RESUMO

This paper shows the ongoing development of two key trends in Russia's housing construction sector: the strengthening role of the state and the formation of market leaders that control its privileged part. The latter are separated from the rest of the market by ever-deeper institutional barriers. These barriers arise due to "special" relations of such companies with the largest banks, their advantages in obtaining financing in the securities markets, better terms of interaction with development institutions, primarily JSC DOM.RF, PPK Fund for the Protection of Rights of Citizens Participating in Shared Construction, the Housing and Utilities Fund, with adequate access to the instruments of state guarantees, and other conditions provided by these institutions. It is shown that such an institutional mechanism provides the best conditions for achieving national goals in the housing sector. Conclusions are made regarding the risks and limitations in terms of achieving the goals of the National Project, Housing and Urban Environment, as well as the advisability of developing a strategy for transforming temporary anticrisis measures of state support into a long-term policy of public-private partnership and maintaining the multistructural character of the industry and market economy.

RESUMO

A combination of advanced light engineering concepts enables a substantial improvement in photon extraction efficiency of micro-cavity-based single-photon sources in the telecom O-band at ∼1.3 µm. We employ a broadband bottom distributed Bragg reflector (DBR) and a top DBR formed in a dielectric micropillar with an additional circular Bragg grating in the lateral plane. This device design includes a doped layer in pin-configuration to allow for electric carrier injection. It provides broadband (∼8-10 nm) emission enhancement with an overall photon-extraction efficiency of ∼83% into the upper hemisphere and photon-extraction efficiency of ∼79% within numerical aperture NA=0.7. The efficiency of photon coupling to a single-mode fiber reaches 11% for SMF28 fiber (with NA=0.12), exceeds 22% for 980HP fiber (with NA=0.2) and reaches ∼40% for HNA fiber (with NA=0.42) as demonstrated by 3D finite-difference time-domain modeling.

This work is based on the analysis of data doing 409 patients with polyps in the stomach. Among them 200 men (48.9%) and 209 women (51.1%) aged 18 to 91 years. The average age of men was 67.4 years, and women 66.8 years. All these patients were find 1034 neoplasms of various sizes. At all of this patients polyps were identified on a background of chronic gastritis, as a rule, has the character of atrophic process. Histological research was conducted 775 (74.6%) polyps, of which focal hyperplasia and hyperplastic polyps detected in 49% of cases, and adenomas in 51% (including adenomas with varying degrees of dyspiasia, and prostatic adenocarcinoma cells). Endoscopic polypectomy performed 367 patients, and the remaining 42 was merely the dynamic monitoring of the identified entities. In 79 (28.8%) patients after polypectomy noted relapse formations and in 10 cases (3.65%) diagnosed with cancer in the intervention area. 4 (0.98%) patients from 409 cancer in polyps detected during the initial survey and in 4 patients the cancer originated in the not associated with polyps of stomach. As a result of the research of the dependence of the size of neoplasms on the nature of destruction and histological structure. For most existing polyps characteristic of the stability of their morphological structure and the absence of its transformation.

It has been suggested that exertional rhabdomyolysis (ER) and malignant hyperthermia (MH) are related syndromes. We hypothesize that patients with unexplained ER harbor mutations in the ryanodine receptor gene type 1 (RYR1), a primary gene implicated in MH, and therefore ER patients are at increased risk for MH. Although there are reported cases of MH in individuals of African descent, there are no data available on molecular characterization of these patients. We analyzed RYR1 in six, unrelated African American men with unexplained ER, who were subsequently diagnosed as MH susceptible (MHS) by the Caffeine Halothane Contracture Test. Three novel and two variants, previously reported in Caucasian MHS subjects, were found in five studied patients. The novel variants were highly conserved amino acids and were absent among 230 control subjects of various ethnic backgrounds. These results emphasize the importance of performing muscle contracture testing and RYR1 mutation screening in patients with unexplained ER. The MHS-associated variant Ala1352Gly was identified as a polymorphism predominant in individuals of African descent. Our data underscore the need for investigating RYR1 across different ethnic groups and will contribute to interpretation of genetic screening results of individuals at risk for MH.

The crystal structure of [C(n)mim]NTf(2) (n = 2, 4, 6) was studied for the first time simultaneously by X-ray diffraction method and IR spectroscopy. The temperature-dependent IR spectrum for crystalline [C(4)mim]NTf(2) was demonstrated to correlate with both the X-ray data and the calorimetric results obtained earlier. Therefore, it was found that IR spectroscopy is able to establish the correspondence between the X-ray and the calorimetric data in this case. The joint use of X-ray diffraction, IR spectroscopy, and quantum-chemical calculations allowed us to determine the structure of all [C(2)mim]NTf(2) crystalline modifications obtained earlier by adiabatic calorimetry measurements. Thus, a new approach for the future identification of ionic liquid crystal structure by use of temperature-dependent infrared spectroscopy is suggested and justified.

