Objective: Chikungunya virus (CHIKV) causes persistent arthritis, and our prior study showed that approximately one third of CHIKV arthritis patients had exacerbated arthritis associated with exercise. The underlying mechanism of exercise-associated chikungunya arthritis flare (EACAF) is unknown, and this analysis aimed to examine the regulatory T-cell immune response related to CHIKV arthritis flares. Methods: In our study, 124 Colombian patients with a history of CHIKV infection four years prior were enrolled and 113 cases with serologically confirmed CHIKV IgG were used in this analysis. Patient information was gathered via questionnaires, and blood samples were taken to identify total live peripheral blood mononuclear cells, CD4+ cells, T regulatory cells, and their immune markers. We compared outcomes in CHIKV patients with (n = 38) vs. without (n = 75) EACAF using t-tests to assess means and the Fisher's exact test, chi-squared to evaluate categorical variables, and Kruskal-Wallis tests in the setting of skewed distributions (SAS 9.3). Results: 33.6% of CHIKV cases reported worsening arthritis with exercise. EACAF patients reported higher global assessments of arthritis disease ranging from 0-100 (71.2 ± 19.7 vs. 59.9 ± 28.0, p=0.03). EACAF patients had lower ratios of T regulatory (Treg)/CD4+ T-cells (1.95 ± 0.73 vs. 2.4 ± 1.29, p = 0.04) and lower percentage of GARP (glycoprotein-A repetitions predominant) expression per Treg (0.13 ± 0.0.33 vs. 0.16 ± 0.24 p= 0.020). Conclusion: These findings suggest relative decreases in GARP expression may indicate a decreased level of immune suppression. Treg populations in patients with CHIKV arthritis may contribute to arthritis flares during exercise, though current research is conflicting.

Abstract Introduction: Posterior fossa tumors are common in the pediatric population and require adequate characterization by means of structural magnetic resonance imaging (MRI) and advanced MRI techniques to achieve an appropriate therapeutic approach. Objectives: To determine the usefulness of apparent diffusion coefficient (ADC) values for the differential diagnosis of posterior fossa tumors in the pediatric population treated at a reference hospital in Bogotá D.C., Colombia. Materials and methods: Diagnostic accuracy study carried out in 28 pediatric patients diagnosed with posterior fossa tumor between 2017 and 2019 at the Fundación Hospital de la Misericordia, a quaternary care institution. ADC values were measured and compared with histopathological diagnosis as gold standard, obtaining sensitivity, specificity, and positive and negative predictive values. Differences between medians were determined using the Kruskall-Wallis test. The p value between the quantitative ADC value and the gold standard was calculated using Pearson's chi-squared test, with a significance level of p<0.05. Results: The mean age of the participants was 83.9 months (SD=57 months), and 64.28% of them were boys. Medulloblastoma was the most frequent tumor (39.29%). For the diagnosis of medulloblastoma, an ADC value of 0.6210x10-3mm2/s was established, obtaining a sensitivity and specificity of 81.82% and 76.47%, respectively. For the diagnosis of pilocytic astrocytoma, an ADC of 1.03x10-3mm2/s was determined, with a sensitivity of 66.67% and a specificity of 89.40%. Conclusions: ADC value is useful to classify and differentiate posterior fossa tumors in the Colombian pediatric population, showing an inversely proportional relationship with the tumor grade.


