The authors present results of studies in a group of 58 children aged 4-36 months in whom food allergy has been diagnosed. Esophageal 24-hour pH-metric studies revealed the presence of gastroesophageal reflux (GER) in 27 cases (46.5%). In 13 cases it was primary and independent of the allergic disease, and in 14 children it showed features of secondary gastroesophageal reflux. Conservative treatment appeared ineffective more frequently in the group of infants with primary reflux (in 23.1% of cases) than in children with secondary reflux and food allergy (in 7.1% of cases). In 4 infants fundoplication according to Nissen was performed.

Twenty three children with fractures of the lateral condyle of the humerus were analyzed. In 20 cases surgery was performed 0 to 8 days after injury. The 3 delayed cases were operated because of one nonunion, one pseudoarthrosis, and one displaced union. All cases which underwent early surgical had a very good outcome with a follow-up observation period ranging from 8 months to 7 years. On long term follow-up of the delayed cases outcome was good in 2 cases. In one case with a 10 year history of pseudoarthrosis, the surgery resulted in a slight improvement. Basing on our observations, the authors believe early surgical intervention in indicated in order to achieve good outcomes. Delayed surgical treatment is justified in cases with elbow dysfunction.

Lymphocyte subpopulations and autologous mixed lymphocyte reaction (AMLR) studied in 13 hereditary sphereocytosis and 21 postsplenectomy patients; the operation was performed because of hereditary spherocytosis. Significant increases of Th cells and the Th/Ts ratio have been noted in children with hereditary spherocytosis. Splenectomy because of spherocytosis has resulted in an increase in Ts cell number and diminution of Th/Ts ratio. Hereditary spherocytosis patients showed both diminished natural killer cell activity and AMLR proliferation. Splenectomy normalized the above parameters. The presented data demonstrated disturbances in T lymphocyte number and functions both before and after splenectomy in children with hereditary spherocytosis.

Splenectomized individuals have been reported to have many immunological abnormalities. We have studied the proliferative response of T cells in AMLR of nine children who had undergone splenectomy after trauma. T cells from splenectomized patients showed increased proliferative response in AMLR as compared with that observed in parallelly studied controls (38.480 +/- 19.700 vs 11.160 +/- 3.760 dpm). The percentage of CD4 cells was diminished in the group of splenectomized patients. We found strong correlation between AMLR and time after splenectomy (r = -0.81, p less than 0.01). Possible mechanism of elevated AMLR following splenectomy and hypotheses on generation of AMLR are discussed.
