RESUMO

BACKGROUND: Interferon-gamma release assays (IGRAs) may be useful in diagnosing latent tuberculous infection (LTBI) in inmates; however, published experience in these settings is limited. OBJECTIVE: To identify variables associated with IGRA positivity among Canadian federal inmates with positive tuberculin skin test (TST) results. DESIGN: On intake, TST-positive (≥10 mm) inmates were offered an IGRA (QuantiFERON(®)-TB Gold), and demographic and historical data were collected. IGRA-positive and -negative inmates were compared using the χ(2) test and multivariable logistic regression; the final model's goodness of fit was assessed using Hosmer-Lemeshow test and area under the receiver operating characteristic curve (AUC). RESULTS: Of 96 TST-positive inmates, 31 (32.3%) were IGRA-positive. Variables associated with positive IGRA were age >45 years (11/20 vs. 20/75, P = 0.016) and previous LTBI treatment (9/20 vs. 13/55, P = 0.032) in univariate analysis, and being from a country with a moderate or high estimated tuberculosis (TB) incidence (OR 3.5, 95%CI 1.3-9.4, P = 0.013) and absence of bacille Calmette-Guérin (BCG) vaccination (OR 3.3, 95%CI 1.2-9.0, P = 0.017) in multivariable analysis. The data fit the model well, classifying the group better than chance alone (AUC 0.67, P = 0.007). CONCLUSION: High discordance with TST, particularly among BCG-vaccinated inmates and those from low TB incidence countries, suggest that IGRA may be useful in Canadian federal penitentiary screening programmes.

Previous reports have documented an improvement in night vision among Jamaican fishermen after ingestion of a crude tincture of herbal cannabis, while two members of this group noted that Moroccan fishermen and mountain dwellers observe an analogous improvement after smoking kif, sifted Cannabis sativa mixed with tobacco (Nicotiana rustica). Field-testing of night vision has become possible with a portable device, the LKC Technologies Scotopic Sensitivity Tester-1 (SST-1). This study examines the results of double-blinded graduated THC administration 0-20 mg (as Marinol) versus placebo in one subject on measures of dark adaptometry and scotopic sensitivity. Analogous field studies were performed in Morocco with the SST-1 in three subjects before and after smoking kif. In both test situations, improvements in night vision measures were noted after THC or cannabis. It is believed that this effect is dose-dependent and cannabinoid-mediated at the retinal level. Further testing may assess possible clinical application of these results in retinitis pigmentosa or other conditions.

This study examined the ability of four measures of suboptimal performance to correctly classify four groups of subjects (normal controls, uncoached malingering, coached malingering, and head injured). Only the Portland Digit Recognition Test-Computerized (PDRT-C) identified simulating malingerers with greater than chance accuracy while minimizing false positives. Coached subjects were better able than their uncoached counterparts to avoid detection on all measures. In an additional analysis, a discriminant function using the response latency and total correct scores from the PDRT-C identified 70% of the coached malingerers on cross validation. The three other tests (Nonverbal Forced Choice Test, 21-Item Test, and Dot Counting Test) failed to obtain a satisfactory classification rate for the malingering groups as a whole and coached malingerers in particular.

RESUMO

Six mentally retarded adults, equally divided into two treatment groups, were provided with individualized social skills training programs. Treatment, evaluated via a multiple-baseline design strategy, was sequentially and cumulatively applied across target behaviors over a four-week intervention period. Behavioral observation probes and social validation measures served as the primary dependent variables. Results indicated that (a) treatment was effective for virtually all behaviors across all subjects, (b) improvements occurred for both training and generalization scenes, and (c) behavioral performance was maintained one month following the termination of treatment.

RESUMO

Nineteen subtrochanteric fractures were treated at Charlotte Memorial Hospital and Medical Center between March 1973 and March 1976 by open reduction and internal fixation using the Zickel nail. Postoperative roentgenograms showed the cross-nail exiting the femoral neck in three of the cases, but this later proved not to be significant. Of the 16 patients available for follow-up all were ambulatory unassisted or with a walker within two months of surgery. Radiographically evident healing averaged 4.1 months. There were no non-unions and no infections. Our data support the use of the Zickel nail in the treatment of subtrochanteric fractures.

RESUMO

Coxa magna is probably due to a period of relative hyperemia of the capial femur epiphysis between 2 and 8 years of age. The peripheral cartilage of the metaphysis and epiphysis proliferate as a response to reactive hyperemia secondary to synovitis. We have observed 11 patients with coxa magna and have followed five in detail. Good function is expected in childhood and adolescence, but traumatic arthritis will probably occur as these patients get older.
