RESUMO

BACKGROUND: Physical education (PE) classes in schools are considered relevant to implement interventions, especially focused on physical activity. However, evidence overviews on how PE classes contribute to general health (physical, social, affective, and cognitive domains) are still needed. Thus, we summarized evidence synthesis (eg, systematic reviews) that addressed the contribution of PE classes to the health of school-aged children and adolescents. METHODS: We performed a scoping review with searches in 8 databases and institutional websites to find systematic reviews or meta-analyses that answered this review's research question. Data charting form included the identification of the study, health outcomes, and PE classes' strategies (policies and environment, curriculum, appropriate instructions, and evaluation). An interactive process was performed to build the evidence summary. RESULTS: An initial search yielded 2264 titles, and 49 systematic reviews (including 11 with meta-analysis) were included in this review. Most documents reported the main benefits of PE classes on physical domain outcomes (eg, physical activity, cardiorespiratory fitness, body mass index, and fundamental motor skills). However, evidence on the benefits of PE classes in affective (eg, enjoyment, motivation, and autonomy); social (eg, cooperation, problem-solving, and making friends); and cognitive (eg, memory, attention, concentration, and decision making) domains were found. Strategies on PE classes for health benefits were highlighted. CONCLUSIONS: These elements were detailed in the evidence summary, which may be considered to guide researchers, teachers, and practitioners to define research and practice priorities on PE class interventions for health in the school context.

RESUMO

In 2018, the World Health Organization (WHO) launched a Global Action Plan on Physical Activity (PA), which included 20 policy actions for creating active societies, environments, people and systems. The objective of this scoping review was to summarize the themes/contents of national PA policies/plans conforming to the WHO's proposals and the country's economy. This review followed Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines for scoping reviews. A systematic search of electronic databases (Web of Science, Medline/PubMed, LILACS, PsycINFO, Scopus and SPORTDiscus) and 441 government documents/websites from 215 countries/territories was conducted (February 2021). Documents on national-level policies, published in English, Spanish and Portuguese since 2000, were eligible. The information on content and structure was systematically extracted and summarized into dimensions proposed by the WHO: active societies, environments, people and systems. The search identified 888 article references and 586 potentially relevant documents. After the screening, 84 policy documents from 64 countries were eligible. Most documents (n = 46) provided detailed PA policies/plans amid other health topics (e.g. non-communicable diseases, named 'general documents'), and 38 were PA-specific. The content analysis merged 54 visions, 65 missions, 108 principles, 119 objectives, 53 priorities, 105 targets, 126 indicators and 1780 actions/strategies from 38 PA-specific and 46 general documents. Among the PA-specific documents, the active system's dimension was the most contemplated in the principles (n = 43), priorities (n = 51) and action/strategies (n = 530) elements. At the same time, the objectives (n = 39), targets (n = 52) and indicators (n = 58) presented contents more frequently related to the active people dimension. For the general documents, all principles (n = 4), objectives (n = 14) and priorities (n = 7) were related to the dimension of active people, while target (n = 51), indicator (n = 53) and actions/strategies (n = 292) elements presented content related to all dimensions. The increase in countries with national PA policies/plans should be followed by improving the current ones because important dimensions seem to be not considered in these documents. This will facilitate a global PA agenda that considers the complexity and multidimensionality of PA promotion.

RESUMO

The aims of the study were to verify the effect of an intervention on the stage of behavior change for TV viewing and to identify the possible mediating role of knowledge on screen time guidelines among Brazilian students. This study is a multicomponent school-based intervention conducted with 727 students (54.3% female, 13 years; 427 in the intervention group and 300 in the control group). The intervention was performed in 2017 with baseline and post-intervention assessments (over one academic year). Teacher training was carried out at the beginning of the intervention along with en-vironmental improvements, while educational curriculum such as the delivery of folders and posters were performed throughout the year. The stages of behavior change for TV viewing and knowledge of screen time guidelines were measured by a self-reported questionnaire and the structural equation modeling approach was performed. The intervention had no significant effects on stages of behavior change for TV (p = 0.744) nor did it result in significant changes on knowledge of screen time guide-lines (p = 0.741). While there was no mediation between knowledge of screen time guidelines and the effect of the intervention on stages of behavior change for TV (95%CI: -0.45;0.63), an associa-tion was found between knowledge of screen time guidelines and stages of behavior change for TV (p<0.001). In conclusion, intervention had no significant effect on the stages of behavior change for TV and no mediating effect was observed. However, enhancing awareness on screen time guidelines may positively impact the adolescent's intention to reduce TV time

