Introdução: a hidradenite supurativa é uma doença inflamatória crônica e recidivante. Altas taxas de recorrência podem persistir mesmo após ampla excisão cirúrgica local. Objetivo: avaliar os principais fatores de risco para complicações pós-operatórias da hidradenite supurativa. Metodologia: foram utilizadas referências em artigos científicos nas bases de dados Google, PubMed, LILACS e SciELO nas línguas portuguesa e inglesa, no período de 2000 a 2021. Resultados: os principais fatores que influenciam os resultados pós-operatórios estão relacionados a técnica cirúrgica empregada, gravidade da doença, comorbidades dos pacientes e terapia clínica adjuvante


Introduction: Hidradenitis Suppurativa is a chronic inflamatory and relapsing disease. High levels of recurrence can persist even after wide local excision. Objective: Identify the main risk factors for postoperative complications of Hidradentis Suppurativa. Methods: We included journal articles with data from Google, Pubmed, Lilacs, and Scielo, in Portuguese and English, from 2000 to 2021. Results: The main risk factors that influence postoperative outcomes are related to surgical technique, disease severity, comorbidities, and adjuvant clinical therapy

Objetivo:Analisar as relações entre a qualidade de vida (QV) de pessoas com estomia com indicadores sociodemográficos, clínicos, de estilo de vida, de saneamento e moradia. Métodos: Estudo transversal com amostra de 106 indivíduos com estomia entrevistados de maio a dezembro de 2019. Utilizou-se um questionário sociodemográfico e clínico e o City of Hope - Quality of Life - Ostomy Questionnary. Resultados: O bem-estar espiritual (7,71±1,09) foi o domínio com melhor performance. A QV não diferiu entre homens e mulheres (p = 0,372), porém esteve associada à escolaridade (< 0,001) e renda familiar (p = 0,025), ao diabetes (p = 0,008) e etilismo (p = 0,044), às condições da água para consumo (p < 0,001), ao destino do lixo (p = 0,021), em ter energia elétrica (p = 0,034), ao tipo de moradia (p = 0,026) e ao número de cômodos (p = 0,023) e tipo de cobertura da habitação (p = 0,021). Conclusão: Piores indicadores socioeconômicos, de saneamento básico e moradia, parecem impactar negativamente a QV de pessoas com estomias.


Objectives:To analyze the relationships between the quality of life (QOL) of people with stoma with sociodemographic, clinical, lifestyle, sanitation, and housing indicators. Method: Cross-sectional study with a sample of 106 individuals with stoma interviewed from May to December 2019. A sociodemographic and clinical questionnaire and the City of Hope Quality of Life Ostomy Questionnaire were used. Results: Spiritual well-being (7.71 ± 1.09) was the best performing domain. Quality of life did not differ between men and women (p = 0.372), but was associated with education (< 0.001) and family income (p = 0.025), diabetes (p = 0.008) and alcoholism (p = 0.044), drinking water conditions (p < 0.001), garbage disposal (p = 0.021), having electricity (p = 0.034), housing type (p = 0.026), number of rooms (p = 0.023), and housing coverage (p = 0.021). Conclusion: worse socioeconomic, sanitation, and housing indicators appear to negatively impact the QOL of people with stomata


Objetivo:Analizar las relaciones entre la calidad de vida (CV) de las personas con estoma con indicadores sociodemográficos, clínicos, de estilo de vida, saneamiento y vivienda. Métodos: Estudio transversal con una muestra de 106 ostomizados, entrevistados de mayo a noviembre de 2019. Se utilizó un cuestionario sociodemográfico y clínico y el Cuestionario City of Hope - Quality of Life - Ostomy. Resultados: El bienestar espiritual (7,71 ± 1,09) fue el dominio con mejor desempeño. La CV no difirió entre hombres y mujeres (p = 0,372), pero se asoció con la educación (< 0,001) y los ingresos familiares(p = 0,025), la diabetes (p = 0,008) y el alcoholismo (p = 0,044), con las condiciones de agua potable (p <0,001), el destino de la basura (p = 0,021), la disponibilidad de electricidad (p = 0,034), el tipo de vivienda (p = 0,026) y el número de habitaciones (p = 0,023) y tipo de cobertura de vivienda (p = 0,021). Conclusión: Peores indicadores socioeconómicos, saneamiento básico, vivienda, parecen impactar negativamente la calidad de vida de las personas con ostomía.

