RESUMO

This study tested the generality of Problem Behavior Theory (PBT) in explaining adolescents' problem behavior in Iran. Data were collected from 392 adolescents (Mage = 15.97, SD = 1.12, 55.4% girls) who completed the Adolescent Health and Development Questionnaire (AHDQ) to assess the individual vulnerability, opportunity risk availability, perceived support, and delinquent behaviors. Results indicated that individual vulnerability and opportunity risk availability had a significant relationship with delinquent behaviors and a significant interaction with perceived support in their influence on delinquent behaviors. Further, perceived support was negatively associated with delinquent behaviors. Our results were consistent with PBT's explanatory model for adolescents' problem behavior in Western countries and are informative about problem behavior involvement among Iranian adolescents and the design of interventions.

Shift work is an inconsistent and atypical work schedule. This study aimed to investigate the influence of shift work on the Quality of Sleep (QOS) and Executive Functions (EF). Thirty shift workers and thirty day workers first completed a demographic questionnaire. They then were tested using the Pittsburgh Sleep Quality Index (PSQI) and EF tests, including the Corsi Block-Tapping Task (CBTT), Berg's Card Sorting Task (BCST), and the Continuous Performance Task (CPT). Results were subjected to non-parametric Chi-Square Tests, the Mann-Whitney U Test, and Independent T-Tests. Shift workers had significantly poorer sleep quality than day workers, which was shown in PSQI global scale (p = 0.001), sleep duration (p = 0.042), habitual sleep efficiency (p = 0.021), and sleep disturbance (p = 0.021). Concerning EF tests, shift workers performed significantly poorer on CBTT (p = 0.019) and BCST (p = 0.015, 0.047) compared with day workers. Significant differences were also observed between shift workers and day workers in terms of variables of omission errors (p = 0.037) and commission errors (p = 0.041) on CPT, but no significant difference was found between shift workers and day workers in reaction time (p = 0.561). Shift work impaired EF. These findings are related to shift workers' poorer sleep and its detrimental effects on areas of the brain, which are critical for EF, such as the prefrontal area. Our results suggest the evaluation and implication of practices and policies to assuage the consequences of working in shifts.

RESUMO

The Wish to be Dead Scale (WDS) was administered to a convenience sample of 200 Iranian psychiatric outpatients. Using a Principal Component Analysis, two factors were identified, labeled Lack of purpose in life (F1), and Lack of interest in living (F2). The WDS had good reliability and significant positive correlations with scores on the Beck Suicide Ideation Scale and with other measures of mental ill-health. This study provides evidence of the usefulness of the WDS for assessing psychiatric patients.

The Wish to be Dead Scale (WDS) is a new scale to measure precursors to suicidal ideation, and the aim of the present study was to examine the psychometric characteristics of a Farsi version of the WDS. The sample was a convenience sample of 145 Iranian female undergraduates and postgraduates selected from different faculties at Iran University of Medical Sciences, Iran. Using a principal component analysis and a varimax rotation with Kaiser normalization, three factors were identified and labeled: (a) lack of purpose and usefulness in life, (b) lack of interest in living, and (c) fantasizing about being dead. The WDS had good inter-item and test-retest reliability and significant positive correlations with scores on the Kessler Psychological Distress Scale-10 and the Rosenberg Self-Esteem Scale, and negative correlations with scores on the Adult Hope Scale, the Satisfaction with Life Scale, the General Self-Efficacy Scale, the Love of Life Scale, the Life Orientation Test, and the Oxford Happiness Questionnaire. We conclude that the WDS may prove to be useful in clinical practice and research into suicide.

