PURPOSE: Over the past few decades, the incidence of cancer among adolescents and young adults (AYA) has been increasing. The impact of behaviors, such as physical activity (PA) and nutrition, on disease progression, prognosis, and overall health and quality of life for AYA cancer survivors is of significant importance. This systematic review aims to evaluate the effectiveness of PA and diet interventions for AYA cancer survivors and to critically evaluate existing literature, gaps, and limitations. METHODS: A search of literature was conducted in PubMed, Science Direct, Scopus, and Google Scholar following the PRISMA guidelines. Twenty-two studies were included from online databases from 2012 to 2022, 13 of which were randomized controlled trials. RESULTS: Most interventions were related to PA, with only four studies including nutrition or Diet interventions. The interventions were generally feasible and acceptable to AYA cancer survivors, and digitally based PA interventions were commonly used. PA interventions mainly comprised aerobic and resistance training and were individualized. Overall, this review found various PA and diet interventions for AYA cancer survivors that were feasible and well-accepted, but gaps in knowledge and design still exist. CONCLUSIONS: This systematic review underscores the importance of conducting more research on diet interventions for YCS. PROSPERO REGISTRATION: https://www.crd.york.ac.uk/prospero/#aboutregpage.

INTRODUCTION: An acute discontinuation of tizanidine, an alpha-2-agonistic muscle relaxant, is associated with reflex tachycardia, hypertension, tremor, hypertonicity and anxiety. CASE REPORT: We describe a 53-year-old patient with broken-heart syndrome, who developed serious tizanidine withdrawal symptoms after high-dosed long-term treatment within the framework of stress cardiomyopathy. CONCLUSION: Central muscle relaxants like tizanidine might have an impact on the development of delirium. Tizanidine withdrawal should be considered in patients who manifest signs and symptoms of withdrawal from medications. The drug should be gradually reduced in dosage under observation by a psychiatrist. When prescribing tizanidine, the possible pharmacological side effects and interactions should be taken into careful account.

In areas without newborn screening for severe combined immunodeficiency (SCID), disease-defining infections may lead to diagnosis, and in some cases, may not be identified prior to the first year of life. We describe a female infant who presented with disseminated vaccine-acquired varicella (VZV) and vaccine-acquired rubella infections at 13 months of age. Immunological evaluations demonstrated neutropenia, isolated CD4 lymphocytopenia, the presence of CD8(+) T cells, poor lymphocyte proliferation, hypergammaglobulinaemia and poor specific antibody production to VZV infection and routine immunizations. A combination of whole exome sequencing and custom-designed chromosomal microarray with exon coverage of primary immunodeficiency genes detected compound heterozygous mutations (one single nucleotide variant and one intragenic copy number variant involving one exon) within the IL7R gene. Mosaicism for wild-type allele (20-30%) was detected in pretransplant blood and buccal DNA and maternal engraftment (5-10%) demonstrated in pretransplant blood DNA. This may be responsible for the patient's unusual immunological phenotype compared to classical interleukin (IL)-7Rα deficiency. Disseminated VZV was controlled with anti-viral and immune-based therapy, and umbilical cord blood stem cell transplantation was successful. Retrospectively performed T cell receptor excision circle (TREC) analyses completed on neonatal Guthrie cards identified absent TREC. This case emphasizes the danger of live viral vaccination in severe combined immunodeficiency (SCID) patients and the importance of newborn screening to identify patients prior to high-risk exposures. It also illustrates the value of aggressive pathogen identification and treatment, the influence newborn screening can have on morbidity and mortality and the significant impact of newer genomic diagnostic tools in identifying the underlying genetic aetiology for SCID patients.

The stratification of reticulorumen (RR) contents has been described in domestic ruminants, but suspected to be absent in certain wild ruminant species. To investigate how such stratification could be quantified, we tested variables indicating stratification in three oxen with rumen cannulae, fed once daily for 3 h, one of three different forages (fresh grass FG, grass hay GH, lucerne hay LH). Ingesta samples from dorsal rumen (DR), ventral rumen (VR) and reticulum (RET) were taken at 0, 3, 6, 12 and 24 h after the last meal and analyzed for dry matter (DM) concentration, mean particle size and proportion of floating/sedimenting particles. Viscosity was measured in centrifuged RR fluid. There was no relevant variation in rumen fluid viscosity over time or with feeding regime. Dry matter content in DR was always higher than in VR or RET and increased after feeding. When LH and FG were fed, DR always contained larger particles than VR, except at 6 and 12 h of feeding GH. The proportion of floating particles was higher in DR than VR except at GH 6 h and GR between 6 and 12 h. The floating particle fraction mostly contained larger particles, except for GH 3 h in both DR and VR and for FG 3-12 h in DR. Changes in the distribution of particle size and proportion of floating particles over time comply with the theoretical course of fermentative digestion for different forages, but make these variables unreliable indicators of stratification in less controlled settings, such as investigations in wild ruminants. In contrast, viscosity of rumen fluids appears constant over time and feeding regime and might be used as a species-specific variable. The difference in DM content in DR and VR is a reliable indicator for RR contents stratification characterized by an oversaturation of the contents with fluid, leading to lower VR DM contents.

