RESUMO

The optimal enhancement of broadband optical pulses in a passive resonator requires a seeding pulse train with a specific carrier-envelope-offset frequency. Here, we control the phase of the cavity mirrors to tune the offset frequency for which a given comb is optimally enhanced. This enables the enhancement of a zero-offset-frequency train of sub-30-fs pulses to multi-kW average powers. The combination of pulse duration, power, and zero phase slip constitutes a crucial step toward the generation of attosecond pulses at multi-10-MHz repetition rates. In addition, this control affords the enhancement of pulses generated by difference-frequency mixing, e.g., for mid-infrared spectroscopy.

RESUMO

We combine high-finesse optical resonators and spatial-spectral interferometry to a highly phase-sensitive investigation technique for nonlinear light-matter interactions. We experimentally validate an ab initio model for the nonlinear response of a resonator housing a gas target, permitting the global optimization of intracavity conversion processes like high-order harmonic generation. We predict the feasibility of driving intracavity high-order harmonic generation far beyond intensity limitations observed in state-of-the-art systems by exploiting the intracavity nonlinearity to compress the pulses in time.

Thermal lensing poses a serious challenge for the power scaling of enhancement cavities, in particular when these contain transmissive elements. We demonstrate the compensation of the lensing induced by thermal deformations of the cavity mirrors with the thermal lensing in a thin Brewster plate. Using forced convection to fine-tune the lensing in the plate, we achieve average powers of up to 160 kW for 250-MHz-repetition-rate picosecond pulses with a power-independent mode size. Furthermore, we show that the susceptibility of the cavity mode size to thermal lensing allows highly sensitive absorption measurements.

We investigate power scaling of ultrashort-pulse enhancement cavities. We propose a model for the sensitivity of a cavity design to thermal deformations of the mirrors due to the high circulating powers. Using this model and optimized cavity mirrors, we demonstrate 400 kW of average power with 250 fs pulses and 670 kW with 10 ps pulses at a central wavelength of 1040 nm and a repetition rate of 250 MHz. These results represent an average power improvement of one order of magnitude compared to state-of-the-art systems with similar pulse durations and will thus benefit numerous applications such as the further scaling of tabletop sources of hard x rays (via Thomson scattering of relativistic electrons) and of soft x rays (via high harmonic generation).

Lung transplantation is a therapeutic option for patients with end-stage lung diseases. Selection of candidates requires careful consideration of the disease-specific indications and contraindications for transplantation. Advances have been made in candidate selection via the ability to prognosticate outcomes of various lung diseases and through the implementation of the lung allocation score (LAS) with specific consideration of the degree of urgency and good postoperative survival rate, after neglecting the waiting time. This system has resulted in decreased mortality on the waiting list for lung transplantation. The availability of donor organs can possibly be increased by implementation of ex vivo lung perfusion as an alternative to conventional organ preservation. Risk factors for poor outcomes post-lung transplantation have been identified and understanding of the physiological, cellular and molecular mechanisms responsible for lung and airway damage has been extensively expanded. Primary graft dysfunction, infectious diseases, acute rejection, antibody-mediated rejection, lymphocytic bronchiolitis, obliterative bronchiolitis, restrictive allograft syndrome, and chronic lung allograft dysfunction are well defined complications and continue to be common causes of morbidity and mortality. This article provides a comprehensive update on these topics for the non-transplantation clinician.

RESUMO

The process of high harmonic generation allows for coherent transfer of infrared laser light to the extreme ultraviolet spectral range opening a variety of applications. The low conversion efficiency of this process calls for optimization or higher repetition rate intense ultrashort pulse lasers. Here we present state-of-the-art fiber laser systems for the generation of high harmonics up to 1 MHz repetition rate. We perform measurements of the average power with a calibrated spectrometer and achieved µW harmonics between 45 nm and 61 nm (H23-H17) at a repetition rate of 50 kHz. Additionally, we show the potential for few-cycle pulses at high average power and repetition rate that may enable water-window harmonics at unprecedented repetition rate.