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory disease of widely varying presentation, which determines functional and psychological impairment, with a high negative impact on patients' quality of life. Therefore, knowing the patient's perception of their health status is of fundamental importance for understanding the real impact of PsA. Given this context, the European League Against Rheumatism (EULAR) recently developed the Psoriatic Arthritis Impact of Disease (PsAID) - instrument to specifically assess the impact of PsA for the patient. OBJECTIVE: Validate the brazilian portuguese version of PsAID-12 (Psoriatic Arthritis Impact of Disease) and to verify its interpretability in clinical practice, through its relation with measures of psoriatic arthritis activity. METHODS: A multicenter cross-sectional study, which recruited 160 patients, who met the Classification criteria for Psoriatic Arthritis (CASPAR), in six Brazilian centers of rheumatology. Reliability was assessed by Cronbach's alpha coefficient and by the intraclass correlation coefficient (ICC). The construct validity was evaluated by exploratory factorial analysis and also by Spearman correlation with other PROMs and measures of disease activity evaluation. RESULTS: Of the total number of participants, 50% were female, with a mean age (SD) of 54.0 ± 11.2 years; 68% had only peripheral arthritis and 32% had pure or mixed axial involvement. The majority (67.7%) of the patients were using biological treatment. The reliability of internal consistency (alpha-Cronbach = 0.93) and test-retest (ICC = 0.996) were good. Factor analysis revealed two factors, named physical and psychosocial, which included the skin evaluation item. PsAID-12 correlated significantly with other PROMs, demonstrating good construct validity. PsAID-12 was also significantly associated with the disease activity assessment instruments (DAS28-ESR, ASDAS, and BASDAI) and the MDA status: "Minimum Disease Activity". Fibromyalgia did not significantly affect the final PsAID-12 score. CONCLUSION: The brazilian version of PsAID-12 has been shown to be a reliable and valid measure of the impact of the disease in patients with psoriatic arthritis. Moreover, it associated significantly with the scores of disease activity assessment.

RESUMO Objetivo: analisar as prescrições de medicamentos potencialmente perigosos e identificar as práticas adotadas na dispensação em hospital universitário público de alta complexidade, propondo estratégias para prevenção de eventos adversos. Métodos: estudo transversal realizado com 566 prescrições, em três turnos, no período de abril a dezembro de 2016. A identificação de práticas para dispensação foi por observação direta com aplicação de checklist baseado no protocolo do Ministério da Saúde (MS) /Agência Nacional de Vigilância Sanitária (ANVISA). Utilizou-se teste não paramétrico do qui-quadrado de independência para avaliar a associação entre prescrições com e sem medicamentos potencialmente perigosos e unidades de internação. Resultados: mais da metade das prescrições (56,6%) continha dois ou mais medicamentos potencialmente perigosos e quase todos injetáveis (95,4%), principalmente analgésicos opioides (31,2%), glicose 50% (24,7%) e insulina NPH e regular (24,3%). A taxa dos prescritos correspondeu a 18,2%. As principais práticas que representaram riscos foram: distribuição coletiva de cloreto de potássio e insulina; falta de etiquetas de alerta; dupla conferência inexistente; uso de fontes de interrupção/distração em 43,9% das prescrições atendidas. Conclusão: apesar de serem frequentes em mais da metade das prescrições, as práticas adotadas na dispensação de medicamentos potencialmente perigosos foram insuficientes para uma dispensação segura, podendo comprometer a administração desses medicamentos e causar danos aos pacientes. É importante a implementação do protocolo do MS/ANVISA para prevenção de erros de medicação, em consonância com o terceiro desafio global da Organização Mundial da Saúde (OMS).

