RESUMO

Importance: People with serious mental illness (SMI), defined as a diagnosis of schizophrenia spectrum disorder, bipolar disorder, or disabling major depressive disorder) die approximately 10 to 25 years earlier than the general population. Objective: To develop the first-ever lived experience-led research agenda to address early mortality in people with SMI. Evidence Review: A virtual 2-day roundtable comprising 40 individuals convened on May 24 and May 26, 2022, and used a virtual Delphi method to arrive at expert group consensus. Participants responded to 6 rounds of virtual Delphi discussion via email that prioritized research topics and agreement on recommendations. The roundtable was composed of individuals with lived experience of mental health and/or substance misuse, peer support specialists, recovery coaches, parents and caregivers of people with SMI, researchers and clinician-scientists with and without lived experience, policy makers, and patient-led organizations. Twenty-two of 28 (78.6%) of the authors who provided data represented people with lived experiences. Roundtable members were selected by reviewing the peer-reviewed and gray literature on early mortality and SMI, direct email, and snowball sampling. Findings: The following recommendations are presented in order of priority as identified by the roundtable participants: (1) improve the empirical understanding of the direct and indirect social and biological contributions of trauma on morbidity and early mortality; (2) advance the role of family, extended families, and informal supporters; (3) recognize the importance of co-occurring disorders and early mortality; (4) redefine clinical education to reduce stigma and support clinicians through technological advancements to improve diagnostic accuracy; (5) examine outcomes meaningful to people with an SMI diagnosis, such as loneliness and sense of belonging, and stigma and their complex relationship with early mortality; (6) advance the science of pharmaceuticals, drug discovery, and choice in medication use; (7) use precision medicine to inform treatment; and (8) redefine the terms system literacy and health literacy. Conclusions and Relevance: The recommendations of this roundtable are a starting point for changing practice and highlighting lived experience-led research priorities as an option to move the field forward.

Introducción: Los síndromes craneofaciales congénitos han sido ampliamente descritos debido a su naturaleza precursora de alteraciones en el sistema cráneo-cérvico-mandibular; específicamente las malformaciones a nivel cóndilo-mandibular, como la hipoplasia o aplasia de estas estructuras, comprometen una de las funciones vitales para el ser humano: la alimentación. Con el objetivo de establecer una guía de práctica clínica, se proponen lineamientos que promuevan la práctica fonoaudiológica basada en la evidencia. Pacientes y métodos: Se realiza una revisión sistemática de tipo scoping review en las principales bases de datos biomédicas, en búsqueda de artículos científicos dirigidos a la evaluación de la alimentación y deglución en lactantes menores con hipoplasia y aplasia cóndilo-mandibular asociadas a síndromes craneofaciales. Resultados:Se seleccionaron 37 artículos de tipo ensayo clínico aleatorizado, estudio de casos, estudio ecológico, metaanálisis y revisión sistemática, cuyo contenido es presentado y organizado dentro de un formato de evaluación que busca la obtención de información anatomofuncional de los lactantes menores para el desarrollo de su evaluación clínica. Conclusiones: Se resalta una reducida cantidad y difusión de evidencia fonoaudiológica al aplicar los parámetros de búsqueda, encontrando que los criterios de evaluación específicos ante esta patología no difieren en gran medida a los de la población pediátrica en general.(AU)


Introduction: Congenital craniofacial syndromes have been widely described due to their precursor nature of alterations in the cranio-cervico-mandibular system; specifically, condyle-mandibular malformations such as hypoplasia or aplasia of these structures, compromise one of the vital functions for humans, nutrition. In order to establish a clinical practice guide, guidelines that promote evidence-based speech therapy practice are proposed. Patients and methods: A scoping review is carried out in the main biomedical databases, in search of scientific articles aimed at evaluating feeding and swallowing in newborn and young infants with condyle-mandibular hypoplasia and aplasia associated with craniofacial syndromes. Results: Thirty-seven articles of a randomized clinical trial, case study, ecological study, meta-analysis, and systematic review were selected, this content is presented and organized using an evaluation format that seeks to obtain anatomical and functional information on newborn and young infants for the development of a clinical assessment. Conclusions: A reduced amount and dissemination of speech therapy evidence is highlighted when applying the search parameters, finding that the specific evaluation criteria for this pathology do not differ greatly from those of the pediatric population in general.(AU)

Background: The current United States asthma management guidelines recommend usage of the Air Quality Index (AQI) for outdoor activity modification when air pollution is high. Little is known about parental knowledge and usage of air quality including the AQI in managing childhood asthma. Methods: Forty parents (or legal guardians) of children with persistent asthma completed a questionnaire designed to assess 4 areas related to outdoor air pollution: awareness, perception, behavioral modification, and prior healthcare provider discussion. Descriptive statistics were obtained and Fisher's exact test was used for analysis of behavioral change by selected variables. Results: Almost all parents reported awareness of air quality alerts or AQI, however, only 20% checked the AQI on the AirNow app or website. Most parents reported air pollution as a trigger (65%), yet few parents reported behavioral modification of their child's outdoor activity based on the perception of poor air quality (43%) or based on AQI or alerts (40%). Over half of parents reported a healthcare provider had ever discussed air pollution as a trigger, with few parents (23%) reporting recommendations for behavior change. Perception of air pollution as a trigger, healthcare provider discussion and recommendations, and usage of AirNow were associated with increased reported activity change. Conclusion: Healthcare providers should discuss outdoor air pollution during asthma management in children and should discuss AirNow as a source for AQI information and behavioral recommendations.

