RESUMO

The Questionnaire for Patient-Surgeon Relationship (Q-PASREL) is a French "Patient-Reported Experience Measure" for hand surgery patients. It is the only one which considers the impact of the patient-surgeon relationship on time to return to work and cooperation by the surgeon for administrative issues. It has been shown that a good Q-PASREL score is associated with shorter sick leave and faster return to work. To make this instrument available to more countries, we translated the Q-PASREL into six languages (English, Spanish, German, Italian, Arabic and Persian), following a validated "translation and cultural adaptation" process guideline. This process includes multiple forward and backward translations, discussions and reconciliations with final harmonization and cognitive debriefing. For each language, a team was set up, comprising a key in-country hand surgery consultant, native target-language speaker and fluent in French, and several forward and back translators. The final translated versions were reviewed and approved by the project manager. The six versions of Q-PASREL are now available in the appendices of this publication.

RESUMO

Primary trigger finger is a common hand disorder for which nonoperative treatment or release of A1 pulley is usually effective. For resistant or recurrent cases, there are different surgical techniques including partial or complete opening of A2 pulley, reduction tenoplasty, and resection of the ulnar slip of the flexor digitorum superficialis tendon. Here, we present our minimally invasive technique for ulnar superficial slip resection surgery. Our indications are the patients with persistent residual proximal interphalangeal joint contracture after A1 pulley release and also the recurrent cases. Then, we report the clinical outcomes of our patients operated using this technique.

RESUMO

Pull-out suture fixation is widely used to fix zone one flexor tendon injuries. The original technique of Bunnell generally has good results. However, in some cases, it may result in complications, including nail deformity and infection. The all-inside techniques for fixation of the tendon at this zone cannot totally replace the Pull-out technique. In this article, we propose a modification of the pull-out technique, which can reduce the complications and report the results of our patients' series.

Guideline-based clinical decision support systems (CDSSs) need the most recent evidence for reliable performance, making the provision of regularly updated clinical practice guidelines (CPGs) a major issue. Some international guidelines are renewed in short intervals and can be used for checking the status of given national guidelines with regard to the most recent evidence. Considering the volume of medical data and the number of CPGs published, computerized comparison of clinical guidelines can be an effective method. We performed a scoping review to evaluate the methods used for comparing two CPGs. We searched for methods for extracting CPG components and for methods used for comparing CPGs at different levels of abstraction. In each case, computerized and semi-computerized methods were recognized. Expert knowledge has yet a determinant role for assessing the comparisons, this role being more prominent for the extraction of semantic rules and the resolution of inconsistencies.

RESUMO

Isometric ligament reconstruction with two graft limbs is used for chronic ligament injuries. Our technique facilitates proper tensioning. It is described in thumb metacarpophalangeal joint ulnar collateral ligament reconstruction. After passing through a tunnel in the base of the phalanx, the two limbs of the graft are docked inside the metacarpal socket. The graft is then fixed under continuous manual tensioning using a Pushlock® anchor. This technique is easy and reproducible; introduction inside the tunnel does not compromise the graft and tensioning is under continuous control. This technique is applicable to any hand, wrist, or elbow ligament reconstruction.