Resultados: más de la mitad de las prescripciones (56.6%) contenían dos o más fármacos potencialmente peligrosos , la mayoría inyectables (95.4%), principalmente analgésicos opioides (31.2%), glucosa 50% (24.7%) e insulina NPH y regular (24.3%). La tasa de prescripción correspondió al 18,2%. Las principales prácticas que representaban riesgos fueron: distribución colectiva de cloruro de potasio e insulina; falta de etiquetas de advertencia; falta de doble verificación; uso de fuentes de interrupción / distracción en el 43,9% de las prescripciones atendidas. Conclusión: a pesar de ser frecuente en más de la mitad de las prescripciones, las prácticas adoptadas en la dispensación de fármacos potencialmente peligrosos fueron insuficientes para la dispensación segura, lo que podría comprometer la administración de estos medicamentos y perjudicar a los pacientes. Es importante implementar el protocolo MS/ANVISA para prevenir errores de medicación, en línea con el tercer desafío global de la Organización Mundial de la Salud (OMS).


ABSTRACT Objective: to analyze the prescriptions of high alert medications and to identify the practices adopted in the dispensing in a high complexity public university hospital, proposing strategies to prevent adverse events. Methods: a cross-sectional study carried out with 566 prescriptions, in three shifts, from April to December 2016. The identification of practices for dispensing was by direct observation with the application of a checklist based on the protocol of the Ministry of Health (MH)/National Health Surveillance Agency(ANVISA) (Ministério da Saúde/Agência Nacional de Vigilância Sanitária). A non-parametric chi-square test of independence was used to assess the association between prescriptions with and without high alert medications and inpatient units. Results: more than half of the prescriptions (56.6%) contained two or more high alert medications and almost all were injectable (95.4%), mainly opioid analgesics (31.2%), glucose 50% (24.7%), and NPH and regular insulin (24.3%). The prescription rate corresponded to 18.2%. The main practices that represented risks were the following: collective distribution of potassium chloride and insulin; lack of warning labels; non-existent double check; presence of interruption/distraction sources in 43.9% of the prescriptions met. Conclusion: despite being frequent in more than half of the prescriptions, the practices adopted in the dispensing of high alert medications were insufficient for a safe dispensing, which could compromise the administration of these drugs and cause harm to the patients. It is important to implement the MH/ANVISA protocol for preventing medication errors, in consonance with the third global challenge of the World Health Organization (WHO).

Dermatomyositis is an inflammatory autoimmune disorder that affects the skin, muscles, and blood vessels. The prevailing clinical manifestations are intense erythema, typically around the eyes and dorsum of the hands, accompanied by proximal muscle weakness. There is one clinical form, called amyopathic dermatomyositis, which lacks muscular symptoms and laboratory evidence of muscle damage.

THE AUTHORS PRESENT TWO CASES OF SYPHILIS: one mimicking reactive arthritis and the other Mucha-Habermann disease. Both reports illustrate syphilis as 'the great imitator', a description given by Sir William Osler, and call attention to the strong need for awareness among physicians of all specialties, especially the younger ones, who are not used to seeing this increasingly prevalent disease, as it once was in the past.

The work orchestrated by these peptides is essential for the communication between resident inflammatory cells (keratinocytes and endothelial cells) and infiltrating cells (neutrophils, lymphocytes, Langerhans cells). This is a complex network due to redundancy, synergism and, sometimes, the antagonism of cytokines, which prevents full understanding of the pathogenesis of the disease. Currently, it seems premature to try to establish a main actor, but TNFalpha participates in all stages of psoriatic plaque development, as we shall see.

A introdução de novos métodos moleculares de investigação ampliou muito o conhecimento sobre o papel das citocinas em diversas doenças, entre elas a psoríase. O trabalho orquestrado desses polipeptídeos é fundamental na comunicação entre as células inflamatórias residentes (queratinócitos e células endoteliais) e infiltrantes (neutrófilos, linfócitos, células de Langerhans). Trata-se de uma rede complexa devido à redundância, ao sinergismo e, por vezes, ao antagonismo das citocinas, o que dificulta a compreensão da fisiopatogenia da doença a partir de um mecanismo linear. No momento atual, parece precoce tentar estabelecer um regente, mas o TNF-alfa se destaca em todos os passos do desenvolvimento da placa psoriásica, como veremos a seguir.

