RESUMO

IgG4-related disease is a fibroinflammatory condition that affects many different organs. The common pathological findings of this disease include storiform fibrosis and dense infiltration of lymphocytes rich in IgG4-positive plasma cells. We herein presents the diagnosis and treatment of a case with a newly recognized condition - IgG4-related pachymeningitis.

A 69-year-old man presented with multiple spontaneous bruises in the past 2 weeks. Several large-sized hematomas were found on examination. The initial investigation revealed a prolonged activated partial thromboplastin time (aPTT) with normal platelet count and international normalized ratio. Further investigation revealed a low factor VIII activity secondary to presence of factor VIII inhibitor, making the diagnosis of acquired hemophilia A. Further work-up revealed that pernicious anemia was present and acted as an associated disease. After steroids therapy, his aPTT was normalized and the factor VIII inhibitor titer became undetectable. 2 months later, a relapse occurred and new hematomas appeared at his retropharyngeal space and left arm. His bleeding was controlled by administration of recombinant factor VIIa, and a combined therapy of intravenous steroids and rituximab was given to eradicate the inhibitor. The approach to workup of bleeding disorders as well as treatment of acquired hemophilia A are herein discussed.

OBJECTIVE: Using a novel monoclonal antibody (mAb Das-1) that specifically reacts with colon epithelium, we examined if there is a phenotypic change of small intestinal enterocytes toward colonocytes in small intestinal neoplastic tissue. METHODS: Tissue sections of the small intestine consisting of adenomas (n = 20, five with histories of familial polyposis), adenocarcinomas (eight primary and one metastatic from colon). carcinoids (n = 2), and hyperplastic polyps (n = 3) were examined by a sensitive immunoperoxidase assay using mAb Das-1 (IgM isotype). Normal jejunal (n = 10) and colonic (n = 10) biopsy specimens were also included as additional controls. RESULTS: mAb Das-1 reacted with normal colonic epithelium but not with jejunal mucosa. However, mAb Das-1 reacted strongly with each of the five adenomas (100%) from patients with histories of familial polyposis, but only five of 15 (33%) of the adenomas from nonfamilial polyposis patients, and each of the eight (100%) adenocarcinomas of the small intestine (p < 0.001). The reactivity with the adenomas from nonfamilial polyposis patients was very focal, whereas in the adenomas with familial polyposis the reactivity was more extensive. Each of the eight carcinomas reacted strongly with mAb Das-1. Adjacent normal small intestinal mucosa did not react. Hyperplastic polyps and the carcinoids did not react with mAb Das-1. CONCLUSION: These data demonstrate a phenotypic change in small intestinal epithelium toward the colonic phenotype, particularly in familial polyposis and in adenocarcinomas. mAb Das-1 may be clinically useful in identifying small intestinal adenomas with "high risk" for malignancy, such as in familial polyposis.

Although consumers say they are concerned about nutrition and are aware that eating a healthful diet is important for good health, this knowledge does not always translate into healthful diet behaviors or motivate behavior change. In an effort to better understand consumer attitudes about nutrition and to explore alternatives for communicating dietary advice in language that is meaningful and motivates behavior change, the International Food Information Council (IFIC) conducted qualitative research with consumers (using focus groups) and registered dietitians (using telephone interviews) in 1998 and 1999. Results of the research are presented using dietary fat as a case study. Findings from the IFIC research were reported to the Dietary Guidelines Advisory Committee to assist the Committee in developing meaningful and action-oriented dietary advice related to dietary fat for inclusion in the 2000 Dietary Guidelines for Americans that would be motivating and easy for consumers to implement. The recommendation to moderate fat intake in the new dietary guideline, "Choose a diet that is low in saturated fat and cholesterol and moderate in total fat" is consistent with communication recommendations in the IFIC research. Further, the moderate fat message is empowering because it suggests an achievable dietary regimen and reduces guilt and worry about foods. It allows flexibility to enjoy desired foods and promotes using common sense when it comes to diet. Several issues emerged from the IFIC research that apply to general nutrition communications with consumers, whether it be through national nutrition recommendations or in one-on-one counseling situations: to be effective, messages to consumers about nutrition, and specifically dietary fat, must address sources of discomfort about dietary choices; they must engender a sense of empowerment; and they should motivate both by providing clear information that propels toward taking action and appeals to the need to make personal choices.

