RESUMO

The global pandemic of metabolic diseases has increased the incidence of hepatocellular carcinoma (HCC) in the context of non-alcoholic steatohepatitis (NASH). The downregulation of the E3 ubiquitin ligase TRIM21 has been linked to poor prognosis in different cancers including HCC. In order to investigate the role of TRIM21 in liver cancer progression on NASH, Trim21+/+ and Trim21-/- male mice were injected with streptozotocin at the neonatal stage. The hypoinsulinemic mice were then fed with a high-fat high-cholesterol diet (HFHCD) for 4, 8 or 12 weeks. All mice developed NASH which systematically resulted in HCC progression. Interestingly, compared to the Trim21+/+ control mice, liver damage was worsened in Trim21-/- mice, with more HCC nodules found after 12 weeks on HFHCD. Immune population analysis in the spleen and liver revealed a higher proportion of CD4+PD-1+ and CD8+PD-1+ T cells in Trim21-/- mice. The liver and HCC tumors of Trim21-/- mice also exhibited an increase in the number of PD-L1+ and CD68+ PD-L1+ cells. Thus, TRIM21 limits the emergence of HCC nodules in mice with NASH by potentially restricting the expression of PD-1 in lymphocytes and PD-L1 in tumors.

C3G is a Rap1 GEF that plays a pivotal role in platelet-mediated processes such as angiogenesis, tumor growth, and metastasis by modulating the platelet secretome. Here, we explore the mechanisms through which C3G governs platelet secretion. For this, we utilized animal models featuring either overexpression or deletion of C3G in platelets, as well as PC12 cell clones expressing C3G mutants. We found that C3G specifically regulates α-granule secretion via PKCδ, but it does not affect δ-granules or lysosomes. C3G activated RalA through a GEF-dependent mechanism, facilitating vesicle docking, while interfering with the formation of the trans-SNARE complex, thereby restricting vesicle fusion. Furthermore, C3G promotes the formation of lamellipodia during platelet spreading on specific substrates by enhancing actin polymerization via Src and Rac1-Arp2/3 pathways, but not Rap1. Consequently, C3G deletion in platelets favored kiss-and-run exocytosis. C3G also controlled granule secretion in PC12 cells, including pore formation. Additionally, C3G-deficient platelets exhibited reduced phosphatidylserine exposure, resulting in decreased thrombin generation, which along with defective actin polymerization and spreading, led to impaired clot retraction. In summary, platelet C3G plays a dual role by facilitating platelet spreading and clot retraction through the promotion of outside-in signaling while concurrently downregulating α-granule secretion by restricting granule fusion.

Six Functionalized Activated Carbon Cloths (FACCs) were designed to obtain fundamental information for training a Bayesian Regularized Artificial Neural Network (BRANN) capable of predicting adsorption capacity of the FACCs to synthesize tailor-made materials with potential application as dialysis membranes. Characterization studies showed that FACCs have a high surface area (1354-2073 m2 g-1) associated with increased microporosity (W0, average: 0.57 cm3 g-1). Materials are carbonaceous, with a carbon content between 69 and 92%. Chemical treatments modify the pHpzc of materials between 4.1 and 7.8 due to incorporating functional groups on the surface (C=O, -COOH, -OH, -NH, -NH2). Uremic toxins tests showed a high elimination rate of p-cresol (73 mg g-1) and creatinine (90 mg g-1) which is not affected by the matrix (aqueous solution and simulated serum). However, in the case of uric acid, adsorption capacity decreased from 143 mg g-1 to 71 mg g-1, respectively. When comparing the kinetic constants of the adsorption studies in simulated serum versus the studies in aqueous solution, it can be seen that this does not undergo significant changes (0.02 min-1), evidencing the versatility of the material to work in different matrices. The previous studies, in combination with characterization of the materials, allowed to establish the adsorption mechanism. Thus, it permitted to train the BRANN to obtain mathematical models capable to predict the kinetic adsorption of the toxins studied. It is concluded that the predominant adsorption mechanism is due to π-π interactions between the adsorbate unsaturations with the material's pseudo-graphitic planes. Results show that FACCs are promising materials for hemodialysis membranes. Finally, taking into consideration the adsorption capacities and rates, as well as the semiquantitative analysis of the environmental impact associated with the preparation of the adsorbents, the best adsorbent (CC, Eco-Scale = 91.5) was selected. The studies presented show that the material is eco-friendly and highly efficient in the elimination of uremic toxins.