Dowling-Degos disease (DDD) is a rare genetic disease of the skin (reticulate pigmented anomaly), clinically characterized by flexural brown pigmented reticulate macules, comedo-like papules on the back, neck and pitted perioral or facial scars. We present the case of a 51 year-old man with macrocomedo-like lesions, pitted scars, cysts, hyperpigmented macules in his back, chest, axillae, neck, groin and face. The patient reported having two children, three brothers and a father with a similar condition. The histopathology of the skin biopsies was very characteristic of Dowling-Degos disease, showing dilated follicular, fingerlike projections called rete ridges (dermal pegs), with thinning of the suprapapillary plates, resulting in an "antler-like" pattern and increased pigmentation of the basal layer.

A dermatite seborreica é uma doença eritêmato-escamativa de caráter crônico-recidivante que acomete entre 1 e 3 por cento da população geral dos Estados Unidos. Possui dois picos de incidência - o primeiro, durante os três primeiros meses de vida, e o segundo, a partir da puberdade, atingindo seu ápice entre os 40 e 60 anos de idade. Os indivíduos HIV positivos têm maior prevalência da doença, que apresenta maior intensidade e tendência à refratariedade ao tratamento. Doenças neurológicas e outras doenças crônicas também estão associadas ao desenvolvimento da dermatite seborreica. Como mecanismo fisiopatogênico, reconhece-se que o fungo Malassezia sp., presente na pele de indivíduos suscetíveis, leve a uma irritação não-imunogênica a partir da produção de metabólitos à base de ácidos graxos insaturados deixados na superfície cutânea. Este artigo faz uma revisão da literatura sobre dermatite seborreica, com ênfase nos aspectos imunogenéticos, formas clínicas e tratamento.


Seborrheic dermatitis is a chronic relapsing erythematous scaly skin disease, the prevalence of which is around 1 to 3 percent of the general population in the United States. It has two incidence peaks, the first in the first three months of life and the second beginning at puberty and reaching its apex at 40 to 60 years of age. The prevalence of seborrheic dermatitis is higher in HIV-positive individuals and the condition tends to be more intense and refractory to treatment in these patients. Neurological disorders and other chronic diseases are also associated with the onset of seborrheic dermatitis. The currently accepted theory on the pathogenesis of this disease advocates that yeast of Malassezia spp., present on the skin surface of susceptible individuals, leads to a non-immunogenic irritation due to the production of unsaturated fatty acids deposited on the skin surface. This article provides a review of the literature on seborrheic dermatitis, focusing on immunogenetics, the clinical forms of the disease and its treatment.

A doença de Dowling-Degos é uma genodermatose rara que consiste numa desordem pigmentar reticulada. Caracteriza-se pela presença de máculas hiperpigmentadas nas regiões flexurais com distribuição em rede; lesões tipo comedão no dorso e na região cervical; e cicatrizes cribriformes na face, particularmente periorais. Apresentamos um caso de um paciente de 51 anos, masculino, com lesões tipo macrocomedões, cicatrizes cribriformes, cistos e máculas hipercrômicas no dorso, tórax anterior, axilas, pescoço, região genital e face. Relatava ter dois filhos, três irmãos e o pai com quadro semelhante. As biópsias de pele foram características da doença de Dowling-Degos, mostrando dilatação folicular, epiderme digitiforme, com áreas de aspecto de "chifre de veado" e focos de hiperpigmentação da camada basal.

Paraneoplasias are frequently the first sign of a subjacent malignant tumor. Although relatively rare, they need to be recognized by dermatologists to make an early diagnosis and improve the prognosis related especially to the neoplasia. This contribution presents the morphologic aspects and the differential diagnosis of the main paraneoplasias, which include acanthosis nigricans, tripe palms, Leser-Trélat sign, acquired ichthyosis, acquired hypertrichosis lanuginosa, pityriasis rotunda, erythema gyratum repens, palmo-plantar keratoderma, Bazex syndrome, and dermatomyositis, hoping to contribute to the familiarity of dermatologists with the identification and early diagnosis of this group.

