RESUMO

PURPOSE: This paper presents a deep learning model for use in the automated segmentation of metastatic brain tumors and associated perilesional edema. METHODS: The model was trained using Gamma Knife surgical data (90 MRI sets from 46 patients), including the initial treatment plan and follow-up images (T1-weighted contrast-enhanced (T1cWI) and T2-weighted images (T2WI)) manually annotated by neurosurgeons to indicate the target tumor and edema regions. A mask region-based convolutional neural network was used to extract brain parenchyma, after which the DeepMedic 3D convolutional neural network was in the segmentation of tumors and edemas. RESULTS: Five-fold cross-validation demonstrated the efficacy of the brain parenchyma extraction model, achieving a Dice similarity coefficient of 96.4%. The segmentation models used for metastatic tumors and brain edema achieved Dice similarity coefficients of 71.6% and 85.1%, respectively. This study also presents an intuitive graphical user interface to facilitate the use of these models in clinical analysis. CONCLUSION: This paper introduces a deep learning model for the automated segmentation and quantification of brain metastatic tumors and perilesional edema trained using only T1cWI and T2WI. This technique could facilitate further research on metastatic tumors and perilesional edema as well as other intracranial lesions.

In mirror-symmetric systems, there is a possibility of the realization of extended gapless electronic states characterized as nodal lines or rings. Strain induced modifications to these states lead to the emergence of different classes of nodal rings with qualitatively different physical properties. Here we study optical response and the electromagnetic wave propagation in type I nodal ring semimetals, in which the low-energy quasiparticle dispersion is parabolic in momentum k_{x} and k_{y} and is linear in k_{z}. This leads to a highly anisotropic dielectric permittivity tensor in which the optical response is plasmonic in one spatial direction and dielectric in the other two directions. The resulting normal modes (polaritons) in the bulk material become hyperbolic over a broad frequency range, which is furthermore tunable by the doping level. The propagation, reflection, and polarization properties of the hyperbolic polaritons not only provide valuable information about the electronic structure of these fascinating materials in the most interesting region near the nodal rings but also pave the way to tunable hyperbolic materials with applications ranging from anomalous refraction and waveguiding to perfect absorption in ultrathin subwavelength films.

We systematically investigate the ground state phase diagram and the finite temperature phase transitions for a Rydberg-dressed Fermi gas loaded in a bilayer optical lattice. When an effective finite-ranged attraction is induced, our self-consistent mean-field calculation shows that the gapped topological (p-wave) superfluids in each layer are coupled together by thes-wave pairing in an intermediate inter-layer distance with a spontaneously modulated phases between these two order parameters. The obtained ground state is a gapless topological superfluid with quantized topological charges characterizing the gapless points, leading to a zero energy flat band at the edges. Finally, we calculate the finite temperature phase diagrams of this two-dimensional gapless superfluid and observe two distinct critical temperatures, demonstrating the fruitful many-body effects on a paired topological superfluids.

A proximity effect facilitates the penetration of Cooper pairs that permits superconductivity in a normal metal, offering a promising approach to turn heterogeneous materials into superconductors and develop exceptional quantum phenomena. Here, we have systematically investigated proximity-induced anisotropic superconductivity in a monolayer Ni-Pb binary alloy by combining scanning tunneling microscopy/spectroscopy (STM/STS) with theoretical calculations. By means of high-temperature growth, the (33×33)R30o Ni-Pb surface alloy has been fabricated on Pb(111) and the appearance of a domain boundary as well as a structural phase transition can be deduced from a half-unit-cell lattice displacement. Given the high spatial and energy resolution, tunneling conductance (dI/dU) spectra have resolved the reduced but anisotropic superconducting gap ΔNiPb ≈ 1.0 meV, in stark contrast to the isotropic ΔPb ≈ 1.3 meV. In addition, the higher density of states at the Fermi energy (D(EF)) of the Ni-Pb surface alloy results in an enhancement of coherence peak height. According to the same Tc ≈ 7.1 K with Pb(111) from the temperature-dependent ΔNiPb and the short decay length Ld ≈ 3.55 nm from the spatially monotonic decrease of ΔNiPb, both results are supportive of a proximity-induced superconductivity. Despite a lack of a bulk counterpart, the atomically thick Ni-Pb bimetallic compound opens a pathway to engineer superconducting properties down to the two-dimensional limit, giving rise to the emergence of anisotropic superconductivity via a proximity effect.

We characterize the early stages of the approach to equilibrium in isolated quantum systems through the evolution of the entanglement spectrum. We find that the entanglement spectrum of a subsystem evolves with three distinct timescales. First, on an o(1) timescale, independent of system or subsystem size and the details of the dynamics, the entanglement spectrum develops nearest-neighbor level repulsion. The second timescale sets in when the light cone has traversed the subsystem. Between these two times, the density of states of the reduced density matrix takes a universal, scale-free 1/f form; thus, random-matrix theory captures the local statistics of the entanglement spectrum but not its global structure. The third time scale is that on which the entanglement saturates; this occurs well after the light cone traverses the subsystem. Between the second and third times, the entanglement spectrum compresses to its thermal Marchenko-Pastur form. These features hold for chaotic Hamiltonian and Floquet dynamics as well as a range of quantum circuit models.

