RESUMO

Numerous available results of investigations confirmed the important role of prophylactic bilateral laparoscopic salpingo-oophorectomy and contralateral prophylactic subcutaneous mastectomy in decrease of risks of malignant tumors of the reproductive system when mutation carriers of BRCA1 and BRCA2 presented. Recommendations to surgical treatment of hereditary breast cancer are considered as a very difficult task which should be based on risks and advantages of prophylactic surgical interventions. Presented surgical operations could be applied in mutation carriers of reparation DNA genes and patients with hereditary breast cancer aimed to improve the indices of general survival rate and quality of life.

RESUMO

An absence of breast is the irreversible cosmetic and physical defect, which reduces the self-assessment and psyco-emotional status of women. Nowadays the necessity of reconstruction after mastectomy will gain more popularity and plays a fundamental role in psychological and social rehabilitation of breast cancer patients. The article analyzes the experience of reconstructive plasty of 26 patients with I-III stages of breast cancer.

RESUMO

The oncological breast surgeries with single-stage replacement of the lost volume by means of autogeneous tissue using distant flaps grafting (particularly by skin--muscular flaps of the latissimus dorsi) allow obtaining satisfactory and stable aesthetic results without the decrease of indices of relapse-free and general survival. The early results of the application of the skin-muscular flaps of the latissimus dorsi for single-stage breast reconstruction after radical breast resection are presented in this work in 3 patients with breast cancer. The technique allows performance of the extensive radical resection with single-stage grafting in patients with breast cancer and obtaining an objective clinical response against the background of neoadjuvant therapy with subsequent applying radiation therapy and systemic treatment.

Hereditary breast-ovarian cancer syndrome contributes to as much as 5-7% of breast cancer (BC) and 10-15% of ovarian cancer (OC) incidence. Mutations in the "canonical" genesBRCA1andBRCA2occur in 20-30% of affected pedigrees. In addition toBRCA1andBRCA2 mutations, germ-line lesions in theCHEK2,NBS1, andPALB2genes also contribute to familial BC clustering. The epidemiology of hereditary breast-ovarian cancer in Russia has some specific features. The impact of the "founder" effect is surprisingly remarkable: a single mutation,BRCA15382insC, accounts for the vast majority ofBRCA1defects across the country. In addition, there are two other recurrentBRCA1alleles:BRCA14153delA andBRCA1185delAG. BesidesBRCA1, in Russia breast cancer is often caused by germ-line alterations in theCHEK2andNBS1genes. In contrast toBRCA1andBRCA2, theCHEK2andNBS1heterozygosity does not significantly increase the OC risk. Several Russian breast cancer clinics recently started to investigate the efficacy of cisplatin in the therapy ofBRCA1-related cancers; initial results show a unique sensitivity ofBRCA1-associated tumours to this compound.

RESUMO

The study was concerned with biological features of inoperable breast cancer and their prognostic significance for patients under 35. That age was identified as a prognosticator for breast cancer: high-grade (G3) malignancies, vascular invasion and EIC+component were relatively more frequent in such patients. Also, those with HER2/neu hyperexpression had worst prognosis.