The aim of this study was to describe the psychometric properties of the Farsi Form of the Arabic Scale of Death Anxiety (ASDA). The original scale was first translated into Farsi by language experts using the back translation procedure and then administered to a total of 252 Iranian college students and 52 psychiatric outpatients from psychiatric and psychological clinics. The one-week test-retest reliability of the Farsi version in a sample of college students was 0.78, indicating good temporal stability and corroborating the trait-like nature of scores. Cronbach's α was 0.90 for the college students and 0.92 for the psychiatric outpatients, indicating high internal consistency. Scale scores correlated 0.46 with Death Obsession Scale scores, 0.56 with Death Depression Scale scores, 0.41 with Death Anxiety Scale scores, and 0.40 with Wish to be Dead Scale scores, indicating good construct and criterion-related validity. A principal component analysis with a Varimax rotation yielded four factors in the sample of Iranian college students, indicating a lack of homogeneity in the content of the scale. Male students obtained a significant higher mean score than did females. It was concluded that the Farsi ASDA had good internal consistency, temporal stability, criterion-related validity, and a factor structure reflecting important features of death anxiety. In general, the Farsi ASDA could be recommended for use in research on death anxiety among Iranian college students and psychiatric outpatients.

BACKGROUND: In response to high burden of opioid abuse in Iran, Ministry of Health has launched a large-scale opioid maintenance treatment program, delivered through a network of certified drug treatment centers. To promote opioid pharmacotherapies, there is an urgent need to develop and introduce evidence-based psychosocial interventions into the network. PATIENTS AND METHODS: This is a randomized clinical trial (RCT) to investigate feasibility and effectiveness of adding mindfulness-based group therapy to opioid pharmacotherapies as compared to opioid pharmacotherapies alone. The primary outcomes were treatment retention and percentage of weekly morphine, methamphetamine, and benzodiazepine negative tests. DISCUSSION: This is the first RCT that explores the effectiveness of mindfulness-based relapse prevention group therapy among opioid dependent clients in Iran. The feasibility of group therapy and comparison of outcomes in intervention and control groups should be discussed in the outcome article.

BACKGROUND: A few studies on short-term psychodynamic approach have been conducted on social phobia. OBJECTIVES: In this study, the effectiveness of short-term psychodynamic psychotherapy on the treatment of social phobia has been compared to the effectiveness of sertraline and waiting list. MATERIALS AND METHODS: In this randomized-controlled trial study, 13 male students were treated with short-term dynamic psychotherapy (McCullough method) lasting 25 sessions, 11 students received sertraline for 12 weeks, and 14 students, as the waiting list, received no intervention for 8 weeks. Participants completed the Social Phobia Inventory (SPIN) as primary efficacy variable 4 times, and were rated with Clinical Global Impression scale (CGI) and Global Assessment of Functioning (GAF) as secondary efficacy variables. The data were analyzed with analysis of variance (ANOVA), analysis of covariance (ANCOVA), general linear model repeated measures analysis of variance and Fisher exact test. RESULTS: ANCOVA showed significant differences between groups based on SPIN scores (F = 23.51, Sig. = 0.001) and Bonferroni test, as post hoc compression, showed means of both short-term dynamic therapy and sertraline therapy groups were significantly different from waiting list mean (STDP-WL: [Formula: see text] = 15.76, Sig. = 0.001), (MED-WL: [Formula: see text] = 15.91, Sig. = 0.001). Mean of SPIN scores was not significantly different between short-term dynamic psychotherapy and pharmacotherapy groups. In both treatment groups, means of SPIN scores significantly decreased in posttest, but not in waiting. These results repeated with GAF and CGI scores. CONCLUSIONS: The results indicated that short-term dynamic psychotherapy sertraline are effective in decreasing social phobia symptoms and were superior to control group.

OBJECTIVE: This study investigated the effectiveness of mindfulness-based group therapy (MBGT) compared to the usual opioid dependence treatment (TAU).Thirty outpatients meeting the DSM-IV-TR criteria for opioid dependence from Iranian National Center for Addiction Studies (INCAS) were randomly assigned into experimental (Mindfulness-Based Group Therapy) and control groups (the Usual Treatment).The experimental group undertook eight weeks of intervention, but the control group received the usual treatment according to the INCAS program. METHODS: The Five Factor Mindfulness Questionnaire (FFMQ) and the Addiction Sevier Index (ASI) were administered at pre-treatment and post-treatment assessment periods. Thirteen patients from the experimental group and 15 from the control group completed post-test assessments. RESULTS: The results of MANCOVA revealed an increase in mean scores in observing, describing, acting with awareness, non-judging, non-reacting, and decrease in mean scores of alcohol and opium in MBGT patient group. CONCLUSION: The effectiveness of MBGT, compared to the usual treatment, was discussed in this paper as a selective protocol in the health care setting for substance use disorders.

