In present study, Water Quality Index (WQI) has been assessed of the Rawal Lake which is a major source of drinking water for people in the Federal Capital, Islamabad, and its adjacent city Rawalpindi in Pakistan. For this, the principal component analysis (PCA) and WQI were applied as an integrated approach to quantitatively explore difference based on spatial variation in 11 water quality parameters of the five major feeding tributaries of the Rawal Lake, Pakistan. The results of temperature in water, total dissolved solids, pH, electrical conductivity, chlorides and sulfates were well within the allowable World Health Organisation's (WHO) limits. However, the heavy metals like cadmium and lead were above permissible limits by the WHO in tributaries of Bari Imam and Rumli. Moreover, this has been proven by the Pearson correlation which suggested strong positive correlation (0.910*) between lead and cadmium. The results of present study were subjected to statistical analysis, i.e., PCA which gave three major factors contributing 96.5% of the total variance. For factor 1, pH, TDS, alkalinity, chlorides, sulfates and zinc have highest factor loading values (>0.60) and presented that these parameters were among the most significant parameters of first factor. As per the WQI results, the water was categorised in two major classes indicating that water of Bari Imam and Rumli is highly contaminated with heavy metals and totally unsuitable for drinking purposes. Based on the results of the present study, it is suggested to make heavy metals consideration as an integrated component in future planning for maintaining water quality of the Rawal Lake and its tributaries.

Global warming is a wide-scale problem and soil carbon sequestration is its local scale, natural solution. Role of soil as carbon sink has been researched extensively but the knowledge regarding the role of soil variables in predicting soil carbon uptake and its retention is scarce. The current study predicts SOC stocks in the topsoil of Islamabad-Rawalpindi region keeping the soil properties as explanatory variables and applying the partial least square regression model on two different seasons' datasets. Samples collected from the twin cities of Islamabad and Rawalpindi were tested for soil color, texture, moisture-content, SOM, bulk density, soil pH, EC, SOC, sulphates, nitrates, phosphates, fluorides, calcium, magnesium, sodium, potassium, and heavy metals (nickel, chromium, cadmium, copper and manganese) by applying standard protocols. Afterwards, PLSR was applied to predict the SOC-stocks. Although, current SOC stocks, ranged from 2.4 to 42.5 Mg/hectare, but the outcomes of PLSR projected that if soil variables remain unaltered, the SOC stocks would be likely to get concentrated around 10 Mg/hectare in the region. The study also identified variable importance for both seasons' datasets so that noisy variables in the datasets could be ruled out in future researches and precise and accurate estimations could be made.

Analysis of cancer biomarkers has enormous promise for advancing our molecular understanding of illness and facilitating more precise and timely diagnosis and follow-up care. MicroRNA, exosomes, ctDNA, CTCs, and proteins are only some of the circulating biomarkers that can be detected by liquid biopsy instead of the more intrusive and time-consuming process of doing a tissue biopsy. As the cancer diagnosis bio-markers reveal ultra-low levels in the early stages of the disease, highly sensitive approaches are urgently required. Researchers have taken an interest in a optical biosensor for detecting cancer biomarkers as a potential tool for early disease diagnosis. These techniques have the potential to aid in the development of effective treatments, ultimately leading to a higher rate of patient survival. This review briefly discuss the i) understanding of cancer and biomarkers for early diagonosis purpose ii) Molecular methods and ii) biosensor-based diagnostics. The reseach primary focus on advancement in biosensor design using various concepts ie., Electrochemical, Chemiluminescence and Colorimetric, Surface plasmons (SP), Surface plasmon resonance (SPR), localized surface plasmon resonance (LSPR), Fluorescence, Fiber-based sensors, Terahertz based biosensors, and Surface enhanced Raman spectroscopy (SERS). As a result of the local electric field amplification around plasmonic (usually gold and silver) nanostructures, surface-enhanced Raman spectroscopy (SERS) has emerged as a rapid, selective, and sensitive alternative to conventional laboratory analytical methods, making significant strides in a number of biosensing applications but still under developing stage to be used as diagnostic tool in clinical research.