Background & Aims: Metabolic dysfunction-associated steatotic liver disease (MASLD) results in steatosis, inflammation (steatohepatitis), and fibrosis. Patients with MASLD more likely develop liver injury in coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). As viral RNA has been identified in liver tissues, we studied expression levels and cellular sources of the viral receptor angiotensin-converting enzyme 2 (ACE2) and coreceptors in MASLD and fibroinflammatory liver diseases. Methods: We built a transcriptomic MASLD meta-dataset (N = 243) to study SARS-CoV-2 receptor expression and verified results in 161 additional cases of fibroinflammatory liver diseases. We assessed the fibroinflammatory microenvironment by deconvoluting immune cell populations. We studied the cellular sources of ACE2 by multiplex immunohistochemistry followed by high-resolution confocal microscopy (N = 9 fatty livers; N = 7 controls), meta-analysis of two single-cell RNA sequencing datasets (N = 5 cirrhotic livers; N = 14 normal livers), and bulk transcriptomics from 745 primary cell samples. In vitro, we tested ACE2 mRNA expression in primary human hepatocytes treated with inflammatory cytokines, bacterial lipopolysaccharides, or long-chain fatty acids. Results: We detected ACE2 at the apical and basal poles of hepatocyte chords, in CLEC4M+ liver sinusoidal endothelial cells, the lumen of ABCC2+ bile canaliculi, HepPar-1+-TMPRSS2+ hepatocytes, cholangiocytes, and CD34+ capillary vessels. ACE2 steeply increased between 30 and 50 years of age; was related to liver fat area, inflammation, high immune reactivity, and fibrogenesis; and was upregulated in steatohepatitis. Although ACE2 mRNA was unmodified in alcoholic or viral hepatitis, it was upregulated in fibroinflammatory livers from overweight patients. In vitro, treatment of primary human hepatocytes with inflammatory cytokines alone downregulated but long chain fatty acids upregulated ACE2 mRNA expression. Conclusions: Lipid overload in fatty liver disease leads to an increased availability of ACE2 receptors. Impact and implications: COVID-19 can be a deadly disease in vulnerable individuals. Patients with fatty liver disease are at a higher risk of experiencing severe COVID-19 and liver injury. Recent studies have indicated that one of the reasons for this vulnerability is the presence of a key cell surface protein called ACE2, which serves as the main SARS-CoV-2 virus receptor. We describe the cellular sources of ACE2 in the liver. In patients with fatty liver disease, ACE2 levels increase with age, liver fat content, fibroinflammatory changes, enhanced positive immune checkpoint levels, and innate immune reactivity. Moreover, we show that long chain fatty acids can induce ACE2 expression in primary human hepatocytes. Understanding the cellular sources of ACE2 in the liver and the factors that influence its availability is crucial. This knowledge will guide further research and help protect potentially vulnerable patients through timely vaccination boosters, dietary adjustments, and improved hygiene practices.

Introducción. El deporte universitario da respuesta a diversas necesidades y expectativas de los estudiantes. Dadas las características de este, se plantean problemas potenciales a la salud. La Universidad de Santander campus Bucaramanga (UDES) brinda espacios de bienestar para la participación deportiva, pero no cuenta con un servicio acorde a las necesidades de los deportistas universitarios. Objetivo. Describir el proceso de creación e institucionalización de un modelo de atención integral al deportista para la UDES. Métodos. Estudio cualitativo de diseño Investigación-Acción, el cual es desarrollado dentro de la comunidad de deportistas universitarios, construido en cuatro fases, partiendo de un diagnóstico poblacional con la aplicación de una entrevista semiestructurada, en segundo lugar, una revisión documental para identificar modelos de atención al deportista. Para el análisis de resultados se genera una matriz para su triangulación. Por último, se genera un plan de acción y construcción del modelo de atención integral al deportista. Resultados. Un total de 92 respuestas a la encuesta envida, encontrando convergencias, divergencias y convergencias. En la revisión bibliográfica se evidencio las necesidades del deporte universitario. Finalmente se dio un plan de acción para cubrir estas necesidades. Conclusiones. El modelo MAID UDES logra cumplir las expectativas de la comunidad deportiva universitaria.