The medical-and-economic efficiency, involving the traditional, newly designed and tested methodic approaches, was evaluated in respect to 19 inhospital department of a multidiscipline hospital in the city of Yekaterinburg. A necessity was proven to use the integral indices in assessing the medical efficiency. The economic and medical efficiencies were found to coincide in 62.5% of cases, which is indicative of that it is not always the case, when the amount of the spent means directly influence the activity output. The study results showed that the technology of integral assessments of the medical-and-economic activity efficiency enables the hospital's manager to define the best and worst functioning units and to make subsequently the appropriate correcting managerial decisions.

RESUMO

A comprehensive study of the efficiency of activities of 5 principle therapeutic-and-preventive institutions (TPI) was conducted in a typical Ural industrial region by using the traditional and newly developed approaches. Both the medical and economic efficiency of their activities was found to be not high in a majority of them. The analysis results were used to work out a scientifically substantiated program for reforming the health care system of the region.

RESUMO

The article contains the technologies of working out a program of analysis to assess the health of the children population in the Sverdlovsk region as well as to assess the related basic factors. The program is addressed to the managers of the regional hospital for children of the ministry for health of the Sverdlovsk region, and its purpose is to effectively manage the children's health condition and to regulate the medical-aid system. It is noteworthy, that the program can be adapted to the conditions of other subjects of the Russian Federation. The program makes it possible to accumulate the limited health care resources and to communicate, on the subject-oriented basis, with the municipal health-care authorities.

RESUMO

We present a new thermodynamic model for the mechanism of activation and regulation of cell proliferation in the G1 stage. In this model, the interactions between growth factors and transmembrane proteins play a crucial role in cell growth control for a normal tissue-culture system. We calculate a phase diagram of normal and malignant transformed states of a cell signal transduction system. We propose thermodynamic reasons why cancer cells can continually grow without activation by the growth factors.

RESUMO

Reformation of therapeutic and prophylactic institutions attached to various institutions and ministries is and important problem of public health at the modern stage of its development. A model developed and tried in Magnitogorsk can serve as a perspective trend of such reforms. A medical institution with mixed form of property has been created. The institution was set up by administration of the territory and a plant (Magnitogorsk metallurgical plant). Creation of a new health center as a non-commercial institution promoted its integration in the municipal public health system; the institution possesses all the potentialities of a budget organization and retains close contact with the plant. Such a solution of the problem improved the financial status of the health center and promoted its adaptation to marketing conditions. Attraction of additional finances from industry to municipal public health allowed the administration of the health center start and carry out internal restructuring aimed at priority development of outpatient care, restructuring of the bed fund, technological updating, and, in general, more rational utilization of the available resources.

The bis(2,6-dioxopiperazine)s are a structurally and mechanistically unique class of topoisomerase II inhibitors that do not bind DNA and that do not stabilize topoisomerase II-DNA strand passing intermediates ("cleavable complexes"). The most effective topoisomerase II inhibitor in the bis(2,6-dioxopiperazine) series is ICRF-193 (meso or S*, R* isomer), with a meso 2,3-butanediyl linker connecting the dioxopiperazine rings. The two enantiomeric diastereomers, (R,R) and (S,S), of ICRF-193 possessing the two optically active 2,3-butanediyl linkers have been prepared from their respective optically pure 2,4-diaminobutanes via 2,3-diaminobutane-N,N,N',N'-tetraacetic acid, esterification, and imide formation. Both in vivo and in vitro assays for catalytic inhibition of topoisomerase II were employed to show that the (S,S)- and (R,R)-isomers are almost inactive as topoisomerase II inhibitors. The data indicate that the meso stereochemistry of the alkanediyl linker is crucial for activity and provides additional evidence that the cytotoxicity of the bis(2,6-dioxopiperazine)s is due to their ability to inhibit topoisomerase II.

The activities of a large bureau of pathology using local computer network are analyzed. Every pathologist has at his working place a personal computer switched in the local computer network. The latter and relevant program is employed for all cases of surgical and biopsy material. The advantages of such work are described.

Curacin A, the major lipid constituent of a strain of the marine cyanobacterium Lyngbya majuscula obtained off the coast of Curaçao, is a potent antimitotic agent that we have previously shown to inhibit microtubule assembly and colchicine binding to tubulin. In the present study, we report that curacin A probably binds in the colchicine site because it competitively inhibits the binding of [3H]colchicine to tubulin with an apparent Ki value of 0.6 microM and stimulates tubulin-dependent GTP hydrolysis, as do most other colchicine-site agents. The binding of curacin A to tubulin resembled the binding reactions of combretastatin A-4 and podophyllotoxin in contrast to that of colchicine in that it occurred as extensively on ice as at higher temperatures. However, once bound, the dissociation rate of curacin A from tubulin is very slow, more closely resembling that observed with colchicinoids (thiocolchicine was the drug examined) than the faster dissociation that occurs with combretastatin A-4 and podophyllotoxin. Because the molecular structure of curacin A is so different from that of previously described colchicine-site drugs (e.g., there is no aromatic moiety, and there are only two conjugated double bonds in its linear hydrocarbon chain), we have been examining the activities of natural isomers and synthetic derivatives. So far, only modest enhancement or reduction of activity has been observed with a variety of structural changes.