OBJECTIVE: The purpose of the present study was to examine the cognitive, emotional and behavioral differences between obese and normal weight women. METHODS: The participants consisted of 60 obese women (BMI ≥ 35, Mean age 35.83) who were candidates for bariatric surgery and 60 normal weight women (BMI ≤ 24.90, Mean age 33.38) who were selected through convenient sampling method. The two groups were matched with respect to age and education. Measures included the Young Schema Questionnaire-Short Form, Difficulties in Emotion Regulation Scale (DERS) and the Binge Eating Scale. Independent sample t test and Mann-Whitney U tests were conducted to compare the two groups' scores on early maladaptive schemas, DER and binge eating. RESULTS: Results indicated that obese women candidate for bariatric surgery scored significantly higher on emotional deprivation, mistrust, failure, dependency, enmeshment, self-sacrifice, especially abandonment, social isolation, vulnerability, self-control and subjugation schemas, but not on shame, entitlement, emotional inhibition and unrelenting standards. Obese women also displayed higher scores on three subscales of DERS, i.e., goal, impulse and strategies. BE scores were significantly higher in obese women than normal weight ones. CONCLUSION: The findings showed that obese women candidates for bariatric surgery suffered from more cognitive, emotional and behavioral vulnerability compared to women with normal weight. Addressing these vulnerabilities among obese women could improve outcomes of weight loss surgeries and cognitive behavioral interventions so that weight regain is minimized and better outcomes are achieved.

RESUMO

A brief account of the three stages of development of a new mental health law in Iran is given. At each stage, the expert opinions of mental health professionals and lawyers interested in the rights of psychiatric patients were obtained. The final draft of the law consists of six sections and 50 articles. It has been submitted for ratification by Parliament.

BACKGROUND: Acceptance-based behavior therapy (ABBT) was developed based on the theory that generalized anxiety disorder (GAD) is maintained through a reactive and fused relationship with internal experiences and a tendency toward experiential avoidance and behavioral restriction. ABBT specifically targets these elements. Here, we aimed to compare ABBT to the applied relaxation (AR), which is the most utilized psychological therapy for GAD. MATERIALS AND METHODS: This study was a randomized clinical trial study. The sample included 18 GAD patients who were referred by an expert psychiatrist to Psychosomatic Research Center of Isfahan University of Medical Sciences. Patients were assigned into two groups (ABBT and AR group). Both groups received routine drug therapies by psychiatrists. The ABBT and AR were applied in 12 weekly sessions. The instruments used in the study included Valued Living Questionnaire, Action, and Acceptance Questionnaire, and Short-Form Health Survey-12 revised Version (SF-12V2). The data were analyzed using the multivariate analysis of variance. RESULTS: No significant differences were found between ABBT and AR groups in their acceptance of internal experiences, engagement in meaningful activities and quality of life (P > 0.05). CONCLUSION: The current study compared ABBT to the most utilized psychological therapy for GAD; i.e., AR. ABBT and AR have similar efficacy on acceptance of internal experiences, valued actions and quality of life.

OBJECTIVE: The present study aimed to compare the effectiveness of metacognitive therapy (MCT) and cognitive-behavior therapy (CBT) in treating Iranian patients with major depressive disorder (MDD). METHODS: Thirty three outpatients meeting DSM-IV-TR criteria for MDD without any other axis I and II disorders were randomly assigned to one of three treatment conditions, i.e. MCT, CBT and pharmacotherapy. The Beck Depression Inventory-II-Second Edition (BDI-II), Beck Anxiety Inventory (BAI), Ruminative Response Scale (RRS) and Dysfunctional Attitude Scale (DAS) were administered for pre-treatment, post-treatment and follow-up. Data were analyzed by repeated measures analysis of variance (ANOVA). RESULTS: Based on repeated measures ANOVA, all the participants demonstrated improvement in depression, anxiety, dysfunctional attitude and ruminative response. Based on percentage results, all the patients in MCT and CBT groups showed significant improvement at post-treatment phase. CONCLUSIONS: MCT and CBT were more effective than pharmacotherapy alone In treatment of MDD. DECLARATION OF INTEREST: None.