INTRODUCTION: This study examined the association between simultaneity of four health-risk behaviours, namely, low levels of moderate-to-vigorous physical activity (insufficient MVPA: <420 min/week), tobacco use, alcohol consumption, and excessive television (TV)-(>2 h/d of TV viewing) and self-rated health (SRH) in Brazilian adolescents. METHODS: We used data of 100,551 adolescents from the National School Health Survey, a national cross-sectional study carried out in 2015. Association between simultaneity of health risk behaviours (i.e. the ratio between observed and expected prevalence rates) and SRH was examined using logistic regression models. RESULTS: The majority of the participants were female (51.9%) and 14 years of age (50.6%), and 26% of the participants' SRH ranged from 'average' to 'extremely poor'. Those who engaged in the following combinations of health-risk behaviours had higher odds of worse SRH than their healthier counterparts: insufficient MVPA and tobacco use (odds ratio-OR: 2.0, 95% confidence interval [CI]: 1.4 to 3.0); insufficient MVPA and alcohol consumption (OR: 1.6, 95%CI: 1.3 to 1.9); insufficient MVPA and >2 h/day of TV viewing (OR: 1.3, 95%CI: 1.1 to 1.6); insufficient MVPA, tobacco use and alcohol consumption (OR: 2.1, 95%CI: 1.7, to 2.7); and insufficient MVPA, alcohol consumption and >2 h/day of TV viewing (OR: 1.6, 95%CI: 1.4 to 2.0). CONCLUSIONS: Insufficient MVPA, alcohol consumption, and other health-risk behaviours were associated with worse SRH in Brazilian adolescents.

RESUMO

BACKGROUND: Understanding which strategies have been recommended for the promotion of active and healthy lifestyles through physical education (PE) classes can guide PE policies and practice. Therefore, we summarized worldwide recommendations regarding strategies for PE classes that have aimed to promote active and healthy lifestyles among school-aged children and adolescents. METHODS: The Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines were utilized. A literature search was carried out in June 2020 in eight peer-reviewed literature databases, in addition to searches in institutional and personal libraries. The eligibility criteria included any online document that included recommendations targeting any dimension of PE classes (e.g., policy and environment, curriculum, appropriate instruction, student assessment, and strategies that interact with PE) published since 2000. RESULTS: In total, 2,408 potentially eligible documents were screened. Of these, 63 were included in the final analysis. The recommended strategies were as follows: six referred to policy and environment (valuing PE, higher frequency and duration of classes, inclusive PE classes, mandatory daily classes, evaluation of PE classes, and qualified teachers), five to curriculum (structure, type of content, cross-cutting themes, and components that improve PE classes), four to appropriate instruction (promotion of physical activities, inclusion of social issues, employment of the use of innovative technologies, and organization of the teaching-learning process), and three to student assessment (understanding human movement concepts, evaluation of contents, and assessment methods to develop an active and healthy lifestyle). CONCLUSION: Twenty-one strategies recommended for PE classes linked to five dimensions aimed at different target populations were identified. Over half were linked to the dimensions of policy and environment and appropriate instruction. PE is recommended to be mandatory and valued at all educational levels, with weekly frequency that contributes to an active and healthy lifestyle. This review shows that guaranteeing different experiences beyond sports, improving social inclusion, using innovative technologies, and providing adequate materials and spaces to be important challenges and ways to guide policies, programs, and new research in this field of knowledge. RESUMO

