BACKGROUND: The isolation of cell-free DNA (cfDNA) from the bloodstream can be used to detect and analyze somatic alterations in circulating tumor DNA (ctDNA), and multiple cfDNA-targeted sequencing panels are now commercially available for Food and Drug Administration (FDA)-approved biomarker indications to guide treatment. More recently, cfDNA fragmentation patterns have emerged as a tool to infer epigenomic and transcriptomic information. However, most of these analyses used whole-genome sequencing, which is insufficient to identify FDA-approved biomarker indications in a cost-effective manner. PATIENTS AND METHODS: We used machine learning models of fragmentation patterns at the first coding exon in standard targeted cancer gene cfDNA sequencing panels to distinguish between cancer and non-cancer patients, as well as the specific tumor type and subtype. We assessed this approach in two independent cohorts: a published cohort from GRAIL (breast, lung, and prostate cancers, non-cancer, n = 198) and an institutional cohort from the University of Wisconsin (UW; breast, lung, prostate, bladder cancers, n = 320). Each cohort was split 70%/30% into training and validation sets. RESULTS: In the UW cohort, training cross-validated accuracy was 82.1%, and accuracy in the independent validation cohort was 86.6% despite a median ctDNA fraction of only 0.06. In the GRAIL cohort, to assess how this approach performs in very low ctDNA fractions, training and independent validation were split based on ctDNA fraction. Training cross-validated accuracy was 80.6%, and accuracy in the independent validation cohort was 76.3%. In the validation cohort where the ctDNA fractions were all <0.05 and as low as 0.0003, the cancer versus non-cancer area under the curve was 0.99. CONCLUSIONS: To our knowledge, this is the first study to demonstrate that sequencing from targeted cfDNA panels can be utilized to analyze fragmentation patterns to classify cancer types, dramatically expanding the potential capabilities of existing clinically used panels at minimal additional cost.

INTRODUCTION: Neuroendocrine prostate cancer (NEPC) is an aggressive late-stage variant of PC that is often androgen-receptor negative. Most clinicians believe the VTE rate with NEPC is higher than with standard metastatic castration-resistant PC (mCRPC), but NEPC tends to present with bulkier visceral disease and include platinum chemotherapy unlike standard PC. In many solid tumors, a more aggressive phenotype correlates with increased VTE risk and elevated expression of coagulation factors. We previously reported on the differential expression of thrombin and tissue factor (TF) in NEPC versus localized PC and benign prostate tissue with a small NEPC cohort (N=7), which showed overexpression of prothrombin and reduced expression of TF in NEPC. AIM: To compare the expression of coagulation factors of NEPC vs mCRPC (and localized PC control) in an expanded datase. MATERIALS AND METHODS: Fresh frozen tissue biopsies were collected and separated into three cohorts based on pathology: localized PC (N=68), standard mCRPC (N=32), and NEPC (N=21). RNA was isolated and next generation paired-end mRNA sequencing was performed on Illumina Sequencers. F2 Prothrombin (F2), tissue factor (F3), carboxypeptidase (CPB2), fibrinogen (FGG, FGA), PAR-1 (F2R), and PAR-2 (F2RL1) were compared by Wilcoxon tests. RESULTS: Prothrombin had significantly higher expression in NEPC versus standard mCRPC (p <0.001). NEPC trended towards higher expression of CPB2 (p=0.1) and lower expression of F3 (p=0.23) and F2RL1 (p=0.14) compared to mCRPC. Compared to localized PC, both types of advanced disease (NEPC and mCRPC) overexpressed F2, FGA, FGB, and CPB2 (p<0.001) and had decreased expression of F3 and F2RL1 (p <0.001). CONCLUSIONS: Prothrombin is reliably overexpressed in NEPC vs mCRPC and localized PC. Advanced disease (regardless of subtype) is associated with significantly higher expression of prothrombin, fibrinogen, and carboxypeptidase and lower expression of TF and PAR-2. It is possible that there may be PC-specific differences with aggressive disease associated with the thrombin axis vs the more common TF/PAR2 axis commonly seen in other advanced solid tumors. Further research is required to understand these differences in biology and resulting thrombotic and hemostatic outcomes.