OBJECTIVE: Acceptance-based behavior therapy (ABBT) is a new psychotherapy for generalized anxiety disorder (GAD). The current study intended to compare severity of symptoms, worry and quality of life of GAD female patients between ABBT and control. METHODS: This study was a randomized clinical trial. The sample included 18 women with GAD referred to psychiatrists in Isfahan, Iran. Patients were assigned in 2 groups randomly (ABBT and control group without any psychotherapy). Both groups received medication. The intervention in ABBT group was conducted based on Roemer & Orsillo's manual for Acceptance-based Behavior Therapy for GAD. 12 therapeutic sessions administered in Shariati psychiatric clinic of Isfahan. The instruments included the GAD-7 Inventory, Penn State Worry Questionnaire (PSWQ) and Short Form Health Survey -12 revised Version (SF-12V2). The data were analyzed using the Multivariate Analysis of Variance (MANCOVA). RESULTS: Overall, clients receiving ABBT compared to control group reported a significantly decrease in severity of GAD symptoms, and improve in quality of life at post- treatment state. They reported decrease in severity of worry but it was not statistically significant compared to control group. Conclusion : ABBT was effective in alleviating symptoms of GAD.

OBJECTIVE: This study has been conducted to investigate the efficacy of short-term dynamic psychotherapy on Love Trauma Syndrome (LTS) in female students. LTS includes a constellation of intensive signs and symptoms which appear following the breakdown of a romantic relationship after a long time. It interrupts person's function in many areas (academic, social or professional) and leads to maladaptive reactions. METHODS: This study was a multiple baseline single-case experimental study. The sample included five cases who were selected based on purposeful sampling procedure. The intervention was conducted based on McCullough's manual for short-term dynamic psychotherapy. In general, the study consisted of three stages including the baseline evaluation, the intervention period, and follow-up. The instruments included the Love Trauma Inventory (LTI), Beck Depression Inventory (BDI-II), Global Assessment Function) GAF) of DSM-IV and Millon Personality Inventory MCMI-II the data were analyzed using the clinical significance method and the recovery percent formula. RESULTS: All subjects who completed the treatment showed significant improvement in their symptoms including depression and general functioning. CONCLUSION: It appears that McCullough's short term anxiety-regulating psychotherapy is effective in treating LTS.

BACKGROUND: Cancer is one of the leading causes of mortality and morbidity worldwide. The incidence of cancer has increased markedly in recent decades in most countries. Studies have shown that diseases such as cancer affect the individuals' quality of life. METHODS: The sample of study consisted of 384 patients selected through non-random convenient sampling procedure from three general hospitals and outpatient clinics in Isfahan and Tehran. The measures used in the study included a demographic questionnaire, the Iranian version of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30), the Cancer Coping Questionnaire, and the Religious Attitude Questionnaire. RESULTS: The results revealed significant correlation between patients' scores on the total scale of the Cancer Coping Questionnaire and their scores on the Global health status/Quality of Life. Significant correlations were also found between patients' scores on the Religious Attitude Questionnaire and various scales of the Quality of Life Questionnaire. However, no significant correlations were found between Cancer Coping and Religious Attitude measures in any type of cancer except for the prostate cancer. CONCLUSIONS: Religious attitude was a significant and important factor in coping with cancer. In addition, patients' quality of life correlated significantly with religious attitude as well as cancer coping measures. However, the results did not show any significant relationship between religious attitude and cancer coping measures except in patients with prostate cancer. The findings of this study are consistent with other studies that have shown significant correlations between religiosity and spirituality and quality of life in patients with life threatening diseases.