The hypothesis was tested that constant, long-term inhibition of cell proliferation is the primary cause of cellular senescence. Mouse L-929 fibroblasts were maintained in confluent cultures for periods of 6 and 12 months with cell viability maintained by regular replacement of medium and serum. The mitotic activity of the cell population under these conditions was one mitosis per 10(4) cells. No changes in the duration of the cell cycle were observed when, following long-term quiescence, the cell were replated and grown at low cell density. The results do not support the hypothesis that prolonged suppression of cell proliferation induces cell senescence.

Clinico-instrumental examination of 226 patients showed that there were no fatal outcomes directly after endoscopic papillosphincterotomy, the number of complications was approximately the same as after transduodenal operations on the major duodenal papilla but they took a much easier course. The high effectiveness of an endoscopic operation was confirmed in the immediate postoperative period (absence of biliary hypertension, jaundice, pancreatitis, cholangitis). In the late-term periods after EPST restenosis was rarely encountered (3.9%); duodeno-biliary reflux caused no serious disorders, while the functional activity of the sphincter Oddi which had been operated on was partly maintained in 84.2% of patients.

A consultative centre for intensive pediatrics (cip) is a center for rendering aid to children in urgent conditions who are hundreds kilometers from specialized help. Such aid is typically rendered by: 1)moving the patient to the another hospital, 2) the specialized team going out by plane, or 3) consulting the local physician by phone. The complexities of the cip dispatcher-consultation of the local physician entail a real-time system that provides a consultant with simultaneous operation with a computer and a local doctor over a phone. In 1987, a computer program, DINAR-1, was used. The core of this program was the scale of a patient's state severity "FHD-5.4" and essential elements of informative services: both a reference subsystem and one accumulating and analyzing data about cip activities, treated patients, and faults in the system of medical care. Two years of DINAR-usage showed the necessity of strengthening the decision support system for cip consultant. In 1989, a new program variant, DINAR-2, was completed. During the new variant's creation, we developed a model of decision making under conditions of information distortion (e.G., as a result of the subconscious influence of diagnostical hypotheses). This model was necessary for DINAR-2, because the most part of all information for decision making had subjective issue. Distinctions between levels of medical aid at the spot (in local hospitals) demanded the special assessment of a patient's state severity in order to adequately choose between tactical activities of the CIP. The severity of a patient's state original index depended on necessary therapy type; volume was developed. Special methodological interaction expedients between the user and the decision making system were verified in DINAR-2. The following improvements were also developed: subsystem of tuning on a definite area (1990); block of analytical final instructions on a patient's treatment (1991); visualization of introduced information (1991); determination of factors causing aggravation of a patient state (1992); and specialized subsystem of consultation of neonatals (1993). During the work, CIP got a new important function. CIP became a methodical center of the medical children aid. In keeping with this, a collection and analysis of information for regional health ministry became important part of DINAR-2. It is especially connected with touched discovery and analysis of medical defects. A system of child's death-rate analysis was especially created to make impartial decisions (1990). Expert subsystems had also an original continuation. This subsystem became a basis of the new system DINAR-created for the common pediatric physician who is attending a severely ill child. A principle addition to this system is a more complicated method of tactic solution choice. The technology of CIP supported by DINAR-systems had been bought by 35 different regions of the former USSR by 1994 (1989: one region; 1990: 2; 1991: 5; 1992: 10; 1993: 17 regions). The total population of all these regions is more than 59,000,000 people, and the total area is more than 11 million square kilometers. The main direction of DINAR's development is the creation of a regional computer network, allowing DINAR-to communicate with DINAR-in local hospitals. The first place for the usage of such a network is going to be the Big Urals Region. Big Ural consists of eight states. The total population of this territory is 28 million people and total area is 3.1 million square km. Computer networks will link eight regional CIPs to one another and the medical analytical center. The CIPs will have to be connected with 259 local hospitals. At present, the DINAR-2-CIP is installed in six states of the Big Ural, and DINAR-H is installed in four hospitals of the Sverdlovsk state.

A hypothesis was tested about a long-term limitation of cell proliferation as the primary cause of their senescence and, consequently, the senescence of the whole organism. Both half-yearly and yearly maintenance of mouse fibroblast-like cells L-929 in a high density permanent culture with low level of mitotic activity (less than 1 mitosis per 10,000 cells) has not revealed any irreversible changes in their proliferative features and has not confirmed the hypothesis under examination.

Tuberous sclerosis is a rare congenital disease characterized by developmental defects of the central nervous system, skin and inner organs with the development of tumours. One case of tuberous sclerosis is described in a 45-year-old female with the involvement of the brain, eyes, skin, kidneys and lungs. The cause of death was uremia as a consequence of large kidney angiomyolipomas.