The case of a 74-year-old woman with past history of hypertension and cerebrovascular accident admitted with pneumonia, dehydration, hypernatremia and severe hypokalemic alkalosis is presented. After correction of the hypertonic dehydration, the hypokalemia and alkalosis persisted in spite of aggressive potassium supplementation and the patient became hypertensive. Mineralocorticoid excess was suspected and excluded after extensive endocrinological testing. The use of aldactone failed to revert the abnormalities. Triamterene administration corrected the electrolytes and acid base aberrations, and dramatically improved the blood pressure control. This clinical picture is compatible with the diagnosis of Liddle's syndrome. Our patient exemplifies the unique occurrence of hypokalemic metabolic alkalosis in association with volume contraction at the start of the hospitalization and volume expansion later on her course.

The media is consistently ranked by the public as their primary source of nutrition and food information. To address the question of what the public is hearing about food, nutrition, and health through the news media, the International Food Information Council Foundation commissioned the Center for Media and Public Affairs to conduct a content analysis of nutrition and food safety stories. The study examined media coverage from 53 print and electronic news outlets during a 3-month period from May through July 1995. Specific criteria were applied to select the sample of food and nutrition stories (n = 979) by length of coverage during the reporting period. Coverage related to food policy or regulation, food programs, individual food companies or products, advertising or marketing, and economic adulteration or tampering were excluded from the sample. The analysis of media coverage of food and nutrition topics revealed a twofold emphasis on dietary fat over other topics. Consumption of dietary fat was mentioned in almost half of all reports, apart from discussions of body fat, weight, or obesity as a health issue. The content analysis also showed an inverse relationship between media stories of foods by food group compared with the recommended number of servings in the Food Guide Pyramid. During an era in which Americans receive their health, nutrition, and food messages from the media rather than from health professionals such as physicians and dietitians, there are multiple challenges for communicating with the public. Our media research and its findings offer approaches for dietetics practitioners to use when providing nutrition and food recommendations and education for clients and the public. These include communicating science, providing context, reinforcing basic messages and guidelines, and promoting action and behavior change to clients and consumers.

Nutrition communications in recent years have placed a priority on reducing dietary fat. Survey findings suggest this priority has fostered consumer obsession with and confusion about dietary fat and contributed to misperceptions about healthful eating. The obsession, confusion, and misperceptions about dietary fat and healthful eating, in turn, have created obstacles to achieving dietary goals. This overview of consumer knowledge, attitudes, and behaviors regarding dietary fat may allow dietetics and other professionals to tailor nutrition communication efforts toward clearing the confusion and better fostering success in reaching dietary goals. Nutrition communicators are encouraged to work together to restore reason to nutrition messages and recommendations and joy to food and eating in an effort to help consumers truly achieve nutrition and health goals.

RESUMO

We describe a type III carpotarsal osteolysis patient with nephropathy who has been maintained on chronic hemodialysis for 12 years. The diagnosis of osteolysis was based on x-ray findings and bone biopsy. At the age of 12 years, she developed renal disease that progressed; she was placed on chronic dialysis at the age of 18 years. Blood pressure was controlled by a medical regimen, and her anemia has benefitted from erythropoietin up to a level of 28% to 30%. Her osteolysis has stabilized since the age of 20 years; she is presently 30 years old. The major problem affecting the patient's clinical outcome is hyperphosphatemia.

