RESUMO

Omega-3 polyunsaturated fatty acids (PUFAs), especially eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA) have been associated with slower rates of cognitive decline. We investigated the association between omega-3 PUFAs and cognitive function in patients with Alzheimer's disease (AD) receiving acetylcholinesterase inhibitors (AChEIs). This was a prospective cohort study using registered data. Patients with AD receiving AChEIs were recruited from 1 May 2016 to 30 April 2019 and were followed up for two years. Their daily diet record and blood concentration of omega-3 PUFAs were analyzed. Multiple linear and binary logistic regression was used to determine the factors associated with cognitive decline (continuous and dichotomized cognitive change). In the research, 129 patients with AD were identified with a mean age of 76.5 ± 6.6. Patients with AD with lower baseline omega-3 PUFAs levels were associated with a higher risk of cognitive decline than those with higher levels (odds ratio [OR] = 1.067, 95% confidence interval [CI]: 1.012, 1.125; p = 0.016) after adjustment. Patients with AD with a lower baseline DHA (OR = 1.131, 95% CI: 1.020, 1.254; p = 0.020), but not EPA, were associated with a higher risk of cognitive decline. We found that higher Mini-Nutritional Assessment scores (beta = -0.383, 95% CI = -0.182--0.048, p = 0.001) and total fat (beta = -0.248, 95% CI = -0.067--0.003, p = 0.031) were independently associated with slow cognitive decline in patients with AD receiving AChEIs. The baseline blood levels of omega-3 PUFAs were associated with cognitive decline in patients with AD receiving AChEIs. Future randomized controlled trials are needed to clarify whether this association is causal.

In this study, we proposed a U-shaped optical fiber probe fabricated using a flame heating method. The probe was packaged in glass tube to reduce human factors during experimental testing of the probe as a glucose sensor. The U-shaped fiber probe was found to have high sensitivity in detecting the very small molecule. When the sensor was dipped in solutions with different refractive indexes, its wavelength or transmission loss changed. We used electrostatic self-assembly to bond gold nanoparticles and glucose oxidase (GOD) onto the sensor’s surface. The results over five cycles of the experiment showed that, as the glucose concentration increased, the refractive index of the sensor decreased and its spectrum wavelength shifted. The best wavelength sensitivity was 2.899 nm/%, and the linearity was 0.9771. The best transmission loss sensitivity was 5.101 dB/%, and the linearity was 0.9734. Therefore, the proposed U-shaped optical fiber probe with gold nanoparticles and GOD has good potential for use as a blood sugar sensor in the future.

The standard treatment for overactive bladder starts with patient education and behavior therapies, followed by antimuscarinic agents. For patients with urgency urinary incontinence refractory to antimuscarinic therapy, currently both American Urological Association (AUA) and European Association of Urology (EAU) guidelines suggested that intravesical injection of botulinum toxin A should be offered. The mechanism of botulinum toxin A includes inhibition of vesicular release of neurotransmitters and the axonal expression of capsaicin and purinergic receptors in the suburothelium, as well as attenuation of central sensitization. Multiple randomized, placebo-controlled trials demonstrated that botulinum toxin A to be an effective treatment for patients with refractory idiopathic or neurogenic detrusor overactivity. The urinary incontinence episodes, maximum cystometric capacity, and maximum detrusor pressure were improved greater by botulinum toxin A compared to placebo. The adverse effects of botulinum toxin A, such as urinary retention and urinary tract infection, were primarily localized to the lower urinary tract. Therefore, botulinum toxin A offers an effective treatment option for patients with refractory overactive bladder.

In eukaryotic cells, mitochondria form a dynamic interconnected network to respond to changing needs at different subcellular locations. A fundamental yet unanswered question regarding this network is whether, and if so how, local fusion and fission of individual mitochondria affect their global distribution. To address this question, we developed high-resolution computational image analysis techniques to examine the relations between mitochondrial fusion/fission and spatial distribution within the axon of Drosophila larval neurons. We found that stationary and moving mitochondria underwent fusion and fission regularly but followed different spatial distribution patterns and exhibited different morphology. Disruption of inner membrane fusion by knockdown of dOpa1, Drosophila Optic Atrophy 1, not only increased the spatial density of stationary and moving mitochondria but also changed their spatial distributions and morphology differentially. Knockdown of dOpa1 also impaired axonal transport of mitochondria. But the changed spatial distributions of mitochondria resulted primarily from disruption of inner membrane fusion because knockdown of Milton, a mitochondrial kinesin-1 adapter, caused similar transport velocity impairment but different spatial distributions. Together, our data reveals that stationary mitochondria within the axon interconnect with moving mitochondria through fusion and fission and that local inner membrane fusion between individual mitochondria mediates their global distribution.

Insulin-gold nanoclusters exhibit outstanding biocompatibility, photostability, and fluorescence quantum efficiency. However, they have never been used in superresolution microscopy, which requires nonlinear switching or saturation of fluorescence. Here we examine the fluorescence and stimulated emission depletion properties of gold nanoclusters. Their bleaching rate is very slow, demonstrating superior photostability. Surprisingly, however, the best depletion efficiency is less than 70%, whereas the depletion intensity requirement is much higher than the expectation from a simple two-level model. Fluorescence lifetime measurement revealed two distinct lifetime components, which indicate intersystem and reverse intersystem crossing during excitation. Based on population dynamic calculation, excellent agreement of the maximal depletion efficiency is found. Our work not only features the first examination of STED with metallic clusters, but also reveals the significance of molecular transition dynamics when considering a STED labeling.

