RESUMO

We report a case of traumatic pseudoaneurysm of the superficial temporal artery (STA) in an 87-year-old man. Surgical treatment was undertaken after failure of ultrasound-guided compression. In this report, we review briefly the pathogenic mechanism, presentation, diagnosis and treatment of this type of aneurysm. Traumatic pseudoaneurysms of the STA are rare injuries, generally occurring after a recent (weeks to months) episode of blunt head injury, and primarily in a young to middle-aged (60 %) male (80 %) population. Clinical diagnosis is straightforward when the patient presents with a pulsating painful mass in the STA area and a history of trauma. Doppler ultrasound, CT scan and MRI are useful to confirm diagnosis and exclude the other possible diagnoses (i.e. hematoma, abscess, lipoma, cyst, meningocele, etc.). Surgery remains the treatment of choice for these lesions, however, other treatments have been proposed: endovascular coil embolization, percutaneous thrombin embolization or ultrasound-guided compression.

RESUMO

Labiaplasty is the surgical term for labia minora reduction. This procedure has become popular with plastic surgery patients because physical discomfort and cosmetic concerns are virtually always combined. Demand for this procedure is becoming common amongst very young patients who appear to have nurtured a complex since their early adolescent years, disturbing their love life. There are several possible corrective surgical techniques, the postoperative courses of which vary significantly. The traditional method (longitudinal resection) remains the most widely used, but requires resection of the larger part of the labia minora, which is why in our opinion the V plasty technique seems more appropriate. The benefits, technical details, complications and postoperative course of the V plasty technique are detailed by the author in this article.

RESUMO

Cases of Mycoplasma hominis infections after allograft are rare. We report a case of M. hominis wound infection after a vascular allograft. The allograft was positive before having any contact with the recipient, and our investigation suggests that M. hominis may have been transmitted from the donor to the recipient. It is not clear, however, whether specific diagnosis of M. hominis should be performed on tissue before grafting in order to prevent such donor-to-host transmission.

OBJECTIVE: To measure cosmetic surgery patients' state of psychological vulnerability. METHOD: A multicentre prospective study was carried out in three hospital departments. One hundred and three patients scheduled for cosmetic surgery were examined using a structured interview and using three assessment scales: the MADRS (Montgomery and Asberg Depression Rating Scale), the SISST (Social Interaction Self Statement Test) and the EQ-5D (EuroQol) which measures quality of life. RESULTS: The MADRS index was higher than that of the control group (p<0.01) with 20% depressive patients. SISST: the social anxiety was greater than that of the control group (p<0.001). The EQ-5D visual analogue scale average was 77.39% indicating that there was no significant difference when compared with the control group, but the descriptive EQ-5D revealed an overrepresentation of the anxiety/depression category (p<0.01); 50% had already taken psychotropic treatment of which 27% were an antidepressant. CONCLUSION: The cosmetic surgery population presents a significant state of psychological vulnerability.

The authors present from a series of 949 implants their method to calculate the life span of a mammary implant in the framework of breast reconstruction after cancer. In this statistical study, they have calculated the median life span of breast implants (loss of half of staff) by distinguishing it according to each type of implant (content, brand, indication...). The global median life span of a breast implant is 127 months. The median life span of a silicone gel-filled implant is superior than a saline implant because the frequency of deflation of saline implants is more important than the first one, despite that the real rupture percentage of silicone gel-filled implants is under-evaluated by the number of asymptomatic rupture. For saline implants, the median life span is clearly decreased by the initial under-inflation (108 months against 127 months) doubling the secondary deflation risk. In this series, the authors have been able to compare the evolution of implants according to their initial indication (reconstruction or aesthetic) and sometimes at a same patient, they have not observed significant difference of the median life span over a period of five years for an implant used in breast reconstruction after cancer or for an implant used in symmetricalization. In this series, the life span of saline implants is significant different in function of the brand of the implant demonstrating the comparative study usefulness by brands.
