Introduction@#In the Philippines, essential eye care services are not easily accessible especially for those in lower income groups, putting public elementary school students at risk for underdiagnosis of problems in visual acuity. The objective of this investigation was to determine prevalence of undetected refractive errors among public elementary students using the Welch Allyn Spot™ Vision Screener.@*Methods @#A cross-sectional design was used in this investigation to estimate the prevalence of uncorrected refractive errors in the first grade students in San Perfecto Elementary School using Welch Allyn Spot™ Vision Screener (Photoscreener). Eligible students who gave informed consent and assent answered a questionnaire and underwent an eye examination. The prevalence of undetected errors of refraction were computed for the sample population and for selected demographic variables.@*Results @#Approximately one out of four students (24.53%) had errors of refraction, with the most common type being astigmatism (22.64%), followed by myopia (3.77%) and hyperopia (2.83%).@*Conclusion @#This study revealed a high prevalence of undetected refractive errors among school-age children, higher than current published data (5%) in the Philippines.

RESUMO

In this study, we employ a thiol-functionalized polymer (P3HT-SH) as a leverage to tailor the nanomorphology and electronic coupling in polymer-nanocrystal composites for hybrid solar cells. The presence of the thiol functional group allows for a highly crystalline semiconducting polymer film at low thiol content and allows for improved nanomorphologies in hybrid organic-inorganic systems when employing non-toxic bismuth sulfide nanocrystals. The exciton dissociation efficiency and carrier dynamics at this hybrid heterojunction are investigated through photoluminescence quenching and transient absorption spectroscopy measurements, revealing a larger degree of polaron formation when P3HT-SH is employed, suggesting an increased electronic interaction between the metal chalcogenide nanocrystals and the thiol-functionalized P3HT. The fabricated photovoltaic devices show 15% higher power conversion efficiencies as a result of the improved nanomorphology and better charge transfer mechanism together with the higher open circuit voltages arising from the deeper energy levels of P3HT-SH.

RESUMO

The light originating from seven light-emitting diodes of different colours is guided, one at a time, into a measuring cell by means of a fibre optic coupler. Detection is carried out with photodiodes which are connected to a log-ratio amplifier yielding direct absorbance readings. Optical filters are used to narrow the emission band from blue light emitting diodes as these bands are relatively wide compared to those of the emitters of other colours. An inexpensive and compact multi-wavelength photometer covering the visible range is thus obtained, which in many cases can replace a conventional spectrophotometer for absorbance measurements. The performance for a range of commonly used photometric analytical procedures is described and compared to conventional measurements with a spectrophotometer.

RESUMO

This paper presents recent innovations in organizing provider work and changes in organizational structure that promote: (1) reduction in fragmentation of care (2) improved continuity and quality of care and (3) improved cost-containment efforts and (4) better decision making. Data is also given on resource utilization. It is anticipated that these models of care and services are prototypes of future models of care for the elderly as this population increases in number. Data is also presented that suggests that, eventually, increases in taxes and social security contributions and/or the rationing of health care may be necessary to meet pensioner health needs.

RESUMO

In Denmark, legislation having the aim to help the elderly to be active as long as possible and prevent or delay institutionalization, is responsible for a number of innovative programs for this segment of the population. It is now the law that no more nursing homes will be constructed in Denmark. Free home nursing and permanent home help and night patrols for home nursing and help is a strategy to delay or prevent institutionalization. In the municipality of Skaevinge, a nursing home was converted to sheltered housing for the same patients and others from the community. Emphasis is on self care and autonomy in decision making. There are no fixed routines or schedules as found in the traditional nursing home. Examples of programs in facilities are given that have health professionals jointly planning and coordinating the delivery of their services that reduce fragmentation and duplication.

Both Denmark and Sweden have achieved a reduction in institutionalization of elderly individuals; 23 percent nursing home care in Denmark from 1980-89 and 34 percent overall institutionalization in Sweden from 1970-1985. This was accomplished by increasing home care and housing with services and adaptations. Further increases in growth of the elderly population combined with modest economic expansion will be the forces responsible for finding additional alternatives to costly institutional care. Community-based services and care have not been demonstrated at this time to be less costly than institutional care. An illustration of an innovative model of nursing home care in Denmark is described. The medical model of care was abandoned in favor of a self-care model for the purpose of fostering independent living and decision-making as long as possible. In Sweden, a reduction in demand for beds in a long-term care hospital has been achieved through a tightly coordinated system of care and services among the various health and social service professionals.

To identify the principal neutralization determinant (PND) of simian immunodeficiency virus (SIV), antisera were generated using recombinant gp110 [the SIV analog of the human immunodeficiency virus type 1 (HIV-1) external envelope glycoprotein, gp120], gp140, several large recombinant and proteolytic envelope fragments, and synthetic peptides of the SIVmac251 isolate. When purified under conditions that retain its native structure, gp110 bound CD4 and elicited antisera that neutralized SIVmac251 with high titer. Native gp110 also completely inhibited neutralizing antibody in sera from SIVmac251-infected macaques. In contrast, denatured gp110 and gp140, large envelope fragments, and synthetic peptides (including peptides analogous to the HIV-1 PND) elicited very low or undetectable neutralizing antibody titers and did not inhibit neutralizing antibody in infected macaque sera. Enzymatically deglycosylated gp110 efficiently absorbed neutralizing antibodies from macaque sera, showing that neutralizing antibodies primarily bind the protein backbone. A 45-kDa protease digest product, mapping to the carboxyl-terminal third of gp110, also completely absorbed neutralizing antibodies from infected macaque sera. These results show that the PND(s) of this SIV isolate depends on the native conformation and that linear peptides corresponding to the V3 loop of SIV envelope, in contrast to that of HIV-1, do not elicit neutralizing antibody. This may affect the usefulness of SIVmac for evaluating HIV-1 envelope vaccine approaches that rely on eliciting neutralizing antibody.

Where once these facilities were only concerned with health-related issues, they are now concerned with their status as businesses. These facilities now utilize business functions such as marketing, advertising, sales, and strategic planning. This article explains how health care facilities can use structural entrepreneurship in order to meet the business-related challenges of the future.

RESUMO

PPOs are designed to promote competition among providers for the purpose of enhancing cost containment. Business and industry have demonstrated an active interest in PPOs and report savings as a consequence of purchasing health services for employees from these new alternative care organizations. To be effective in reducing the rate of escalation of personal health care expenditures will require demonstration to the business community that they are able to provide quality care at less cost. This will require a strong utilization review program and a need for providers to share some risk as an incentive to be cost efficient. Furthermore, any federal or state legislation or regulation should discourage overburdensome costs for PPOs to do business.