Background. University sports respond to various needs and expectations of students. Given its characteristics, potential health problems arise. The University of Santander Bucaramanga campus (UDES) provides wellness spaces for sports participation, but does not have a service according to the needs of university athletes. Objective. Describe the process of creation and institutionalization of a comprehensive athlete care model for the UDES. Methods. Qualitative study of Action Research design, which is developed within the community of university athletes, built in four phases, starting from a population diagnosis with the application of a semi-structured interview, secondly, a documentary review to identify models of attention to the athlete. For the analysis of results, a matrix is generated for triangulation. Finally, an action plan is generated and construction of the comprehensive care model for the athlete. Results. A total of 92 responses to the envida survey, finding convergences, divergences and convergences. In the bibliographic review, the needs of university sports were evident. Finally, an action plan was given to cover these needs. Conclusions. The MAID UDES model manages to meet the expectations of the UDES university sports community.

In the present research work, an algorithm of artificial neural network (ANN) has been developed based on the processing of digital images of Persian lemons with the aim of optimizing the quality control of the product. For this purpose, the physical properties (weight, thickness of the peel, diameter, length, and color) of 90 lemons selected from the company Esperanza de San José Ornelas SPR de RL (Jalisco, Mexico) were studied, which were divided into three groups (Category "extra", Category I, and Category II) according to their characteristics. The parameters of weight (26.50 ± 3.00 g), diameter/length (0.92 ± 0.08) and thickness of the peel (1.50 ± 0.29 mm) did not present significant differences between groups. On the other hand, the color (determined by the RGB and HSV models) presents statistically significant changes between groups. Due to the above, the proposed ANN correctly classifies 96.60% of the data obtained for each of the groups studied. Once the ANN was trained, its application was tested in an automatic classification process. For this purpose, a prototype based on the operation of a stepper motor was simulated using Simulink from Matlab, which is connected to three ideal switches powered by three variable pulse generators that receive the information from an ANN and provide the corresponding signal for the motor to turn to a specific position. Manual classification is a process that requires expert personnel and is prone to human error. The scientific development presented shows an alternative for the automation of the process using low-cost computational tools as a potential alternative.

The present research shows a robust isotopic ratio characterization of Carbon-13 (δ13CVPDB) in congeneric compounds such as methanol, n-propanol, isoamyl alcohol, ethyl lactate, ethyl acetate, ethanol, and acetaldehyde in representative samples (n = 69) of Tequila 100% agave silver class (TSC), employing gas chromatography/combustion/isotope-ratio mass spectrometry (GC/C/IRMS). From the information obtained, the construction of a radial plot attributable to the isotopic fingerprint of TSC was achieved. With this information, a diagnostic test was designed to determine the authenticity of TSC, comparing alcoholic beverages from other agave species as non-authentic samples. The sensitivity of the test was 94.2%; the specificity was 83.3%. Additionally, non-authentic samples were analyzed that meet all the criteria established in the regulations. The results obtained show that the GC/C/IRMS analytical technique and designed diagnostic test are useful as auxiliary parameters to determine the authenticity of the beverage, thus managing to determine the adulteration or falsification of the product.

The study of the evolution of color of alcoholic beverages subjected to accelerate maturation process using heat-treated French oak wood fragments is presented. The results show that it is possible to obtain tonalities like aged beverages in 4 weeks. In this sense, the fragments conditioned at 150 °C (light toasted) proportionated colors like white wine, pale straw, and pale gold. On the other hand, the fragments that received a heat treatment at 200 °C (medium toasted) present yellow tones such as old gold, amber, and deep gold. Finally, the fragments treated at 250 °C (heavily toasted) are those with the most intense yellow tones, classified as sweet chestnut, sherry, russet, muscat, and tawny. The studies of kinetic maturation concluded that the mathematical model of parabolic diffusion could correctly describe the process. Based on this, it is concluded that the heat treatment increases the cavities of the most exposed surface of the wood, increasing the maximum humidity of the materials by 20 %; in such a way that during the first two weeks, there is a diffusion of the solution to the active sites. Wood bioactive compounds on the outer surface achieve a rapid extraction, such as flavonoids, which oxidize rapidly within the solution, generating an increase in yellow color. The previous results were corroborated in a real case analysis using Tequila which can be concluded that the proposed process can give the beverage similar colors to an aged, extra-aged, and ultra-aged class in less than 4 weeks.