Background: Nanotechnology finds broad applications in the field of nanomedicine, an emerging new field used for diagnosis, treatment, prevention of diseases, and improvement of health. Objectives: To synthesize silver nanoparticles (AgNPs) from Withania somnifera and Fagonia indica and to carry out their antimicrobial, insecticidal, and phytotoxic activities, a step toward the new range of nanomedicines. Methods: Silver nanoparticles were synthesized from Withania somnifera and Fagonia indica by chemical reduction method, and further biological activities of these nanoparticles were compared with crude methanolic extract, prepared through cold maceration process, at the concentration of 50 mg/ml. Results: Among all tested bacterial pathogens, crude extract of W. somnifera showed a statistically high significant inhibition zone in millimeter against Pseudomonas aeruginosa (21; p < 0.01). AgNPs showed highly significant result against Streptococcus pneumonia (14; p < 0.01). In comparison with crude extracts, AgNPs showed statistically significant (p < 0.01) results against S. pneumonia (AgNPs, 14; crude, 8.33 mm). Crude extract showed significant inhibition zone against two bacterial strains, P. aeruginosa (crude, 21; AgNPs, 11.67 mm) and Klebsiella pneumoniae (crude, 11.33; AgNPs, 8 mm). Crude extracts of F. indica showed the significant activity against Vibrio cholera (p < 0.01; 11.33 mm). Silver nanoparticles of F. indica exhibited the highest significant activity against Aspergillus flavus and Fusarium oxysporum while AgNPs of W. somnifera were active only against A. flavus. Extracts of W. somnifera and F. indica showed increasing phytotoxic activity with increasing concentrations. The highest significant inhibition was obtained for crude extract (46.7) and AgNPs (45.7) of F. indica at 1000 µg/ml. Insecticidal activity of crude and AgNPs of both plants showed significant inhibition against all tested insects with increasing time intervals, and the highest significant result was obtained at 72 hours with a value of p < 0.01 except T. castaneum. Conclusions: Both crude and AgNPs showed potent activity; however, in comparison, silver nanoparticles showed slightly enhanced activity. Crude and AgNPs of both plants showed good phytotoxic and insecticidal inhibition. Antimicrobial studies of AgNPs on diseases causing pathogens open a door for new antimicrobial agents and could be the answer to antibiotic resistance after further analysis.

A polyherbal formulation consisting of Mentha piperita L., Camellia sinensis L. Kuntze, and Elettaria cardamomum (L.) Maton with a ratio of 10 : 5 : 2, respectively, was recommended for curing nausea, vomiting, and diarrhea. Experimental validation is crucial to affirm its therapeutic property leads toward the development of modified antidiarrheal agents. This research aimed to investigate the in vivo antidiarrheal efficacy of traditionally used polyherbal recipe in a castor oil-induced animal model. Moreover, the study also presents the elemental screening and in vivo toxicity of tested polyherbal recipe. Individual plant parts of the polyherbal recipe were mixed according to the traditional prescription ratio, and hydromethanolic extract was prepared by the cold maceration process. The antidiarrheal activity was assessed by castor oil induction method, charcoal meal test, and enteropooling procedure in Sprague-Dawley rats. Elemental analysis and in vivo subacute toxicity were carried out, followed by biochemical, hematological, and histopathological analyses. Polyherbal extract significantly delayed the diarrhea onset in a dose-dependent manner and showed marked inhibition at 200 and 400 mg/kg. Fecal weight was reduced significantly (p < 0.05) at 200 mg/kg (0.26 ± 0.25) in comparison with the control (1.63 ± 0.15). The diarrhea score was zero at a concentration of 200 and 400 mg/kg. Antienteropooling effect of the extract was greater than that of loperamide. Following subacute toxicity, all the treated rats were normal, survived, and showed no changes in behavior. There were no significant differences between values of blood parameters in both the control and extract-treated groups except a significant decrease in monocytes (control 8.4; polyherbal 2.2). Elemental analysis showed a slight increase in the amount of manganese (Mn, 8.076 ppm) as compared to the WHO recommended level (2 ppm). Traditionally used polyherbal recipe is effective and safe for combating diarrheal diseases. In vivo evidence supported the use, safety, and efficacy of the polyherbal recipe that has been used as an alternative medicine for diarrhea in the study area. Inhibition of castor oil-induced diarrhea and antisecretory effect of the studied polyherbal recipe makes it a potent antidiarrheal drug without no or limited toxic effects at the tested dose after further analysis.