BACKGROUND: The objective of this study was to verify the reliability, discriminatory power and construct validity of the Kidscreen-27 questionnaire in Brazilian adolescents. METHODS: Adolescents that participated of the pilot study (210 adolescents; 52.9% boys; 13.7 years old) and of the baseline (816 participants; 52.7% girls; 13.1 years old) of the Movimente Project in 2016/2017 composed the sample of the present study. This project was carried out in six public schools in the city of Florianópolis, Santa Catarina, Brazil. Test-retest reproducibility was assessed by the intraclass correlation coefficient and Gwet coefficient; internal consistency through McDonald's Omega; Hankins' Delta G coefficient verified the scale's discriminatory power and; confirmatory factor analysis to assess construct validity. RESULTS: Reproducibility values ranged from 0.71 to 0.78 for the dimensions (ICC), and ranged from 0.60 to 0.83 for the items (Gwet). McDonald's Ômega (0.82-0.91) for internal consistency measures. Discriminatory power ranging from 0.94 for the dimension Social Support and Friends to 0.98 for Psychological Well-Being. The factorial loads were > 0.40, except for item 19 (0.36). The fit quality indicators of the model were adequate (X2[df] = 1022.89 [311], p < 0.001; RMSEA = 0.053 (0.049-0.087); CFI = 0.988; TLI = 0.987), confirming the five-factor structure originally proposed. CONCLUSIONS: The Brazilian-version Kidscreen-27 achieved good levels of reproducibility, internal consistency, discriminatory power and construct validity. Its use is adequate to measure the health-related quality of life of adolescents in the Brazilian context.

RESUMO

Schools have been the main context for physical activity (PA) and sedentary behavior (SB) interventions among adolescents, but there is inconsistent evidence on whether they also improve dimensions of the health-related quality of life (HRQoL). The aim of this study was to evaluate the effects of a school-based active lifestyle intervention on dimensions of HRQoL. A secondary aim was to verify whether sex, age, and HRQoL at baseline were moderators of the intervention effect. A cluster-randomized controlled trial was conducted at three control and three intervention schools in Florianopolis, Brazil. All students from 7th to 9th grade were invited to participate. A school year intervention, designed primarily to increase PA and reduce SB, included strategies focused on (i) teacher training on PA, SB, and nutrition, and availability of teaching materials related to these contents; (ii) environmental improvements (i.e., creation and revitalization of spaces for the practice of PA in school); and (iii) education strategies, with the availability of folders and posters regarding PA, SB, and nutrition. Participants and the research staffs were not blinded to group assignment, but a standardized evaluation protocol was applied at baseline and after the intervention (March and November 2017) using the KIDSCREEN-27 to assess HRQoL across five dimensions. Mixed linear models were performed to evaluate the effect of the Movimente intervention on the five HRQoL dimensions. Of the 921 students who answered the questionnaire at baseline, 300 and 434 completed the study in control and intervention groups, respectively (dropouts: 20%). The results revealed no significant effects of the intervention on any HRQoL dimensions. A reduction of the school environment dimension was observed in both the control (-2.44; 95% CI: -3.41 to -1.48) and intervention groups (-2.09; 95% CI: -2.89 to -1.30). Sensitivity analyses showed that students in the highest baseline tertiles of HRQoL in any dimension had a reduction in their respective scores from pre- to post-intervention in both school groups. In conclusion, our results demonstrated no intervention effect on HRQoL dimensions and those students with the highest levels of HRQoL at baseline on all dimensions reduced from pre to post-intervention. CLINICAL TRIAL REGISTRATION: The trial is registered at the Clinical Trial Registry (Trial ID: NCT02944318; date of registration: October 18, 2016).

Physical Education (PE) classes represent an appropriate time to know and experience the plurality of the human movement, favoring the promotion of a healthy and active life. The importance of PE was recognized by the Physical Activity Guide for the Brazilian Population, which dedicated a chapter to guide the school community on the health benefits and successful strategies adopted in PE classes. This study aimed to show the process of elaborating Brazilian recommendations for PE for the Brazilian population, more specifically students, teachers, parents and guardians, and managers. This chapter of the Guide was developed by ten Brazilian researchers, including a spokeswoman of the Ministry of Health. Three syntheses of evidence, focus groups/interviews and public consultation were carried out to construct the recommendations. Data from 49 systematic reviews on the health benefits of PE and 22 interventions in PE classes with Brazilian students were summarized. Also, 63 documents were synthesized with strategies recommended for PE classes, focusing on the dimensions: politics and environment; curriculum; appropriate instruction; evaluation; and strategies that interact with PE. The focus groups and the public consultation were the basis for the elaboration and improvement of the final version of the recommendations. It is expected that these recommendations can assist the creation of public policies, the improvement of practice PE and the need to conduct future studies in this area of knowledge

