incident CVD (n = 232), comprised of coronary heart disease and stroke, during an average 6-y follow-up. RESULTS: Mean levels of all 3 dietary scores were significantly different across the 6 Hispanic/Latino background groups (all P < 0.001), with the highest (i.e., healthiest) in those of Mexican background and lowest in those of Puerto Rican background. Compared with non-mainland-US-born Hispanics/Latinos, mainland-US-born Hispanics/Latinos had significantly lower dietary scores (P < 0.001). Differences in dietary scores between mainland-US-born and non-mainland-US-born Hispanics/Latinos were majorly driven by differences in dietary intakes of healthy plant-based foods. After adjusting for multiple covariates, significantly lower risk ratios (95% CI) of CVD were observed for 1-SD increments of the dietary scores, with 0.74 (0.60, 0.91) for aMED, 0.80 (0.63, 1.00) for HEI-2015, and 0.74 (0.60, 0.93) for hPDI. CONCLUSIONS: Although adherence to healthy eating patterns varied by Hispanic/Latino backgrounds and generations, greater adherence to these eating patterns was associated with lower risk of CVD across diverse US Hispanics/Latinos.

Children and adolescents with physical disabilities have motor and social-emotional challenges that interfere with their health-related quality of life and put them at greater risk of developing secondary conditions. Moreover, services that provide them therapies are more likely to be restricted, especially for the low-income population. There must be broader actions towards health promotion, offering not only means for physical habilitation and rehabilitation but for social and emotional improvements as well. This goal is attainable by adaptive sports and recreational activities where physical conditioning is accompanied by an improvement in self-esteem and social benefits. With the COVID-19 pandemic and social isolation, children and adolescents with physical disabilities were even further deprived of assistance. Our aim was to report the efforts of a non-governmental sports organization in maintaining physical and psychological care through virtual consultations and to analyze the perceptions of those affected by the process. Crianças e adolescentes com deficiência física apresentam desafios motores e socioemocionais que interferem na qualidade de vida relacionada à saúde e os colocam em maior risco de desenvolver doenças secundárias. Além disso, estão mais sujeitos a restrições de serviços adequados que ofereçam terapias, principalmente para a população de baixa renda social. Torna-se necessário oferecer ações voltadas para a promoção da saúde em um sentido mais amplo, oferecendo não só meios de habilitação e reabilitação física, mas também de ganhos sociais e emocionais. Este objetivo pode ser atingido com esportes adaptados e atividades recreativas nos quais o condicionamento físico é acompanhado por ganhos em autoestima e benefícios sociais. Com a pandemia de COVID-19 e o isolamento social, crianças e adolescentes com deficiência física ficaram ainda mais privados de assistência. Relatamos aqui o esforço de uma organização não-governamental esportiva em manter o atendimento físico e psicológico por meio de consultas virtuais, e avaliamos as percepções das partes interessadas no processo. Rodent models in rats, mice, and guinea pigs have been extremely helpful to gain insight into pregnancy physiology and pathologies-related. Moreover, they have allowed understanding the mechanism that links an adverse intrauterine environment with the origin of adult disease. In this regard, the effects of diverse maternal conditions, such as undernutrition, obesity, hypoxia, and hyperandrogenism on placental function and its long-term consequences for the offspring, have been widely analyzed through rodents models involving dietary manipulations, modifications in environmental oxygen, surgical and pharmacological procedures that reduce uteroplacental blood flow and administrations of exogenous testosterone and dihydrotestosterone (DHT) mimicking maternal androgen excess. Both in human and in rodent models, these interventions induce modifications of placental morphology, transport of glucose, amino acid, and fatty acids, steroid synthesis, and signaling pathways control placental function. These changes are associated with the increase of pro-inflammatory and oxidative stress markers. For its part, offspring exhibit alterations in organs involved in metabolic control such as the hypothalamus, adipose tissue, liver, skeletal muscle, and pancreas altering the intake and preferences for certain foods, the metabolism of glucose and lipid, and hormonal function leading to fat accumulation, insulin resistance, fatty liver, dyslipidemia, and elevated glucose levels. Therefore, the present review discusses the evidence emerging from rodent models that relate maternal nutrition, hypoxia, and androgen exposure to the maternal mechanisms that lead to fetal programming and their metabolic consequences in postnatal life.
RESUMO
BACKGROUND: Non-alcoholic fatty liver disease (NAFLD) is characterized by hepatic accretion of triacylglycerides in the absence of alcohol intake that may progress to steatohepatitis, fibrosis and cirrhosis, becoming the main cause of chronic liver disease. This article discusses recent data concerning the use of dietary polyphenols in the prevention and treatment of NAFLD in vitro, in vivo, and in clinical trials. METHODS: Study searches were performed using the PubMed database from the National Library of Medicine-National Institutes of Health. RESULTS: Polyphenols exert beneficial effects in NAFLD, with positive outcomes being related to body weight gain, insulin resistance, liver fat accumulation, oxidative stress, proinflammatory status, mitochondrial dysfunction and ER stress. Data reported for hydroxytyrosol suggest that the activation of the hepatic PPAR-α-FGF21-AMPK-PGC-1α signaling cascade is associated with fatty acid oxidation enhancement, de novo lipogenesis diminution and recovery of mitochondrial function, a contention that is supported by the actions of several polyphenols on specific components of this signaling pathway. Besides, polyphenols downregulate NF-κB, suppressing the pro-inflammatory state developed in NAFLD and upregulate liver Nrf2, increasing the cellular antioxidant potential. The latter feature of polyphenols is contributed by chelation of pro-oxidant trace elements, reduction of free radicals to stable forms and inhibition of free radical generating systems. CONCLUSION: Polyphenols are relevant bioactive compounds in terms of prevention and treatment of NAFLD, which exhibit low bioavailability and instability in biological systems that could limit their health effects. These drawbacks reinforce the necessity of further studies to improve the efficacy of polyphenol formulations for human interventions.