Emerging antibiotic resistance has become a cosmopolitan problem and evoking researchers to search for new antimicrobials from natural constituents. The present study was intended to test the antimicrobial potential of traditionally used unexplored polyherbal recipes for curing digestive ailments. A total of 25 plants species were combined in different ratios to form 14 polyherbal recipes. After collecting and grinding plant parts, methanolic extracts of 14 polyherbal recipes were prepared by the cold maceration process. Antibacterial and antifungal activity of the polyherbal extracts was checked by agar well diffusion method at a concentration of 50 mg/ ml while minimum inhibitory concentration (MIC) was determined by serial dilution method. Polyherbal recipes B and D showed significant inhibition zone each against Vibrio cholerae (25.63; p < 0.001). Recipe G (23.33; p < 0.001) showed better efficacy against Escherichia coli. Recipe E and G significantly inhibited Proteus species (28.33; 24.33; p < 0.001). Recipe B was highly effective against Salmonella typhi. Recipe C, A and F had significant antifungal affect and inhibited Aspergillus nigar (28.67; p < 0.05), Aspergillus fumigatus (27; p < 0.01) Trichoderma (30; p < 0.001), Rhizopus (19.67; p < 0.01), and Fusarium graminearum (28.67;p < 0.001). Polyherbal formulations A, B, D, K, and N were active with the lowest concentration. MIC ranges within 3.12-25 mg/ml while minimum bactericidal concentration (MBC) between 12.5 and 50 mg/ml. Polyherbal recipes' A, B, D, G, K and N have enhanced antimicrobial potential with better efficacy than tested antibiotics and should be evaluated for further scientific validation.

This study examined the relationships between perfectionism, self-esteem, and the will to win and the effects of gender and the level of achievement on these variables. A total of 318 adolescents in the age group of 12-19 years (M = 16.10 ± 1.01) completed the self-esteem questionnaire, the will-to-win questionnaire, and the perfectionism inventory. Interstate level (ISL) athletes obtained higher scores than interdistrict level (IDL) athletes on the following variables: self-esteem, the will to win, and four of the eight dimensions of perfectionism (i.e., concern over mistakes, the need for approval, organization, and planfulness). Further, male athletes obtained higher self-esteem and perfectionism (i.e., the need for approval and rumination) scores than female athletes. Self-esteem, the will to win, and the dimensions of perfectionism were positively and significantly interrelated. However, one dimension, namely, perceived parental pressure, was unrelated to any factor except striving for excellence. Further, the will to win, concern over mistakes, high standard for others, and planfulness were unrelated to striving for excellence. The results of the discriminant analysis revealed that there was no significant difference between ISL and IDL athletes (variance explained = 9.480%). Finally, using path analysis showed that Model 3 (perfectionism self-esteem will-to-win) has provided good model fit such as Bentler's comparative fit index (CFI) (0.987), Tucker-Lewis index (TLI) (0.876), normed fit index (NFI) (0.973), and the root mean square error of approximation (RMSEA) (0.097).

