RESUMO

AIMS: The authors investigated associations of work related risk factors with self perceived health as less than "good" and psychological distress among Italian women flight attendants. METHODS: The authors conducted a cross sectional survey on health and mental health among 1955 former and current flight attendants, using a postal questionnaire. RESULTS: More current than former flight attendants reported self perceived health as fair to poor and psychological distress measured as a GHQ-12 score of six or more. Among current flight attendants, reporting health as fair to poor was associated with low job satisfaction (OR 1.89) and recent experiences of sexual harassment by passengers (OR 2.83). Psychological distress was associated with low job satisfaction (OR 2.38) and frequent tension with partner over childcare (OR 1.79). CONCLUSIONS: Perceived health as fair to poor and psychological distress were greater among current flight attendants and were related to job characteristics and family difficulties. Perceived poor health has been shown in the literature to be related to mortality, high job strain, and early retirement, and psychological distress is associated with work absence. The effect of sexual harassment by passengers on perceived health of flight attendants may be relevant to other working women dealing with the public. The health effects of family/work conflicts, low job satisfaction, and sexual harassment should be explored more in depth, using qualitative as well as quantitative methods among working women in various occupations.

RESUMO

There are few human data on low-dose-rate-radiation exposure and the consequent acute and late effects. This fact makes it difficult to assess health risks due to radiation in the space environment, especially for long-term missions. Epidemiological data on civilian flight personnel cohorts can provide information on effects due to the low-dose and low-dose rate mixed high- and low-LET radiation environment in the earth's atmosphere. The physical characteristics of the radiation environment of the atmosphere make the results of the studies of commercial flight personnel relevant to the studies of activities in space. The cooperative international effort now in progress to investigate dose reconstructions will contribute to our understanding of radiation risks for space exploration.

RESUMO

A study of the effects of exposures of civilian aviation flight personnel to atmospheric ionizing radiation (including high-energy neutrons) is being performed. The results of previous studies and of the criteria required for a more satisfactory outcome in future studies are presented, along with a description of the protocol for the Italian national study. A description of the cohort is given in terms of its size, composition and member eligibility. The various ways of determining the exposure and the health status of past and current aircrew members are discussed, and follow-up procedures are described. An overview of the data management and processing philosophy adopted in the Italian study is given with regard to flight routes, radiation dose evaluation along the flight paths, and construction of exposure matrices. Other studies of potential interest are also discussed. The study is still in progress, so the results are preliminary.

RESUMO

Human data on low dose rate radiation exposure and its effects are not readily available. A huge amount of such data may be obtained through flight personnel cohorts, in the form of epidemiological studies on delayed health effects induced by the cosmic-ray generated atmospheric ionizing radiation, to which flight personnel are exposed all throughout their work activity. All the available results from different studies on flight personnel exposure have been combined in various ways to evaluate the association between atmospheric ionizing radiation environment and health risks and to assess directions for future investigations.

RESUMO

A study, currently in progress, on the radiation exposure and the associated biomedical effects due to the atmospheric ionizing radiation environment for the Italian civilian aviation flight personnel is sketched. After a presentation of the considered data sources, a description of the cohort is given, in terms of criteria for eligibility, and cohort construction, size and composition. Then the protocol for the Italian study is presented: the various ways of investigating the exposure and the health status of past and currently employed aircrew members and follow-up procedures are shown. An overview is given of the data management and processing philosophy with regards to flight routes, radiation dose evaluation along the flight path and exposure matrix building, as adopted in the Italian study. Potential side studies of interest are also shown.

Fibrin and fibrinogen degradation products in the cerebrospinal fluid (CSF-FDP) were first studied in a group of 29 patients observed during the first and the second week after subarachnoid hemorrhage (SAH), then in a second group of 26 patients for a total of 55 patients. In the latter group only the first FDP value obtained as soon as possible after SAH was taken in consideration. In the whole series of 55 patients several noteworthy factors were found: 1) FDP determination should be performed as soon as possible after SAH; 2) CSF-FDP at or above 40, 80 micrograms/ml was found both in the patients with severe neurological deficits and in those with cerebral ischemia (statistically significant); 3) the significance of CSF-FDP in patients who rebled was also evaluated. In conclusion CSF-FDP could be considered useful in predicting cerebral ischemia.

We have characterized a beta 112 Arg hemoglobin in an individual from Naples, Italy, with minimal clinical problems. Blood tests revealed only slight reticulocytosis and hemoglobin instability. Furthermore, high value of alkali resistance tests for Hb F were observed. Isoelectricfocusing of globins showed the occurrence of a band migrating between the normal alpha and beta globin chains. The fairly stable variant chain was purified by fast protein liquid chromatography. A mass map of the tryptic digest was obtained by fast atom bombardment mass spectrometry clearly showing that we were dealing with a beta chain variant. However, the peptide 105-120 was missing and two new ones were present, i.e.: 105-112 and 113-120; we assumed these peptides to be generated because of the substitution of 112 Cys with an arginine residue. Further confirmation stemmed from the fast atom bombardment mass spectra of the tryptic digest submitted to a single Edman degradation step and to carboxypeptidase B further hydrolysis. The beta-globin chain variant was thus mass mapped to an extent of about 98%. Such a variant, named Hb Indianapolis, was first reported by Adams et al, as an extremely unstable variant producing the phenotype of a severe beta-thalassemia. Contrary to the findings of the above authors the occurrence of the same variant in a clinically normal individual from a Spanish family has recently been reported. Because the clinical manifestations in the latter case are similar to those observed by us, the conclusion can be drawn that beta 112 Arg hemoglobin is not a biologically unstable variant but should be regarded as belonging to the class of unstable hemoglobins giving rise to only marginal clinical problems.