Resumen Introducción. Los tumores de la fosa posterior son frecuentes en población pediátrica y requieren de una adecuada caracterización mediante resonancia magnética (RM) estructural y técnicas avanzadas de RM para lograr un enfoque terapéutico apropiado. Objetivo. Determinar la utilidad de los valores del coeficiente de difusión aparente (ADC cuantitativo) en el diagnóstico diferencial de los tumores de la fosa posterior en población pediátrica de un hospital de referencia en Bogotá D.C., Colombia. Materiales y métodos. Estudio de validez de prueba diagnóstica realizado en 28 pacientes pediátricos diagnosticados con tumor de fosa posterior entre 2017 y 2019 en la Fundación Hospital Pediátrico de la Misericordia, hospital de IV nivel de complejidad. Se midieron los valores del ADC cuantitativo, los cuales fueron comparados con el diagnóstico histopatológico como estándar de oro, obteniendo datos de sensibilidad, especificidad, valores predictivos positivos y negativos. Las diferencias entre medianas fueron determinadas mediante la prueba de Kruskall-Wallis. El valor p entre el valor del ADC cuantitativo y el estándar de oro se calculó con la prueba X2 de Pearson, con un nivel de significancia de p<0.05. Resultados. La edad media fue 83.9 meses (DE=57 meses) y 64.28% fueron niños. El tumor más frecuente fue el meduloblastoma (39.29%). Para el diagnóstico de meduloblastoma se estableció un valor ADC cuantitativo de 0.6210x10-3mm2/s, obteniéndose una sensibilidad y especificidad de 81.82% y 76.47%, respectivamente, y para el diagnóstico de astrocitoma pilocítico, un ADC cuantitativo de 1.03x10-3mm2/s, con una sensibilidad de 66.67% y una especificidad de 89.40%. Conclusiones. El valor del ADC cuantitativo es útil para clasificar y diferenciar los tumores de la fosa posterior en población pediátrica colombiana, mostrando una relación inversamente proporcional con el grado tumoral.

OBJECTIVE: The primary objective of this research was to explore the link between sleep and flare pain associated with chikungunya virus (CHIKV) infection. The secondary objective was to investigate if cytokines and T regulatory (Treg) cells have an influence on this relationship. METHODS: A cross-sectional study was performed using data collected in Barranquilla, Colombia, which enrolled patients with and without chronic arthritis with a history of chikungunya infection. Flare severity was measured by a version of the Outcome Measures in Rheumatoid Arthritis Clinical Trials (OMERACT) flare questionnaire adapted for CHIKV arthritis, including metrics for pain, difficulty with physical activity, fatigue, stiffness and difficulty maintaining social activities due to arthritis that contribute to flare severity. In addition, four sleep disturbance items, five inflammatory cytokine levels, four anti-inflammatory cytokine levels, and six Treg levels were measured. Then, multivariable linear regression models were used to test the direct and indirect effects of flare-pain on sleep disturbance, and to determine whether this relationship was mediated by cytokines or Tregs. Finally, the SAS CALIS procedure was used to test path models showing possible causal effects with mediators and confounds. RESULTS: The analysis showed that sleep disturbance is positively correlated with CHIKV arthritis flare pain, and that it is a significant predictor of flare severity after adjusting for demographic variables, cytokine, and T cell levels. Further, neither T cells nor cytokines mediate the pain/sleep relationship in CHIKV arthritis. CONCLUSION: There is a strong association between sleep disturbance and arthritis flare pain and severity; however, this relationship is not mediated by cytokines or T cells. Since this study is unable to determine causation, further research is needed to determine the mechanism underlying the relationship between sleep disturbances and CHIKV arthritis flares.

Cattle of Colombian creole breed Chino Santandereano in the raising phase were subjected to stabling for 100 days in order to determine the productive, nutritional, and metabolic capacity when exposed to different degrees of protein-energy supplementation. Sixteen whole males with an initial mean weight of 377.69 ± 16.55 kg were used in completely randomized delineation with four treatments and four repetitions per treatment, as follows: UNS, not supplemented; low, supplemented with amounts relative to 0.5% of body weight; medium, supplemented with amounts relative to 1.0% of the live weight; and high, supplemented with amounts relative to 1.5% of body weight. At the end of each experimental period, the animals were weighed, and samples of feed, feces, blood, and urine were collected to determine the performance, consumption, and digestibility of the nutritional components, and nitrogen balance. In the greater performance, consumption, and digestibility of DM, OM, C-NFCP, EE, and NFC, the concentration of ureic nitrogen in blood and urine (P <0.05) was observed in supplemented animals when contrasted with UNS. Bovine Chinese Santandereano presents high productive, nutritional, and metabolic potential as a response to high levels of protein-energy supplementation.