RESUMO

The first St Gallen Advanced Prostate Cancer Consensus Conference (APCCC) Expert Panel identified and reviewed the available evidence for the ten most important areas of controversy in advanced prostate cancer (APC) management. The successful registration of several drugs for castration-resistant prostate cancer and the recent studies of chemo-hormonal therapy in men with castration-naïve prostate cancer have led to considerable uncertainty as to the best treatment choices, sequence of treatment options and appropriate patient selection. Management recommendations based on expert opinion, and not based on a critical review of the available evidence, are presented. The various recommendations carried differing degrees of support, as reflected in the wording of the article text and in the detailed voting results recorded in supplementary Material, available at Annals of Oncology online. Detailed decisions on treatment as always will involve consideration of disease extent and location, prior treatments, host factors, patient preferences as well as logistical and economic constraints. Inclusion of men with APC in clinical trials should be encouraged.

BACKGROUND: Mineral trioxide aggregate (MTA) has been used in a variety of surgical and non-surgical endodontic applications. The aim of this study was to evaluate the gene expression and protein production of TNF-α, IL-1ß and IL-6, as well as the gene expression of RANKL and OPG using both commercial and experimental MTA in macrophage cell cultures. METHODS: Peritoneal macrophage cell culture was performed. Viability, gene expression of cytokines, RANKL and OPG, and protein levels in experimental- and commercial-grey MTA co-cultured with peritoneal macrophages was determined by tryptan blue, real time PCR and ELISA. RESULTS: The expression of TNF-α for both commercial and experimental MTA was higher, while the expression of IL-1ß and IL-6 was similar when compared to the negative control. At protein expression level, no differences were observed between the negative control and cements. RANKL did not show a significant improvement in gene expression when compared with the negative control, but OPG expression in cement samples was higher when compared to the negative control. CONCLUSIONS: This study suggests that commercial and experimental MTA promotes anti-inflammatory processes, as well as bone healing capacity.

RESUMO

Quercetin, rutin, naringin, hesperidin and chrysin were tested as substrates for chloroperoxidase to produce reactive quinones to graft onto chitosan. Quercetin and rutin quinones were successfully chemically attached to low molecular weight chitosan. The quercetin-modified chitosan showed an enhancement of plastic, antioxidant and antimicrobial properties as well as of thermal degradability. Finally, chitosan-quercetin films visibly decreased enzymatic oxidation when applied to Opuntia ficus indica cladodes.

Objetivo: Describir en pacientes con cáncer glótico temprano tratado con cirugía y radioterapia, la calidad y funcionalidad de la voz. Diseño: Estudio de serie de casos. Materiales y métodos: Los parámetros objetivos de la voz fueron evaluados con análisis acústico. La funcionalidad de la voz fue evaluada con el índice de incapacidad vocal (VHI). El análisis estadístico fue realizado con SPSS 11.5 y SuperSMITHWeibull. Resultados: 12 sujetos que recibieron tratamiento para cáncer glótico temprano fueron incluidos en la muestra. El seguimiento clínico fue de 53.1 meses en promedio. El control de la enfermedad se observó en 100% de los casos, sin recaídas. El análisis acústico evaluó lafrecuencia fundamental de la voz, que fue normal en 63,6% de los casos; adicionalmente las medidas del Jitter y Shimmer fueron anormales en todos los pacientes. El valor del índice de incapacidad mostró una disfuncionalidad leve en 58,4% de los casos y normal en 41,6% de los casos. Conclusionese importancia clínica: El manejo del cáncer glótico temprano con cirugía y radioterapia resultó en una alteración de los parámetros objetivos de la voz en todos los sujetos; sin embargo, en nuestra muestra los pacientes refieren poca o ninguna disfuncionalidad en la vida diaria.


Objective: To describe on an adult population with early glottic cancer, treated with surgery and radiotherapy, the voice quality and functional outcome. Design: Case series study. Materials andMethods: Voice parameters were assessed using acoustic analysis. Voice functional outcome was evaluated with the voice handicap index (VHI). Statistical analysis was performed using the statistical software SPSS 11.5 and SuperSMITHWeibull. Results: 12 subjects that received treatment for early glottic cancer were included in the sample. The subjects were followed up during an average of 53.1 months. Effective control of disease without relapse was observed in 100% of the cases. The acoustic analysis assessed the fundamental frequency which was normal in 63,63% of the cases. Moreover, the mean of the Jitter and Shimmer was abnormal in the whole sample. The voice handicap index revealed a mild impairment in 58,4% of subjects, and was normal in 41,6% of cases. Conclusion and clinical significance: The management of early glottic cancer with surgery and radiotherapy resulted in alterations of objective voice parameters in the whole sample, however the patients report mild or none voice dysfunction on their daily life.

