Background: In older people, a notable research gap exists regarding the intricate dynamics between frailty, seasonal sensitivity, and health-related quality of life (HRQoL). This study aimed to determine the association between frailty, seasonal sensitivity, and HRQoL in older people from high southern latitudes. Methods: A cross-sectional observational study was conducted. Frailty, seasonal sensitivity, and HRQoL measurements were self-reported by participants through questionnaires. A total of 118 older people were recruited from a local community. The participants were selected through intentional non-probabilistic sampling. Results: The adjusted models showed a trend where lower education was associated with a higher risk of frailty (BF = 0.218). For frailty and HRQoL, we observed a trend suggesting that HRQoL decreases with increasing severity of frailty (BF = 1.76). In addition, we observed a linear effect based on the severity of seasonal sensitivity, meaning that older people with higher perceived severity report a proportional decrease in HRQoL (BF = 6.66). Conclusion: Sociodemographic factors, such as lower education levels, have increased the risk of frailty. At the same time, frailty and seasonal sensitivity perceived severity were associated with a lower HRQoL in older people.

B cells generate antibodies that provide protection from infection, but also cause pathology in autoimmune and allergic conditions. Antigen-specific B cells can be detected by binding their surface antibody receptors with native antigens conjugated to fluorescent probes, a technique that has revealed substantial insight into B cell activation and function. This protocol describes the process of generating fluorescent antigen tetramer probes and delineates a process of enriching large samples based on antigen-specificity for high-resolution analyses of the antigen-specific B cell repertoire. Enrichment of tetramer-binding cells allows for detection of antigen-specific B cells as rare as 1 in 100 million cells, providing sufficient resolution to study naive B cells and IgE-expressing cells by flow cytometry. The generation of antigen tetramers involves antigen biotinylation, assessment of biotin:antigen ratio for optimal tetramer loading and polymerization around a streptavidin-fluorophore backbone. We also describe the construction of a control tetramer to exclude B cells binding to the tetramer backbone. We provide a framework to validate whether tetramer probes are detecting true antigen-specific B cells and discuss considerations for experimental design. This protocol can be performed by researchers trained in basic biomedical/immunological research techniques, using instrumentation commonly found in most laboratories. Constructing the antigen and control tetramers takes 9 h, though their specificity should be assessed before experimentation and may take weeks to months depending on the method of validation. Sample enrichment requires ~2 h but is generally time and cost neutral as fewer cells are run through the flow cytometer.

OBJECTIVE: To report a series of atypical presentations of Aicardi-Goutières syndrome. METHODS: Clinical, neuroimaging, and genetic data. RESULTS: We report a series of six unrelated patients (five males) with a subacute loss of developmental milestones, pyramidal signs, and regression of communication abilities, with onset at ages ranging from 7 to 20 months, reaching a nadir after 4 to 24 weeks. A remarkable improvement of lost abilities occurred in the follow-up, and they remained with residual spasticity and dysarthria but preserved cognitive function. Immunization or febrile illness occurred before disease onset in all patients. CSF was normal in two patients, and in four, borderline or mild lymphocytosis was present. A brain CT scan disclosed a subtle basal ganglia calcification in one of six patients. Brain MRI showed asymmetric signal abnormalities of white matter with centrum semi-ovale involvement in five patients and a diffuse white matter abnormality with contrast enhancement in one. Four patients were diagnosed and treated for acute demyelinating encephalomyelitis (ADEM). Brain imaging was markedly improved with one year or more of follow-up (average of 7 years), but patients remained with residual spasticity and dysarthria without cognitive impairment. Demyelination relapse occurred in a single patient four years after the first event. Whole-exome sequencing (WES) was performed in all patients: four of them disclosed biallelic pathogenic variants in RNASEH2B (three homozygous p.Ala177Thr and one compound heterozygous p.Ala177Thr/p.Gln58*) and in two of them the same homozygous deleterious variants in RNASEH2A (p.Ala249Val). CONCLUSIONS: This report expands the phenotype of AGS to include subacute developmental regression with partial clinical and neuroimaging improvement. Those clinical features might be misdiagnosed as ADEM.