Laparoscopic surgery is normally performed under general anesthesia, but regional techniques like thoracic epidural and lumbar spinal have been emerging and found beneficial. We performed a clinical case study of segmental thoracic spinal anaesthesia in a healthy patient. We selected an ASA grade I patient undergoing elective laparoscopic cholecystectomy and gave spinal anesthetic in T10-11 interspace using 1 ml of bupivacaine 5 mg ml-1 mixed with 0.5 ml of fentanyl 50 µg ml-1. Other drugs were only given (systemically) to manage patient anxiety, pain, nausea, hypotension, or pruritus during or after surgery. The patient was reviewed 2 days postoperatively in ward. The thoracic spinal anesthetia was performed easily in the patient. Some discomfort which was readily treated with 1mg midazolam and 20 mg ketamine intravenously. There was no neurological deficit and hemodynamic parameters were in normal range intra and post-operatively and recovery was uneventful. We used a narrow gauze (26G) spinal needle which minimized the trauma to the patient and the chances of PDPH, which was more if 16 or 18G epidural needle had been used and could have increased further if there have been accidental dura puncture. Also using spinal anesthesia was economical although it should be done cautiously as we are giving spinal anesthesia above the level of termination of spinal cord.

Soil providing a wide array of ecosystem services is subjected to quality deterioration due to natural and anthropogenic factors. Most of the soils in Pakistan have poor status of available plant nutrients and cannot support optimum levels of crop productivity. The present study statistically analyzed ten soil quality parameters in five subwatersheds (Bari Imam, Chattar, Rumli, Shahdra, and Shahpur) of the Rawal Lake. Analysis of variance (ANOVA), cluster analysis (CA), and principal component analysis (PCA) were performed to evaluate correlation in soil quality parameters on spatiotemporal and vertical scales. Soil organic matter, electrical conductivity, nitrates, and sulfates were found to be lower than that required for good quality soil. Soil pH showed significant difference (p < 0.05) in mean values at different sampling sites and sampling months indicating that it is affected and determined by land uses and seasons. Pearson correlation revealed a strong positive correlation (r = 0.437) between nitrates and organic matter. Application of principal component analysis resulted in three major factors contributing 76 % of the total variance. For factor 1, temperature, sand, silt, clay, and nitrates had the highest factor loading values (>0.75) and indicated that these were the most influential parameters of first factor or component. Cluster analysis separated five sampling sites into three statistically significant clusters: I (Shahdra-Bari Imam), II (Chattar), and III (Shahpur-Rumli). Among the five sites, Shahdra was found to have good quality soil followed by Bari Imam. The present study illustrated the usefulness of multivariate statistical approaches for the analysis and interpretation of complex datasets to understand variations in soil quality for effective watershed management.

ETHNOPHARMACOLOGICAL RELEVANCE: Substantially, plants produce chemicals such as primary and secondary metabolites, which have significant applications in modern therapy. Indigenous people mostly rely on traditional medicines derived from medicinal plants. These plants have the capacity to absorb a variety of toxic elements. The ingestion of such plants for medicinal purpose can have imperative side effects. Hence, with regard to the toxicological consideration of medicinal plants, an effort has been made to review the elemental contents of ethno medicinally important plants of Pakistan and to highlight the existing gaps in knowledge of the safety and efficacy of traditional herbal medications. MATERIALS AND METHODS: Literature related to the elemental contents of ethno medicinal plants was acquired by utilizing electronic databases. We reviewed only macro-elemental and trace elemental contents of 69 medicinal plant taxa, which are traditionally used in Pakistan for the treatment of sundry ailments, including anemia, jaundice, cancer, piles, diarrhea, dysentery, headache, diabetes, asthma, blood purification, sedative and ulcer. RESULTS: A majority of plants showed elemental contents above the permissible levels as recommended by the World health organization (WHO). As an example, the concentrations of Cadmium (Cd) and Lead (Pb) were reportedly found higher than the WHO permissible levels in 43 and 42 medicinal plants, respectively. More specifically, the concentrations of Pb (54ppm: Silybum marianum) and Cd (5.25ppm: Artemisia herba-alba) were found highest in the Asteraceae family. CONCLUSIONS: The reported medicinal plants contain a higher amount of trace and toxic elements. Intake of these plants as traditional medicines may trigger the accumulation of trace and toxic elements in human bodies, which can cause different types of diseases. Thus, a clear understanding about the nature of toxic substances and factors affecting their concentrations in traditional medicines are essential prerequisites for efficacious herbal therapeutics with lesser or no side effects.