Currently, water pollution represents a serious environmental threat, causing an impact not only to fauna and flora but also to human health. Among these pollutants, inorganic and organic pollutants are predominantly important representing high toxicity and persistence and being difficult to treat using current methodologies. For this reason, several research groups are searching for strategies to detect and remedy contaminated water bodies and effluents. Due to the above, a current review of the state of the situation has been carried out. The results obtained show that in the American continent a high diversity of contaminants is present in the water bodies affecting several aspects, in which in some cases, there exists alternatives to realize the remediation of contaminated water. It is concluded that the actual challenge is to establish sanitation measures at the local level based on the specific needs of the geographical area of interest. Therefore, water treatment plants must be designed according to the contaminants present in the water of the region and tailored to the needs of the population of interest.

BACKGROUND AND AIMS: HCC, the third leading cause of cancer-related death, arises in the context of liver fibrosis. Although HCC is generally poorly fibrogenic, some tumors harbor focal intratumor extracellular matrix (ECM) deposits called "fibrous nests." To date, the molecular composition and clinical relevance of these ECM deposits have not been fully defined. APPROACH AND RESULTS: We performed quantitative matrisome analysis by tandem mass tags mass spectrometry in 20 human cancer specific matrisome (HCCs) with high or low-grade intratumor fibrosis and matched nontumor tissues, as well as in 12 livers from mice treated with vehicle, carbon tetrachloride, or diethylnitrosamine. We found 94 ECM proteins differentially abundant between high and low-grade fibrous nests, including interstitial and basement membrane components, such as several collagens, glycoproteins, proteoglycans, enzymes involved in ECM stabilization and degradation, and growth factors. Pathway analysis revealed a metabolic switch in high-grade fibrosis, with enhanced glycolysis and decreased oxidative phosphorylation. Integrating the quantitative proteomics with transcriptomics from HCCs and nontumor livers (n = 2,285 samples), we identified a subgroup of fibrous nest HCCs, characterized by cancer-specific ECM remodeling, expression of the WNT/TGFB (S1) subclass signature, and poor patient outcome. Fibrous nest HCCs abundantly expressed an 11-fibrous-nest - protein signature, associated with poor patient outcome, by multivariate Cox analysis, and validated by multiplex immunohistochemistry. CONCLUSIONS: Matrisome analysis highlighted cancer-specific ECM deposits, typical of the WNT/TGFB HCC subclass, associated with poor patient outcomes. Hence, histologic reporting of intratumor fibrosis in HCC is of clinical relevance.

Currently, the authenticity and traceability of Tequila are determined in an inspection process carried out by the Tequila Regulatory Council. However, in recent years, the authorities have seized illegal alcoholic products that are marketed as Tequila without being so, making it necessary to strengthen the current methods of detecting counterfeiting and/or adulteration. Therefore, it is important to establish a review of the current analytical techniques that have been proposed to solve this problem. In this review, emphasis is placed on the analysis of the analytical techniques that have been used to consolidate a profile of authenticity and quality in Tequila, thus highlighting new auxiliary analytical techniques to the current verification process, establishing future validation opportunities in terms of international quality control. The use of isotopic ratios stands out as the most robust technique because it establishes the type of sugar source used and the maturation time of the manufacturing process.

