RESUMO

Objectif. Décrire les caractéristiques épidémiologiques et microbiologiques des pieds diabétiques compliqués d'ostéite à Abidjan. Méthodes. Il s'agit d'une étude transversale, rétrospective ayant concerné 71 prélèvements bactériologiques (écouvillonnage) suivis d'une culture en présence d'un pied diabétique avec ostéite, colligés dans le service d'endocrinologie ­ Diabétologie du CHU de Yopougon (Abidjan) de 2002 à 2012. Résultats. La population d'étude était constituée de 71 patients dont 92,2% diabétiques de type 2, d'âge moyen 56,6± 12,6 ans, de durée moyenne d'évolution du diabète 10,4±8 avec une moyenne glycémique à 3,33±1,54 g/l et au stade de multiples complications du diabète. Le facteur déclenchant de la plaie était souvent un traumatisme (21,1%), une phlyctène spontanée (21,1%) ou un pied d'athlète (12,2%). Parmi les 71 patients avec pieds diabétiques compliqués d'ostéite ayant bénéficié d'un prélèvement, 51 cultures ont été positives soit 71,83 % des cas. Elles ont été le plus souvent monomicrobiennes 42 soit 82,3% des cas. A l'issue des examens microbiologiques 61 germes ont été isolés repartis selon les familles, les espèces et sous espèces. Parmi ces germes 60,65% étaient des bacilles Gram négatif, dont 86,5 % d'Entérobactéries, et 39,35% des Cocci Gram positif. La recherche de germes anaérobies n'a pas êté réalisée. En fonction des espèces, Escherichia coli a été isolé dans 35,13% des cas et les staphylocoques aureus ont été isolés dans 18% des cas. Au total 52,7% dessouches d'entérobactéries étaient résistantes aux quinolones et 22,7 des souches de Staphylocoque étaient méticillino-résistants. Conclusion. L'écologie bactérienne des pieds diabétiques compliqués d'ostéite est dominée par des souches d'entérobactéries résistantes aux antibiotiques usuels

RESUMO

Le traitement de la maladie de Blount infantile négligée est complexe. Une patiente avec une forme bilatérale a consulté à l'âge de 12 ans. Il a été réalisé pour chaque genou une double ostéotomie tibiale dans le même temps opératoire. Un délai d'un an séparait les deux interventions. Il s'agissait d'une ostéotomie épiphysaire de relèvement du plateau tibial interne et d'une ostéotomie de valgisation sous la tubérosité tibiale antérieure. Le vide créé par le relèvement a été comblé par les fragments tibial et péronier. Cette double ostéotomie tibiale était instrumentée. Au denier recul de cinq ans (à droite) et quatre ans(à gauche),la patiente était satisfaite avec de bons axes. Il n'y avait pas de raccourcissement

RESUMO

Introduction: L'intervention de Sauvé-Kapandji consiste à réaliser une arthrodèse radio-ulnaire distale associée à une pseudarthrose intentionnelle de l'ulna. Le but de cette étude était d'évaluer les résultats obtenus avec cette technique dans le traitement des désordres post-traumatiques du poignet. Matériels et méthodes: Cette étude rétrospective réalisée entre janvier 2005 et décembre 2014 a concerné huit patients. La lésion était une séquelle d'une fracture de Galéazzi (n=5) , un cal vicieux d'une fracture de l'extrémité inférieure du radius(n=2), et une arthrose post-traumatique du poignet(n=1). Tous les poignets étaient douloureux, avec limitation de la mobilité et restriction de la force de serrage de la main. Résultats: Au recul moyen de 16 mois (extrême 8 et 36 mois) la douleur a été soulagée chez six patients. La pronation moyenne a été améliorée de 46° à 75°, la supination de 51° à 77°, la flexion de 44,5° à.48°, et l'extension de 24° à 54°. L'angle d'inclinaison radiale moyen a été amélioré de 18° à 20°, l'angle d'inclinaison de la glène moyen de 9,75°à 12°, et l'index radio-ulnaire distale de + 3 mm à +1mm. La force de serrage a été satisfaisante chez six patients cas et mauvaise chez deux. Toutes les arthrodèses ont fusionné. Conclusion: La technique de Sauvé Kapandji a permis le soulagement de la douleur, le renforcement de la mobilité et de la force de préhension chez la majorité des patients

OBJECTIVE: To evaluate the results of multiple closed intramedullary Kirschner wiring via a supracondylar entry point for humeral shaft fractures. PATIENTS AND METHODS: The charts of 37 patients with humeral shaft fractures treated with the Hackethal's technique between January 2007 and December 2011 were reviewed retrospectively. The operation was performed with the patient lying in supine (n = 22) or lateral (n = 15) position. The elbow was flexed over an articulated support with the arm kept in a vertical position. Thirty-three patients were available for final evaluation with a mean follow-up delay of 14 (range, 6-24) months. We were concerned about fracture union, range of motion of the shoulder and the elbow, and complications. Final evaluation used the criteria by Qidwai. RESULTS: Bone union rate was 94%. Restriction of ranges of motion of the shoulder more than 20° was noticed in two patients due to protruding wires. Three patients developed limitation of elbow extension owing to backing out of the wires. The overall results were excellent (n = 26; 79%), good (n = 4; 12%), and poor (n = 3; 9%). CONCLUSION: Closed Hackethal's technique using K-wires gives satisfactory results in terms of bone union and elbow and shoulder function in selected humeral shaft fractures. The articulated support precludes the transolecranon traction.