Objective: To evaluate the impact of a functional power threshold test (FTP) on cardiac autonomic regulation indicators in high performance cyclists.Methods: A total of 12 male elite cyclists (mean age 36.1 ± 11.2 years) were recruited. Body composition parameters were measured usingbioimpedancemetry and heart rate variability (HRV) before and after the application of the FTP assessment. Results: We observed that a greater sympathetic nervous system (SNS) index and Stress index on baseline were correlated with a smaller decrease in theparasympathetic nervous system (PNS) activity in response to the FTP test (ρ= 0.69, p = 0.013). Concerning morphological parameters, the skeletalmuscle index (SMI) was the only one that was inversely correlated with ∆PNS (ρ= -0.69, p = 0.02) whereas the muscle-bone index (MBI) displayed apositive correlation with ∆SNS (ρ = 0.82, p = 0.001). In fully adjusted models we found that waist-to-hip ratio (β= 7.90, CI95%[4.16, 11.63], t(8) = 4.88, p =0.001) and SMI significantly influenced ∆PNS (β = -1.38, CI95%[-1.84, -0.92], t(8) = -6.94, p < 0.001), whereas MBI (β= 10.26, CI95%[8.10, 12.42], t(8) =10.96, p < 0.001) and the interaction between the latter and Power achieved during FTP influenced ∆SNS (β = -0.05, CI95%[-0.09, -4.99e-03], t(8) = -2.56, p= 0.033). Conclusion: Our findings indicate that the SMI had a negative effect on the ∆PNS, while the MBI was positively correlated with the ∆SNS in cyclists. Thesefindings suggest that a higher SMI and MBI could have a detrimental impact on the cardiac autonomic response to maximal aerobic exercise in high-performance cyclists, such as FTP.(AU)


Objetivo: Evaluar el impacto de una prueba de umbral de potencia funcional (FTP) sobre los indicadores de regulación autonómica cardiaca en ciclistasde alto rendimiento. Métodos: Se reclutó a un total de 12 ciclistas de élite masculinos (edad media 36.1 ± 11.2 años). Se midieron los parámetros de composición corporalmediante bioimpedanciometría y la variabilidad de la frecuencia cardiaca (HRV) antes y después de la aplicación de la evaluación del FTP. Resultados: Observamos que un mayor índice del sistema nervioso simpático (SNS) e índice de estrés basalmente se correlacionaron con una menordisminución de la actividad del sistema nervioso parasimpático (PNS) en respuesta a la prueba FTP (ρ= 0.69, p = 0.013). En cuanto a los parámetrosmorfológicos, el índice músculo esquelético (SMI) fue el único que se correlacionó inversamente con el ∆PNS (ρ= -0.69, p = 0.02) mientras que el índicemúsculo-hueso (MBI) mostró una correlación positiva con ∆SNS (ρ = 0.82, p = 0.001). Collective actions occur all around the world and, in the last few years, even more frequently. Previous literature has mainly focused on the antecedents of collective actions, but less attention has been given to the consequences of participating in collective action. Moreover, it is still an open question how the consequences of collective action might differ, depending on whether the actions are perceived to succeed or fail. In two studies we seek to address this gap using innovative experimental studies. In Study 1 (N = 368) we manipulated the perceptions of success and failure of a collective action in the context of a real social movement, the Chilean student movement from last decade. In Study 2 (N = 169), in addition to manipulating the outcome, we manipulated actual participation, using a mock environmental organization aiming to create awareness in authorities, to test the causal effect of both participation and success/failure on empowerment, group efficacy, and intentions of future involvement in normative and non-normative collective actions. Results show that current and past participation predict overall participation in the future, however, in Study 2 the manipulated participation was associated with having less intentions of participating in the future. In both studies, perception of success increases group efficacy. In Study 1, we found that when facing failure, participants increase their willingness to participate more in the future as opposed to non-participants that actually decrease theirs. In Study 2, however, failure increases the perception of efficacy for those with a history of non-normative participation. Altogether these results highlight the moderating role of the outcome of collective action to understand the effect of participation on future participation. We discuss these results in light of the methodological innovation and the real world setting in which our studies were conducted.

BACKGROUND: Currently, and to the best of our knowledge, there is no standardized protocol to measure the effect of low- to moderate-intensity physical exercise on autonomic modulation focused in older people. AIM: Validate a test-retest short-term exercise protocol for measuring the autonomic response through HRV in older people. METHODS: A test-retest study design was used. The participants were selected through intentional non-probabilistic sampling. A total of 105 older people (male: 21.9%; female: 78.1%) were recruited from a local community. The assessment protocol evaluated HRV before and immediately after the 2-min step test. It was performed twice on the same day, considering a time of three chronological hours between the two measurements. RESULTS: The posterior distribution of estimated responses in the Bayesian framework suggests moderate to strong evidence favoring a null effect between measurements. In addition, there was moderate to robust agreement between heart rate variability (HRV) indices and assessments, except for low frequency and very low frequency, which showed weak agreement. CONCLUSIONS: Our results provide moderate to strong evidence for using HRV to measure cardiac autonomic response to moderate exercise, suggesting that it is sufficiently reliable to show similar results to those shown in this test-retest protocol.