The incidence, clinical manifestations, morbidity, and mortality of all adult hypernatremic patients hospitalized during a 6-month period were studied. The impact of age on this parameter was evaluated, and the velocity of correction that produced best clinical results was established. Records of all patients who were admitted or developed hypernatremia (Na+ > 150 mEq/L) were reviewed. Demographic characteristics of age, gender, associated diagnosis, length of stay, source of admission, treatment, and outcome were recorded. Of 3209 hospitalizations, 111 patients were hypernatremic (3.46%). Sixty-five were admitted with hypernatremia, and 45 developed hypernatremia while hospitalized. Fifteen had recurrent episodes of hypernatremia. Forty-nine had associated hypokalemia and six had hypercalcemia. The etiology was multifactorial and varied with age. Correction of the hypernatremia within 4 days produced significant improvement in mental status. The overall mortality was 48.6%, and age did not favorably influence mortality. Hypernatremia is a common disorder of elderly hospitalized patients, associated with high mortality and morbidity. Other electrolyte disorders that impair the kidney concentrating ability frequently are observed. The etiology is multiple, and febrile illness due to bacterial infections is the most common cause among the elderly. Prompt treatment of infections and increased water intake in this group of patients could prevent its development. Correction over a 72-hour period significantly improved recovery of mental functions.

Although the prevalence of overweight and obesity among US children is a concern for many health care professionals, incidence rates over time seem to be variable, depending on the assessment measurements used. It is difficult to determine the associated health implications of pediatric obesity or overweight, especially the type that might result in adult obesity or overweight. This review examines the various factors that contribute to the weight and fitness status of children, including anthropometric factors, nutrient intake, the level of physical activity, and nutrition knowledge. Nutrient intake data of the past decade show that the energy and fat intakes of children in the United States have been fairly constant. However, data also indicate that their physical activity has declined. The data strongly suggest that the apparent prevalence of pediatric overweight may not be so much a function of nutrient intake as of a decrease in physical activity leading to an imbalance of energy input and output.

This study was undertaken to evaluate the relationship between prevalence of hypokalemia and the age and sex of patients. Serum potassium concentrations of 872 patients were measured, and the prevalence of hypokalemia was compared between age and sex groups. Hypokalemia was more common in patients 65 years or older than in younger groups (P < .001), with a significantly higher frequency in female patients (P < .003). The finding was more common in elderly female patients than in elderly males (P < .002). No statistically significant difference was noted in the frequency of hypokalemia in elderly male patients compared with younger patients of either sex. The predominance of hypokalemia in elderly groups was chiefly attributable to a marked preponderance in elderly female patients over all other groups. Neither younger females nor elderly males were at increased risk. The preponderance in elderly females was not dependent on diuretic usage. It may have been due to age- and sex-associated differences in body mass composition, which result in a physiologically low total exchangeable body potassium in elderly females, placing this group at greater risk for developing hypokalemia.

Peritonitis in continuous ambulatory peritoneal dialysis (CAPD) patients is rarely caused by group B beta-hemolytic streptococcus species. We describe a 52-year-old man with chronic glomerulonephritis who developed a fatal peritonitis due to streptococcus group B in the absence of predisposing factors such as diabetes mellitus, malignancy, human immunodeficiency virus (HIV) infection, or liver disease. This report suggests that although beta-hemolytic streptococcus is a rare cause of peritonitis, the severity of the infection may be overwhelming and may rapidly lead to serious consequences and death.

The clinical characteristics and outcome of patients with peptic ulcer disease (PUD) in association with chronic renal insufficiency were compared with those of patients who had PUD without renal impairment. The number of patients hospitalized with PUD was 150, which represented 1.3% of all hospital admissions to Kingsbrook Jewish Medical Center over the two-year period August 1986 to August 1988. The incidence of PUD among the patients with end-stage renal disease was 2.6%. Mortality was significantly higher in patients with serum creatinine greater than 264 mumol/L (Group A) than in patients with serum creatinine less than 264 mumol/L (Group B) (p = 0.02). A striking finding was the presence of giant (greater than 2 cm in width) gastric and duodenal ulcers and ulcers of the second portion of the duodenum in both groups. The difference in number of giant ulcers in Group A and Group B was statistically significant (p less than 0.001). Moreover, the patients in Group A had a higher incidence of upper gastrointestinal bleeding and mortality than those in Group B. Group A patients also had a greater need for surgery than Group B patients. Both groups were predominantly elderly with a mean age greater than 65 years, which may have contributed to the increased incidence of complications and mortality.