Introducción: el MTA es un material cuya materia prima es el cemnto portland y es usado para inúmeros procedimientos endodónticos. Objetivo: el objetivo de este trabajo fue de comprobar la semejanza entre MTA-Angelus y el cemento portland Yura, S.A. Materiale y Métodos: para determinar la composición química y la estructura, se analizaron ambos cementos pormedio de difracción de rayos-X y microestructura, para el análisis de la capcidad de sellado apical de ambos cementos se empleó la técnica de microfiltración usando como colorante el azul de metilen. Para el análisis de la capacidad antimicrobiana de ambos cementos se utilizaron cepas bacterianas y levaduras orales, observándose la inhibición del crecimiento de los microorgnismos y su capacidad microbicida por la observación del agregados microbianos y por turbidez, resectivamente. Todos procedimientos fueron realizados en triplicido. El análisis estadistico empleado para evaluar la microinfiltración fue la prueba One-way ANOVA y la prueba T (p<0,05) y para evaluar la capacidad antimicriobiana se empleóla prueba de Kruskal-Wallis (p<0,05). Resultados: los resultados obtenidos mostraron que el MTA-Angeles y cemento portland Yura S.A posen los mismos elementos química a excepción el bismuto en el cemento portald, no hubo diferencia en la microfiltración entre el MTA-Angelus y cemnto portland Yura, S.A; y se observo también que ambo cementos tuvieron un gran capacidad antimicrobiana. Conclusión el Cemento portland Yura, S.A posee caraterísticas físticas, químicas y antimirobinas, semejanes al MTA-Angelus (AU)


Introduction: the MTA is material whose raw materia is portland cemnt and s used to inmunerable endodontics procedures. Aim: the aim of this study was to verify the similarity between the MTA-Angelus and portland cement Yura, S.A. Materials and Methods: to asses the chemical composition and structure, both cements were analyzed throuh X-ray diffraction and microructre; to he aanalysis of apical sealing was employed the mcorfiltration technique, using methylene blue stain. The antimicrobial ability ws analzed employig oral bacteria and oral yeasts observing the inhibition growth through the microbial aggreagates and the microbial viaility through the microbial turbidity. All expeiences were performed intriplicate. The statistical analysis employed to discrimintae differentes on microfiltration ws performed using One way- ANOVA and t-test (p<0,05)., and the kruskal-Wallis test for antimicrobial ability (p<0,05). Results: The results showed that the MTA-Angelus and portland cement Yura, S.A possess the same cheical elements with the exception of bismuth in the portland cement, there was no difference inmicroleakage between the MTA-Angelus and portland cement Yura, S.A, and it was also noted that both cements had a great antimicrobial capacity. Conclusión: The Portland Cement Yura, S. A possesses similar physics, chemical, and antimicrobial characteristisc that the MTA-Angelus (AU)

Objetivos: Analizar la relación existente entre el tipo de Atención Primaria que el paciente tiene asignada (centro de salud o centro no reconvertido) y el uso de un Servicio de Urgencias Hospitalario. Métodos: Diseño: estudio transversal. Período de estudio: 1 noviembre2000 a 31 octubre 2001. Entorno: Área de Urgencias del Hospital Médico Quirúrgico 'Ruiz de Alda', Granada. Población de estudio: Muestra aleatoria estratificada por semanas y días de todos los usuarios del servicio de urgencias, mayores de 14 años y pertenecientes a la población de referencia. Se recogió información sobre variables demográficas, hora y forma de acceso a la urgencia, tipo, características y utilización de la atención primaria asignada, modalidad de derivación a urgencias, motivo de consulta y adecuación de la urgencia. Se han calculado OR crudas, estratificadas y ajustadas para estudiar la asociación entre tipo de atención primaria: Centro de Salud o Centro No Reconvertido y la inadecuación de la consulta a urgencias. Resultados y conclusión: La atención primaria en centros no reconvertidos se asoció con una frecuencia de uso inadecuado de la urgencia1,27 veces mayor que la atención en centros de salud (IC95%=1,01-1,59). Esta asociación aumentó al estratificar por determinadas variables, siendo más elevada en los estratos ligados a una mayor utilización de los servicios de atención primaria, lo que podría suponer mejor capacidad resolutiva para los centros de salud respecto a los centros no reconvertidos (AU)