Since mean nitrate concentration along single river channels increases significantly with percent arable land use upstream of sampling points and autumn/early winter flushes in nitrate concentration are widespread, it is generally concluded that farmers contribute most of the nitrate. For the River Derwent in North Yorkshire, the correlation between nitrate concentration and percent arable land use is much poorer when tributary data are included in the equation, because of greater variations in dilution by water draining upland areas and in other N input sources. For the whole river system therefore, percent upland moorland/rough grazing land cover is an appreciably better predictor than percent arable land use for nitrate concentration. Upland land use encompasses the higher precipitation and runoff in such areas, and the subsequent greater dilution downstream of both arable land runoff and effluent from treatment works, as well as an inverse correlation to percent arable land use. This is strongly supported by the observation that, for the Derwent, Meteorological Office rainfall data alone proved even better than percent moorland rough grazing for predicting nitrate concentration. The dilution effect is therefore substantial but highly seasonal; lower runoff and dilution in summer offset the lower leaching losses from arable land, and higher dilution and runoff in winter offset greater nitrate leaching losses from arable soils. Because of this, coupled to improved efficiency of nitrogen fertilizer use, seasonality trends in nitrate concentrations that were pronounced a decade ago now have all but disappeared in the catchment.

This study investigates the drinking water (groundwater and surface water) quality and potential risk assessment along mafic and ultramafic rocks in the Swat district of Khyber Pakhtunkhwa Provence, Pakistan. For this purpose, 82 groundwater and 33 surface water samples were collected and analyzed for physico-chemical parameters. Results showed that the majority of the physico-chemical parameters were found to be within the drinking water guidelines set by the World Health Organization. However, major cationic metals such as magnesium (Mg), and trace metals (TM) including iron (Fe), manganese (Mn), nickel (Ni), chromium (Cr) and cobalt (Co) showed exceeded concentrations in 13%, 4%, 2%, 20%, 20% and 55% of water samples, respectively. Health risk assessment revealed that the non-carcinogenic effects or hazard quotient values through the oral ingestion pathway of water consumption for the TM (viz., Fe, Cr and Mn) were found to be greater than 1, could result in chronic risk to the exposed population. Results of statistical analyses revealed that mafic and ultramafic rocks are the main sources of metal contamination in drinking water, especially Ni and Cr. Both Ni and Cr have toxic health effects and therefore this study suggests that contaminated sites should be avoided or treated for drinking and domestic purposes.

The present study was designed to document detailed ethnogynaecological knowledge of selected remote regions of Pashtun's tribe in northwest Pakistan. Semistructured questionnaires were designed to collect ethnogynaecological and ethnographic data. Total of 51 medicinal plants belonging to 36 families were documented that were used by the women of studied regions for the treatment of 9 types of gynaecological complaints. Majority of the plants (19) were found used against menses followed by 11 plants each for gonorrhea and pregnancy. Bannu region has high number of gynaecological plants (22) followed by Karak (15). Women of the regions mostly used whole plants (33%) and leaves (31%) for various ethnomedicinal preparation of gynae. Fic results showed that all ailments in different areas scored high consensus ranges between 0.6 and 1.00. Majority of the female respondents (44%) were aged between 61 and 70 years, of which most were illiterate. Women in the remote regions of Pakistan have tremendous traditional knowledge in utilizing medicinal plants for their reproductive health. Plants with high Fic values should be cross-checked for their in vitro and in vivo validation. Young girls should be educated on the importance of ethnogynaecological practices to conserve this valuable knowledge.