Censuses and other surveys responsible for gathering socioeconomic data are expensive and time consuming. For this reason, in poor and developing countries there often is a long gap between these surveys, which hinders the appropriate formulation of public policies as well as the development of researches. One possible approach to overcome this challenge for some socioeconomic indicators is to use satellite imagery to estimate these variables, although it is not possible to replace demographic census surveys completely due to its territorial coverage, level of disaggregation of information and large set of information. Even though using orbital images properly requires, at least, a basic remote sensing knowledge level, these images have the advantage of being commonly free and easy to access. In this paper, we use daytime and nighttime satellite imagery and apply a transfer learning technique to estimate average income, GDP per capita and a constructed water index at the city level in two Brazilian states, Bahia and Rio Grande do Sul. The transfer learning approach could explain up to 64% of the variation in city-level variables depending on the state and variable. Although data from different countries may be considerably different, results are consistent with the literature and encouraging as it is a first analysis of its kind for Brazil.

O estudo discute as competências envolvidas na condução de rodas de conversas. Trata-se de um relato analítico e reflexivo desenvolvido a partir das experiências dos estudantes da disciplina de Promoção da Saúde de um programa de Mestrado Profissional em Saúde da Família, Mato Grosso do Sul, Brasil. As experiências foram registradas no formato de relatórios de experiências e enfatizaram o uso de rodas de conversas em Unidades de Saúde da Família. A partir do corpus documental, foram levantados o público- alvo, temáticas/objetivos e quatro competências aplicadas para o desenvolvimento das rodas de conversa, a saber: aprender a conhecer, aprender a fazer, aprender a conviver, e aprender a ser. Observou-se que o público-alvo e as temáticas desenvolvidas nas rodas de conversas foram relacionados a situação epidemiológica do território. Ainda, foi identificado que a técnica é frequentemente baseada no referencial teórico de Paulo Freire, com etapas sistematizadas. As competências de saber conhecer e saber fazer foram desafiantes. As competências saber conviver e ser destacaram- se positivamente nas experiências descritas. Por fim, as experiências revelaram que há competências que precisam ser aprimoradas para a condução de rodas de conversas, com vistas a alcançar o seu potencial transformador.

Urolithiasis is a prevalent urological condition characterized by stone formation in the urinary tract, but stones weighing more than 100 g are rare. The Mayan and Mestizo populations in Yucatan have been identified as being at an increased risk of urolithiasis because of the coexistence of environmental, genetic, metabolic, and lifestyle risk factors. The patient's occupation may play a significant role in enhancing these factors. Here, we report the case of a Mayan gardener with a giant kidney stone weighing 1,154 g, one of the largest ever reported from Mexico.

Virus-like particles (VLPs) have been gaining attention as potential platforms for delivery of cargos in nanomedicine. Although animal viruses are largely selected due to their immunostimulatory capacities, VLPs from plant viruses constitute a promising alternative to be considered. VLPs derived from Turnip mosaic virus (TuMV) have proven to present a tridimensional structure suited to display molecules of interest on their surface, making them interesting tools to be studied in theragnostic strategies. Here, we study their potential in the treatment of food allergy by genetically coupling TuMV-derived VLPs to Pru p 3, one of the most dominant allergens in Mediterranean climates. VLPs-Pru p 3 were generated by cloning a synthetic gene encoding the TuMV coat protein and Pru p 3, separated by a linker, into a transient high-expression vector, followed by agroinfiltration in Nicotiana benthamiana plants. The generated fusion protein self-assembled in planta to form the VLPs, which were purified by exclusion chromatography. Their elongated morphology was confirmed by electron microscopy and their size (~400 nm), and monodispersity was confirmed by dynamic light scattering. Initial in vitro characterization confirmed that they were able to induce proliferation of human immune cells. This proliferative capability was enhanced when coupled with the natural lipid ligand of Pru p 3. The resultant formulation, called VLP-Complex, was also able to be transported by intestinal epithelial cells, without affecting the monolayer integrity. In light of all these results, VLP-Complex was furtherly tested in a mouse model of food allergy. Sublingual administration of VLP-Complex could effectively reduce some serological markers associated with allergic responses in mice, such as anti-Pru p 3 sIgE and sIgG2a. Noteworthy, no associated macroscopic, nephritic, or hepatic toxicity was detected, as assessed by weight, blood urea nitrogen (BUN) and galectin-3 analyses, respectively. Our results highlight the standardized production of allergen-coated TuMV-VLPs in N. benthamiana plants. The resulting formula exerts notable immunomodulatory properties without the need for potentially hazardous adjuvants. Accordingly, no detectable toxicity associated to their administration was detected. As a result, we propose them as good candidates to be furtherly studied in the treatment of immune-based pathologies.