OBJECTIVES: Review clinical results of the treatment for acetabular fractures using the pararectus approach and analyze surgical variations of the traditional approach. DESIGN: Retrospective. SETTING: Tertiary referral hospital. PATIENTS: 46 patients over 65 years of age who sustained an acetabular fracture and underwent surgery using the pararectus approach. INTERVENTION: Fractures were treated using a pararectus approach. Three variations of the original technique were performed: (1) Ligature of the deep iliac circumflex artery and vein, (2) separation of the psoas and iliacus muscles, and (3) isolation of the spermatic cord in men and round ligament in women together with the iliac and epigastric vessels. MAIN OUTCOME MEASUREMENTS: Outcomes measures included surgical, demographic, and clinical data, and information related to follow-up. RESULTS: Duration of surgery, 125 minutes (95-210). Quality of reduction on postoperative computed tomography (CT) scan; anatomic in 22 patients (47.8%), incomplete in 16 (34.8%), and poor in 8 (17.4%). In patients in whom the hip was preserved (n = 41), functional status was excellent in 15 patients (36.5%), good in 17 (41.4%), fair in 6 (14.7%), and poor in 3 (7.4%), with mean functional score of 16 points (7-18). Seven patients (15.2%) developed posttraumatic osteoarthritis and 4 of these patients underwent total hip replacement. CONCLUSIONS: This study reports positive outcomes in fracture reduction and clinical outcomes with low complications in older patients who suffered acetabular fractures and were treated using a pararectus approach. Small variations in the technique, such as those proposed in this study, may help to widen access to the surgical site and simplify the technique. LEVEL OF EVIDENCE: Therapeutic Level IV. Objective: To determine the effectiveness and safety of infliximab and etanercept biosimilar drugs in patients diagnosed with rheumatoid arthritis, ankylosing spondylitis, ulcerative colitis, and psoriasis in a specialized institution in Colombia, between 2015 and 2019. Methods: A retrospective study in patients treated with infliximab and etanercept biosimilar drugs treated in an institution specializing in the management of rheumatological diseases, to verify the clinimetric indicators of effectiveness and reports of adverse drug reactions. Clinical, sociodemographic, and pharmacological variables were identified over 5 years of follow-up. Results: 207 patients were identified with a mean age of 48.7 ± 15.1 years, 61.4% were women. Of the patients, 58.0% (n = 120) used infliximab and 42.0% (n = 87) etanercept. It was found that 46 (22.2%) patients had adverse drug reactions. At the end of the observation period, 61.6% (n = 72) of the patients with RA had achieved control of the disease (mild activity or remission), and 57.9% (n = 117) had problems with access to and persistence with therapy. Conclusion: In a group of patients treated in Colombia, the biosimilars of infliximab and etanercept showed proportions of effectiveness and safety comparable to the reference drugs, but lack of adherence to treatment was quite common.


Objetivo: Determinar la efectividad y la seguridad de medicamentos biosimilares de infliximab y etanercept en pacientes con diagnóstico de artritis reumatoide, espondilitis anquilosante, colitis ulcerativa y psoriasis en una institución especializada de Colombia, entre los arios 2015 y 2019. Métodos: Estudio retrospectivo, en pacientes tratados con infliximab y etanercept biosimilares, atendidos en una institución especializada en el manejo de enfermedades reumatológicas, para verificar los indicadores clinimétricos de efectividad y reportes de reacciones adversas medicamentosas. Se identificaron variables clínicas, sociodemográficas y farmacológicas durante cinco años de seguimiento. Resultados: Se identificaron 207 pacientes, con una edad media de 48,7 ± 15,1 años, el 61,4% de los cuales eran mujeres. El 58% (n = 120) de los pacientes utilizó infliximab y el 42% (n = 87) etanercept. Se encontró que 46 (22,2%) pacientes presentaron reacciones adversas al medicamento. Al final del periodo de observación, un 61,6% (n = 72) de los pacientes con AR había alcanzado el control de la enfermedad (actividad leve o remisión) y, en general, el 57,9% (n = 117) tuvo problemas de acceso y persistencia a la terapia. Conclusión: En un grupo de pacientes tratados en Colombia, los biosimilares de infliximab y etanercept mostraron proporciones de efectividad y seguridad comparables a los medicamentos de referencia, pero fue bastante común la falta de adherencia al tratamiento.

