RESUMO

The content of hormones that regulate carbohydrate metabolism was studied during the early neonatal period in 80 full-term neonates with intrauterine hypotrophy. Early application to the breast (2 to 6 hours after the birth) was shown to promote the normalization of the hormonal content. The levels of blood serum C-peptide in the newborn depend on the degree of the rise of the mother's body weight during pregnancy and the presence of toxicosis. The levels of cortisol, somatotropic hormone, immunoreactive insulin and C-peptide were determined by the degree of morphological immaturity of the tissues whereas the content of STH and cortisol by the intensity of hypotrophy as well. The moment of the birth and the early neonatal period of children with intrauterine hypotrophy is characterized by a decrease of the activity of lactate dehydrogenase, alpha-hydroxybutyrate dehydrogenase, creatine kinase and aspartate aminotransferase.

RESUMO

Investigation of the insulin-binding activity of insulin receptors (IR) in insulin treated pregnant women with insulin dependent diabetes mellitus with different requirement in insulin has shown that insulin-dependent diabetes in pregnant women in characterized by heterogeneity. Insulin therapy gets to normal the activity of insulin receptors in patients with diabetes mellitus with an increased affinity to IR, and causes no increase in the level of IR in patients in whom it was not raised. Insulin-binding activity in the latter may be associated with metabolic derangement caused by insulin insufficiency.

RESUMO

Overall 22 neonates born due to plural pregnancy (twins, gestation 38-40 weeks) were examined for the clinical characteristics of the early neonatal period. Blood samples were analyzed, measurements were made of blood serum cortisol, immunoreactive insulin (IRI) and somatotropic hormone (STH). The findings were correlated to those obtained in controls, namely in 57 healthy neonates born due to single pregnancy. Alterations in the IRI content in twins turned out similar to those obtained by the authors in neonates with intrauterine hypotrophy. Cortisol was discovered to fall within the first day of life. The mean STH in twin children was much lower. The data obtained enable the neonate twins to be classified with the group at risk for development of hormonal abnormalities during the neonatal period.

Using a radioimmunoassay a low C-peptide fasting level was revealed in children, pregnant and lying-in women as well as in patients with insulin-dependent diabetes mellitus. After breakfast and insulin administration with curative purposes the IRI concentration in children increased whereas the C-peptide level changed insignificantly. Changes of the insulin secretion were more noticeable in severe diabetes mellitus with vascular complications and in disease decompensation. The atherogenic nature of the lipid metabolism (an increase in the cholesterol, triglyceride and beta-lipoprotein levels), changes in the liver and a tendency to vascular involvement are the results of insulin effect inadequacy. Such metabolic derangements in pregnant women create unfavorable conditions for the development of fetus and may lead to early atherogenic processes.

A combined study of the blood content of catecholamines (C) and of their urinary excretion in 42 patients with primary forms of myodystrophies and denervated amyotrophies allowed adequate identification of the features of the sympathoadrenal system functioning under conditions of the myodystrophic process. The study revealed changes in C levels. The primary forms of progressive muscular dystrophies (PMD) were associated with increased blood levels of adrenalin (A) while denervated amyotrophies were related to elevated concentrations of noradrenalin (NA). A statistically significant increase in A excretion with urine was found in Duchenne's PMD and in Kugelberg-Welander's spinal amyotrophy, i. e. the disorders with the most malignant course among all the groups studied. NA concentrations were lowered in all forms of PMD with the exception of Charcot-Marie's neural amyotrophy.
