RESUMO

Different variants of omentoduodeno- and omentogastroplasty were applied in 69 patients during performance of gastric resection, perforative ulcer excision for the peritonitis dissemination prophylaxis. In 18 patients was used the polychlorvinyl drain, newly constructed by the authors. In 124 patients gastric resection according to Haberer with an oblique gastroduodenoanastomosis formation, the Treitz ligament dissection and duodenojejunal flexure descending were performed for the peritonitis dissemination prophylaxis.

RESUMO

On the basis of the experience with treatment of 19,346 patients with acute appendicitis, the authors have established that pain in the right inguinal region, rigidity of muscles of the anterior abdominal wall, and a Volkovich-Kocher symptom were the most informative symptoms. The Shchetkin-Blumberg symptom caused by the development of an inflammatory process in the peritoneum is not an appendicular symptom; the Rovzing, Voskresensky, Obraztsov symptoms, not being the specific symptoms of acute appendicitis, are of essential informative value only in combination with the other symptoms of the disease.

RESUMO

A new method of plasty of hernial hilus for femoral hernias is proposed which consists in sutureless fixation of allotransplants made of silicone having the form of a mushroom-shaped element. The transplant gives good and reliable closure of the hernial hilus without changing the interrelation of tissues. The duration of the operative procedure becomes considerably shorter, any injuries of large vessels were excluded as well as the development of ligature fistulas, suppuration of the wounds and recurrent hernias. An analysis of nearest and long-term results of treatment of 68 patients operated upon shows high efficiency of the method proposed.

RESUMO

The clinico-roentgenological signs of acute commissural ileus were analysed in 226 patients. Of them, 192 were operated on. In 6.4% of the patients in plain roentgenography, the signs of acute ileus were absent, in 27.7%--they were not prominent. Study of the barium suspension passage through the small intestine, as the authors consider, is indicated in unmarked clinical features of the disease, and as well in its short duration (less than 6 h) with the aim of establishing a degree of ileus (complete, or partial).

RESUMO

The authors observed 41 patients with pseudotuberculosis. In 32 patients, the abdominal syndrome predominated in the clinical course. For this reason, the appendectomy was performed in 30 of them. In 9 patients, the lingering, relapsing course of pseudotuberculosis was noted after the operation.

RESUMO

The analysis of the causes of 45 (0.16%) eventrations which occurred after 27,438 emergency laparotomies was carried out. The authors attach the most importance in the occurrence of this complication to the development of a purulent-necrotic process in the wound of the abdominal wall, which spreads from outside inside in relation to the abdominal wall layers. Most often, eventration occurs at day 6-8 after the operation. Prophylaxis of postoperative phlegmona in a wound is the most effective measure for prevention of eventration.

RESUMO

An observation of 184 patients treated for different intestinal fistulas allowed to conclude that the developed and introduced into clinic methods of hermetization of incomplete fistulas with the help of a porolon sponge and treatment of complete fistulas by special devices of original construction facilitates valuable preparation of patients to surgery and improves results of the surgical treatment of the disease.

RESUMO

On the basis of the experimental investigation and clinical observations, the authors consider that the formation of inter-intestinal end-to-end anastomosis with two-layer interrupted suture of non-absorbable material creates the prerequisites for the development of suture failure and anastomotic stenosis. The removal of the mesothelium of the sutured intestinal surfaces leads to the early formation of adhesions between the organs. The first-turn placing of the internal layer of sutures with tightening the threads provides their timely eruption and rejection. The suturing of the mesenterial sites of the intestine with the surfaces covered by serous membrane increases the safety of anastomotic sutures and creates convenience for the work of a surgeon.

RESUMO

The analysis of the results of treating 47 patients with complications of colonic diverticulosis was carried out. The diagnosis of the disease and its complications should be based on the strict consideration of the data of history, clinical manifestations with mandatory use of a roentgenologic method at the acute period. In perforation of diverticulum, the extraperitonization of a site of the colon is an operation of choice.