We report on a picosecond time-resolved x-ray magnetic circular dichroic-photoelectron emission microscopy study of the evolution of the magnetization components of a microstructured permalloy platelet comprising three cross-tie domain walls. A laser-excited photoswitch has been used to apply a triangular 80 Oe, 160 ps magnetic pulse. Micromagnetic calculations agree well with the experimental results, both in time and frequency, illustrating the large angle precession in the magnetic domains with magnetization perpendicular to the applied pulse, and showing how the magnetic vortices revert their core magnetization while the antivortices remain unaffected.

This report summarizes data obtained via a mailed questionnaire from 129 Department of Veterans Affairs (VA) hospitals regarding current practices in the care of central venous catheters (CVCs) used for total parenteral nutrition (TPN). The size of VA hospitals' acute medical-surgical beds ranged from 14 to 1320 (median 168) beds. Over 6000 patients annually received CVCs for TPN. Hospitals reported using triple-lumen catheters most frequently as their CVC for TPN (80.3%). A povidone-iodine scrub was used to prepare the skin for CVC insertion by 72.6% of reporting hospitals. Sixty percent of hospitals used transparent polyurethane dressings. Care of CVCs varied among hospitals. Catheter-related infection and sepsis rates were within the national average, although < 50% of responding hospitals provided data on these outcomes. The results of this survey point to the need for a national standardized database relative to patients receiving TPN via a CVC.

Bilateral subclavian vein thrombi developed, in the presence of a Groshong catheter, in a patient with the acquired immunodeficiency syndrome (AIDS) and disseminated cytomegalovirus infection. Thrombosis is an uncommon complication of AIDS, and bilateral subclavian thrombi are rare. Although localized, unilateral thrombi occur in patients with indwelling central venous catheters, unexpectedly extensive bilateral clot formation is seen to develop in patients with human immunodeficiency virus infection. Potentially predisposing factors for thrombosis in AIDS include central venous catheterization, malignancy, elevated anticardiolipin antibodies, alteration of vascular endothelial cell function reflected in low free protein S levels, and enhanced endothelial procoagulant activity related to cytomegalovirus. Anecdotal data strongly suggest that cytomegalovirus enhances the risk for thrombosis, but studies are needed to confirm the association. Physicians should be aware of the enhanced risk for extensive thrombi in AIDS, especially in patients with disseminated cytomegalovirus infection.

In experiments on competition between Pratylenchus neglectus and Meloidogyne chitwoodi in barley, the species that parasitized the roots first inhibited penetration by the latter species. Prior presence of P. neglectus impeded the development of M. chitwoodi. Pratylenchus neglectus reduced egg production, final population levels, and reproductive index of M. chitwoodi. The reduction was linearly related to initial population densities of P. neglectus. Initial population densities of M. chitwoodi had no effect on final population levels of P. neglectus. Carbon assimilation by barley plants was reduced when either nematode species was present alone, but not when both were present together. Both nematode species assimilated lower amounts of carbon when present together than when present alone. A split-root experiment demonstrated that translocatable chemicals were not involved in the competition between the two species.

Lipoxygenase activity with linoleic acid as substrate and the immunologically detectable amount of lipoxygenase protein were estimated in the course of in vitro maturation of rabbit reticulocytes withdrawn at the sixth day of an experimental bleeding anaemia. With unseparated cell mixture there was a significant increase in the lipoxygenase activity of 67 +/- 15% after a maturation period of 4 h followed by a decrease up to the initial level. The maturational changes were more pronounced when the fraction of youngest reticulocytes after buoyant density separation in a serum albumin gradient was used, whereas the cells of medium density failed to show the intermittent increase. The lipoxygenase activity was largely paralleled by the amount of lipoxygenase protein. The increase of lipoxygenase was prevented by either anaerobiosis or addition of oligomycin. The protein synthesis was greatly decreased after 4 h of incubation. The decline of the amount of lipoxygenase between 4 and 24 h incubation is probably largely caused by proteolysis. The results support former interpretations as to the synthesis and biological dynamics of lipoxygenase in reticulocytes.

RESUMO

Facial pain is a perplexing problem confronting all who practice the healing arts. The purpose of this article is to concentrate on one aspect of facial pain--trigeminal neuralgia. An overview of this entity is presented by demonstrating current concepts in its etiology, diagnosis, and treatment. Classically, trigeminal neuralgia has been described as a paroxysmal, lancinating, knifelike pain which is limited to the anatomic pathways of the fifth cranial nerve. It is a chronic facial pain that is amenable to medical and surgical treatment when correctly diagnosed. A knowledge of the anatomy of the fifth cranial nerve is essential for a correct diagnosis of trigeminal neuralgia. A description of the anatomy is not within the scope of this article, and the reader is referred to any of the standard anatomy textbooks for review.