The mold Alternaria alternata is one of the main sources of asthma exacerbation, being its major allergen, Alt a 1, indispensable for its development. The main objective of this work was to answer two main questions: 1) can Alt a 1 by itself (without any other context) induce an asthmatic profile in vivo?; and 2) Which molecular mechanisms take place during this phenomenon? To answer both questions, we have developed a mouse model of allergic asthma using only Alt a 1 for mice sensitization. We also made use of in-vitro cellular models and computational studies to support some aspects of our hypothesis. Our results showed that Alt a 1 can induce an asthmatic phenotype, promoting tissue remodeling and infiltration of CD45+ cells, especially eosinophils and macrophages (Siglec F+ and F4/80+). Also, we have found that Alt a 1 sensitization is mediated by the TLR4-macrophage axis.

Lipid Transfer Proteins (LTPs) have been described as one of the most prevalent and cross-reactive allergen families in the general population. They are widely distributed among the plant kingdom, as well as in different plant organs ranging from pollen to fruits. Thus, they can initiate allergic reactions with very different outcomes, such as asthma and food allergy. Several mouse models have been developed to unravel the mechanisms that lead LTPs to promote such strong sensitization patterns. Interestingly, the union of certain ligands can strengthen the allergenic capacity of LTPs, suggesting that not only is the protein relevant in the sensitization process, but also the ligands that LTPs carry in their cavity. In fact, different LTPs with pro-allergenic capacity have been shown to transport similar ligands, thus positioning lipids in a central role during the first stages of the allergic response. Here, we offer the latest advances in the use of experimental animals to study the topic, remarking differences among them and providing future researchers a tool to choose the most suitable model to achieve their goals. Also, recent results derived from metabolomic studies in humans are included, highlighting how allergic diseases alter the lipidic metabolism toward a pathogenic state in the individual. Altogether, this review offers a comprehensive body of work that sums up the background evidence supporting the role of lipids as modulators of allergic diseases. Studying the role of lipids during allergic sensitization might broaden our understanding of the molecular events leading to tolerance breakdown in the epithelium, thus helping us to understand how allergy is initiated and established in the individuals.

INTRODUCCIÓN. La incorporación de nuevas tecnologías como la hemodiafiltración en línea, han mejorado parámetros metabólicos/nutricionales en los pacientes que se encontraban en hemodiálisis convencional; en la actualidad no existen datos registrados en la población ecuatoriana que se encuentra sometida a esta clase de tecnologías. OBJETIVO. Comparar la evolución clínico-metabólica de pacientes que estaban en hemodiálisis convencional y cambiaron a hemodiafiltración en línea, determinar si es favorable la migración de la terapia hemodialítica difusiva a convectiva y establecer si el cambio de terapia dialítica ocasionó resultados favorables. MATERIALES Y MÉTODOS. Estudio analítico retrospectivo. Población y muestra de 38 pacientes enfermos renales crónicos en terapia de sustitución renal modalidad hemodiálisis convencional que cambiaron a hemodiafiltración en línea, independientemente del tiempo de diagnóstico y tratamiento en la unidad de hemodiálisis del Hospital de Especialidades Carlos Andrade Marín, Quito-Ecuador, durante el periodo marzo 2016 a marzo 2017. RESULTADOS. Los efectos nutricionales y metabólicos pudieron denotar mayor ponderación de resultados favorables en la modalidad de hemodiafiltración. En la estabilidad hemodinámica y la dosis de diálisis se evidenció una leve superioridad en la modalidad de hemodiafiltración en comparación a la Hemodiálisis. En las dosis administradas de Calcio, Hierro, Eritropoyetina y Calcitriol no existieron diferencias significativas entre las dos modalidades de tratamientos. CONCLUSIÓN. El cambio de modalidad de Hemodiálisis convencional a Hemodiafiltración en línea fue favorable, y mejoró los parámetros clínicos/metabólicos de los pacientes que requieren terapia de sustitución renal.