BACKGROUND: In sub-Sahara Africa, the strong involvement of the family in multidimensional cares/supports of elderly is often presented like the family ensures almost everything to these people. Thus, few studies have focused on unmet needs of the elderly in their family or above. This study was conducted in Bobo-Dioulasso to identify those needs. METHODS: This is a longitudinal study including 58 people or 15 elderly and 43 caregivers from 15 families in Bobo-Dioulasso. In addition to regular observations of these families during 1 year, we conducted in-depth individual interview with each participant at the beginning and at the end of the study. The data were analyzed using QSR NVivo 8 software. RESULTS: A priori, respondents let believe that there is no unmet functional needs of the elderly in their family. However, the food, the first and main functional need of the elderly is not qualitatively satisfied by their family as well as other equipments or health needs. The quality of social cares/supports, biomedical cares and community supports are insufficient when these cares/supports are provided. The family demands many free or subsidized services to public or community structures then they are not currently available. DISCUSSION: In a context of widespread poverty, it is difficult for each actor of the social system of maintaining elderly in functional autonomy to provide services/supports of optimal quality. A synergy of action will reduce the unmet needs of the elderly in Bobo-Dioulasso.

Objective: The purpose of this study is to present our experience with the use of primary unreamed and unlocked nailing by the curved Kuntscher nail for open tibial fractures.Design: A retrospective study carried out between 1993 and 2003.Setting: Department of Orthopaedics Surgery; Yopougon Teaching Hospital; Abidjan; Coted'Ivoire.Patients and methods: A total of 154 open tibial shaft fractures in 149 patients were operated on by primary stabilisation with an unreamed and unlocked Kuntscher nail. Postoperatively; patients wore a leg plaster cast. Results: There were 124 men and 25 women with a mean age of 35 (16 to 72) years. Fractures werecaused in the majority of cases (130 cases; 87.2) by traffic accidents. Fractures were classified according to Cauchoix system into 96 grade I; 55 grade II; and 3 grade III. The duration of follow-up averaged nine (3 to 25) months. Infections (softtissue and bone) were encountered in 30 (19.5 ) fractures. 113 fractures (86.25 ) were primarily united with an average of 4.7 (two to six) months.Bone union was delayed for seven (5.3 ) fractures. Eleven (8.45 ) fractures developed nonunion. Malunion in valgus 10o was seen in three patients and in varus 10oin two. Rotational malalignment 10o was encountered in five patients

RESUMO

A case of bilateral proximal metaphyseal fracture of the humerus in a 3 year-old child victim of a traffic road accident is presented. The fractures were managed conservatively under general anesthesia with an excellent remodeling at the fOllow-up of 2 years and 8 months.

RESUMO

The authors report a case of bilateral fracture of the thigh bone that occurred in a 22 year old man with multiple exostosis. Clinical deformities, radiology and histology confirmed the diagnosis. Traumatic fractures were caused by a road accident. The treatment, because of deformities, consisted of an open fire synthesis of the both thigh bones with Küntscher's nails. The post-operative evolution was marked by rapid healing of fractures. The scarcity of this genetic affection and the symmetrical lesions have lead the authors to conduct a literature review.

RESUMO

The research of the predominant occlusal function during mandibular movements of laterality at the topic African melanoderme, drove authors of this survey to: select 52 healthy topics (11 women and 41 men), aged of 22 to 33 years old observe and note at every topic the presence and the nature of contacts or their possible absence of the working side during the movement of deduction. Observations done as well directly in mouth, that on models put in semi-adaptable articulators, permitted to note that the function of posterior group is more the recovered so much at women that at men, at the young adults and at adults.

RESUMO

We report a study of 15 cases of tuberculous hips with sickle-cell disease who presented during 1991-1993. Although the osteonecrosis was long-standing, biopsy was nearly always required to reveal the more recent tuberculous infection. Management consisted of 6 months of anti-tuberculous chemotherapy with appropriate palliative surgery 5-8 weeks after the start of drug treatment. The operative techniques which we used are described. The results were good both post-operatively, and in 12 patients followed-up at an average of 3 years. We recommend this combined management for the treatment of secondary tuberculous infections of hips previously damaged by sickle-cell disease.