BACKGROUND: Grana and starch are major functional structures for photosynthesis and energy storage of plant, respectively. Both exhibit highly ordered molecular structures and appear as micrometer-sized granules inside chloroplasts. In order to distinguish grana and starch, we used multiphoton microscopy, with simultaneous acquisition of two-photon fluorescence (2PF) and second harmonic generation (SHG) signals. SHG is sensitive to crystallized structures while 2PF selectively reveals the distribution of chlorophyll. RESULT: Three distinct microstructures with different contrasts were observed, i.e. "SHG dominates", "2PF dominates", and "SHG collocated with 2PF". It is known that starch and grana both emit SHG due to their highly crystallized structures, and no autofluorescence is emitted from starch, so the "SHG dominates" contrast should correspond to starch. The contrast of "SHG collocated with 2PF" is assigned to be grana, which exhibit crystallized structure with autofluorescent chlorophyll. The "2PF dominates" contrast should correspond to stroma thylakoid, which is a non-packed membrane structure with chrolophyll. The contrast assignment is further supported by fluorescence lifetime measurement. CONCLUSION: We have demonstrated a straightforward and noninvasive method to identify the distribution of grana and starch within an intact leaf. By merging the 2PF and SHG images, grana, starch and stroma thylakoid can be visually distinguished. This approach can be extended to the observation of 3D grana distribution and their dynamics in living plants.

We report the favorable anatomic and functional outcome of photodynamic therapy with verteporfin in a case of chronic central serous chorioretinopathy complicated with choroidal neovascularization (CNV). This 37-year-old Chinese male with bilateral chronic central serous chorioretinopathy presented with central scotoma, reduced vision and metamorphopsia in his right eye. Fluorescein angiography (FA) disclosed macular hemorrhage, exudate and subfoveal classic CNV. Photodynamic therapy (PDT) with verteporfin was applied to the CNV according to standard protocol with 2.2-mm spot size. Best-corrected visual acuity (BCVA) improved from 6/20 to 6/10 1 month after PDT. BCVA recovered to 6/7.5 without leakage on FA 3 months after PDT. Neither recurrent CNV lesion nor new hemorrhage was noted over 12 months of follow-up. Short-term results suggest that PDT with verteporfin for CNV secondary to central serous chorioretinopathy is a safe and effective treatment modality.

PURPOSE: To study the prevalence and risk factors of intraocular pressure (IOP) elevation and refractory IOP elevation after a single 4 mg intravitreal triamcinolone acetonide (IVTA) injection in a Chinese population. DESIGN: Retrospective, observational case series. METHODS: Retrospective chart review of all the patients who received an IVTA injection at one medical center from January 2003 through June 2005. A postoperative IOP increase of more than 6 mm Hg was defined as an IOP elevation. Postoperative IOP elevation that could not be controlled by glaucoma medications was defined as refractory IOP elevation. RESULTS: Sixty-four (43.5%) of the 147 enrolled patients experienced IOP elevation. Males had a higher risk of IOP elevation, with an odds ratio (OR) of 3.17 (95% confidence interval [CI], 1.38 to 7.27; P = .006), after adjusting for age, glaucoma history, and diagnosis of retinal disease. Patients 55 years of age and younger had a larger magnitude of IOP elevation than those older than 55 years (11.2 +/- 10.1 mm Hg vs 7.3 +/- 7.7 mm Hg; P = .020). Ten patients (6.8%) had refractory IOP elevation, and those 55 years of age or younger had a higher risk of refractory IOP elevation compared with those older than 55 years, with an OR of 8.16 (95% CI, 1.67 to 39.81; P = .009), after adjusting for preoperative IOP and diagnosis of retinal disease. CONCLUSIONS: Elevated IOP after IVTA injection is common in this Chinese population. Male patients have a higher risk of IOP elevation. Younger patients should be monitored carefully after IVTA injection because they exhibit a greater magnitude of IOP elevation and have an increased chance of developing refractory IOP elevation.

The prognosis of double penetrating ocular trauma in children is usually guarded. We report the good anatomic and functional outcome in a child with double penetrating ocular trauma associated with retinal detachment and traumatic endophthalmitis. A 5-year-old boy presented to the emergency room with pain and tearing in his left eye after a penetrating ocular injury by a rusted steel wire. After examination under anesthesia, both entrance and exit wounds in the sclera were found, and were complicated with inferior retinal detachment. There were signs of infection such as corneal edema, increased cell, flare and hypopyon in the anterior chamber, and vitreous opacity. The post-traumatic endophthalmitis was successfully treated with prompt intravenous and intravitreal antibiotics injection. The patient subsequently underwent scleral buckling and transpupillary indirect laser photocoagulation operation on the 10th day after trauma. The retina was completely reattached 2 weeks later, and his vision recovered to 6/10 in his left eye after a 6-month follow-up. Prompt use of antibiotics and meticulous surgical intervention are essential in the successful management of such patients.