INTRODUCTION: Roxadustat is an oral hypoxia-inducible factor prolyl hydroxylase inhibitor that stimulates erythropoiesis and improves iron metabolism. We assessed the efficacy and tolerability of roxadustat in patients with chronic kidney disease (CKD)-related anemia not on dialysis. METHODS: ANDES was a global Phase 3 randomized study in which adults with stage 3-5 CKD not on dialysis received roxadustat or placebo. Patients were initially dosed thrice weekly; dose was titrated to achieve a hemoglobin level ≥11.0 g/dl, followed by titration for maintenance. The primary endpoints were change in hemoglobin (weeks 28-52) and proportion of patients achieving a hemoglobin response (hemoglobin ≥11.0 g/dl and increase ≥1.0 g/dl [baseline >8.0 g/dl], or increase ≥2.0 g/dl [baseline ≤8.0 g/dl]) (week 24). Treatment-emergent adverse events (TEAEs) and serious adverse events (TESAEs) were recorded. RESULTS: In roxadustat (n = 616) and placebo (n = 306) groups, hemoglobin mean (SD) change from baseline over weeks 28-52 was significantly larger for roxadustat (2.00 [0.95]) versus placebo (0.16 [0.90]), corresponding to least-squares mean difference of 1.85 g/dl (95% confidence interval [CI] 1.74-1.97; P < 0.0001). The proportion of patients achieving a response at week 24 was larger for roxadustat (86.0%; 95% CI 83.0%-88.7%) versus placebo (6.6%; 95% CI 4.1%-9.9%; P < 0.0001). The proportion of patients receiving rescue therapy at week 52 was smaller for roxadustat (8.9%) versus placebo (28.9%); hazard ratio, 0.19 (95% CI 0.14-0.28; P < .0001). The incidences of TEAEs and TESAEs were comparable. CONCLUSION: This study showed that roxadustat corrected and maintained hemoglobin and was well tolerated in patients with CKD-related anemia not on dialysis (ClinicalTrials.gov NCT01750190).

At the end of 2019, in Wuhan, China, the outbreak of a new coronavirus began and quickly spread throughout the world infecting and claiming thousands of lives. To date, certain comorbidities are known to be risk factors for unsatisfactory disease outcomes, but little has been reported regarding hemodialysis patients despite being a population at high risk of infection, complications, and death. Here we describe the clinical course, clinical manifestations and complications of COVID-19 in seven patients on permanent hemodialysis. We also make recommendations for the management of patients with chronic kidney disease.

Se ha encontrado que ciertas comorbilidades constituyen factores de riesgo para resultados poco satisfactorios de la enfermedad, pero es poco lo que se ha descrito sobre pacientes en hemodiálisis, a pesar de tratarse de una población de alto riesgo de infección, complicaciones y muerte. En este artículo se describe el curso clínico, las manifestaciones clínicas y las complicaciones de la COVID-19 en siete pacientes en hemodiálisis permanente y se hacen recomendaciones para el manejo de pacientes con enfermedad renal crónica.

INTRODUCTION/OBJECTIVES: To characterize the importance of musculoskeletal stiffness in a cohort of chikungunya patients with chronic joint symptoms. METHOD: Eighty-two patients were followed up 3 years after chikungunya infection. Tender and swollen joint counts, a pain intensity scale, Health Assessment Questionnaire-Disability Index (HAQ-DI), and the EuroQol EQ-5D quality of life instrument were completed. A musculoskeletal stiffness questionnaire provided scores for overall stiffness and its components: stiffness severity, physical impact, and psychosocial impact. RESULTS: Patients had a mean age 51 ± 14 years. Sixty-seven patients were still experiencing chronic arthralgia. Musculoskeletal stiffness was reported by 43/67 patients with arthralgia and 3/15 patients without arthralgia. A physical impact of stiffness was reported by 87% patients and psychosocial impact by 71% patients. Mean tender joint count in patients reporting arthralgia was 6 ± 7, mean pain intensity 65 ± 20 out of 100, mean HAQ-DI was 0.54 ± 0.52, and mean EQ-VAS global health perception was 68 ± 62 out of 100. Stiffness severity was correlated with tender joint counts (ρ = 0.46) and pain intensity (ρ = 0.40). All three measures were equally well correlated with the EuroQol-VAS global health perception. Pain and tender joints were better correlated with the HAQ-DI (ρ = 0.68 and ρ = 0.63), but stiffness was more strongly correlated with several quality of life domains, including mobility. Swollen joints were a poor predictor of outcomes. CONCLUSIONS: Musculoskeletal stiffness following chikungunya infection is distinct from arthralgia. It does not always occur in the same patients or with a corresponding intensity. Joint pain and stiffness may be independently associated with disability and quality of life assessments.