BACKGROUND: Himalayan region of Pakistan has been known as a rich source for valuable medicinal plants. The present work is the documentation of indigenous knowledge of highly utilized plants as natural remedy by the local inhabitants of the northwest part of the country. METHODOLOGY: Data on highly utilized medicinal plants were collected during May-September 2012 using semi-structured questionnaire from the elders, local herbalists and medicinal plants collectors. RESULTS: In total about 53, plants belonging to 38, families were highly utilized in the study area, of which majority of those (43), were medicinal plants. The most frequent part used in preparation of different medicinal remedies is leaves (53%). Most of the medicinal plant species (14) were used as antipyretic. The inhabitants of the study area were producing ethno-medicines mostly in crushed form (26%). Most of the remedies (74%), were taken orally within the study area. Ten species were found most valuable from the local's perspective such as Bergenia ciliata, Hedera nepalensis and Viola canescens. CONCLUSION: In conclusion, traditional healers depend largely on naturally growing medicinal plant species, which are under severe threat. This study can provide a basis for further phyto-chemical and pharmacological studies on the region's medicinal plants. Moreover, documentation of indigenous recipes would serve as preservation of the knowledge for the generations to come.

OBJECTIVES: Caralluma is a xerophytic genus used as traditional medicine for the treatment of diabetes, inflammation, leprosy, obesity and rheumatism. Objectives of this review are to establish a relationship between traditional uses and scientific studies by critically evaluating the available fragmented literature on ethnobotany, pharmacology, phytochemistry and toxicology of genus Caralluma. KEY FINDINGS: Ethnomedical uses of Caralluma have been recorded from various countries such as China, India, Iran and Pakistan for six major classes of diseases including diabetes and gastrointestinal disorders. This review indicated the efficacy of genus Caralluma in several in vitro and in vivo pharmacological properties such as antimicrobial, antioxidant and anticancer activity. These bioactivity might be due to the presence of certain classes of compounds in genus Caralluma including pregnane glycosides, flavonoid glycosides and flavones. SUMMARY: Traditional uses and scientific evaluation of Caralluma indicates that it is one of the most widely used genus in some parts of the world. Further studies on the structural activity relationship of some of the isolated compound may improve their biological potency as well as scientific exploitation of traditional uses of the genus.

Long-term spatial and temporal variations in nitrate-N concentrations along the River Derwent have been examined using Environment Agency data to investigate the relative importance of impacts of atmospheric N deposition, land use, and changes in management. Where moorland and rough grazing dominate upstream of Forge Valley and Malton, over the 20 years since 1988 mean nitrate-N concentrations were initially increasing significantly, but are now levelling off, with peaks at ca. 4.5 mg Nl(-1). As expected in a catchment in a nitrate vulnerable zone (NVZ), more agricultural land use increases mean nitrate concentrations and the occurrence of distinct winter maxima, though the latter have become markedly less pronounced since 2001. It is suggested that this improvement is a combined effect of imposition of NVZ designation in the lower reaches in 2002, animal number declines associated with the Foot & Mouth outbreak in the region in 2001, and the impact of farmers' responses to increasing fertilizer prices and to beneficial pollutant mineral N inputs from the atmosphere. Minima in nitrate-N concentrations in summer have become much less pronounced over the past decade and are typically ca. 60% higher in concentration than a decade earlier. This probably is attributable to the effects of pollutant-N leaching to depths in soil below the rooting zone when near surface biotic uptake is low in winter. The resultant N mineralization in summer enhances summer nitrate leaching. The Derwent is a relatively clean river; however, its entire catchment was designated justifiably as a NVZ in January 2009, apparently based upon a projected 95 percentile nitrate-N concentration >11.29 mg l(-1) for 2010 based upon forward projection of data from 1990 to 2004 for Derwent Bridge. A survey of water quality in March 2009 showed that some agricultural areas are still making a significant contribution to the total nitrate level well downstream, at the point responsible for implementation of NVZ status. At 3 of the 29 sites sampled, nitrate concentration exceeded 60 mg l(-1).