Because the older group of the population is becoming more numerous, we see a high prevalence in drug adverse reactions among the elderly. Polypharmacy, which is the use of five or more medications, is one reason why this group has a greater risk of adverse drug reactions. Cutaneous adverse reactions to drugs are not always life threatening, but they can be an important factor for a poor quality of life among older patients. The potential benefits of appropriately prescribed medications are unquestionable, but the possibility of an adverse reaction must be recognized and prevented in older people so they can have a better quality of life.

Seborrheic dermatitis is a chronic relapsing erythematous scaly skin disease, the prevalence of which is around 1 to 3% of the general population in the United States. It has two incidence peaks, the first in the first three months of life and the second beginning at puberty and reaching its apex at 40 to 60 years of age. The prevalence of seborrheic dermatitis is higher in HIV-positive individuals and the condition tends to be more intense and refractory to treatment in these patients. Neurological disorders and other chronic diseases are also associated with the onset of seborrheic dermatitis. The currently accepted theory on the pathogenesis of this disease advocates that yeast of Malassezia spp., present on the skin surface of susceptible individuals, leads to a non-immunogenic irritation due to the production of unsaturated fatty acids deposited on the skin surface. This article provides a review of the literature on seborrheic dermatitis, focusing on immunogenetics, the clinical forms of the disease and its treatment.

Although many classifications include oral lesions of lichen planus in the category of a premalignant condition, there is still much discussion about whether the mucous membranes lesions should be characterized as an intrinsically premalignant condition or merely as a facilitator of the action of a carcinogenic factor. The possibility that an epidermoid carcinoma can emerge at the site of lichen planus lesions, mainly in mucous membranes, has been shown; however, several published cases omit information about other potential risk factors. This prevents a complete analysis of the triggering relationship between lichen planus and squamous cell carcinoma. This contribution reviews the literature on this subject. The question of whether oral or vulval lichen planus, or both, are premalignant conditions will only be answered after prospective studies with large samples and extensive follow-up are performed, taking into consideration the great variety of risk factors involved, together with the establishment of a consensus in relation to the points still without agreement.

Recent studies have suggested that mast cell numbers are increased in the skin of patients with cutaneous mucinosis and that these cells may have an important role in angiogenesis and production of mucin. Then, skin biopsies from 30 patients with cutaneous mucinosis (papular mucinosis, focal mucinosis, and mucinosis associated with lupus erythematosus) and from 10 healthy subjects were analyzed. Mast cells and blood vessels were immunolabeled with anti-tryptase and anti-CD34 antibodies, respectively, and then quantified stereologically. Counting was performed in papillary and reticular dermis. An increase in the number of mast cells was observed in the skin of patients with cutaneous mucinosis compared with the control group. Only minimal differences were observed in vessel stereology. There was no correlation between the increase in the number of mast cells and the number of blood vessels in the patients studied. There was no significant difference in the numbers of mast cells or blood vessels between the 3 subgroups of cutaneous mucinosis. Although many clinical forms of mucinosis have been described, neither mast cell number nor vessel distribution seems to distinguish the 3 different forms studied here.

Acne is a chronic disease of the pilosebaceous follicle that affects mainly adolescents. It causes polymorph cutaneous lesions that may leave scars after regression. It is characterized by alternation of periods of exacerbation and stability. Spontaneous regression usually occurs after age 20, but some patients may continue suffering during adult life. Diagnosis is clinical and based on the patient's age at the time that the lesions first appear, and on its polymorphism and type of lesions and location. Some patients also need a hormonal and, sometimes, psychological evaluation. The right treatment for the right patient is key to treating acne safely.

Granulocytic sarcoma is an extramedullary tumor of immature cells of granulocytic series, generally associated to acute myelogenous leukemia. The skin is one of the most commonly affected sites. Granulocytic sarcoma can complicate myelodysplastic syndromes and is considered a sign of poor prognosis. They are often misdiagnosed with non-Hodgkin lymphoma of the lymphoblastic type, Burkitt lymphoma and large cell lymphoma. In children, the differential diagnoses also include small, round cell tumors. It is important to diagnose these lesions early because they can precede peripheral blood and bone marrow transformation to acute myelogenous leukemia. We report a case of an elderly patient with myelodysplastic syndrome who developed multiple cutaneous granulocytic sarcoma lesions and discuss prognostic and treatment implications.
