RESUMO

BACKGROUND: Paracoccidioidomycosis is an important medical and social problem mainly in rural areas of Brazil, because of the high incidence of the diseases, its long clinical evolution, frequent recurrences and sequels leading to anatomical and functional incapacities. MAIN OBSERVATION: We present a 73-year-old patient with paracoccidioidomycosis showing significant lymph node manifestations, which are only common in children and teenagers. CONCLUSIONS: Paracoccidioidomycosis may have a long incubation period, and it can be diagnosed outside of the endemic regions, where it was acquired. Thus, all dermatologists should maintain a high index of suspicion, especially in unusual cases of extensive lymph node enlargement in the elderly patients.

RESUMO

Prader-Willi syndrome (PWS) is a multisystemic genetic disease associated with structural, behavioral, and intellectual disorders. The clinical appearance of PWS includes dysmorphic facial features with almond-shaped eyes, a narrow bifrontal diameter, and a thin upper lip; short stature; central obesity; and small hands and feet. We present a case of a 27-year-old woman with PWS and describe the typical clinical features and cutaneous manifestations of PWS.

RESUMO

Esporotricose é a micose subcutânea mais comum da America Latina, causada pelo fungo dimórfico Sporothrix schenkii. A transmissão ocorre por trauma com plantas, solo, mordida ou arranhaduras de animais. O gato domestico é um importante transmissor da doença para homens, sendo responsável pela epidemia de esporotricose no Rio de Janeiro. As lesões cutâneas podem ser pápulas, placas, nódulos ou úlceras e fazem diagnóstico diferencial com outras doenças infecciosas, câncer ou metástases cutâneas. Relatamos o caso de uma paciente jovem com lesão úlcero-vegetante na mama direita, sem história de mordida ou arranhao de felino. Diante dessa localização peculiar, é imprescindível fazer o diagnóstico diferencial com metástase cutânea de câncer de mama.


Sporotrichosis is the most common subcutaneous mycosis in Latin America and it is caused by the dimorphic fungus Sporothrix schenkii. The transmission occurs by trauma with plants, contact with soil, bite or scratches from animals. The domestic cat is an important transmitter of the disease for men, being responsible for the epidemic of sporotrichosis in Rio de Janeiro, Brazil. Skin lesions may be papules, plaques, nodules or ulcers and make differential diagnosis with other infectious diseases, cancer or cutaneous metastasis. We report the case of a young patient with ulcerative-vegetative lesion in the right breast with no history of bite or scratch from cat. Given this unusual situation, it is essential to make the differential diagnosis with cutaneous metastasis of breast cancer.