Coexistence of topological elements in topological metals/semimetals (TMs) has gradually attracted attention. However, non-topological factors always interfere with the Fermi surface and cover interesting topological properties. Here, we find that Ba3Si4 is a "clean" TM which contains coexisting nodal-chain networks, intersecting nodal rings (INRs) and triple points, in the absence of spin-orbit coupling (SOC). Moreover, the nodal rings in the topological phase exhibit diverse types: from type-I and type-II to type-III rings according to band dispersions. All of the topological elements are generated by crossings of three energy bands, and thus they are correlated rather than mutually independent. When some structural symmetries are eliminated by an external strain, the topological phase evolves into another phase including a Hopf link, a one-dimensional nodal chain and new INRs.

Current theories of superfluidity are based on the idea of a coherent quantum state with topologically protected quantized circulation. When this topological protection is absent, as in the case of ^{3}He-A, the coherent quantum state no longer supports persistent superflow. Here, we argue that the loss of topological protection in a superconductor gives rise to an insulating ground state. We specifically introduce the concept of a Skyrme insulator to describe the coherent dielectric state that results from the topological failure of superflow carried by a complex-vector order parameter. We apply this idea to the case of SmB_{6}, arguing that the observation of a diamagnetic Fermi surface within an insulating bulk can be understood as a realization of this state. Our theory enables us to understand the linear specific heat of SmB_{6} in terms of a neutral Majorana Fermi sea and leads us to predict that in low fields of order a Gauss, SmB_{6} will develop a Meissner effect.

INTRODUCTION: The diagnosis of urinary retention in stroke patients is challenging because their symptoms may be subtle, or go unnoticed. Furthermore, patients suffering from aphasia or impaired cognition often have difficulties communicating urination problems, leading to their urinary retention being overlooked. CASE REPORT: We report a 53-year-old man with bilateral pontine hemorrhage resulting in incomplete locked-in syndrome. During the patient's hospital course, he developed 3 episodes of rapid onset of profuse unilateral hyperhidrosis. All 3 episodes were found to be induced by an overly distended bladder and each time the unilateral hyperhidrosis resolved promptly after bladder drainage. CONCLUSIONS: This case provides the first description of unilateral hyperhidrosis as a manifestation of urinary retention in a stroke patient. It is important for clinicians to recognize this condition so that appropriate management can be implemented in time to prevent potential detrusor injury and renal impairment caused by bladder overdistension.

BACKGROUND: It is generally understood that postoperative C5 palsy can occur with anterior or posterior decompression surgery, but functional measures of the palsy have not been well documented. This study aimed to investigate the incidence of C5 palsy in different surgical procedures, examine the correlations between muscle strength, upper extremity functional measures, and health-related quality of life, and to observe potential risk factors contributing to C5 palsy. METHODS: Our investigation involved a retrospective study design. A total of 364 patients who underwent decompression surgery were indicated within the selected exclusion criteria. Additionally, 12 C5 palsy patients were recruited. The relationships between the manual muscle test (MMT), the action research arm test (ARAT), the Jebsen test of hand function (JTHF), and the European quality of life-5 dimensions (EQ-5D) were studied, and univariate analyses were performed to search possible risk factors and recovery investigation. RESULTS: The data analyzed in the 12 cases and C5 palsy incidences (3.3%) were: 0.7% in anterior procedures (n = 2), 8.8% in posterior procedures (n = 6), and 36.4% in combined procedures (n = 4). Moderate-to-high correlations were observed between the ARAT, JTHF, EQ-5D visual analog scale scores, and MMT (r = 0.636-0.899). There were significant differences in patient age, etiology of cervical lesion, variable decompression procedures, and the number of decompression levels between the C5 palsy and non-C5 palsy groups. For female patients (p = 0.018) and number of decompression levels (p = 0.028), there were significant differences between the complete recovery and the incomplete recovery groups. CONCLUSION: Patients undergoing combined anterior-posterior decompression surgery had the highest incidence of C5 palsy, and correlations between the ARAT, JTHF, EQ-5D visual analog scale clinical tools, and MMT scores supported these findings. Female status and lower decompression levels could also be predictive factors for complete recovery, although additional research is needed to substantiate these findings.

Smell is a vital sense for animals. The mainstream explanation of smell is based on recognition of the odorant molecules through characteristics of their surface, e.g., shape, but certain experiments suggest that such recognition is complemented by recognition of vibrational modes. According to this suggestion an olfactory receptor is activated by electron transfer assisted through odorant vibrational excitation. The hundreds to thousands of different olfactory receptors in an animal recognize odorants over a discriminant landscape with surface properties and vibrational frequencies as the two major dimensions. In the present paper we introduce the vibrationally assisted mechanism of olfaction and demonstrate for several odorants that, indeed, a strong enhancement of an electron tunneling rate due to odorant vibrations can arise. We discuss in this regard the influence of odorant deuteration and explain, thereby, recent experiments performed on Drosophila melanogaster. Our demonstration is based on known physical properties of biological electron transfer and on ab initio calculations on odorants carried out for the purpose of the present study. We identify a range of physical characteristics which olfactory receptors and odorants must obey for the vibrationally assisted electron transfer mechanism to function. We argue that the stated characteristics are feasible for realistic olfactory receptors, noting, though, that the receptor structure presently is still unknown, but can be studied through homology modeling.

INTRODUCTION: Sciatic neuropathy is a rare but serious complication following surgery performed in the sitting position. METHODS: We report a case of a 12-year-old boy who developed bilateral sciatic neuropathy after a 9-h-long craniotomy while in the sitting position. RESULTS: Magnetic resonance imaging for the lower extremities revealed bilateral swelling of the sciatic nerves near the ischial tuberosities. CONCLUSION: A heightened awareness of sciatic nerve injury during prolonged surgery in the sitting position may make early diagnosis possible and allow prevention of further neurologic injury.