INTRODUCTION. The incorporation of new technologies such as online haemodiafiltration have improved metabolic/nutritional parameters in patients who were on conventional haemodialysis; At present, there are no registered data on the Ecuadorian population that is subjected to this kind of technology. OBJECTIVE. To compare the clinical-metabolic evolution of patients who were on conventional hemodialysis and changed to online hemodiafiltration, to determine if the migration from diffusive to convective hemodialysis therapy is favorable and to establish if the change in dialysis therapy caused favorable results. MATERIALS AND METHODS. Retrospective analytical study. Population and sample of 38 patients with chronic kidney disease in conventional hemodialysis modality renal replacement therapy who changed to online hemodiafiltration, regardless of the time of diagnosis and treatment in the hemodialysis unit of the Hospital de Especialidades Carlos Andrade Marín, Quito-Ecuador, during the period March 2016 to March 2017. RESULTS. The nutritional and metabolic effects could denote a greater weighting of favorable results in the hemodiafiltration modality. In hemodynamic stability and dialysis dose, a slight superiority was evidenced in the hemodiafiltration modality compared to hemodialysis. In the administered doses of Calcium, Iron, Erythropoietin and Calcitriol there were no significant differences between the two treatment modalities. CONCLUSION. The change of modality from conventional hemodialysis to online hemodiafiltration was favorable, and improved the clinical/metabolic parameters of patients requiring renal replacement therapy.

Blastic plasmacytoid dendritic cell neoplasm is a rare hematologic neoplasm characterized by cutaneous, hematologic, and central nervous system (CNS) involvement with poor prognosis. Diagnosis is made by flow cytometry, although there are no specific markers, making its diagnosis challenging. So far, with the available evidence, acute lymphoid leukemia-type schemes and consolidation with allogeneic transplant seem to become the first-line therapy. With its characterization, new therapies directed toward CD123 and the anti-apoptotic protein Bcl-2 have appeared to prolong the survival of these patients. We present a case of a 27-year-old male patient diagnosed with blastic plasmacytoid dendritic cell neoplasm with unusual CNS manifestations and without skin involvement who achieved complete remission with venetoclax and improvement of neurological symptoms, making him a candidate for hematopoietic stem cell transplant.

Immune reconstitution syndrome (IRIS) is an uncommon complication seen in patients living with human immunodeficiency virus (HIV) characterized by the decline in a pre-existing condition or a new diagnosis of a masked disease. It is associated with a significant inflammatory component that occurs after starting antiretrovirals, being most frequent in those with severe immunosuppression. Thereby, different types of opportunistic diseases such as non-tuberculous mycobacteria are more frequent in this group of patients, especially those with low lymphocyte counts of below 50 cells/µL. Here, we present the case of an HIV-positive patient who developed lymphadenitis caused by Mycobacterium intracellulare as an unmasked IRIS after initiating treatment for HIV.

Allergic sensitization is initiated by protein and epithelia interaction, although the molecular mechanisms leading this encounter toward an allergic phenotype remain unknown. Here, we apply the two-hit hypothesis of inflammatory diseases to the study of food allergy sensitization. First, we studied the effects of long-term depilation in mice by analyzing samples at different time points. Several weeks of depilation were needed until clear immunological changes were evidenced, starting with upregulation of NLRP3 protein levels, which was followed by overexpression of Il1b and Il18 transcripts. Secondly, we assessed the effects of allergen addition (in this case, Pru p 3 in complex with its natural lipid ligand) over depilated skin. Systemic sensitization was evaluated by intraperitoneal provocation with Pru p 3 and measure of body temperature. Anaphylaxis was achieved, but only in mice sensitized with Prup3_complex and not treated with the NLRP3 inhibitor MCC950, thus demonstrating the importance of both hits (depilation + allergen addition) in the consecution of the allergic phenotype. In addition, allergen encounter (but not depilation) promoted skin remodeling, as well as CD45+ infiltration not only in the sensitized area (the skin), but across several mucosal tissues (skin, lungs, and gut), furtherly validating the systemization of the response. Finally, a low-scale study with human ILC2s is reported, where we demonstrate that Prup3_complex can induce their phenotype switch (↑CD86, ↑S1P1) when cultured in vitro, although more data is needed to understand the implications of these changes in food allergy development.
