RESUMO

Prussian blue is an iron-cyanide-based pigment steadily becoming a widely used electrochemical sensor in detecting hydrogen peroxide at low concentration levels. Prussian blue nanoparticles (PBNPs) have been extensively studied using traditional ensemble methods, which only provide averaged information. Investigating PBNPs at a single entity level is paramount for correlating the electrochemical activities to particle structures and will shed light on the major factors governing the catalyst activity of these nanoparticles. Here we report on using plasmonic electrochemical microscopy (PEM) to study the electrochemistry of PBNPs at the individual nanoparticle level. First, two types of PBNPs were synthesized; type I synthesized with double precursors method and type II synthesized with polyvinylpyrrolidone (PVP) assisted single precursor method. Second, both PBNPs types were compared on their electrochemical reduction to form Prussian white, and the effect from the different particle structures was investigated. Type I PBNPs provided better PEM sensitivity and were used to study the catalytic reduction of hydrogen peroxide. Progressively decreasing plasmonic signals with respect to increasing hydrogen peroxide concentration were observed, demonstrating the capability of sensing hydrogen peroxide at a single nanoparticle level utilizing this optical imaging technique.

BACKGROUND: Frailty is common and is associated with increased mortality, lower quality of life, and higher readmission rates in cirrhotic patients. Not only are these outcomes important, but further understanding the impact of frailty on a caregiver's life is crucial to better comprehend caregiver burden in cirrhotic patients and develop strategies to improve care for patients and their caregivers. METHODS: A single-center, prospective study was conducted of cirrhotic patients and their caregivers between 4/1/2019 and 11/1/2019. Frailty testing combined aspects from the Fried Frailty Instrument, Short Physical Performance Battery, and activities of daily living. Caregivers completed questionnaires to evaluate caregiver burden using the Zarit Burden Interview (ZBI-12), and perceived social support, using the Interpersonal Support Evaluation List. RESULTS: In total, 94 cirrhotic patients were included, 50% males with a median age of 63.1 years. The most common etiology of cirrhosis was nonalcoholic steatohepatitis. Frailty was prevalent (45.1%). In total, 12.8% of caregivers reported a high burden based on ZBI-12. There was no association between frailty and caregiver burden, hospitalization rates, or death. However, frailty was associated with a higher number of outpatient GI visits (p = 0.002). Lower perceived social support among caregivers was associated with a higher caregiver burden (p < 0.0001). CONCLUSION: Frailty is prevalent in cirrhotic patients but is not associated with higher rates of caregiver burden. Low perceived social support among caregivers, however, was associated with higher caregiver burden. It is important to recognize the impact of caregiver burden on caregivers of cirrhotic patients and ensure caregivers have the appropriate support to mitigate burden.

RESUMO

Historically, systemic treatment of advanced hepatocellular carcinoma was limited to the tyrosine kinase inhibitor sorafenib. With the recent approval of several new agents the armamentarium of treatment options available to providers and patients has expanded. Although these promising advances offer hope for patients with advanced hepatocellular carcinoma, they also present new and challenging adverse effects that threaten to limit their efficacy. Immunotherapy with checkpoint inhibitors introduce immune-related adverse events, which may affect a wide array of organ systems. With prompt recognition, however, common side effects of systemic therapies for hepatocellular carcinoma are predictable, manageable, and many improve with appropriate intervention.

RESUMO

Nanosensors built with pre-pulled glass nanopipettes, including bare or chemically modified nanopipettes and fully or partially filled solid nanoelectrodes, have found applications in chemical and biological sensing via resistive-pulse, current rectification, and electrochemical sensing. These nanosensors are easily fabricated and provide advantages through their needle-like geometry with nanometer-sized tips, making them highly sensitive and suitable for local measurements in extremely small samples. The variety in the geometry and layout have extended sensing capabilities. In this review, we will outline the fundamentals in fabrication, modification, and characterization of those pre-pulled glass nanopipette based nanosensors and highlight the most recent progress in their development and applications in real-time monitoring of biological processes, chemical ion sensing, and single entity analysis.