Aims: To analyze the relationships between Primary Care Pattern(reformed health centre or traditional ambulatory centre) and the utilization of the Hospital Emergency Department. Methods: Design: Cross sectional study. Study period: From November1, 2000 to October 31, 2001. Setting: Emergency Department, Granada Hospital Ruiz de Alda. Study population: Radon sample stratified by week and day of the week chosen between all patients 14 years and oldest attended at the Emergency Department. Variables: Socio-demographic characteristics, time and reasons of the attending, pattern of primary care assigned, frequency and type of primary care consultation, whether patients attended on the own initiative or they are referral by primary care physicians, and if the ED use is appropriate based on explicit criteria. We estimated the crude and stratified odds ratio between primary care pattern and inappropriate use of the ED. Results and Conclusion: Primary care in traditional ambulatory centres was associated with greater frequency of inappropriate ED use (OR1.27; IC95%=1.01-1.59). This figure increased when data were stratified by other variables. The bigger associations were for population strata with high service utilization. This could meant reformed primary care centre have better decisive ability (AU)

Objetivos: Ante la necesidad de mejorar y rentabilizar la utilización de los recursos asistenciales nos planteamos como objetivo el estudio de la población atendida en el Servicio de Urgencias Hospitalario (SUH) de un hospital terciario y las prestaciones realizadas. Métodos: Entorno: Área de Urgencias del Hospital Médico Quirúrgico "Ruiz de Alda". Diseño: estudio transversal (1- XI- 2000 al 31 - X -2001) mediante un muestreo estratificado polietápico. Todas las variables del estudio fueron recogidas de forma concurrente siguiendo el circuito asistencial. Resultados: La población de estudio quedó constituida por 2.829 asistencias repartidas por igual entre ambos sexos. El perfil del usuario responde a un sujeto de 49 años, casado, con estudios primarios, jubilado / trabajador activo / ama de casa (por este orden), proceden en su mayoría de Granada capital o área metropolitana; acudieron al SUH por propia iniciativa, en horario de pleno funcionamiento de su centro de salud. A casi la mitad de los usuarios no se les realizó prueba complementaria o tratamiento alguno, siendo algo más de un tercio procesos catalogados como banales. La mayoría de los pacientes fueron alta a su domicilio. Conclusiones: En términos generales, el patrón de uso y perfil de usuario detectados en nuestra muestra coinciden con la mayoría de estudios consultados, con algunas matizaciones propias de nuestro ámbito geográfico, social y cultural. Al menos en el 40% de las atenciones realizadas en un SUH no se realiza pruebas o terapias hospitalarias, por lo que la atención de estos pacientes deberían encauzarse hacia la atención primaria (AU)


Objetives: Before the need to improve and to profit the utilization of the welfare resources we present us as objective: study of the population attended in ED of a hospital of reference and of the benefits carried out. Methods: Environment: ED of the General and Surgical Hospital Ruiz de Alda. Design: cross study (from 1-11-2000 to 31-10-2001) by means of a sampling stratified in several phases. All the variables of the study were collected of concurrent form continuing the welfare circuit. Results: the population of the study was of 2829 patients, half men an half women. The profile of the user responds to a subjet of 49 years, married, with elementary studies, retired / industrious active / housewife (by this order). They proceed in their majority of the city of Granada or of their metropolitan area, they responded to ED by own initiative and in schedule of full operation of their family physicians. Themselves they were not used diagnostic test neither treatment procedures in almost the half of the users, and something but a third they were catalogued as banal. To the majority of the patients was sent them home after to be attended in ED. Conclusions: in general terms, the boss of use and the profile of the user detected in our sample they coincide with the majority of the studies consulted, with some own particularities of our cultural, social and geographical environment. In but 40 percent of the ED attended they are not carried out test diagnostic neither treatment procedures, for wich these patients should be channeled toward de primary attention (AU)

Temporal fluctuations of the light radiated by a photoemissive source are studied through the cross correlation of output fields. Whereas microscopic reversibility guarantees time-symmetric fluctuations in thermal equilibrium--where detailed balance holds--away from equilibrium time asymmetry is permitted. Examples of time asymmetry in cavity QED are reported.

RESUMO

We report measurements in cavity QED of a wave-particle correlation function which records the conditional time evolution of the field of a fraction of a photon. Detection of a photon prepares a state of well-defined phase that evolves back to equilibrium via a damped vacuum Rabi oscillation. We record the regression of the field amplitude. The recorded correlation function is nonclassical and provides an efficiency independent path to the spectrum of squeezing. Nonclassicality is observed even when the intensity fluctuations are classical.

Se revisan las historias clínicas de 88 pacientes con el diagnóstico de oclusión intestinal por bridas, tratados en el hospital docente clinicoquirúrgico "Joaquín M. Albarrán", entre 1975 y 1980, ambos años inclusive, con el fin de conocer la incidencia de esta afección, la eficacia del tratamiento y la evolución de la enfermedad en estos pacientes. Se informa una elevada mortalidad similar a otros resultados en el tratamiento de la oclusión intestinal(AU)