There is substantial epidemiological and experimental evidence of an obesity-related asthma phenotype. Compared to children of healthy weight, children with obesity are at higher risk of asthma. Children with obesity who have asthma have greater severity and poorer control of their asthma symptoms, more frequent asthma exacerbations, and overall lower asthma-related quality of life than children with asthma who have a healthy weight. In this Review, we examine some of the latest evidence on the characteristics of this phenotype and its main underlying mechanisms, including genetics and genomics, changes in airway mechanics and lung function, sex hormone differences, alterations in immune responses, systemic and airway inflammation, metabolic dysregulation, and modifications in the microbiome. We also review current recommendations for the treatment of these children, including in the management of their asthma, and current evidence for weight loss interventions. We then discuss initial evidence for potential novel therapeutic approaches, such as dietary modifications and supplements, antidiabetic medications, and statins. Finally, we identify knowledge gaps and future directions to improve our understanding of asthma in children with obesity, and to improve outcomes in these susceptible children. We highlight important needs, such as designing paediatric-specific studies, implementing large multicentric trials with standardised interventions and outcomes, and including racial and ethnic groups along with other under-represented populations that are particularly affected by obesity and asthma.

BACKGROUND: Poor diet quality may contribute to the disproportionate asthma burden in Puerto Rican youth. OBJECTIVE: To examine whether an unhealthy diet at one or two study visits conducted over about 5 years was associated with asthma, severe asthma exacerbations, and worse lung function in Puerto Rican youth. METHODS: This was a prospective study of 406 Puerto Rican youth aged 6 to 14 years at a baseline visit and 9 to 20 years at a follow-up visit. As in prior work, diet was assessed using a dietary score ranging from -2 to +2. The exposure of interest was an unhealthy diet, defined as a nonpositive dietary score (0 to -2) at one or both visits. Outcomes of interest were asthma (defined as physician-diagnosed asthma and one of more episode of wheeze in the year before the second visit), one or more severe asthma exacerbation in the year before the second visit, and change in percent predicted lung function measures (FEV1, FVC, and FEV1/FVC) between the first and second visits. RESULTS: In a multivariable analysis, an unhealthy diet at both visits was associated with increased odds of asthma (adjusted odds ratio = 3.38; 95% confidence interval, 1.74-6.57) and severe asthma exacerbations (adjusted odds ratio = 2.65; 95% confidence interval, 1.16-6.03), but not with change in lung function. CONCLUSIONS: An unhealthy diet at both visits was associated with increased odds of asthma and severe asthma exacerbations, compared with a healthy diet at both visits. Our findings support health policies promoting a healthy diet in Puerto Rican youth, a population at high risk for asthma.

RESUMO

Isolated endobronchial inflammatory myofibroblastic tumors (IMT) are rare, accounting for about 1% of primary endobronchial tumors in children. The mainstay of treatment for this tumor has been surgical resection. Recently, the identification of anaplastic lymphoma kinase (ALK) gene mutations in half of IMTs and promising results of treatment with ALK inhibitors in other ALK-positive tumors have opened the possibility of alternative approaches. We present a 4-year-old child with an ALK-positive endobronchial IMT, treated with endoscopic resection and neoadjuvant therapy with crizotinib, without evidence of tumor recurrence 2 years after the initial resection.

Resumen El suicidio en población privada de la libertad (PPL) en centros de reclusión de la ciudad de Bogotá, por la comisión de diferentes delitos, trae consigo una serie de preguntas y cuestionamientos que generan inquietud, duda y en ocasiones temor. El objetivo del estudio fue establecer el estado del arte de los estudios realizados y las tendencias halladas sobre conducta suicida en esta población. En esta investigación se encontrarán los principales estudios, investigaciones y documentos tecnicos realizados por funcionarios y académicos, recopilados en trece documentos. Se pudo establecer que son pocos los documentos encontrados (13), pocas las investigaciones y en su mayoría de tipo descriptivo.


Abstract The suicide in the population deprived of freedom (PPL, for its initials in Spanish) in prisons in the city of Bogotá, for the commission of different crimes, brings with it a series of questions and issues that generate concern, doubt and sometimes fear. The purpose of the study was to establish the state of the art of studies carried out and trends found on suicidal behavior in this population. In this research we will find the main studies, research and technical documents made by officials and academics, compiled in thirteen documents. It could be established that there are few documents found (13), few investigations and most of them are descriptive.