We present simple and compact (1.5m x 0.5m footprint) post-compression of a state-of-the-art fiber chirped pulse amplification system. By using two stage nonlinear compression in noble gas filled hollow core fibers we shorten 1 mJ, 480 fs, 50 kHz pulses. The first stage is a 53 cm long, 200 µm inner diameter fiber filled with xenon with subsequent compression in a chirped mirror compressor. A 20 cm, 200 µm inner diameter fiber filled with argon further broadens the spectrum in a second stage and compression is achieved with another set of chirped mirrors. The average power is 24.5 W/19 W after the first/second stage, respectively. Compression to 35 fs is achieved. Numerical simulations, agreeing well with experimental data, yield a peak power of 5.7 GW at a pulse energy of 380 µJ making this an interesting source for high harmonic generation at high repetition rate and average power.

A renal biopsy demonstrated the presence of both glomerulocystic disease (GCD) and lupus glomerulonephropathy in a patient who was admitted for an unrelated disease and was found to have proteinuria and renal failure. Of 12 adult patients with GCD reported in the literature, some form of glomerulonephropathy was demonstrated in 3. One of the 3 had the membranous type of lupus glomerulonephropathy. When family members of previously reported cases of GCD have been examined, "occult" cases of GCD were frequently identified. GCD is probably a much more frequent disease than the relatively small number of published cases would lead one to believe. In adults GCD is often asymptomatic and will only become symptomatic if another renal disease, such as lupus glomerulonephropathy, is superimposed.

RESUMO

A mass in the left breast of a 32-year-old woman was first diagnosed as sarcoma with rhabdomyosarcomatous differentiation. Subsequent studies demonstrated a malignant epithelial component to be present, changing the diagnosis to carcinosarcoma. This course of events supports the concept that many, if not all, sarcomas of the breast would be mixed tumors with a malignant epithelial component, if search for the epithelial component was extensive. Carcinosarcomas with rhabdomyosarcomatous differentiation in the breast are rare, but like sarcomas elsewhere, they do not metastasize to regional lymph nodes, but disseminate hematogenously, primarily to the lungs.

RESUMO

Membrane-bound cytoplasmic crystals were found in the intrafusal fibers of a muscle spindle from a patient with neurogenic atrophy. The crystals have a periodicity of 10 nm and an intersecting axis angle of approximately 80 degrees. This makes the crystals similar to those described in alveolar soft part sarcoma (ASPS). Because the crystals in ASPS may not be quite as specific as previously believed, and because similar crystals have been described in various other neoplasms, the present findings should not be taken as evidence for a muscle spindle derivation for ASPS.

Biliary tract obstruction in a 30-year-old man was found to be caused by a malignant melanoma in the common bile duct. Melanin pigment was demonstrated by immunohistochemistry and electron microscopy. Extensive search for a primary malignant melanoma elsewhere was unsuccessful. No pigmented lesions had been removed previously. There were junctional changes in the mucosa of the common bile duct close to the tumor. The malignant melanoma in the common bile duct therefore is considered to be primary. Only one other case of primary malignant melanoma in the common bile duct has been described in the literature, whereas metastases to the major bile ducts in one autopsy study of malignant melanoma in the more common locations were found with a frequency of 6 per cent.

A recurrent soft tissue tumor from the groin in a 37-year-old male is described. The tumor cells contain many leptomeric structures, both fibrils and complexes, but none of the other characteristic features of striated muscle: alternating parallel thick and thin myofilaments with Z-line material. A review of one rhabdomyoma and six rhabdomyosarcomas demonstrates that leptomeric structures are present in large amounts in the rhabdomyoma but are very infrequent in only three of the six rhabdomyosarcomas. A careful search for leptomeric structures is indicated when the ultrastructural diagnosis of either rhabdomyoma or rhabdomyosarcoma is being considered.