BACKGROUND: Perioperative decrease of albumin (ΔAlb) appeared as a promising predictor of complications after digestive surgery, but its role after liver surgery remains unclear. This study aimed to analyze whether and how ΔAlb can be used to predict complications after liver surgery. METHODS: A bicentric retrospective analysis of patients undergoing liver surgery (2010-2016) was performed, following TRIPOD guidelines. The preoperative and postoperative difference of albumin was calculated on POD 0 and defined as ΔAlb. Patients with any missing variable were excluded. The primary endpoint was overall complications according to the Clavien classification. A multiparametric algorithm based on ΔAlb was generated to optimize prediction performance. RESULTS: A total of 110 patients were analyzed. At least one complication occurred in 66 (60%) patients. Patients with and without complication showed a ΔAlb of 15.8 vs. 9.5 g/L (p<0.001). Area under ROC curve (AUC) of ΔAlb was 0.75 (p<0.01.). The ΔAlb-based algorithm showed an AUC of 0.84 (p<0.01), significantly improving performance (p=0.03). Multivariable analysis identified ΔAlb as independent predictor of complications (HR, 1.12; 95% CI, 1.01-1.07; p = 0.002). CONCLUSIONS: ΔAlb appeared as a promising predictor independently associated with the risk of complication after liver surgery. The study presents a novel decision-tree based on ΔAlb to anticipate complications.

C3G is a Rap1 guanine nucleotide exchange factor that controls platelet activation, aggregation, and the release of α-granule content. Transgenic expression of C3G in platelets produces a net proangiogenic secretome through the retention of thrombospondin-1. In a physiological context, C3G also promotes megakaryocyte maturation and proplatelet formation, but without affecting mature platelet production. The aim of this work is to investigate whether C3G is involved in pathological megakaryopoiesis, as well as its specific role in platelet mediated angiogenesis and tumor metastasis. Using megakaryocyte-specific C3G knockout and transgenic mouse models, we found that both C3G overexpression and deletion promoted platelet-mediated angiogenesis, induced by tumor cell implantation or hindlimb ischemia, through differential release of proangiogenic and antiangiogenic factors. However, only C3G deletion resulted in a higher recruitment of hemangiocytes from the bone marrow. In addition, C3G null expression enhanced thrombopoietin (TPO)-induced platelet production, associated with reduced TPO plasma levels. Moreover, after 5-fluorouracil-induced platelet depletion and rebound, C3G knockout mice showed a defective return to homeostatic platelet levels, indicating impaired platelet turnover. Mechanistically, C3G promotes c-Mpl ubiquitination by inducing Src-mediated c-Cbl phosphorylation and participates in c-Mpl degradation via the proteasome and lysosome systems, affecting TPO internalization. We also unveiled a positive role of platelet C3G in tumor cell-induced platelet aggregation, which facilitated metastatic cell homing and adhesion. Overall, these findings revealed that C3G plays a crucial role in platelet-mediated angiogenesis and metastasis, as well as in platelet level modulation in response to pathogenic stimuli.

BACKGROUND: Liver resection is a curative treatment for hepatocellular carcinoma (HCC) and an alternative to liver transplantation (LT). However, post-liver resection recurrence rates remain high. This study aimed to determine whether liver stiffness measurement (LSM) correlated with recurrence and to propose a method for predicting HCC recurrence exclusively using pre-liver resection criteria. METHODS: This retrospective monocentric study included patients who had undergone LR liver resection for HCC between 2015 and 2018 and who had (1) preoperative alpha-fetoprotein scores indicating initial transplant viability and (2) available preoperative LSM data. We developed a predictive score for recurrence over time using Cox univariate regression and multivariate analysis with a combination plot before selecting the optimal thresholds (receiver operating characteristic curves + Youden test). RESULTS: Sixty-six patients were included. After an average follow-up of 40 months, the recurrence rate was 45% (n = 30). Three-year overall survival was 88%. Four preoperative variables significantly impacted the time to recurrence: age ≥70 years, LSM ≥11 kPa, international normalized ratio (INR) ≥1.2, and maximum HCC diameter ≥3 cm. By assigning 1 point per positive item, patients with a score <2 (n = 22) demonstrated greater mean overall survival (69.7 vs 54.8 months, P = .02) and disease-free survival (52.2 vs 34.7 months, P = .02) than those with a score ≥2. Patients experiencing early recurrence (<1 year) presented a significantly higher preoperative LSM (P = .06). CONCLUSION: We identified a simple preoperative score predictive of early hepatocellular carcinoma recurrence after liver resection, highlighting the role of liver stiffness. This score could help physicians select patients and make decisions concerning perioperative medical treatment.