RESUMO

It is unknown whether knowledge of guidelines for television (TV) viewing translate into behavior change for TV time among adolescents. The purpose of the present study is to verify whether knowledge of TV recommendations (spending <2 hr/day watching TV) is associated with the stages of behavior change related to TV viewing among adolescents. Schoolchildren (11-16 years) from six schools were recruited. Their knowledge of TV guidelines (correct [<2 hr/day], overestimated [<1 hr/day or never watch TV], underestimated [<3hr/day or watch TV whenever they wanted], and do not know [response option]) and stages of behavior change related to TV (precontemplation, contemplation, preparation, action, and maintenance) were assessed by a questionnaire. Multinomial logistic regression was used to test the association between knowledge and stages of behavior change. A total of 792 adolescents (53% girls) participated, of whom 45% were in the precontemplation stage, while 35% underestimated and 18% did not know the TV guidelines. Those who overestimated the guidelines had higher odds of being in the maintenance stage than did those who knew the recommendations correctly (odds ratio [OR] 2.06), while those who underestimated the guidelines had lower odds of being in the maintenance (OR 0.22) and any other stage compared to those who were aware of the recommendations (the precontemplation stage was the reference). Underestimating the guidelines was associated with being in the precontemplation stage, while overestimating them was associated with the maintenance stage. Interventions should test whether increasing knowledge can help shifting those in the precontemplation and contemplation toward action and maintenance.

RESUMO

Abstract This review updated data on sedentary behavior in Brazilian children and adolescents for the Brazil's Report Card 4.0. The searching was carried out in eight databases (PubMed, Scopus, Web of Science, LILACS, SPORTDiscus, BIREME, Scielo, and Google Scholar), based on the the following criteria: original research; samples including Brazilian children and adolescents; to be a observational survey with the prevalence of at least one component of sedentary behavior. In this updated review were included 118 studies (corresponding to 159 papers), being 71 studies (104 papers) previously reviewed in the Report Card 3.0 and 47 studies (55 papers) found in update from 2018 to 2019. Screen time (34.7%) and TV viewing (28.2%) remains the most investigated components, however, two studies investigated cell phone use, and there was an increase in other types of sedentary behavior such as sitting time (from 9% to 25.6%). We found only four studies involving pre-scholars, but four of them covered almost all age groups. Self-reported questionnaire was the instrument more used; however, increased the studies using accelerometers (from 2 to 8 studies). The cut-off point more frequent was 2 hours/day (47.5%), but the use of other measures doubled. Almost 70% of the studies reported that less than 50% (general range: 9.4% to 97.7%) of individuals had < 2 hours/day of sedentary behavior. The updated review found few studies with prescholars and children; using validated instruments; using accelerometers, with standardization of cutoff points, and prevalences very close to what was observed in the previous review.

OBJECTIVES: To investigate whether psychosocial variables mediate the effect of a multicomponent intervention on screen time reduction among Brazilian students from schools located in vulnerable areas. DESIGN: A cluster-randomized controlled trial with a 4-month follow-up. METHODS: This study was conducted with 1085 students (548 in the intervention group and 537 in the control group), aged 11-17years. The intervention strategies focused on training teachers, increasing opportunities for physical activity at school, and reducing screen time, as well as health education. The questionnaire was administered before and after intervention with questions about the amount of time spent on TV and computer/video games on weekdays and weekend days (combined screen time). The potential psychosocial mediators (attitude, self-efficacy, family and school support) were measured through validated scales. Socioeconomic status was used as control variable. Multilevel mediation analyses were conducted using a product-of-coefficients test. RESULTS: Psychosocial factors were not mediators of the effect of the intervention on screen time. The intervention significantly improved school support for both sexes (boys: 1.307; girls: 0.759; p<0.05) and older students (1.154; p<0.001). Attitude (boys: -0.228; 11-13years: -0.133; 14-17years: -0.152; p<0.05) and self-efficacy scales (boys: -0.040; girls: -0.104; 11-13years: -0.048; 14-17years: -0.100; p<0.05) were associated with reduced screen time. CONCLUSIONS: The intervention produced a significant improvement of school support for both sexes, as well as older students. Enhancing attitude and self-efficacy may be a useful strategy for reducing screen time among boys and students of any age groups. TRIAL REGISTRATION: ClinicalTrials.Gov: NCT02439827. Registration date: May 3, 2015.

