RESUMO

Objective of the investigation was to examine clinical characteristics of peptic duodenal ulcer (DU) associated with arterial hypertension (АH) after acute gastrointestinal bleeding (AGB). The study included 25 patients with AH associated with DU, 45 patients with AH in AGB, 63 patients with AGB without AH, 48 patients with AH without AGB and 69 patients with DU without AH. If AH joins the existing DU, it increases the intensity of pain and dyspeptic syndromes. If DU joins AH, on the other hand, the clinical picture of ulcer disease was unclear. The largest concentration of Helicobacter pylori antibodies was found in patients with AH in AGB. Hypertensive crises were detected more often in combination of AH and DU.

Objectives of the investigation was to examine the dynamics of emotional and autonomic disorders in patients with peptic duodenal ulcer (DU) associated with arterial hypertension after acute gastrointestinal bleeding during training at School of Combined Pathology (SCP). Main group included 25 patients. The comparison group consisted of 26 patients with the matched pathology, sex and age, but not trained at SCP. Results. Both groups of patients demonstrated improvement in emotional sphere and autonomic regulation. However, in patients who had training at the SCP the indicators of emotional status improved earlier and got a greater extent. Optimization of autonomic regulation was identified in 3 months, not six months, like in untrained patients. Conclusion. Training hypertensive patients with peptic DU at the SCP improves the efficiency of emotional state and autonomic regulation.

Aim: Aim. To evaluate lung function in patients with liver cirrhosis of the liver with conservative therapy and surgery tunneling liver. Materials and methods: The study involved 55 patients with liver cirrhosis with portal hypertension. Patients were randomized into 2 groups. In the first group (n = 40) patients received medication. Patients of the second group (n = 15) was performed tunneling liver shunt operation. Results: In patients with liver cirrhosis observed clinical and functional remodeling of the lung appears shortness of breath, coughing, wheezing, and restrictive ventilatory disorders. Conclusions: In patients with cirrhosis of the liver after tunneling is observed normalization of lung ventilation. After conservative treatment reliable normalization of lung function occurs.

AIM: To investigate the blood pressure changes and electrocardiogram parameters for the 1st day after acute gastrointestinal bleeding in patients with arterial hypertension. MATERIAL AND METHODS: Indexes of blood pressure monitoring and electrocardiogram in case of acute gastrointestinal bleeding in patients with arterial hypertension were studied. 45 patients with arterial hypertension and acute gastrointestinal bleeding, 63 patients in acute stage of bleeding without hypertension and 48 patients suffering from hypertension without gastrointestinal bleeding were studied. Parallel simple open controlled investigation included blood pressure monitoring and Holter monitoring. RESULTS: Circadian dynamics of blood pressure in patients with arterial hypertension on the background of gastrointestinal bleeding was noted by reduced variability and insufficient reduction of pressure at night. Diastolic pressure decreased in accordance with severity of blood loss. Holter monitoring revealed high incidence of supraventricular ectopic activity, sinus arrhythmia and significant elongation of the ventricular electrical systole.

RESUMO

Comparison of the state of 83 patients with histologically confirmed sarcoidosis observed with a 10 year interval revealed remission in 47% of the cases. The main factors having negative effect on prognosis of the disease included extrapulmonary symptoms, the use ofcorticosteroids (at all stages, especially at stage I and in Lofgren syndrome) and antituberculosis drugs, positive TB test. Risk factors of relapses were stage II sarcoidosis, the use of systemic corticosteroids in patients with Lofgren syndrome and antituberculosis drugs, initially low FEV1/FVLC ratio and the number of lymphocytes in peripheral blood.

RESUMO

A case of cordarone-induced pneumopathy in a man is reported diagnosed one year after the appearance of its first symptoms. Withdrawal of cordarone and prescription of glucocorticoid therapy resulted in positive dynamics of the patient's condition. Differential diagnostics of this rather common pathology sometimes encounters difficulties. Demonstration of this complication of anti-arrhythmic therapy illustrates the importance of careful analysis of the current and past medical history of a patient with special reference to the drugs used for the treatment.

RESUMO

154 patients with moderate severity bronchial asthma (BA) received treatment including two components: electroaerosols with mineral sodium chloride water (Ust-Kachka resort) (group 1) or ward speleotherapy (group 2), standard antiinflammatory medication with beklametasone dipropionate. (BD). A control group was given BD only. Personality traits and vegetative pattern were studied before and after the treatment. General clinical response was the same in all three groups. Normalization of emotional and vegetative abnormalities was faster in groups where physical factors were employed. These findings show perspectiveness of a new line in the treatment of BA--neurorehabilitation.