La salud mental de los estudiantes de medicina alrededor del mundo ha sido afectada por la presencia de ansiedad y depresión. Según la Organización Mundial de la Salud, más de 260 millones de personas padecen ansiedad y aproximadamente otros 300 millones depresión. Objetivo: Determinar la asociación entre ansiedad y depresión en estudiantes de medicina de la Universidad Ricardo Palma durante el ciclo académico 2021-II. Materiales y Métodos: Estudio observacional, analítico, transversal y cuantitativo. De una población de 2046 estudiantes de medicina del primer al séptimo año de estudios, el tamaño de la muestra obtenida fue de 110 estudiantes. Los datos obtenidos se recolectaron por medio de Google Forms. Resultados: De los estudiantes de medicina el 85.46% manifestó algún grado de ansiedad y 50% manifestó algún grado de depresión. Estas patologías se presentaron con mayor frecuencia en el sexo femenino, nivel socioeconómico bajo y en aquellos que llevaban cursos de clínica. Se encontró asociación estadísticamente significativa entre ansiedad y depresión (p: 0.038), y entre nivel socioeconómico bajo y depresión (p: 0.004). No se encontró asociación estadísticamente significativa entre sexo femenino y depresión, ni entre cursos de pre clínica y depresión. Conclusiones: En los estudiantes de medicina de la Universidad Ricardo Palma se encontró prevalencias de ansiedad y depresión elevadas. Además, se evidencio una asociación estadísticamente significativa entre ansiedad y depresión (p: 0.038). Por ello, se recomienda implementar un acompañamiento psicológico a lo largo de la carrera universitaria de medicina.


The mental health of medical students around the world has been affected by the presence of anxiety and depression. According to the World Health Organization, more than 260 million people suffer from anxiety and approximately 300 million people suffer from depression. Objective: To determine the association between anxiety and depression in medical students at Ricardo Palma University during the 2021-II academic year. Materials and Methods: Observational, analytical, cross-sectional and quantitative study. From a population of 2046 medical students from the first to the seventh year of studies, the size of the sample obtained was 110 students. The data obtained was collected through Google Forms. Results: Of the medical students, 85.46% manifested some degree of anxiety and 50% manifested some degree of depression. These pathologies occurred more frequently in the female sex, low socioeconomic level and in those who had clinical courses. A statistically significant association was found between anxiety and depression (p: 0.038), and between low socioeconomic status and depression (p: 0.004). No statistically significant association was found between female sex and depression, nor between pre-clinical courses and depression. Conclusions: High prevalences of anxiety and depression were found in medical students at the Ricardo Palma University. In addition, a statistically significant association between anxiety and depression was evidenced (p: 0.038). For this reason, it is recommended to implement a psychological accompaniment throughout the medical university career.

Contexto Los pacientes en hemodiálisis son una población susceptible para la enfermedad causada por el SARS-CoV-2, enfermedad por el coronavirus 2019 (COVID-19), siendo el diagnóstico temprano una necesidad urgente por considerarse una población de riesgo por presentar cuadros severos y graves. Objetivo Estimar si la anosmia de inicio súbito permite una detección temprana de la infección por COVID-19, en pacientes de hemodiálisis sin síntomas clásicos (que no presentaron malestar general, síntomas respiratorios o gastrointestinales al inicio de la enfermedad). Metodología Se realizó un estudio de prueba diagnóstica durante los meses de septiembre y octubre del 2020 en tres unidades de hemodiálisis en Colombia. Fueron incluidos pacientes adultos, sin síntomas clásicos para COVID-19. Se excluyó a los pacientes con infección previa por COVID-19 o con antecedentes de alteración del olfato. Se recolectaron datos demográficos, clínicos y de laboratorio, previa firma de consentimiento informado avalado por el comité de ética (CEI-487). La anosmia se exploró con una prueba subjetiva para detección de olores antes del inicio de la hemodiálisis. Resultados Se incluyeron 587 pacientes. La prevalencia de anosmia fue de 0,85 % (cinco pacientes) y una incidencia de COVID-19 de 1,19 % (siete pacientes). No hubo diferencia estadística al comparar las variables demográficas, clínicas y de laboratorio entre pacientes con y sin anosmia. La presencia de anosmia tuvo una sensibilidad y un valor predictivo positivo del 0 %, pero una especificidad del 99,14 % y un valor predictivo negativo del 98,8 %. La exactitud fue del 97,9 %. Conclusiones La prevalencia de anosmia en nuestra población de hemodiálisis fue baja. Se debería indagar en el triaje cualquier síntoma (incluida la alteración del sentido del olfato) como estrategia para detectar de forma temprana casos individuales de COVID-19 o un brote en las unidades de hemodiálisis.


