RESUMO

The expression of TLR4 and HBD3 in epithelial cells of the oral cavity was studied using polymerase chain reaction in real-time in patients with inflammatory and destructive periodontal lesions before and after surgery using osteoplastic material. Analysis of TLR4 and HBD3 expression in epithelial cells demonstrates the key role of the innate immune factors in the pathogenesis of inflammatory and destructive periodontal lesions. TLR4 gene expression was increased 1.5-7.0 times and HBD3 1.5-5.0 times compared to healthy individuals. Surgical treatment resulted in an effective rehabilitation and normalization of innate immunity.

RESUMO

The objective of the study was to define treatment strategy in cases of facial bones bisphosphonate induced osteonecrosis based on the study of the role of conditionally pathogenic oral microorganisms. Three typical clinical cases of bisphosphonate osteonecrosis of the facial bones were analyzed and 15 conditionally pathogenic oral microorganisms were identified in these patients using real-time PCR in saliva, wound and bone samples. A comparative analysis was carried out with purulent-inflammatory diseases of maxillofacial area. The study results proved an important role of conditionally pathogenic microorganisms of the oral cavity in the development of osteonecrosis of the facial bones. Wide range of bacterial species was identified in osteonecrosis of the facial bones patients. While bone tissue is most exposed to microbial communities, surgical treatment results in effective rehabilitation for a long period.

RESUMO

The aim of the study was to assess the effectiveness of fibrinogen/thrombin-coated collagen patch by mucogingival operations in patients with somatic diseases. Twenty-seven patients aged 25 to 45 (15 males and 12 females) with somatic diseases such as arterial hypertension (11 patients), diabetes (8 patients), bleeding disorders (8 patients) underwent Edlan-Mejcher vestibuloplasty. Using fibrinogen/thrombin-coated collagen patch as wound dressing caused marked hemostatic effect in 3 to 5 minutes even in hypertension and bleeding disorder patients. Wound heeling was observed 14 days post-op with no excessive scarring.

RESUMO

The study included 15 patients with purulent inflammatory diseases of maxillofacial area and 25 patients with facial bone necrosis induced by synthetic drugs. Pro- and anti-inflammatory cytokines levels in saliva and wound fluid were analyzed in two groups. The results proved cytokines to play important role in jaw necrosis induced by drugs containing red phosphorus.

RESUMO

Molecular study of congenital immune factors was conducted in 45 patients with inflammatory diseases of maxillofacial area. The study focused on expression of cationic low-molecular peptides α- and Β-defensines in oral mucosa. These peptides are involved in antibacterial activity and regulation of immune reactions. The results showed 20-fold increase in defensines expression in oral mucosa of patients with inflammatory diseases of maxillofacial area when compared to control group (12 healthy individuals) thus proving an important role they play in the development of the disease. Therapy with cytokines may be indicated in these cases.

RESUMO

The role of conditionally pathogenic microflora in the pathogenesis of lower jaw bone traumatic osteomyelites was investigated. The different groups of aerobic and anaerobic microorganisms were detected by real-time PCR. Role of conditionally pathogenic microflora in the pathogenesis of traumatic osteomielytis was shown.