RESUMO

Promoting healthy lifestyle factors (e.g., physical activity, healthy eating, less screen time) among young people is a relevant and challenging step toward reducing non-communicable diseases. This study aimed to evaluate the effect of a multicomponent intervention on lifestyle factors among adolescents from schools in low Human Development Index (HDI < 0.500) areas. The Fortaleça sua Saúde program was conducted with 548 adolescents aged 11⁻18 years old in the intervention group and 537 in the control group. The four-month intervention included strategies focused on training teachers, new opportunities for physical activity in the school environment, and health education strategies for the school community (including parents). Moderate- to-vigorous physical activity level (≥420 min/week), TV watching and computer use/gaming (<2 h/day), daily consumption of fruit juice, fruit, vegetables, soft drinks, savory foods and sweets, and current alcohol and tobacco use were measured before and after intervention. McNemar's test and logistic regression (odds ratio [OR] and a 95% confidence interval [95% CI]) were used, considering p < 0.05. In the intervention schools, a significant increase occurred in the number of adolescents who met physical activity guidelines (5.3%; 95% CI = 0.8; 9.8) and who reported using computer for <2 h a day (8.6%; 95% CI = 3.8; 13.4) after intervention. No changes were observed in the control schools. At the end of the intervention, adolescents from intervention schools were more likely to practice physical activity at recommended levels (OR = 1.44; 95% CI = 1.00; 2.08) than adolescents from control schools. No significant change was observed for the other lifestyle factors. In conclusion, this multicomponent intervention was effective in promoting physical activity among adolescents from vulnerable areas. However, other lifestyle factors showed no significant change after intervention. This study is registered at Clinicaltrials.gov NCT02439827.

RESUMO

This study evaluated whether the relationship between socioeconomic status (SES) and moderate-to-vigorous physical activity (MVPA) is mediated by social support of peers and parents. A cross-sectional study included students (n = 1182) aged 11 to 18 years who were recruited in 2014 from public schools in Fortaleza, Brazil. Participants completed a questionnaire to assess all variables. Mediation analyses were performed to determine the role of parental support and peer support on the relationship between SES and MVPA. Also, moderated-mediation analyses with estimating of conditional indirect effects were performed in order to identify whether gender is a moderator of the mediating effect. A positive and significant association between SES and MVPA was found among Brazilian students, and social support of parents and peers were significant mediators of this relationship. Conditional indirect effects showed that both parental and peer support had a significant indirect effect among boys; only parental support had a significant indirect effect among girls. These results suggest that improving social support can help reduce the impact of SES on physical activity among Brazilian adolescents.

RESUMO

The preference for a specific activity during leisure time can directly influence its practice, both for active and sedentary behaviors. Thus, the purpose of this study was to identify changes after a decade in the preference for leisure activities according to gender and age group between two generations of adolescents from southern Brazil. It was a repeated cross-sectional study (2001-2011). Representative samples of high school students (aged 15-19 years) from public schools in the state of Santa Catarina were evaluated in 2001 (n = 5028) and 2011 (n = 6529). Students answered one question on the preferred leisure activity, organized into six groups of activities. Comparisons between surveys were based on the relative frequency and 95% condence intervals (95% CI). After a decade, there were signicant reductions (95% CI did not overlap) in the preference for practicing physical activities, cultural activities and other activities, while the preference for watching TV, playing video games and using computer increased over time. These trends were similar between boys and girls and between younger (15-16 years-old) and older (17-19 years-old) students. The magnitude of changes was bigger in the proportion of preference for using computer and for practicing physical activities. There was an increase in the preference for leisure activities that involve screen time use, but the preference for other leisure activities (e.g., physical and cultural activities) reduced after a decade. Reasons underlying the change in leisure preferences need to be examined to support future interventions