La salud ambiental es un concepto multifactorial, desarrollado por el logro interdisciplinario nacido a la luz de las comprensiones multidinámicas de sus factores determinantes. Estos aspectos que le conforman, hacen del concepto de salud ambiental una combinación plurifactorial de saberes desarrollados en el tiempo y que son la base de esta construcción conceptual, enriquecida a la luz de la comprensión del hombre, de sus interrelaciones con el medio ambiente y su papel determinante en la preservación del mismo, para beneficio de las presentes y futuras generación. Esta evaluación crítica de nuestro papel como especie en el control y cuidado del medio ambiente, ha favorecido la toma de consciencia de que nuestras acciones son determinantes en resarcir el daño realizado, donde por factores como la contaminación del aire, el agua, la deposición de residuos peligrosos, entre otros, está causando un daño y afectando sensiblemente la salud de la población, con repercusiones a nivel mundial. Por otra parte, esta toma de consciencia está generando acciones muy positivas en pro del ambiente, dando el debido reconocimiento constitucional que la Madre Tierra se merece, siendo reconocida en la Constitución de la República del Ecuador como un Sujeto de Derecho y no como un Objeto, como una contribución de los países Latinoamericanos a las profundas transformaciones que en materia de salud ambiental, requiere el mundo actual, en beneficio directo de la salud pública del planeta y todos los seres vivos que en el habitan(AU)
Environmental health is a multifactorial concept, developed by interdisciplinary achievement born in light of multidynamic understandings of its determining factors. These aspects that make it up, make the concept of environmental health a multifactorial combination of knowledge developed over time and that are the basis of this conceptual construction, enriched in the light of the understanding of man, his interrelationships with the environment and his determining role in its preservation, for the benefit of present and future generations. This critical evaluation of our role as a species in the control and care of the environment has favored the awareness that our actions are decisive in compensating for the damage done, where factors such as air pollution, water, the deposition of hazardous waste, among others, is causing damage and significantly affecting the health of the population, with repercussions worldwide. On the other hand, this awareness is generating very positive actions in favor of the environment, giving due constitutional recognition that Mother Earth deserves, being recognized in the constitution of the Republic of Ecuador as a Subject of Law and not as an Object. , as a contribution of the Latin American countries to the profound transformations that the current world requires in terms of environmental health, for the direct benefit of the public health of the planet and all the living beings that inhabit it(AU)

Los microorganismos son seres vivos, que han ido adaptándose y evolucionando permitiendo su supervivencia, evadiendo las respuesta innata de los hospedadores e incluso modificando sus estructuras biológicas antes moléculas farmacológicas, que se han, en los últimos años, empleado de manera inadecuadas, bien sea por abuso o por deficiencia en el cumplimiento del esquema del tratamiento, o desaciertos para tratar un germen en específico y, como consecuencia, la actual crisis mundial por la resistencia a los antimicrobianos; considerado un problema de salud pública, requiere de esfuerzos multisectoriales e integrales que aporten soluciones mediatas, a corto y mediano plazo, sostenibles en el tiempo. Para ellos, es clave la formación de los nuevos profesionales, en el área de microbiología, de las diferentes carreras biomédicas. Es por ello, el presente estudio identificó las competencias que deben desarrollar los estudiantes, de seis universidades ecuatorianas. Se aplicaron cuestionarios, inquiriendo la importancia atribuida y el grado de desarrollo percibido de cada competencia mediante escala tipo Likert, además de seis dimensiones distribuidas en 40 indicadores. Se identificó mayor desarrollo de competencias en la identificación del patógeno, diagnóstico y análisis e interpretación de los resultados; además las seis dimensiones educativas fueron clasificadas como relevantes. Sin embargo, los resultados sugieren falencia en el desarrollo de competencias en la temática de resistencia antimicrobiana, por lo que se sugiere la revisión de los contenidos programáticos y aplicar nuevas herramientas pedagógicas que permitan al estudiantado aprender a aprender, aplicando un modelo socio-critico, y como base la investigación y el conocimiento científico actualizado(AU)
Microorganisms are living beings, which have been adapting and evolving allowing their survival, evading the innate response of the hosts and even modifying their biological structures before pharmacological molecules, which have, in recent years, been used inappropriately, either by abuse or deficiency in compliance with the treatment scheme, or failure to treat a specific germ and, as a consequence, the current global crisis due to antimicrobial resistance; considered a public health problem, it requires multisectoral and comprehensive efforts that provide immediate, short- and medium-term solutions that are sustainable over time. For them, the training of new professionals in the area of microbiology, of the different biomedical careers, is key. For this reason, this study identified the skills that students from six Ecuadorian universities should develop. Questionnaires were applied, inquiring about the importance attributed and the perceived degree of development of each competency using a Likert-type scale, in addition to six dimensions distributed in 40 indicators. Greater development of skills in the identification of the pathogen, diagnosis and analysis and interpretation of the results was identified; In addition, the six educational dimensions were classified as relevant. However, the results suggest a lack in the development of skills in the subject of antimicrobial resistance, so it is suggested to review the program contents and apply new pedagogical tools that allow students to learn to learn, applying a socio-critical model, and as a basis research and updated scientific knowledge(AU)
