OBJECTIVE: To analyze the long-term postoperative outcomes in patients with cicatricial tracheal stenosis and to determine the indications for various surgical strategies. MATERIAL AND METHODS: There were 976 patients with benign cicatricial tracheal stenosis for the period 2001-2017. Tracheal stenosis occurred after mechanical ventilation and tracheostomy in 910 (93.2%) patients. Other causes were neck trauma, burns, previous surgery or tuberculosis. Idiopathic stenosis was observed in 41 (4.2%) patients. Multiple-stage reconstructive treatment was possible due to benign nature of disease. There were 2.4 operations per a patient, and 976 patients underwent 2327 procedures. Circular tracheal resection was preferred (n=396). RESULTS: Surgical complications occurred in 107 (4.6%) cases, mortality rate - 0.3%. In long-term period, 42 patients died for various causes. In most cases (n=34, 80.9%), mortality was associated with concomitant diseases or consequences of trauma rather cicatricial tracheal stenosis or its treatment. Eight patients died from cicatricial tracheal stenosis or its treatment (7 patients after staged repair, 1 after circular tracheal resection). Four patients died due to asphyxia following T-tube obturation with a tracheobronchial secret or unjustified decannulation. For various reasons, 41 (6.2%) patients continued their treatment in other hospitals (4 patients died). Mortality rate in this group was 9.8%. Favorable long-term outcome was observed in 90.1% of patients, good and unsatisfactory results - in 7.2% and 1.8% of patients, respectively. Circular tracheal resection ensured better functional outcome. CONCLUSION: Surgical treatment of cicatricial tracheal stenosis is associated with low incidence of postoperative complications and mortality. However, further improvement in long-term results is associated with advanced rehabilitation programs for concomitant diseases. Treatment of cicatricial tracheal stenosis should be carried out at specialized hospitals.

557 case records of patients with late postoperative purulent complications (LPC), admitted to department of purulent infection during 10 years were studied (4% of total number of operated patients). The mean duration of hospitalization period of rehospitalized patients made up 14.2 bed days. Mortality rate was 0.4%. 70% of patients with LPC were people of working age. 3 groups of patients were singled out regarding the time after the operation until readmission: group 1 (38.5%) 1 month since the discharge of the patients; group 2 (31.5%) up to 12 months; group 3 (30%) more than 1 year after. In group 1 purulent infection of the wounds predominated, in group 2--infiltrates and mattery fistulas, in group 3--ligature-fistulas, infiltrates of abdominal cavity. LPC after appendectomy was observed in 53% of cases. Among complications purulent infection predominated (60%). 75.8% of patients with LPC needed surgery.

RESUMO

Precision termino-lateral pancreatico-jejunal anastomosis with or without mobilization of the pancreatic duct, with the use of inert suture materials in pancreatoduodenal resection was conducted in 21 patients, 3 of them died. Incompetence of the anastomoses was not encountered. The number of patients treated by radical operation increased almost 2.5 fold, and was 27.4%. The authors consider inclusion of the stump of the gland into digestion together with other conservative and technical measures to be most promising trend in the prevention of incompetence of the anastomoses and postoperative pancreatitis, in improvement of the results of pancreatoduodenal resection, and in expanding the scope of its use.

RESUMO

It was shown in experiments on female rats that sigetin can be used in the treatment of the disturbed sex behavior pattern that occurred because of impaired differentiation of the hypothalamic sex centres in the crucial period of early ontogenesis. Sigetin did not produce any similar effects on intact females. Potential clinical use of sigetin for the treatment of certain groups of sexologic patients is discussed.

RESUMO

About 40 per cent of the uterine preplacental vessels were ligated in rabbits on the 11th or 18th days of gestation. Some of the female rabbits operated upon were given 5 mg of intravenous sigetin twice a day till the end of pregnancy. Investigation of the progeny development, the ability of learning at an age of about 6 months, in particular, showed a somewhat better development and ability of learning in the offsprings of female rabbits receiving sigetin.

RESUMO

Sygethin, an estrogen-like drug, was introduced to rats of the Wistar line during critical periods marking differentiation of hypothalamic sex centers, initial development and differentiation of the mammary glands as well as the time when administration of estrogens caused irreversible changes in the testes. No deviations in the development of test animals similar to those produced by estrogens were disclosed.