Background Hemodialysis patients are susceptible population for COVID-19 the disease caused by SARS-CoV-2, with early diagnosis being an urgent and needed because they are considered a population with high risk for severe and serious conditions. Purpose The objective of our study was to estimate whether sudden-onset anosmia allow to detect COVID-19 infection early in hemodialysis patients without classic symptoms (who did not present malaise, respiratory or gastrointestinal symptoms at the beginning of the disease). Methodology A diagnostic test study was accomplished during the months of September and October 2020 in three hemodialysis units in Colombia. Adult patients without classic symptoms for COVID-19 were included. Patients with previous COVID-19 infection or with a history of smell alteration were excluded. Demographic, clinical, and laboratory data were collected, prior signature of informed consent endorsed by the ethics committee. Anosmia was evaluated with objective test for odor detection before the start of hemodialysis session. Results 587 patients were included. Anosmia prevalence was 0.85% (5 patients) and the incidence of COVID-19 was 1.19% (7 patients). There was no statistically difference in demographic, clinical, and laboratory comparison between patients with and without anosmia. The presence of anosmia had a sensitivity and a positive predictive value of 0%, but a specificity of 99.14% and a negative predictive value of 98.8%. The accuracy was 97.9%. Conclusions The prevalence of anosmia in our hemodialysis population was low. Any symptoms (including altered sense of smell) should be investigated in TRIAGE as a strategy to detect early individual cases of COVID-19 or an outbreak in hemodialysis units.

RESUMO

The aim of this study is to explore the effects of a residential multimodal treatment intervention for an addict population. We gathered participants from the "Programa Base" (n = 166) of the Solidarity and Reinsertion Foundation of Murcia, and assessed the various problematic areas with the EuropASI at baseline level, 6 months and 12 months of treatment. We found improved outcomes in every area except for Legal Status. In addition, we found differences between male and female participants in their baseline evaluation, as well as between completers and non-completers. In conclusion, this data shows us some changes which occurred in individuals with problematic drug use during treatment, going further into the complex social reality which causes great suffering and damage to people and their families.

The interest of consumers to acquire Tequila has caused an increase in its sales. As demand increases, the Tequila industry must obtain its raw material at a constant rate and agave farmers must be prepared to satisfy this supply chain. Because of this, modernization of the strategies used to ensure a planned, scheduled, timely, and predictable production will allow farmers to maintain the current demand for Tequila. This has been evidenced in official historical records from 1999 to 2020 where there is a fluctuation in the price of agave due to supply and demand. Given this scenario, this research shows the development of a multivariable predictive mathematical model that will permit the agave−Tequila production chain to work based on a smart implementation of planned actions to guarantee the agave supply to the Tequila industry. The proposed model has a goodness of fit (R = 0.8676; R¯2 = 0.8609; F(1,20) = 131.01 > F0.01 (1,20) = 8.10) and demonstrates the impact on agave prices is due to several factors: Tequila exports (α = 0.50) > agave plants harvested "jima" (α = 0.44) > dollar exchange (α = 0.43) > Tequila production (α = 0.06) > annual accumulated precipitation (α = 0.05). Nevertheless, the price forecast can be influenced by climate change or economic crises that affect the supply chain. In conclusion, a prediction of agave price stabilization for five years is shown where authorized producers can evaluate future scenarios so that the agave supply chain can be guaranteed for Tequila production, facilitating the decision making regarding its raw material.
