RESUMO

We conducted comparative analysis of treatment 52 patients (7 men and 45 women aged 42-64 years, mean age 51.1 years) with I-II degree arterial hypertension (AH) and type 2 diabetes mellitus with the use of angiotensin converting enzyme inhibitor perindopril and with of other drugs. During 6 months effective control of AH was achieved on both treatments. We assessed changes in target organs at the background of 1 year therapy based on the use of perindopril or other antihypertensive preparations. Favorable changes of 24 hour indexes of arterial pressure accord in the group of perindopril treated patients compared with the group of patients treated with other antihypertensive drugs. Therapy with perindopril exerted organoprotective effect which manifested as lowering of microalbuminuria and augmentation of endothelium dependent vasodilation. We demonstrated ability of perindopril to affect proinflammatory status: to suppress factors of subclinical inflammation (interleukin-6 and C-reactive protein) and to elevate level of anti-inflammatory interleukin-10.

RESUMO

METHODS: Parameters of spirometry, body plethysmography, and pulmonary diffusion capacity were measured in 46 patients (age 35-60 years, 40 men, 6 women) with stage I-II hypertensive disease (HD) without bronchial and pulmonary diseases, systolic dysfunction of left ventricular myocardium, obesity, current or past smoking. RESULTS: Increase of severity of HD and elevation of average values of arterial pressure (AP) was associated with mixed type worsening of parameters of pulmonary ventilation capacity. Negative correlations existed between parameters of 24 hour arterial pressure monitoring and spirometry. Progression of microalbuminuria and endothelial dysfunction was associated with lowering of pulmonary vital capacity, alveolar volume, and diffusion capacity. CONCLUSION: The data presented allow to suggest that lungs can stand as target organ in hypertensive disease.

RESUMO

AIM: To analyze prevalence and structure of thyroid pathology in women of reproductive age living in Shoria mountains. MATERIAL AND METHODS: The examination of 409 women aged 18 to 40 living in mountain Shoria included assessment of social status, heredity, actual nutrition, visit to endocrinologist, ultrasound investigation of the thyroid, tests for thyroid hormones (TTH, T3, T4) in the serum, antibodies to microsomal fractions, iodine concentrations in the morning urine. RESULTS: Significant prevalence of thyroid diseases (62.3%), high rate of diffuse nontoxic goiter of the second degree (18.0%), nodular nontoxic goiter (11.8%), autoimmune thyroiditis (6.3%), hypothyroidism (5.5%), low median of iodine excretion with urine in women refer mountain Shoria to the zone of goiter endemia. CONCLUSION: Among migrants thyroid morbidity is significantly higher than among the native population. Eradication of goiter endemia will promote reproductive health in this region.