The aim of this study was to evaluate the effect of an intervention on screen time and on psychoso-cial factors (Attitude, self-efficacy, support of family and school members) associated with reduced screen time in students. A cluster-randomized controlled trial was conducted in three schools in the intervention group (n = 548) and three in the control group (n = 537). The intervention focused on teacher training, opportunities for practicing physical activity at school and reducing screen time, as well as health education. The questionnaire (applied in July and December 2014) included questions about time using TV and computer / video game on weekdays and weekend days (Combined in full-screen time). The psychosocial factors were obtained through scales. The control variables were school and income. The analysis included binary logistic regression models and generalized linear models. The effect size was calculated with analyzes stratified by sex and age (p < 0.05). The intervention group reduced TV (boys: OR = 2.86; CI95%: 1.06-7.71) time and computer/video game (girls: OR = 3.34; CI95%: 1.14-9.84; 11 to 13 years: OR = 3.08; CI95%: 1.17-8.08) for < 2 hours / day (p < 0.05). Positive effects of the intervention were observed on attitude (among girls and adolescents of both age groups) and school support (all subgroups), with effect sizes ranging from 0.21 to 0.54. We concluded that the intervention caused improvements in certain components of the screen time, as well as attitude and school support. Interventions in the school context can help reduce screen time in the young population

Abstract The aim of this study was to summarize studies examining the prevalence of sedentary behavior among Brazilian children and adolescents. A systematic review conducted on eight databases (PubMed, Scopus, Web of Science, LILACS, SPORTDiscus, BIREME, Scielo, and Google Scholar). The criteria applied were: original research; samples including Brazilian children and adolescents; to be a school- or population-based survey; observational studies using different measures of sedentary behavior; studies showing the prevalence of at least one component of sedentary behavior. Of the 205 studies included, 104 were analyzed. There was a greater concentration of studies in the southern (n=41) and northeastern (n=18) regions. The components more frequently investigated were watching TV (n=36) and screen time (n=32). Only three studies included children under seven years, and the age range more investigated was 10 to 19 years. Most of the studies used a self-reported questionnaire and showed variability in the cut-off point applied (from 2 to 4 hours/day). The prevalence of adolescents who met recommendations ranged from 9.4% to 68.0% for sedentary behavior (<2 hours/day) and from 16.8% to 67.2% for TV viewing (<2 hours/day). The prevalence ranged from 39.1% to 97.7% for computer use and from 47.7% to 98.0% for videogame use. Most of the studies reported that less than half of the adolescents met the recommendations of sedentary behavior and TV viewing. On the other hand, more than half of them devoted less than two hours a day to computer and videogame use.

Revisão sistemática conduzida em oito base de dados (PubMed, Scopus, Web of Science, LILACS, SPORTDiscus, BIREME, Scielo, and Google Scholar).

Abstract Inadequate sleep has been associated with unhealthy behavior in adolescence. This study aimed to estimate the prevalence of negative sleep indicators (perceived poor-quality sleep and insufficient sleep duration) among students and to identify unhealthy behaviors associated with this outcome. This study is part of a school-based, cross-sectional survey conducted in 2011 in adolescents from Santa Catarina State, Brazil. High school students aged 15 to 19 years (n = 6,529) and enrolled in state public schools participated in the study. The students responded to a questionnaire on the number of hours slept (insufficient: < 8 h, sufficient: ≥ 8 h); perceived sleep quality (good or poor); and behavioral variables. Multinomial logistic regression was performed using reports of positive sleep indicators, and negative reports of one or two sleep indicators. Approximately 30% of adolescents reported a negative perception of sleep, indicating poor quality and insufficient duration. The use of computers/videogames (≥ 4 h/d) and excessive consumption of alcoholic beverages, salty snacks or sweets were associated with negative sleep indicators. A high number of young people displayed one or two negative sleep indicators, and it was found an association between these indicators and a number of unhealthy behaviors.