OBJECTIVE: To determine the frequency of chronic joint pain and stiffness 3 years after infection with chikungunya virus (CHIKV) in a Latin American cohort. METHODS: A cross-sectional followup of 120 patients from an initial cohort of 500 patients who reported joint pain 2 years after infection from the Atlántico Department, Colombia. Patients were clinically diagnosed as having CHIKV during the 2014-2015 epidemic, and baseline and followup symptoms at 40 months were evaluated in serologically confirmed cases. RESULTS: Of the initial 500 patients enrolled in the study, 482 had serologically confirmed chikungunya infection. From this group, 123 patients reported joint pain 20 months after infection, and 54% of those patients reported continued joint pain 40 months after infection. Therefore, 1 out of every 8 people who tested serologically positive for CHIKV infection had persistent joint pain 3 years after infection. Participants who followed up in person were predominantly adult (mean ± SD age 51 ± 14 yrs) and female (86%). The most common type of pain reported in these patients at 40 months post-infection was pain with periods of relief and subsequent reoccurrence, and over 75% reported stiffness after immobility, with 39% experiencing morning stiffness. CONCLUSION: To our knowledge, this is the first report to describe persistent joint pain and stiffness 40 months after viral infection. The high frequency of chronic disease highlights the need to develop prevention and treatment methods. Further studies should be conducted to understand the similarities between post-chikungunya joint pain and rheumatoid arthritis.

Abstract This paper reports the case of a toddler with chronic stridor of four months of evolution, treated several times by micro-nebulization (MNB) with beta 2, adrenergics, O2 and corticosteroids with partial improvement. The patient did not have complementary studies nor a clear history of foreign body aspiration (FB). During the last visit to the ER, a neck X-ray revealed a stippling sign in the upper airway. A bronchoscopy was performed, and a sharp foreign body (fishbone) was found in the upper airway, which was subsequently removed. This case is reported due to the unusual evolution of the foreign body in the upper airway, which manifested as a chronic stridor that resolved without further complications.


Resumen Se presenta el caso de una paciente prescolar con estridor crónico de 4 meses de evolución, tratada varias veces mediante micronebulizaciones (MNB) con beta 2, adrenérgicos, O2 y corticoides con mejoría parcial, sin estudios complementarios ni antecedentes claros de atoramiento con cuerpo extraño (CE). Además, se realizó una radiografía de cuello (Rx) que reveló la presencia de imagen en punta de lápiz traqueal y una broncoscopia que comprobó la existencia de un cuerpo extraño puntiagudo (espina de pescado) en la vía aérea superior. Este caso se reportó por su inusual presentación, pues es un CE en la vía aérea superior que se manifestó con estridor crónico, sin mayores complicaciones.

El uso recreacional de drogas psicoactivas es una práctica generalizada en población de adultos jóvenes en el mundo. Existe evidencia de que los opioides y la cocaína producen toxicidad renal. La necrosis tubular aguda, rabdomiolisis y la glomerulonefritis focal segmentaria son las presentaciones más comunes. Reportamos un caso de daño renal agudo en un joven después del uso recreacional de alcohol, cocaína y opioides, cuyos hallazgos patológicos son descritos brevemente en la literatura.


Recreational drug use is a widespread activity in the young adult population. Cocaine and opioids have well described mechanism for renal toxicity, they include local and systemic toxicity, acute tubular necrosis, rhabdomylolysis and focal segmental glomerulosclerosis are among the most common presentations. We report a case of acute kidney injury in a young man after the recreational use of alcohol, cocaine and opioids, the pathological findings are described and a brief review of the literature is presented.
